Abstract

【課題】トモシンセシスの自然な３Ｄ局所化能力を利用する生検システムを提供する。
【解決手段】Ｘ線システムに取り付ける針生検組立体１０である。生検針は検出器及びＸ線源のうちの少なくとも１つに対して傾斜され、生検手術中に、生検装置による干渉を伴わずにＸ線撮像を行い得る。定位生検装置を、直立であれ傾斜であれ、乳房Ｘ線撮像システム、トモシンセシスシステム、及び、乳房Ｘ線撮像システム／トモシンセシスシステムの組み合わせを非限定的に含む、如何なるＸ線システムにも連結し得る。システムは、標的視覚化及び標的局所化の一方又は両方のために、如何なる画像捕捉モード(即ち、偵察、二次元乳房Ｘ線撮像、三次元再構築容積)の使用をも柔軟に支持する。そのような構成を備えることで、様々な異なるＸ線撮像プラットフォームとの使用のために、改良された患者適用範囲を有する。

Description

この出願は、３５ＵＳＣ§１．１１９(ｅ)に従い、参照としてここに引用する２００９年１０月８日に出願された米国仮特許番号６１／２４９，７７２号の優先権を主張する。
マンモグラフィ(乳房Ｘ線撮影)は、乳ガン検診及び診察に使用し得る乳房撮像の確立した方法である。スクリーニング乳房Ｘ線撮影像は、４０歳を超える人口の女性構成員又は遺伝学的に乳ガンの危険性のある者のために、年１回得られるのが好ましい。スクリーニング乳房Ｘ線撮影像中に塊又は石炭化(関心領域)が特定されるならば、患者は更なる診断を必要とし得る。そのような診断は、関心領域を生検し且つ切除組織を分析することを含み得る。
乳房生検(breast biopsy)の間、様々な撮像モダリティ(imaging modality)が歴史的に使用されている。撮像モダリティは、超音波撮像、Ｘ線撮像、及び、磁気共鳴撮像を含む。乳房生検の実施は、典型的には、患者を位置付けること、撮像機器を使用して関心領域を視覚化すること、関心領域の座標を標的化すること、及び、標的化領域から細胞又は組織を回収することを含む。観血手術(open surgery)、細針吸引(fine needle aspiration)、コア針生検(core needle biopsy)、又は、真空補助生検(vacuum assisted
biopsy)を含む様々な方法で、細胞又は組織を回収し得る。最も侵襲的な手術である観血手術は、関心領域の視覚化中にワイヤを乳房内に配置する放射線医によって行われるのが一般的であり、その場合、ワイヤは切除されるべき関心領域内に延びる。次に、患者は手術に移され、関心領域を位置決めするワイヤを使用して、組織が回収(retrieve)される。
細針吸引、針生検、及び、真空補助生検は、観血手術よりも低侵襲的であり、観血手術用の針を必要とせずに細胞及び組織が得られることを可能にする。全てが針生検(needle biopsy)であり、針の大きさ、よって、生検サンプルの対応する大きさ（及び数）が差別化要因である。各手術において、患者は位置付けられ、関心領域が視覚化され、生検装置の針が標的関心領域に進められ、組織が回収される。細針吸引及びコア針生検装置は、典型的には、１つの組織サンプルを回収し、超音波のような撮像モダリティを使用して、標的へのそれらの前進を監視し得る。真空補助生検装置は、一般的には、より大きな針を有し、多数のコアを抽出し得る。
標的領域を三次元容積で視覚化することが望ましいので、一般的には、定位モードのＸ線撮像が乳房生検のために使用される。定位生検は、少なくとも２つの平面内で取られるＸ線画像を使用して容積情報を取得する。次に、Ｘ線画像は、標的領域の深さ又はＺ寸法を決定する視差の原理を使用して処理され、関心領域を三次元空間内に局所化する。
