前記ブドウ糖センサ及び前記ポンプのうちの少なくとも1つから信号を受信するコントローラであって、前記コントローラは、前記ポンプへ信号を発信して、前記被験者の所望のブドウ糖値、送達されるインスリン量、及び測定されるブドウ糖値に基づく前記被験者のモデル予測制御を利用するフィードバックコントローラが決定する量のインスリンを送達するように構成され、かつ前記被験者がインスリンの手動ボーラス投与を開始し、感知又は測定されるブドウ糖値が第1の持続時間内に少なくとも第1の閾値であるときに必ず、少なくとも基礎量のインスリンを送達するように構成される、コントローラと、を備える、糖尿病管理システム。
(2) 前記第1の閾値が血液1デシリットル当たり約120ミリグラムのブドウ糖を含み、前記第1の持続時間が約45分を含む、実施態様1に記載のシステム。
(3) 前記ブドウ糖センサが、一過式ブドウ糖センサ及び連続式ブドウ糖センサのうちの少なくとも1つを含む、実施態様2に記載のシステム。
(4) 輸液ポンプ、コントローラ、及び少なくとも1つのブドウ糖センサを用いて被験者の糖尿病を管理する方法であって、
複数のブドウ糖測定値を提供するために前記被験者体内のブドウ糖値を前記ブドウ糖センサから測定することと、
所定の間隔にわたって前記被験者に送達されるべき算出されたインスリン量を提供するために、前記被験者の代謝状態の推定値から前記ブドウ糖値の傾向を予測するために前記複数のブドウ糖測定値を利用するモデル予測コントローラに基づき、送達のために前記コントローラによってインスリン量を算出することと、
前記ブドウ糖値が第1の時間周期内に少なくとも第1のブドウ糖閾値である間に前記被験者が手動ボーラス投与を開始したかどうか決定することと、
前記決定する工程が真である場合、インスリン送達を少なくとも基礎量に制約することと、を含む、方法。
(5) 輸液ポンプ、コントローラ、及び少なくとも1つのブドウ糖センサを用いて被験者の糖尿病を管理する方法であって、
複数のブドウ糖測定値を提供するために前記被験者体内のブドウ糖値を前記ブドウ糖センサから測定することと、
前記被験者に送達するための基礎比率を算出することと、
前記ブドウ糖値が第1の時間周期内に少なくとも第1のブドウ糖閾値のレベルである状態で前記被験者が手動ボーラス投与を開始したかどうかを決定することと、
前記決定することが真である場合、前記基礎比率でインスリンを送達するように前記輸液ポンプを制約することと、
前記決定することが偽である場合、およそゼロの比率で送達するように前記輸液ポンプを制限することと、
前記制約する工程及び前記制限する工程に基づいてインスリン投与量(insulin dosing)を算出することと、
前記算出する工程によって算出されたインスリン投与量を送達するように前記ポンプに命令することと、を含む、方法。
(7) 前記第1の時間周期が、約15分〜2時間の間の任意の値の持続時間を含む、実施態様6に記載の方法。
(8) 前記所定の間隔が、1分、3分、5分、10分、15分、20分、30分、及びこれらの組み合わせから本質的になる群から選択される間隔を含む、実施態様4に記載の方法。
(9) 前記ブドウ糖センサが、一過式ブドウ糖センサ及び連続式ブドウ糖センサのうちの少なくとも1つを含む、実施態様4又は5に記載の方法。
(10) 前記算出することが、前記被験者の代謝状態の推定値を、前記被験者体内の前記ブドウ糖値のおよそ実時間の測定値から再帰的に決定することを更に含む、実施態様4又は5に記載の方法。
Claims (10)
- 糖尿病管理システムであって、
インスリンを被験者に送達するように構成される輸液ポンプと、
前記被験者体内のブドウ糖値を感知し、前記被験者体内の前記ブドウ糖値を表す出力信号を提供するように構成されるブドウ糖センサと、
前記ブドウ糖センサ及び前記ポンプのうちの少なくとも1つから信号を受信するコントローラであって、前記コントローラは、前記ポンプへ信号を発信して、前記被験者の所望のブドウ糖値、送達されるインスリン量、及び測定されるブドウ糖値に基づく前記被験者のモデル予測制御を利用するフィードバックコントローラが決定する量のインスリンを送達するように構成され、かつ前記被験者がインスリンの手動ボーラス投与を開始し、感知又は測定されるブドウ糖値が第1の持続時間内に少なくとも第1の閾値であるときに必ず、少なくとも基礎量のインスリンを送達するように構成される、コントローラと、を備える、糖尿病管理システム。 - 前記第1の閾値が血液1デシリットル当たり約120ミリグラムのブドウ糖を含み、前記第1の持続時間が約45分を含む、請求項1に記載のシステム。
- 前記ブドウ糖センサが、一過式ブドウ糖センサ及び連続式ブドウ糖センサのうちの少なくとも1つを含む、請求項2に記載のシステム。
- 輸液ポンプ、コントローラ、及び少なくとも1つのブドウ糖センサを用いて被験者の糖尿病を管理する方法であって、
複数のブドウ糖測定値を提供するために前記被験者体内のブドウ糖値を前記ブドウ糖センサから測定することと、
所定の間隔にわたって前記被験者に送達されるべき算出されたインスリン量を提供するために、前記被験者の代謝状態の推定値から前記ブドウ糖値の傾向を予測するために前記複数のブドウ糖測定値を利用するモデル予測コントローラに基づき、送達のために前記コントローラによってインスリン量を算出することと、
前記ブドウ糖値が第1の時間周期内に少なくとも第1のブドウ糖閾値である間に前記被験者が手動ボーラス投与を開始したかどうか決定することと、
前記決定する工程が真である場合、インスリン送達を少なくとも基礎量に制約することと、を含む、方法。 - 輸液ポンプ、コントローラ、及び少なくとも1つのブドウ糖センサを用いて被験者の糖尿病を管理する方法であって、
複数のブドウ糖測定値を提供するために前記被験者体内のブドウ糖値を前記ブドウ糖センサから測定することと、
前記被験者に送達するための基礎比率を算出することと、
前記ブドウ糖値が第1の時間周期内に少なくとも第1のブドウ糖閾値のレベルである状態で前記被験者が手動ボーラス投与を開始したかどうかを決定することと、
前記決定することが真である場合、前記基礎比率でインスリンを送達するように前記輸液ポンプを制約することと、
前記決定することが偽である場合、およそゼロの比率で送達するように前記輸液ポンプを制限することと、
前記制約する工程及び前記制限する工程に基づいてインスリン投与量を算出することと、
前記算出する工程によって算出されたインスリン投与量を送達するように前記ポンプに命令することと、を含む、方法。 - 前記第1のブドウ糖閾値が、血液1デシリットル当たり約80mgのブドウ糖から血液1デシリットル当たり約180mgのブドウ糖の間の任意の濃度のブドウ糖濃度を含む、請求項4又は5に記載の方法。
- 前記第1の時間周期が、約15分〜2時間の間の任意の値の持続時間を含む、請求項6に記載の方法。
- 前記所定の間隔が、1分、3分、5分、10分、15分、20分、30分、及びこれらの組み合わせから本質的になる群から選択される間隔を含む、請求項4に記載の方法。
- 前記ブドウ糖センサが、一過式ブドウ糖センサ及び連続式ブドウ糖センサのうちの少なくとも1つを含む、請求項4又は5に記載の方法。
