本願は、言及することを以ってその開示内容の全てを本明細書の一部とする同日出願の米国特許出願（整理番号：ＩＳＤ‐０８３）（名称：「外科用クリップ（Surgical Clip）」）に関連する。
本発明は、外科器具に関連する。詳細には、本発明は、内視鏡を介して用いる可撓性内視鏡器具に関する。より詳細には、本発明は、管、血管、及び他の組織を締め付け及び／または縫合して組織を固定したり、組織に異物を取り付けたりできる内視鏡に適合した外科用クリップアプライヤーに関する。
外科用クリップは通常、管、血管、及び他の組織を締め付けるために用いられる。加えて、外科用クリップは、縫合やステープル止めが困難な場合に縫合やステープル止めの代わりに組織の出血を抑制するのに有用である。
外科用クリップは通常、クリップアプライヤーで組織に取り付けられる。複数のクリップを発射できる現在入手可能な外科用クリップアプライヤーは全て、トロカールポートまたは切開部からクリップを取り付ける外科部位まで送れるように構成された実質的に硬質な装置である。このような装置が硬質であるのは、硬いプッシュ部材を用いてクリップを押して組織に対してクリップを動かすためである。
可撓性クリップアプライヤー、特に内視鏡の内腔から挿入可能な可撓性クリップアプライヤーの要望が多い。内視鏡を介してクリップを取り付けることができれば、医療上の問題、特に胃腸管の問題についての様々な低侵襲性外科的解決法が可能となる。しかしながら、組織に向かってクリップを前進させたりクリップを成形するのに必要な力を、一般的には金属チューブコイルまたはポリマーチューブで形成される内視鏡装置やカテーテルなどの細長い可撓性装置の先端部に伝達するのは不可能であると一般に考えられている。
一般に、可撓性内視鏡装置（例えば、生検鉗子装置）は、通常はポリマーチューブまたは金属チューブコイルから形成される外側チューブ（外側チューブ部材に引張り応力を付与する力の伝達能力が低い）と、そのチューブ部材に対して長手方向に移動可能な制御要素と、制御要素とチューブ部材の相対運動で作動するようにチューブ部材及び制御要素の先端部に結合されたエンドエフェクタと、ハンドルに対して制御要素を移動させるそのハンドルとを含む。このタイプの可撓性内視鏡器具は、様々な理由から、生成できる押す力が制限されている。第１に、可撓性制御要素（プッシュ要素）を圧迫すると、そのプッシュ要素が装置の外側可撓性シース内で座屈しがちである。座屈に耐えるように比較的大きな直径の可撓性プッシュ要素を用いると、プッシュ要素の剛性が強すぎて内視鏡器具と共に曲がらない可能性がある。第２に、大径の可撓性プッシュ要素は、外側シース内で大きな摩擦力を受け、これによりハンドルからエンドエフェクタに伝達される力が低減してしまう。可撓性プッシュ要素が比較的小さな直径で形成されると、捻れ易いため、力が殆ど先端部に伝達されない。捩れは、内視鏡及びその内腔が曲がりくねった通路に通されることがあるため特に内視鏡器具で問題である。大抵の可撓性装置、特に金属コイルの場合、プッシュ要素に力が加えられると外側シースが伸び始める。これにより、プッシュ要素の力及び相対運動が低減または消失する。これらの理由及びその他の理由から、先端部に比較的大きな押す力を機械的に加えること、具体的にはクリップの取り付けは、可撓性内視鏡器具では不可能であった。
加えて、クリップを取り付ける組織を実質的な圧迫しないことが重要である。ジョーの締付け力で組織を圧迫するが、ジョー組立体の外形寸法が比較的小さいため大きな締付け力を得るのは困難である。すなわち、ジョー組立体のピボットと各ジョーのタングとの間のレバーアームの長さが比較的短いため、ジョー組立体の機械的てこ作用が限定されてしまう。
クリップを配置した後に既知の位置にクリッププッシャーを配置するように適合されたラチェット機構を含む可撓性クリップアプライヤーが、言及することを以ってその開示内容の全てを本明細書の一部とする前記した米国特許出願第１０／３９６，９６２号に開示されている。加えて、このクリップアプライヤーは、一連のクリップが収納される可撓性ハウジングを含む。この可撓性ハウジングは、引張り力がかかっても伸長しない。更に、ジョー組立体は、クリップを取り付ける前の組織の圧迫を容易にするべく比較的大きな機械的てこ作用が得られるように構成されている。
前記した米国特許出願第１０／３９６，９６２号にクリップアプライヤーの開発が開示されているため、本願の目的である改良した可撓性内視鏡クリップアプライヤーを開発した。
従って、本発明の目的は可撓性内視鏡クリップアプライヤーを提供することにある。
本発明の目的はまた、複数のクリップを取り付けることができる可撓性内視鏡クリップアプライヤーを提供することにある。
本発明の別の目的は、ジョーに加えられる力を制限する可撓性内視鏡クリップアプライヤーを提供することにある。