参照としてここに組み込む２００８年２月２１日に出願された特許文献１は、乳房生検中の病巣局所化のためにトモシンセシス(tomosynthesis)を使用するシステム及び方法を記載している。トモシンセシス(トモ)は三次元(３Ｄ)乳房Ｘ線撮像を行う方法である。トモシンセシスは圧縮された乳房を通じる断面スライスの画像を生成し、乳房病理を特定するためにも使用される。トモシンセシスの利点の１つは、画像が三次元であることであるので、関心領域が画像内で決定されるや否や、例えば、スライス画像中のｘ，ｙ座標から、並びに、画像スライス深さ場所によって与えられるｚ又は深さ座標から、乳房中のその正確な３Ｄ座標を計算又は推定し得ることである。トモシンセシスの他の利点は、乳房内の異なる高さにある物体からの画像の抑圧(suppression)によって、物体の高いコントラスト可視性(contrast
visibility)をもたらす能力である。その優れたコントラスト可視性の故に、標準的なＸ線マンモグラフィ、定位装置、又は、超音波を使用しても可視的でなく、ＭＲＩを使用してさえも可視的でない病理が、トモシンセシス画像上に見られることが予期される。

この目的のために、トモシンセシスの自然な３Ｄ局所化能力を利用するトモシンセシスシステムを使用する局所化方法を開発することが望ましい。
本発明の１つの特徴によれば、Ｘ線撮像システムのＸ線源と検出器との間の取付けのために、定位針生検組立体が提供される。定位組立体は、検出器によって定められる平面に対して垂直な平面から偏心した角度で生検装置を支持するための取付けアームを含む。一部の実施態様では、組立体は、乳房への横方向アクセスを許容する、横方向側方アームも含み得る。定位針生検組立体は、組織を切除するために生検中の標的場所への生検装置の自動化された案内のための案内モジュールを含む。生検針は検出器に対して角度付けられるので、生検手術中に、生検装置による干渉を伴わずにＸ線撮像を行い得る。加えて、角度付き生検針は、患者の腋窩及び胸壁への改良されたアクセスを可能にする。
１つの実施態様において、針生検組立体は、特定の生検標的場所に向かう生検装置の自動化された前進を可能にするモータ又は均等機構を含む。加えて、システムは、装置の手動前進を可能にする機構を含む。システムは、自動制御及び手動制御の間の移行のために、使用者が標的に向かう生検経路に沿って停止場所を定めるのを許容する。
１つの実施態様において、針生検組立体は、針生検組立体の上に取り付けられる制御モジュールを含み得る。制御モジュールは、医療人員が、患者から離れることなく、標的に向かう装置の自動動作を制御することを可能にする。一部の実施態様では、制御モジュールは、針及び標的の相対場所のような生検手術に関する情報を表示し得る。例えば、胸壁、乳房プラットフォーム、又は、他の望ましくない位置への針の近接性を警告するために、制御モジュールは使用者に視覚的又は可聴的な警報ももたらし得る。１つの実施態様では、生検装置の意図されない前進を排除するために、制御モジュールの制御ボタンが配置される。
本発明の定位生検装置を、直立であれ傾斜であれ、乳房Ｘ線撮像システム、トモシンセシスシステム、及び、乳房Ｘ線撮像／トモシンセシスシステムの組み合わせを非限定的に含む、如何なるＸ線システムにも連結し得る。システムは、如何なる画像捕捉モード(即ち、偵察、二次元乳房Ｘ線撮像、三次元再構築容積)及び標的視覚化及び標的局所化の一方又は両方のための二次元又は三次元の画像データの如何なる組み合わせの使用をも柔軟に支持する。そのような構成を用いるならば、様々な異なるＸ線撮像プラットフォームとの使用のために、改良された患者適用範囲を有する針生検組立体がもたらされる。
