本発明は、個別の肺区画内の肺疾患のレベルを査定するシステム、方法、装置およびキットを提供する。肺区画は、例えば葉または部分のような、肺の小部分を含む。個別の肺区画をテストし、かつ肺の個別の小部分または区画を反映する疾患パラメータの値を決定することにより、肺系の疾患レベルが、より正確に規定され得る。同様に、さらなる診断情報を提供するために、区画は、別個に画像作成され得る。一旦個別の区画が特徴付けされると、それらは、例えば疾患レベルまたは処置の必要性を反映する多数の変数に基づき、比較および分類され得る。このような比較は、このような変数または画像を視覚的表示装置上に同時表示することによって補助され得る。さらに、同じ診断テストが、全体としての肺、または両肺に対して、かつ全体的肺性能に対する罹病した肺区画の効果を決定するために、行われ得る。さらに、罹病した肺区画は、一時的に分離され、かつ診断テストが、肺性能に対する分離の影響を決定するために、肺残部に対して行われ得る。結果として、最も有益な処置選択肢が選択され得る。
本発明は、患者の疾患レベルの診断、処置および監視または臨時査定に使用し得る、肺の機能および/または外見に関する多数のパラメータの1つまたはそれ以上を測定する肺診断システムを提供する。図1〜2に示すように、気管支内肺診断(EPD)装置102が、このようなシステム100の中心となる。EPD装置102は、意図される用途次第で、本明細書中以下に記載される、多様な機構および特徴を含み得る。この装置は、1つまたはそれ以上の肺通路を通して肺区画に到達するように構成された、図示されるような、肺カテーテル120と接続可能である。ここで、このカテーテル120は、近位端122、遠位端124、そこを通る必要に応じた管腔126、および必要に応じた閉塞部材128を有するものとして示され、管腔126および閉塞部材128は、破線で示される。多様な異なるタイプの肺カテーテル120が、使用され得、その少数の実施形態が、後段で論じられる。
多数の実施形態において、肺カテーテル120は、アクセスカテーテル10を含む。代表的アクセスカテーテル10が、図4に図解されており、かつカテーテル本体12を含む。このカテーテル本体12は、遠位端14、近位端16、その遠位端付近にある膨張式閉塞バルーン18、およびそこを通る少なくとも1つの管腔を有する。通常、このカテーテルは、少なくとも2つの管腔を有し、かつカテーテル10は、中央管腔20と、内部本体部材24およびこの内部本体部材の周りに同軸上に配置された外部本体部材26によって規定される環状管腔22との両方を含む。この環状管腔22は、近位ハブ31のポート30に開口し、かつこの環状管腔22は、バルーン18の膨張を提供する。この中央管腔20は、ハブ31のポート36に開口し、かつこの環状管腔22は、ガイドワイヤを介した任意の導入、吸引、第2カテーテル(例えば、密封カテーテル、測定カテーテル等)の導入を含む、複数の機能を提供する。
いくつかの実施形態において、肺診断システム100の測定構成要素104は、図8に示されるように肺力学ユニット200を含む。明確にするために、肺力学ユニット200は、別個の取り付け可能なユニットとして図解されるが、しかしながらこのユニット200は、EPD装置102の内部にあり得ることが認識され得る。肺力学ユニット200は、肺LNGの区画154の肺力学に関する多数の変数を測定するために使用される。

いくつかの実施形態において、肺診断システム100の測定構成要素104は、図16に示されるように、マッピングユニット700を含む。再び、明確にするために、マッピングユニット700は、別個の取り付け可能なユニットとして図解されるが、しかしながらユニット700は、EPD装置102と一体または内部にあり得ることが認識され得る。マッピングユニット700は、肺カテーテル120の位置を、それが気管支通路を通して導入および進められる時に、決定するために使用される。気管支の解剖学的構造の複数の分岐により、通路内のカテーテル120の位置は、決定することが困難であり得、かつ従って測定される肺区画154も、決定することが困難であり得る。マッピングユニット700の使用により、カテーテル120の位置は、より容易に可視的化され得る。

Claims (10)
- 肺診断システムであって、
肺区画の呼吸特徴を反映する測定データを生成する、少なくとも1つのセンサ;
該肺区画に到達するように構成された肺カテーテルと接続可能な気管支内肺診断装置であって、
該肺カテーテルを通して、該肺区画へ、または該肺区画から、流体または気体を伝達する手段と、
該少なくとも1つのセンサから該測定データを受信する手段と、
該測定データを処理する手段と、
を含み、
該測定データを処理する手段は、肺通路内にある時の該カテーテルの位置を決定する手段を含む、
装置;および
該処理済データを受信する、少なくとも1つのデータ受信構成要素、
を含む肺診断システム。 - 前記呼吸特徴は、圧力、流量、速度、酸素濃度、二酸化炭素濃度、または希ガス濃度を含む、請求項1に記載のシステム。
- 前記測定データを処理する手段は、前記測定データを分析する分析手段を含み、ここで該分析手段は、平均圧力値、容量値、コンプライアンス値、平均一回呼吸量値、抵抗値、またはこれらのいずれかの組み合わせを計算する、請求項1に記載のシステム。
- 前記流体または気体を伝達する手段は、
前記流体または気体の源と、
該流体または気体の流れを生成する手段と
を含む、請求項1に記載のシステム。 - 前記気体は、空気、酸素、二酸化炭素、希ガス、放射線不透過性気体、分極気体またはこれらのうちのいずれかの混合物を含み、ここで、前記流体は、放射線不透過性造影溶液、水、超音波画像作成流体、またはこれらの任意の組み合わせを含む、請求項4に記載のシステム。

を含む、請求項1に記載のシステム。 - 前記測定データを処理するための手段が、
前記流れ測定データから容量測定データを算出するための手段と、
前記圧力測定データおよび該容量測定データから圧力−容量曲線を生成するための手段とを含む、請求項6に記載のシステム。 - 前記肺カテーテルをさらに含む請求項1に記載のシステムであって、
そしてここで、前記少なくとも1つのセンサが、前記肺区画を特徴付ける空気速度測定データを生成する、該カテーテル上もしくは該カテーテル内に配置された少なくとも1つの速度センサを含む、