本発明の更に別の目的は、内視鏡の曲がりくねった通路による外側シースの長さの相対変化を調節すると共に装置のジョーに加えられる力を制限する可撓性内視鏡クリップアプライヤーを提供することにある。
本発明の更なる目的は、ジョーのメカニカルアドバンテージを改善するためにそれぞれ別の軸を中心に回動可能な２つのジョーを有する可撓性内視鏡クリップアプライヤーを提供することにある。
本発明の目的はまた、手動アクチュエータのスムーズな運動によりクリップを排出する可撓性内視鏡クリップアプライヤーを提供することにある。
本発明の更なる目的は、クリップを正確に１回ずつ排出するアクチュエータを有する可撓性内視鏡クリップアプライヤーを提供することにある。
本発明の別の目的は、１つの制御ワイヤを用いてジョーの開閉及びその長軸を中心としたジョーの回動を行う可撓性内視鏡クリップアプライヤーを提供することにある。
本発明の更なる目的は、改善されたジョーを有する可撓性内視鏡クリップアプライヤーを提供することにある。
本発明の別の目的は、最下位のクリップが取り付けられた後、そのクリップから可撓性内視鏡クリップアプライヤーが離れる時に、最下位から２番目のクリップが誤って排出されるのを防止する可撓性内視鏡クリップアプライヤーを提供することにある。
本発明の更に別の目的は、クリップを排出する時にクリップを成形する可撓性内視鏡クリップアプライヤーを提供することにある。
本発明の更に別の目的は、クリップの成形及び取付け工程に亘って実質的に一定の抵抗力を有するハンドルを備えた可撓性内視鏡クリップアプライヤーを提供することにある。
本発明の目的はまた、取り付けられるクリップの組織固定点を示すクリップ成形ジョーを備えたクリップアプライヤーを提供することにある。
本発明の更に別の目的は、コイルを介してエンドエフェクタまでクリップを安定して前進させる機構を備えたクリップアプライヤーを提供することにある。
本発明の別の目的は、アプライヤーのジョーが閉じている場合にのみクリップを発射することができる内視鏡クリップアプライヤーを提供することにある。
本発明の更なる目的は、１回に１つ、すなわちジョーの開閉の間に１つのクリップだけを発射することができる内視鏡クリップアプライヤーを提供することにある。
本発明の更なる目的は、内視鏡クリップアプライヤーに残っているクリップの数を外科医に示すことができる内視鏡クリップアプライヤーを提供することにある。
本発明の更に別の目的は、全てのクリップが排出されると動作を停止する内視鏡クリップアプライヤーを提供することにある。
これらの目的に従い、詳細を後述する本発明の可撓性内視鏡クリップアプライヤーは、基端部及び先端部を有する比較的長い可撓性コイル（またはチューブ）を有する。ここで用いる用語「基端部」は外科医に最も近い端部を指し、用語「先端部」は患者に最も近い端部を指す。手動アクチュエータがコイルの基端部に結合され、一対のジョーがコイルの先端部に結合されている。一連のクリップがジョーに近接したコイル内に受容される。ガレージから閉じたジョー内にクリップが押された時にそのクリップを曲げるためのアンビルがジョーの内部に形成されている。手動アクチュエータは、ジョーを開閉するためのレバー、コイルの長軸を中心にジョー（及びコイルの先端部分）を回動させるためのノブ、及びクリップを排出するためのクランクの３つの制御部を有する。レバーとノブが、１つの第１の制御部材に結合されている。この第１の制御部材は、コイル内を通って、収納されているクリップの基端側まで延びている。クランクが、先端部分にねじ付きの第２の制御部材に結合されている。この第２の制御部材は、コイルの内部を通って、収納されているクリップに近接した位置まで延びている。
現在好適な実施形態に従えば、レバーが、ジョーが閉じる時にジョーに過度の力が加わらないようにする力制限装置に結合されている。この力制限装置はまた、ジョー制御部材に対する外側シースの長さの相対変化を効果的に調節することができる。ノブが、スプラインカップリングを介して第１の制御部材に結合されている。クランクが、トランスミッション及びエネルギー蓄積装置（例えばフライホイール）を介して第２の制御部材に結合されている。
第１の制御部材が、収納されたクリップの基端側まで延び、一対のプルワイヤに結合されたジョイナーに結合されている。これらのプルワイヤは、収納されたクリップの両側に延在し、それぞれが対応する１つのジョーに結合されている。