具体的には、そのような組立体をトモシンセシス撮像システムに容易に統合し得る。トモシンセシス投射画像を所望の定位角度で捕捉することによって、針生検組立体と共に使用するための自動化された定位画像捕捉をもたらすよう、そのようなトモシンセシス撮像システムを直ちに構成し得る。そのようなシステムは、患者露出を削減するという追加的な利点を有する。何故ならば、トモシンセシス投射画像は、一般的には、従来的な乳房Ｘ線撮像よりも低い線量で得られ、従って、(視覚化又は標的化における使用のための)定位容積情報を削減された線量で得ることができる。これらの及び他の利点を以下に更に詳細に記載する。

乳房トモシンセシスシステムは、一般的には、ガントレの回転可能なアームの上に取り付けられるＸ線源と、Ｘ線源がゼロ位置にあるときにＸ線源に対して概ね垂直に位置付けられるＸ線検出器とを含む。トモシンセシス画像の取得中、Ｘ線源は限定的な角度範囲に亘って回転される。Ｘ線源軌道内の様々な地点で、Ｘ線源は活性化され、画像が検出器によって捕捉される。各地点で捕捉される各画像は投射画像と呼ばれる。投射画像から三次元容積を再構築するためにコンピュータプログラムが使用され、三次元容積は病巣検出のために使用される。乳房Ｘ線撮影及びトモシンセシス撮像可能であり且つ本発明を組み込むよう構成し得るＸ線撮像システムの１つの実施例を図１に示す。
乳房Ｘ線撮影／トモシンセシスシステムは、獲得ワークステーション(ＡＷＳ)４と、Ｘ線撮像組立体２を支持するガントリ１とを含むように示されている。そのような乳房Ｘ線撮影／トモシンセシスシステムは、Selenia Dimensionsという商号(trade name)の下にある同一承継人から現在入手可能であるが、本発明の針生検組立体１０を取り付け得る１つのＸ線システムを代表するものであるに過ぎない。ガントリ１は、システムを操作する医療専門家によって制御されるモータによって駆動されるＣアームを支持し、Ｃアームは、ガントリに沿って選択的な高さに上下移動可能である。Ｃアームは、上方端部でＸ線管２ａを支持し、下方端部で乳房トレイ２ｂを支持する。トレイ２ｂは、フラットパネルＸ線画像受容器２ｃを覆い、受容器２ｃは、（トレイ２ｂと受容器２ｃとの間の空間から取り除き得るよう後退可能な）集束散乱防止グリッド２ｄによってトレイから離間する。Ｃアームは、Ｘ線源２ａと乳房トレイ２ａとの間にある圧縮パドル２ｅも支持し、圧縮パドル２ｅは、患者の乳房を圧縮し且つ固定し得るよう、患者の乳房がトレイ２ｂとパドル２ｅとの間に適合し得るように、トレイ２ｂから離れる方向に移動するよう電動化される。パドル２ｅの動作は電動化され、且つ、医療従事者によって制御される。異なる乳房の大きさに適し且つ撮像ニーズ（即ち、スクリーニング又は診察のため）に適するよう、異なる大きさ及び異なる構造のパドル２ｅをガントリの上に適合し得る。加えて、医療専門家は、同一承継人によって現在提供されるSeleniaシステムにおけるように、パドル２ｅを、トレイ２ｂの幅に沿って、パドル２ｅがトレイ２ｂ上で中心化されない乳房の位置と合致する位置に移動し得る。システムは、キーボード４ａ及びトラックボール４ｂ、ディスプレイスクリーン４ｃ、制御及び画像処理設備のようなインターフェース装置を含む制御ステーション４のような、他の構成部品を更に含む。
本発明の１つの特徴によれば、針生検組立体１０を撮像システム２のＸ線源とＸ線検出器との間に容易に取り付け得る。以前の生検組立体と異なり、本発明の針生検組立体は、圧縮装置及びＸ線検出器を含むＸ線システムの既存の構成部品の全てを利用する。結果的に、針生検組立体は、直立針生検能力を様々な現在のＸ線撮像プラットフォームに利用可能にする、低コストの解決策であることが理解されよう。

図３Ａ及び３Ｂは、Ｘ線撮像システムのガントリ上に取り付けられる針生検組立体１０を例示している。針生検組立体の様々な実施態様を直立撮像システム又は傾斜撮像システムのいずれかと共に使用し得る。本出願の目的のために、(Hologic, Inc.によって提供されるDimensions(TM)乳房トモシンセシス撮像システムのような)直立乳房トモシンセシス撮像システムと共に使用するための実施態様を記載する。例示的なトモシンセシス撮像システムは、コーンビーム又は他のＸ線源を支持する管ヘッド３２と、Ｘ線検出器を入れる圧縮プラットフォーム３０とを含み得る。管ヘッド３２は、ガントリ(図示せず)上に回転可能に取り付けられ、管ヘッド３２が図３Ｂ中に破線４１によって概ね示される角軌道に沿って回転することを可能にする。
１つの実施態様において、針生検組立体１０は、針生検組立体をトモシンセシス撮像システムのガントリに取り付けるためのクランプ、フック、又は、他の取付け手段を含む。有利には、そのような再使用は要件ではないが、クランプは、(例えば、フェースシールド等のような)他の取付け装置を支持するガントリの機能に適合される。
図３Ａ及び３Ｂの実施例において、ホルスタ１３は固定角度アーム１２上のホルスタ取付部に連結され、固定角度アーム１２は垂直から１０度だけ偏心した角度で支持アーム１７に固定的に取り付けられるが、偏心角度が異なり得ること及び殆ど偏心角度が設計上の選択の問題であることが直ちに理解されよう。固定角度アーム１２(及び、結果的に、生検装置１５)を角度付けることは、生検装置及びホルスタがＸ線画像中にアーチファクトを導入することなく、生検装置が生検標的領域内の(標的領域５０によって概ね示される)所望の場所まで前進させられることを可能にする。図３Ｂに示されるように、コーンビームＸ線源は標的領域５０内に延びるが、生検装置１５はコーンビーム内に入らない。１０度の定角が開示されているが、本発明は如何なる定角にも限定されないことが留意されるべきであり、選択的な定角は撮像システム及び組織除去工具の特定の幾何学的構成に応じて異なり得ることが理解されよう。最広義には、本発明はＸ線撮像中に針によって引き起こされる視覚的なアーチファクトの導入を限定するよう生検針を角度付けるという着想を包含する。
図４は、本発明の針生検組立体の例示的な実施態様の側面図である。図４において、線Ａは、Ｘ線検出器３０の平面に対して「垂直」な平面内にある。線Ｂは、生検装置の角度変位を例示しており、従って、装置はΘの角度だけ垂直から偏心している。結果的に、生検装置は生検撮像と干渉する。
図４に同様により詳細に示されているのは、針生検組立体１０の例示的な連結機構５０ａ及び５０ｂである。連結機構５０ａ及び５０ｂは、ガントリの相補的特徴と合致するよう構成される。ここでは、ラッチ、フック、スロット等を含む他の種類の連結機構を均等物として置換し得る。
図５は、針生検組立体の例示的な実施態様を示す上面図であり、参照の容易性のために、印の付けられた構成部品のサブセットだけが示されている。この視野から針生検組立体を見るとき、生検装置の角傾斜に起因する針先端の変位Ｎが直ちに明らかである。この図面は、従来技術の定位装置を使用してアクセスするのが以前は困難であった(腋窩組織及び胸壁付近の組織のような)乳房の領域を生検する、この装置の能力を最良に例示している。
図６Ａ乃至６Ｃは、本発明と共に使用し得る様々な圧縮パドルを例示している。生検針を受け入れるための貫通開口を有する、Ｘ線不透過性材料で概ね製造される従来技術の針生検圧縮パドルとは対照的に、圧縮パドル６２Ａ−６２ＣはＸ線透過性であるのが好ましい。スロット６４ａ及び６５ｂが、圧縮パドルがＸ線撮像システムの圧縮装置に取り外し可能に連結されるのを可能にするように位置付けられ、パドルが乳ガンスクリーニングに使用される乳房Ｘ線撮像／トモシンセシスパドルと容易に交換されるのを可能にする。図６Ｂ及び６Ｃに示されるように、異なる大きさの乳房のために使用されるよう、生検圧縮パドルを異なる大きさに製造し得る。開口６６が各生検圧縮パドルに設けられる。開口６６は標的と関連付する乳房の部分が露出されるのを可能にする。図７に示されるように、開口６６を取り囲む圧縮パドル６２ａの部分６８は、生検される乳房の部分に対する圧縮を増大し、それによって、生検領域を安定化するよう成形される。