本発明に従えば、収納されたクリップの基端側でコイルが２つに分かれ、そのコイルの２つの部分が４つの孔を有する剛性部材につながれている。４つの孔の内の１つにねじが設けられている。剛性部材は、ジョイナーの先端側に配置され、第１の制御部材が剛性部材の１つの孔を通っている。このような構成から、第１の制御部材の回動により剛性部材が回動し、これによりコイルの先端部分及びジョーがその長軸を中心に回動する。従って、クリップする組織に対してジョーが閉じる前にジョーの向きを適切に合わせることができる。２つのプルワイヤが剛性部材の他の２つの孔を通り、第２の制御部材のねじ部分が剛性部材のねじ孔に螺合する。このような構成から、ねじ付き制御部材がクランクによって回動されると、このねじ制御部材が先端側に平行移動する。ねじ制御部材は、その先端部がクリッププッシャーに結合している。クリッププッシャーは、軸方向に整列された一連のクリップの最も基端側のクリップに近接して配置される。ねじ制御部材が先端側に平行移動すると、一連のクリップが先端側に移動し、最下位のクリップ（収納されたクリップの最も先端側のクリップ）が閉じたジョーに進入してジョーの内側アンビルによってそのクリップの両端が曲げられて組織に取り付けられる。
第２の制御部材のねじ部分のねじピッチ及びトランスミッションが、クランクの厳密な１回転で正確に１個のクリップが排出されるように構成されている。クランクは、そのクランクに解除可能に係合し、クランクが１回転した後に自動的にクランクを停止させる戻り止めロックを備えるのが好ましい。
更に、本発明に従えば、ジョーは、同一の無性ジョーであって、それぞれのジョーが、先端コイルの先端部でクレビスの互いにずれた軸に回動可能に結合されている。それぞれのジョーは、先端歯及び基端タングを有する。このタングは、１つのプルワイヤに結合され、その長軸の一側に延在する。ジョーの先端歯は、ジョーの長軸の反対側に位置し、ジョーが閉じた時にジョーの左右のずれを防止する。
一連のクリップが、クレビスに結合されその基端側に延びたガレージに収納される。このガレージは、実質的に硬質の矩形構造であって、クリップを適切に整列した状態に維持し、ガレージから閉じたジョーの中にクリップをスムーズに押し出すことができる。ガレージの先端部分には一対の付勢されたストッパーが設けられており、これにより、最下位のクリップが取り付けられてそのクリップからアプライヤーが離れる時に最下位から２番目のクリップがガレージから排出されるのが防止される。代替の実施形態では、このようなストッパーは、ガレージの一部ではなくクレビスの一部として形成される。
現在好適な実施形態に従えば、クランクは、左右両側から使えるように手動アクチュエータの両側に延出している。カウンタ機構がクランクに結合され、ガレージに残っているクリップの数を表示する。ジョーが閉じた時にのみクランクが動作できるように、戻り止めロックをレバーで係合させることができる。クランクはまた、一方向にのみ回動するようにラチェット機構を備えている。カウンタはまた、全てのクリップが排出された後にクランクの回動を防止するストッパーも備えている。
本発明の更なる目的及び利点は、添付の図面を参照しながら以下の詳細な説明を読めば当業者には明らかであろう。
上記した一部の問題を解消した、非同一線上の軸に取り付けられた無性ジョーを備えた内視鏡クリップアプライヤーが提供される。
図１を参照すると、本発明に従った可撓性内視鏡クリップアプライヤー１０が、基端部１４及び先端部１６を有する比較的長い可撓性コイル（またはチューブ）を有する。本明細書に用いる用語「基端側」は医師に最も近いことを意味し、「先端側」は患者に最も近いことを意味する。手動アクチュエータ１８が、コイル１２の基端部１４に結合され、一対のジョー２０がコイル１２の先端部１６に結合されている。コイルは、摩擦低減外側シース（不図示）を有する平坦なワイヤコイルであるのが好ましい。本発明の主な構成要素を基端部から先端部に向かって順に参照して本発明を詳細に説明する。
手動アクチュエータ
図１‐図６に、手動アクチュエータの第１の実施形態が示されている。図３０‐図３７に、手動アクチュエータの現在好適な実施形態が示されており、詳細は後述する。
図１‐図６の手動アクチュエータ１８は、３つの制御部、すなわちジョー２０を開閉するためのレバー２２と、コイルの長軸を中心にジョー２０（及びコイル１２の先端部分）を回動させるためのノブ２４と、クリップを排出するためのクランク２６とを有する。手動アクチュエータ１８は、他の内視鏡アクチュエータに類似した概ねピストン形状を有することを理解されたい。フィンガーグリップ２８が、サムグリップ３０を備えたレバー２２の反対側に設けられている。レバーの係合フック３２とフィンガーグリップの係合フック３４により、図４に示されているようにレバーを固定することができる。当業者であれば、クリップアプライヤー１０の一般的な動作に、ジョーを閉じること（場合によってはレバー２２を固定する）、内視鏡の内腔を介してジョー２０を外科部位まで送ること、図１に示されているようにジョーを開くこと、クリップする組織がジョーの間にくるようにコイル１２の移動とノブ２４の回動によりジョー２０を配置すること、レバー２２を固定して組織に対してジョーを閉じること、クランク２６を回してクリップを取り付けること、クリップ及び組織からジョーを放すことが含まれることを理解できよう。
図１‐図６、特に図３を用いて、手動アクチュエータ１８を詳細に説明する。レバー２２及びノブ２４が、１つの第１の制御部材３６に結合されている。レバー２２は、軸２３を中心に回動可能であり、力制限機構３７によって制御部材３６に結合されている。力制限機構は、ばね３８、基端カップラー３８ａ、力制限キャップ３８ｂ、力制限カップ３８ｃ、オーバーロードナット３８ｄ、先端カップラー３８ｅ、及びオーバーロードシャフト３８ｆを含む。オーバーロードシャフト３８ｆは、オーバーロードナット３８ｄに結合され、ばね３８の内部に延在し、力制限カップ３８ｃの途中まで延びている。
レバー２２は、図２に最も良く示されているリンケージ４４によってばねの先端部に結合されている。具体的には、リンケージ４４は、ばね３８及びその関連要素３８ａ‐３８ｆを受容するＵ型部材４４ａを含む。ピン４４ｂにより、Ｕ型部材４４ａの基端部がレバー２２のスロット２２ａに結合され、第２のピン４４ｃにより、Ｕ型部材４４ａの先端部がオーバーロードナット３８ｄに結合されている。
この構成では、レバー２２がフィンガーグリップ２８に向かって移動すると、リンケージ４４が基端側に移動してオーバーロードナット３８ｄが基端側に移動する。次いで、オーバーロードナット３８ｄが、ばね３８を基端側に押す。これにより、ばね３８がカップ３８ｃを押し、このカップ３８ｃが制限カップ３８ｂを押し、この制限カップ３８ｂが基端カップリング３８ａを押す。基端カップリング３８ａが制御部材３６に固定されているため、基端カップリング３８ａの基端側への移動により制御部材３６が基端側に移動する。従って、事実上、力制限組立体３７全体が基端側に移動し、制御部材３６が基端側に引張られる。
ジョーを閉じる際の任意の時点で、制御部材３６にかかる張力がばね３８の所定の力の限度（例えば、１７ポンド（約７．７３ｋｇ））を超えると、ばね３８の力が打ち負かされてばね３８が収縮し、図３Ａに示されているように制御部材３６が移動しないでオーバーロードシャフト３８ｆ及びオーバーロードナット３８ｄが先端カップラー３８ｅから離れる。このように過度に力がかかった状態では、シャフト３８ｆがオーバーロードカップ３８ｃの中空部３８ｃ’内に受容される。
図３Ａに、ジョーが閉じる際の圧縮された位置にある力制限ばね３８が示されている。このような状態は、制御部材３６がジョーを閉じるのに通常必要な距離を越えて引張られた時に起こる（例えば、ジョーが非常に厚い組織または非常に硬い組織を挟んでいるため、ジョーが閉じることが全くできない場合、またはコイルの曲がりくねった通路により、コイルが実質的に延びて制御部材３６が実質的に短くなり、これによりレバー２２のストロークが長くなってジョーを閉じるために必要なストロークが短くなった場合である）。ばね３８により、ジョー、組織、及び／または制御部材３６の過荷重による損傷が防止され、レバー２２がラッチ位置まで十分に作動することができる。
ジョーが閉じられている図４に示されている位置までレバー２２が移動した後に過剰な力が制御部材３６にかかっても（例えば、曲がりくねった内視鏡通路を介して外科部位までジョーが送られる時）、力制限機構３７が動作して制御部材３６の損傷が防止される。具体的には、図４Ａに示されているように、ジョーが閉じた後に制御ワイヤが先端側に引張られ、制御ワイヤ３６に対する力がばね３８の所定の制限を超えると、制御部材３６に固着された基端カップリング３８ａがキャップ３８ｂ及び力制限カップ３８ｃに対して先端側に移動する。これにより力制限カップが先端側に移動し、ばね３８が、固定されたレバー２２に対するリンケージにより所定の位置に固定されたナット３８ｄに対して圧縮される。
何れの場合も、レバー２２が解放されると、リンケージ４４によってばね３８が伸長し、これによりナット３８ｄが、先端カップリング３８ｅ（制御部材３６に結合されている）に当接するまで先端側に移動する。ナット３８ｄの先端側への移動により、制御部材３６に結合されている先端カップリング３８ｅが先端側に移動し、これにより制御部材３６が先端側に移動する。
現在好適な実施形態に従えば、レバー２２が固定位置に移動すると、ばねは常に圧縮されている。
制御部材３６を回動できるように、ノブ２４が、クランプ２４ｂによって保持された軸受２４ａに取り付けられたスプライン４０を介して制御部材３６に結合されている。制御部材３６の基端部は、シェパードクルック（shepherd's crook）３７ａに曲げられており、ノブ２４に結合されたスプライン４０にスライド可能に係合する。シェパードクルックは図２９に例示されている。ノブ２４の回動により、制御部材３６が回動する。
力制限機構３７では、先端カップリング３８ｅ及び基端カップリング３８ａが力制限組立体３７の残りの要素との間に遊びができるように組み立てられることに留意されたい。この遊びにより回動が容易になるため、予荷重されたばね３８による摩擦がねじれ抵抗に変換されない。