Ｘ線透過性の乳房圧縮パドルを使用する１つの利点は、検出器の全景を可能にすることである。従来技術の針生検構成では、標的局所化に関連する検出器の一部のみが可視的であった。従来技術の生検は圧縮パドルの開口の直ぐ下にある組織の部分に対してのみ行われたので、これは十分であった。しかしながら、本発明の傾斜針は、実際には図５に示されるリーチＮを有する圧縮パドル開口−針の境界を越える生検のために利用可能な組織の量を増大する。よって、上述のように、生検中に腋窩組織及び胸壁付近の組織をより容易に切除し得る。
図８は、図６及び７に示されたようなＸ線透過性生検パドルと共に撮影された乳房の２Ｄ画像を例示している。図８に示されるように、Ｘ線透過性圧縮パドルの１つの利点は、検出器画像全体を獲得ワークステーションインターフェースで見ることができることである。情報に対するアクセスにおいて、Ｘ線医は最早制限されない。結果的に、Ｘ線医は、圧縮パドル開口６６を越えて延びる病巣８０の部分を見ることができる。正しい診断のために全ての組織がアクセスされることを保証するために、本発明の傾斜生検針組立体を使用して乳房のこの部分にアクセスし得る。
上述したような例示的な針生検組立体を概ね以下のように使用し得る。生検の候補者として特定された患者がＸ線撮像システムに位置付けられる。ステップ９１で、生検圧縮パドルが圧縮プラットフォームに向かって下に移動し、患者の乳房を圧縮し、病巣を視覚化するプロセスがステップ９２で開始される。簡潔に上述したように、Ｘ線撮像システムの能力に依存して、偵察画像(scout image)、乳房Ｘ線撮影像、獲得定位画像、獲得トモシンセシス投射画像、トモシンセシス再構築画像、又は、これらの任意の組み合わせを使用して、病巣の視覚化を行い得る。１つの実施態様では、トモシンセシス能力を有するＸ線画像システムを、「定位モード」を含むよう構成可能であり、定位モードが選択されるとき、定位モードは、Ｘ線撮像システムに、典型的な＋／−１５度の定位画像を自動的に回収させ、定位画像に対して適切な撮像処理を行って定位容積を引き出す。そのような実施態様の１つの利点は、投射画像獲得中により低い線量を使用するトモシンセシス撮像システムにおける患者露出を減少し得ることである。
病巣が視覚化されるや否や、ステップ９３で、病巣が標的化される。病巣の標的化は、画像データを使用して病巣の座標を特定すること、及び、当業者に既知の変換技法を使用して、画像のカーテシアン座標系からの座標を傾斜生検組立体の角座標系に変換することを含む。本発明の１つの特徴によれば、病巣を標的化するためにというよりも、病巣を視覚化するために、異なる画像又は画像の組み合わせを使用し得る。例えば、患者が所定位置にいることを保証するために偵察画像が初めに使用され、病巣を視覚化するために一対の定位画像が使用されることを想定のこと。病巣が偵察画像中に発見されるが、両方の定位画像中に発見されないならば、偵察画像を定位画像との組み合わせで使用し得る。その場合には、標的場所情報を引き出すために、病巣が局所化される。従って、上述のように、Ｘ線撮像システムの能力に依存して、偵察画像、乳房Ｘ線撮影像、獲得定位画像、獲得トモシンセシス投射画像、トモシンセシス再構築画像、又は、これらの任意の組み合わせを使用して、病巣標的座標を引き出し得る。
ステップ９４で、標的座標が引き出されるや否や、医療専門家は、生検針を移動するのに必要とされる制御ボタンを押すことによって生検出術を開始し得る。図１０は、制御ユニット２５の例示的なディスプレイ及びボタンを例示している。制御ユニットは、針サイズ、プラットフォーム及びパドルへの距離、針座標、標的座標、標的への近接性、及び、他の関連情報のような、生検に関連する情報を表示するためのディスプレイ２８を含む。制御パネルは、針が乳房プラットフォーム、胸壁、又は、皮膚線に近過ぎるときの警告表示のような、使用者に他の有用な情報も提供し得る。上述のように、警告表示は着色コード付きであってよく、或いは、望ましくない条件の他の視覚表示又は可聴表示を提供してもよい。
上述のように、１つの実施態様において、制御ユニットは、生検組立体を片手で作動させることを可能にするが、偶発的な作動を妨げるよう位置付けられ且つ配置される、ボタン(ボタン２７を含む)も含む。１つの実施態様では、一対の制御ボタンが提供され、一方は制御パネルの正面にあり、他方は制御パネルの背面にある。両方のボタンの同時の押圧を介してのみ、生検組立体動作を活性化（作動）し得る。ここでは、本発明の範囲に影響を及ぼすことなく、操作者の意図を追認するための他の機構を置換し得る。
本発明の１つの特徴によれば、機械的ストップを生検経路内に導入して、生検経路に沿う特定地点での自動針動作を停止し得る。例えば、針が標的の所望範囲内にあるとき、ノブ３３ａ又は３３ｂを介して、針動作の手動制御に切り換えることが望ましくあり得る。或いは、乳房内への針遅い挿入のために解放ブレーキを提供することが望ましくあり得る。１つの実施態様によれば、機械的ストップの数及び場所は、システムの使用者の個別の好みに誂え得るプログラム可能な機能である。ギア掛け前進、ピストン前進等を含む針の機械的前進のための様々な方法が考えられると言えば十分であろう。
図９を今や参照すると、ステップ９５で、針が標的座標まで前進させられるや否や、針が実際に病巣に位置付けられたことを確証するために、画像を獲得し得る。生検針はＸ線撮像システムの視野の外にあるので、そのような画像を干渉なしに得ることができる。ステップ９６で、針が標的にあることが検証されるや否や、組織を切除可能であり、生検は完了する。フロー図に明示的に示されていないが、病巣全体の切除を検証するために、ステップ９５及び９６を反復し得る。
従って、生検針組立体が示され且つ記載された。本発明の他の特徴によれば、針生検制御能力を伴って、獲得ワークステーションのユーザーインターフェースが有利に増大される。針生検組立体と共に使用するために獲得ワークステーションに加え得るユーザーインターフェース機能の実施例を図１１に示す。ユーザーインターフェースは、Ｘ線医が生検中に収集された情報を表示し且つ出力することを制御することを可能にするメニュー及び／又は制御ボタンを含む。標的化工具１１０は使用者が標的情報を検討し、変更し、削除することを可能にする。(定位、トモシンセシス、偵察、乳房Ｘ線撮像等を含む)画像源セレクタ１２０は、Ｘ線医が視覚化又は標的化のためにどの画像を使用するかを選択することを可能にする。画像ビューセレクタ１３０は、Ｘ線医が生検中に獲得し得る画像のうちのいずれかを迅速に引き出し且つ検討することを可能にする。画像のいずれをも画像窓１５０内に引き出し得る。生検装置の種類、標的と圧縮プレート／プラットフォームとの間の相対距離等のような生検に関する他の情報も獲得ワークステーション上に含め得る。ボタン及びアイコンの具体的な配置が図１１の代表的な図面中に示されているが、本発明はそのような情報の如何なる特定の表示に限定されず、他の形態のプルダウンメニュー、ハイパーリンク、アイコン、ボタン、及び、窓が、それらと均等であると考えられることが留意されるべきである。