図４Ｂに、力制限機構１３７の第１の代替の実施形態が示されている。ここでは、コイル１１２に基端ブッシュ１１３が設けられている。手動アクチュエータまたはハンドル１１８が、ブッシュ１１３、ワッシャー／スペーサ１１５、及びそれらの間の圧縮ばね１３８を受容できる大きさの凹部１１９を備えている。当業者であれば、ジョーを閉じる際に障害物によってジョーが完全に閉じるのが妨げられる場合、制御部材１３６に加えられる基端側への力が、先端側クレビス１８２、コイル１１２、及びハンドル１１８にかかる。これにより、ハンドルが、ワッシャー／スペーサ１１５に対して先端方向に等しい反対の力を加える。従って、圧縮ばね１３８の力が過剰になると、コイルの基端部によりブッシュ１３８がばね１３８に向かって移動する。
図１‐図６、特に図４を参照すると、クランク２６が、第２の制御部材４８に結合されたトランスミッション４６に結合されている。具体的には、トランスミッションは、クランクに結合された入力平歯車４７、その入力平歯車に結合されたステップアップ平歯車４９、及びそのステップアップ平歯車に結合されたクラウン歯車５０を含む。このクラウン歯車が、内部にキーが形成されたシリンダ５４に結合されたピニオン５２に係合する。このシリンダの内部にキーが設けられており、このキーに、基端部またはその近傍にシェパードクルック４８ａ（図２９にも例示されている）が形成された第２の制御部材４８が係合する。シリンダ５４は、クランプ５５ａ及び５７ａによって保持された２つの軸受５５及び５７に取り付けられている。第２の制御部材４８（図８‐図１１を用いて詳細に後述する）の先端部分にねじが設けられている。後述する説明から、シリンダ５４が、全てのクリップが排出されるまで第２の制御部材４８が先端側へ移動できる十分な長さを有することを理解できよう。現在好適な実施形態に従えば、制御部材４８は１７‐７ＰＨステンレス鋼ワイヤから形成される。
現在好適な実施形態に従えば、エネルギーを蓄積するフライホイール５６がシリンダ５４に結合されている。別法では、フライホイールとシリンダを１つの成形部品とすることができる。フライホイールが、クランクの動作をスムーズにするため、クランクの回動の開始時に制御部材によりクリップが前進させられ、クランクの回動の最後で制御部材によりエンドエフェクタのアンビルにクリップが押されてクリップが成形され（詳細は後述）、クランクの回動により増大した力を加える必要がない。当業者であれば、効率的にするには、エネルギーの蓄積のために比較的大きな回転質量を有するフライホイールが好ましいことを理解できよう。クランクの回動によってフライホイールが回転すると、所定量のエネルギーが投下され、これによりフライホイールの運動エネルギー（質量×速度）が増大する。時間経過により、このエネルギーの一部が摩擦で失われるが、フライホイールの回転に用いられるエネルギーの一部が運動エネルギーの形で蓄えられる。後に、このエネルギーを直接的な機械伝達によって回収することが可能である。本発明の場合、まずクランク２６が始めに回動する時は、制御部材４８に殆ど抵抗がないため（クリップが容易に前進）、クランクに加えられたエネルギーの殆どがフライホイール５６の回転に利用される。クランクの回動が終了に近づくと、クリップが最終形状に曲げられるサイクルの終了に近づくため、制御部材４８によってねじれ抵抗が蓄積される。この時点で、フライホイールの運動エネルギーが利用され、クランクサイクルの残りが容易になる。本発明に従えば、フライホイール５６は、クランク２６に加えられる力が１つのクリップを排出するためのクランク２６の全運動に亘って実質的に均等（例えば、２５％を超えて変動しない）になるように選択されるのが好ましい。
図５及び図６に最も良く示されているように、クランク２６が戻り止めロック５８をクランク２６が備えている。この戻り止めロックは、回動を開始前に解除しなければならず、クランク２６が１回転するとそのクランクを自動的に固定する。クランク２６は、逆回転を防止するラチェット機構（不図示）を備えるのが好ましい。クランクはまた、ジョーが閉じるまでその回動を防止するロック（不図示）を備えるのが好ましい。クランクはまた、入力平歯車に結合して、クランクが回転した回転数をカウントして排出されたクリップの数を示すことができる回転カウンタ（不図示）を備えることもできる。また回転カウンタを用いて、全てのクリップが排出されたらクランクが回転できないようにすることもできる。理想的には、クランクの回転と回転の間にジョーが開閉しないでクランクが２回転するのを防止するロックアウト機構をクランクに設ける。