上記実施態様は、針が乳房の上に概ね頭蓋の尾側向きに位置付けられるときに心材(core)を得るための針生検組立体を記載し且つ例示したが、本発明の１つの特徴によれば、横方向生検アームの取付けを通じて、乳房への横方向アクセスを達成し得る。横方向生検を遂行する能力は、薄い乳房を有する患者にとって特に重要である。何故ならば、圧縮は、乳房の厚みを、生検針の遠位先端での組織切除ポートを組織内に十分に挿入し得ない程度まで減少し得るからである。図１２Ａ−１２Ｃは、横方向生検アーム２００で増強された本発明の針生検組立体の実施態様の様々な図面を例示している。図１２Ａは、増強組立体の正面図であり、図１２Ｂは、増強組立体の斜視図であり、図１２Ｃは、増強組立体の側面図である。横方向生検アームは、検出器と実質的に平行な線に沿って生検針を支持し、それによって、乳房組織の横方向切除を許容する。図１２Ａ−１２Ｃの実施態様において、横方向生検アームは、ホルスタ取付板２１０と、指動輪(サムホイール)２２０とを含む。指動輪２２０の回転は、図１２Ａ中に概ねＸとして示される方向における針の手動並進（平行移動）をもたらす。Ｘ平面内の手動並進が示されているが、制御を案内モジュール１９からホルスタ板２１０に拡張することによって、自動並進も達成し得ることが理解されよう。
針が頭蓋−尾側向きにあると想定して、針がＸ、Ｙ、及び、Ｚ方向においてどの程度まで遠くに移動すべきかを定める、病巣の標的化が常に行われる。本発明の１つの特徴によれば、適切な病巣局所化のために、案内モジュール１９が案内座標系を適宜調節することを可能にするよう、横方向生検アームがホルスタ取付部１３と連結されるとき、ホルスタ取付部上の電気接続は、横方向生検アーム上のコネクタを据え付ける。
従って、多数の利点を有する傾斜針生検組立体が示され且つ記載された。生検針は検出器及びＸ線源の少なくとも一方に対して傾斜させられるので、生検装置による干渉なく生検手術中にＸ線撮像を行い得る。加えて、傾斜生検針は、患者の腋窩及び胸壁への改良されたアクセスを可能にする。本発明の定位生検装置を、直立であれ傾斜であれ、乳房Ｘ線撮像システム、トモシンセシスシステム、及び、乳房Ｘ線撮像システム／トモシンセシスシステムの組み合わせを含む、如何なるＸ線システムにも連結し得るが、それらのシステムに限定されない。システムは、標的視覚化及び標的局所化の一方又は両方のために、如何なるモードの画像捕捉の使用(即ち、偵察、二次元乳房Ｘ線撮像、三次元再構築容積)をも柔軟に支持する。そのような構成を用いるならば、様々な異なるＸ線撮像プラットフォームとの使用のために、改良された患者適用範囲(patient coverage)を有する針生検組立体が提供される。
例示的な実施態様を記載したが、上述の実施例は例示的であるに過ぎないこと、及び、他の実施例も付属の請求項の範囲内に含まれることを理解し得るであろう。

Claims (12)

  1. − 一端にＸ線源を含み他端にＸ線検出器を含むＣアームと、
    − 概ね前記Ｘ線源と前記Ｘ線検出器との間で前記Ｃアームに解放可能に連結される針生検組立体とを含み、
    前記Ｘ線源は、トモシンセシス撮像のために前記Ｘ線検出器に対する角軌道に沿って回転するように構成され、
    前記針生検組立体は、
    − 固定支持アームと、
    − 前記Ｘ線検出器の平面から偏心した角度で前記固定支持アームに連結される角度支持アームと、
    − 該角度支持アームに連結される生検装置とを含み、該生検装置は、針を含み、該針は、前記Ｘ線検出器の前記平面から偏心した角度で位置付けられる、
    直立Ｘ線撮像及び生検システム。
  2. ホルスタ取付部を更に含み、該ホルスタ取付部は、前記ホルスタ取付部に連結される前記生検装置を標的場所に位置付けるために前記角度支持アームに移動可能に連結される、請求項１に記載の直立Ｘ線撮像及び生検システム。
  3. 前記針生検組立体の前記固定支持アームと支持構造との間に連結される案内モジュールを更に含み、該案内モジュールは、前記標的場所の座標を決定し、前記決定される座標への前記ホルスタ取付部及び連結される生検装置の自動化された動作を制御する、請求項２に記載の直立Ｘ線撮像及び生検システム。