図５及び図６に例示されている実施形態に従えば、クランク２６は、周方向に離間した複数の外周フィンガーグリップ２６ａ及び刻み付き外周面２６ｂを有する。クランクハンドル２６ｃを、所望に応じてクランクをノブのように回動できるように場合によっては取り外し可能にする。戻り止めロック５８は、フランジ５８ｂを有するプッシュボタン５８ａ、フランジ５８ｄを有するロックピン５８ｃ、及びばね５８ｅを含む。ロックピン５８ｃは、段付き孔５８ｆに配置され、クランク２６の段付き孔２６ｄ内にばね５８ｅによって付勢されている。ボタン５８ａを押すと、固定ピン５８ｃがばね５８ｅに向かって移動して孔２６ｄから出るため、クランクが自由に回動することができる。
例示的な実施形態に従えば、クランクが１回転すると、トランスミッションにより第２の制御部材が５８．１８７５回、回転する。制御部材のねじのピッチにより、クランクが１回転すると制御部材が０．２８５インチ（７．２３９ｍｍ）前進する。歯車及びねじピッチは、特定のクリップの長さに対して選択される。現在好適な実施形態に従えば、異なった長さのクリップに対応するためにはクラウン歯車を変更（歯の数を増減）するだけで良い。
図１‐図４Ａに例示されているように、手動アクチュエータ１８の先端部は、コイルの基端部に取り付けられたコイルコネクタ（不図示）を保持する一対の垂直スロット１５を有する。
制御部材
ここで図７‐図１０を参照されたい。制御部材３６及び４８が、手動アクチュエータ１８の先端部に結合された可撓性コイル１２の内部に延在する。本発明に従えば、コイル１２は、剛性部材６０によって互いに結合された基端部分１２ａ及び先端部分１２ｂの２つの部分を有する。剛性部材６０は、実質的に円筒状であって、端部６０‐２及び６０‐３よりも直径の大きな中心部分６０‐１を有する。端部６０‐２及び６０‐３は、コイル１２ａ及び１２ｂ内に適合する寸法であり、中心部分６０‐１は、コイル１２ａ及び１２ｂの外径と実質的に同じ外径寸法を有する。剛性部材６０は４つの孔６０ａ‐６０ｄを有する。その１つの孔６０ｄは、ねじを備えており、第２の制御部材４８のねじ部分に螺合する。剛性部材６０がコイル１２ａ及び１２ｂに対して固定されているため、このねじの螺合により、第２の制御部材４８がクランク２６（図２）によって回転すると、第２の制御部材４８が剛性成部材６０の内部を先端側に移動することを理解されたい。
剛性部材６０の他の２つの孔６０ａ及び６０ｂにより、図１６及び図１７を用いて詳細を説明する一対のプルワイヤ６２及び６４が通過できる。プルワイヤの基端部は、４つの孔６６ａ‐６６ｄを有するジョイナー６６に結合されている。一方のプルワイヤが孔６６ａに結合され、他方のプルワイヤが孔６６ｂに結合されている。第１の制御部材３６が孔６６ｃ内を通ってその孔６６ｃに結合され、ねじ付き制御部材４８が孔６６ｄを自由に通過する。この方式では、第１の制御部材３６が長手方向に移動すると、プルワイヤ６２及び６４が長手方向に移動する。ジョイナー６６から延出した第１の制御部材３６の一部３６ｂが剛性部材６０の孔６０ｃ内に延びている。この部分３６ｂの長さは、制御部材３６の移動範囲に亘って孔６０ｃに係合するのに十分である。この方式では、ノブ２４（図１）によって制御部材３６が回動すると剛性部材６０が回動し、これによりコイル１２の先端部分１２ｂが回動し、コイルの長軸を中心にジョー２０及び格納されているクリップが回動する。
図８に最も良く示されているように、制御部材３６及び４８は、コイル１２の基端部分１２ａの内側の２内腔可撓性シース６８によって保護されている。シース６８により、制御部材とコイルの内部との摩擦が低減される。シース６８はまた、制御部材の座屈またはねじれを防止する。また、摩擦低減シースを、コイルの全外面に沿って設けて、コイルとそのコイルを送る内視鏡の内腔との摩擦を低減し、内視鏡の内腔が損傷しないようにするのが好ましいことを理解されたい。
現在好適な実施形態に従えば、両制御部材は、可撓性を付与するために先端部で直径が小さく、トルクの伝達を最適化するために基端部で直径が大きい。
プッシャー
ここで図１１‐図１３を参照されたい。ねじ付きの第２の制御部材４８の先端部がクリッププッシャー７０に結合されている。図１２に最も良く示されているように、プッシャー７０は、概ね直線状の部材であって、ノッチ７０ｂが横断する軸外孔７０ａを有する。内側に湾曲し先端側に延びた一対のフィンガー７０ｃ及び７０ｄが、肩７０ｆ及び７０ｇによって先端開口７０ｅから離れている。図１１に最も良く示されているように、ねじ付き制御部材４８の先端部が孔７０ａ内に延び、ノッチ７０ｂ内に保持されたシリンダ７２に結合されている。シリンダ７２は、制御部材４８に対して加締めるまたは溶接することができる。プッシャーと制御部材の結合は、制御部材がプッシャーに対して自由に回動できる結合である。図１１に最も良く示されているように、制御部材４８の先端部４８ｂが尖っており、孔７０ａの先端部に円錐壁７０ｈが画定されている。円錐壁７０ｈは、その頂角が先端４８ｂの頂角よりも大きい。上記した説明から、当業者には、制御部材４８が回転すると、制御部材４８が先端側に前進し、プッシャーが先端側に押し出されることを理解できよう。制御部材４８とプッシャー７０の摩擦係合が、円錐壁７０ｈの頂点と先端４８ｂの小さな係合面積に限定されていることを理解できよう。
図１３に最も良く示されているように、クリッププッシャー７０は、ジョー組立体２０の基端側に軸方向に整列して収納された一連のクリップ７６の最も基端側のクリップ７４に近接して配置されている。制御部材４８が先端側に移動すると、最下位のクリップ７８（一連のクリップの先端のクリップ）が閉じたジョーの中に入り、ジョーの内側アンビルによってクリップの先端部が曲がってクリップが組織に取り付けられるまで、収納された一連のクリップ７６が先端側に移動する。図１３に、クリップ７８が取り付けられた後の開いたジョーが示されている。
図１１に最も良く示されているように、クリップ７４（他の全てのクリップと同一）は、プッシャー７０の開口７０ｅ及び肩７０ｆ及び７０ｇに係合する基端尾部７４ａを有する。フィンガー７０ｃ及び７０ｄが、クリップが垂直方向に移動するのを制限し、プッシャーがクリップを軽く保持することができ、これにより組み立ての際のクリップの装着が容易になる。クリップの更なる詳細については、前記した米国特許出願（整理番号：ＩＳＤ‐０８３）を参照されたい。
当業者であれば、ねじの配置及び構造を変更して同一または同様の結果を得られることを理解できよう。例えば、ねじ付き制御部材を先端側に前進させるように構成するのではなく、ねじ制御部材が平行移動しないようにプッシャーにねじを形成することができる。このような構成では、ねじ付き制御部材の回転により、プッシャーが制御部材に沿って平行移動する。
ガレージ
図１３及び図１６に最も良く示されているように、一連のクリップ７６が、ジョー組立体２０の基端側のコイル１２の先端部分１２ｂの内側のガレージ８０内に受容されている。ガレージ８０の詳細は図１４‐図１７に示されている。ガレージ８０は通常、複数の平行な側壁８０ａ‐８０ｌ、及び側壁に対して垂直な先端側に延びた一対のフィンガー８０ｍ及び８０ｎを含む。各側壁は、複数の外側を向いたスペーサ（例えば、８０ｃ‐１、８０ｃ−２、８０ｃ‐３、８０ｃ‐４）を有する。これらのスペーサは、コイルの内面に係合し、プルワイヤ６２及び６４（図１６を参照）が通過できるようにコイルとガレージとの間の空間を確保している。外側に向いたスペーサを形成することにより、細いストリップ（例えば、８０ａｂ、８０ｂｃ、８０ｃｄなど）になり、ガレージに可撓性が付与される。器具の先端部における可撓性は、内視鏡が反転した場合に重要であろう。図１６に最も良く示されているように、クリップがガレージ内に配置されると、この細い領域で１つのクリップが別のクリップに対して当接する。従って、クリップがその当接部で曲がることができる。
図１５に最も良く示されているように、先端側に延びたフィンガー８０ｍ及び８０ｎはそれぞれ、外向きに延びた先端リップ８０ｍ‐１及び８０ｎ‐１と、内向きに延びた基端ストッパー８０ｍ‐２及び８０ｎ‐２を有する。フィンガー８０ｍ及び８０ｎが、クレビスに対してガレージの向きを合わせるのに役立つ。先端リップは、後述するようにガレージのクレビスに対する係合を助け、基端ストッパーは、後述するように最下位から２番目のクリップの不所望の動きを防止する。対向した平行なフィンガー８０ｐと８０ｑの対が、フィンガー８０ｍ及び８０ｎの平面に垂直な平面に離間して位置している。図１７に最も示されているように、これらのフィンガー８０ｐ及び８０ｑは、詳細を後述するように基端カラー８０ｒから延出してクレビスに係合する。
現在好適な実施形態に従えば、ガレージは、スタンプされ折り返された単体のステンレス鋼から形成される。
我々の初期のクリップアプライヤーとは異なり、クリップを引き戻すことができるようにクリップを互いに連結する必要がない。また、どのような時も１つのクリップも引き戻す必要がない。
ジョー組立体
図１７‐図２７に、クレビス８２及び一対のジョー８４及び８６を含むジョー組立体２０の詳細が例示されている。各ジョーは、無性噛み合いジョーすなわち各ジョーが同一であって、互いに噛み合うように構成されている。各ジョーは、基端タング８４ａ及び８６ａ、ジョーの長軸の両側の互いに半ピッチずれた側歯８４ｂ及び８６ｂ、先端歯８４ｃ及び８６ｃ、及び取付け孔８４ｄ及び８６ｄを含む。ジョーは、それぞれの取付け孔を介してクレビスに結合されている。図２７に最も示されているように、クレビス８２は、２つの軸がずれたボス８２ａ及び８２ｂを有する。これらのボスに、ジョーがリベットによって取り付けられ、所定の位置に保持されている。プルワイヤ６２及び６４の先端部がドッグレッグに曲げられ、ジョーのそれぞれのタング８４ａ及び８６ａに結合されている。従って、プルワイヤの先端側への移動により、図１８及び図１９に示されているようにジョーが開き、プルワイヤの基端側への移動により、図２０及び図２１に示されているようにジョーが閉じることを理解されたい。ずれたボスを用いることにより、ジョーのメカニカルアドバンテージが増大する。クレビスはまた、一対のストッパー８２ｅ及び８２ｆ（図２６に最も良く示されている）を備えている。これらのストッパーは、ジョーのタングの耳８４ａ‐１及び８６ａ‐１に係合して、閉じる時にジョーが軸から４５度〜６０度回動することを可能にし、またジョーの約１０度の過剰な回動を可能にする。これにより、閉じたジョーが内視鏡の内腔内を介して曲がった通路を移動することができる。