  4. 前記標的場所の前記座標は、前記Ｘ線撮像システムによって獲得されるＸ線画像データを用いて決定される、請求項１乃至３のうちのいずれか１項に記載の直立Ｘ線撮像及び生検システム。
  5. 前記Ｘ線画像データは、偵察画像、乳房Ｘ線写真、定位画像、トモシンセシス投射画像のような、二次元画像データを含む、請求項４に記載の直立Ｘ線撮像及び生検システム。
  6. 前記Ｘ線画像データは、定位容積データ及びトモシンセシス再構築データのような、三次元画像データを含む、請求項４に記載の直立Ｘ線撮像及び生検システム。
  7. 前記Ｘ線画像データは、二次元画像データ及び三次元画像データの組み合わせを含む、請求項４に記載の直立Ｘ線撮像及び生検システム。
  8. 前記固定支持アームと前記角度支持アームとの間の前記角度を調節するための調節機構を更に含む、請求項１乃至７のうちのいずれか１項に記載の直立Ｘ線撮像及び生検システム。
  9. 横方向生検の間に生検針を支持するために、前記組立体に連結される横方向生検アームを更に含む、請求項１乃至８のうちのいずれか１項に記載の直立Ｘ線撮像及び生検システム。
  10. 当該直立Ｘ線撮像及び生検システムによって捕捉されるトモシンセシス画像を検討するために当該直立Ｘ線撮像及び生検システムに連結される獲得ワークステーションを更に含み、該獲得ワークステーションは、当該直立Ｘ線撮像及び生検システムの生検モードと関連付けられるユーザーインターフェースを有し、該ユーザーインターフェースは、生検中に病巣を視覚化すること及び標的化することのうちの少なくとも一方における使用のために画像データを選択するための機構を有する、請求項１乃至９のうちのいずれか１項に記載の直立Ｘ線撮像及び生検システム。
  11. 一端にＸ線源を含み他端にＸ線検出器を含むＣアームを有する種類の直立Ｘ線撮像及び生検システムのための針生検組立体であって、
    前記Ｘ線源は、トモシンセシス撮像のために前記Ｘ線検出器に対する角軌道に沿って回転するように構成され、当該針生検組立体は、概ね前記Ｘ線源と前記Ｘ線検出器との間で前記Ｃアームに解放可能に連結され、前記針生検組立体は、
    固定支持アームと、
    前記Ｘ線検出器の平面から偏心した角度で前記固定支持アームに連結される角度支持アームと、
    該角度支持アームに連結される生検装置とを含み、該生検装置は、針を含み、該針は、前記Ｘ線検出器の前記平面から偏心した角度で位置付けられる、
    針生検組立体。
  12. 直立Ｘ線撮像及び生検システムを用いて乳房生検を行う方法であって、
    前記直立Ｘ線撮像及び生検システムは、
    一端にＸ線源を有し他端にＸ線検出器を有するＣアームと、
    概ね前記Ｘ線源と前記Ｘ線検出器との間で前記Ｃアームに解放可能に連結される針生検組立体とを含み、
    前記Ｘ線源は、トモシンセシス撮像のために前記Ｘ線検出器に対する角軌道に沿って回転するように構成され、
    前記針生検組立体は、固定支持アームと、前記Ｘ線検出器の平面から偏心した角度で前記固定支持アームに連結される角度支持アームと、該角度支持アームに連結される生検装置とを有し、該生検装置は、針を含み、該針は、前記Ｘ線検出器の前記平面から偏心した角度で位置付けられ、
    当該方法は、
    生検されるべき乳房を圧縮するステップと、
    前記乳房内の病巣を視覚化するステップと、
    前記病巣の座標を決定することを含む、前記視覚化される病巣を標的化するステップと、
    前記Ｘ線検出器の平面から偏心した角度で前記生検装置の針を前記標的化される病巣に向かって前進させるステップと、
    前記病巣を切除するステップとを含む、
    方法。