図１９に最も良く示されているように、それぞれのジョーの先端歯と基端タングは、ジョー組立体の長軸の反対側に位置する。この構成により、エンドエフェクタの構成に安定性が付与される。具体的には、ジョーとクレビスとの間に一定の遊びがあり、ジョーが回動して容易に開閉することができる。この遊びにより、ジョーがクレビスに対して水平方向に振動し、ジョーをずらすことが可能になる。例示されている実施形態では、ジョーは、閉じられると互いに離れるのではなく、水平方向に互いに押圧される。これは、プルワイヤによって生じる水平方向のモーメントに対して成形アンビルが反作用するためである。
先端歯の内面が、成形アンビルであって、図２０及び図２３に示されているようにクリップの２つの尖叉を約９０度〜１８０度曲げることができる。具体的には、図１９及び図２１に示されているように、先端歯が互いに平行な２つの湾曲通路を画定している。これにより、クリップの２つの尖叉が平行な半円に曲げられる。先端歯はまた、歯が接触する点がクリップの尖叉が組織に刺入する位置に近接しているため組織固定点を表示する役割も果たす。
図２３に最も良く示されているように、最先端（最下位）のクリップ７８が取り付けられたら、隣すなわち最下位から２番目のクリップ７８ａが、ガレージ８０のストッパー８０ｍ‐２及び８０ｎ‐２によって保持され、クリップ７８ａの尖叉がクリップ７８の尾部を保持する。ジョーが開くと、ジョー組立体が最下位から２番目のクリップ７８ａを解放しないで最下位のクリップ７８から離れることができる。図１８に最も良く示されているように、ジョーが開いた状態で、最下位から２番目のクリップ７８ａの尖叉がクレビス８２の先端フィン８２ｃ及び８２ｄによって保護されている。しかしながら、クリップが取り付けられるまでクレビスからクリップが延出しないようにクレビスの寸法を変えれば、これらのフィン８２ｃ及び８２ｄが必要なくなる。クレビスの他の形態（例えば図２６）にはフィンが示されていない。
図１７に示されているように、ガレージは、クレビスに対して固定されるように３つの位置でクレビスに係合している。先端側の係合は、先端リップ８０ｍ‐１及び８０ｎ‐１とクレビス８２のリップ８２ｅ及び８２ｆの係合によって達成されている（図２２に最も良く示されている）。基端側の係合は、フィンガー８０ｐ及び８０ｑと側面凹部すなわち孔８２ｇ及び８２ｈの係合によって達成されている。第３の係合は、コイルの先端部とクレビスコアとの間に基端カラー８０ｒを挟んで達成される。
代替の実施形態：セルフプッシュクリップ
図２８‐図２８ｂを参照すると、クリップ３０１及びクリップ前進機構の代替の実施形態が示されている。クリップ３０１は、基端部にねじ孔３０１ａが設けられているという点を除いて、上記したクリップと実質的に同じ構造を有する。図２８に、回動可能な制御部材１４８のねじ端部１４８ａに螺合した複数のクリップ３０１‐３０４が示されている。制御部材１４８は、コイルまたはアクチュエータに対して平行移動しないように取り付けられるという点を除いて、上記した制御部材４８に類似している。具体的には、制御部材１４８は、基端コイルと先端コイル（不図示）との間に位置するスラスト軸受３１２内に配置されたスラストカラー３１０に取り付けられている。制御部材１４８が回転すると、制御部材とのねじ係合によりガレージ（不図示）が平行移動して、クリップ３０１‐３０４は回転しない。
現在好適な手動アクチュエータ
図３０‐図３７に、現在好適な手動アクチュエータ４１８が例示されている。４００が加算された類似の参照番号は、図１‐図６の手動アクチュエータ１８の類似した構成部品を指す。手動アクチュエータ４１８は、レバー４２２、ノブ４２４、及びクランク４２６を含む。この実施形態の第１の態様に従えば、第２のクランク４２６’が、アクチュエータの反対側に設けられている。図３０及び図３２に最も良く示されているように、クランク４２６及び４２６’は、ギザギザが付けられていて、クランク２６のようにクランクハンドルを備えていない。図３１‐図３３に示されているように、アクチュエータ４１８はアクチュエータ１８と同様に、フィンガーグリップ４２８、及びサムグリップ４３０を有するレバー４２２を備えている。係合フック４３２及び４３４により、図３２に示されているようにレバーを閉じた位置に解除可能に固定することができる。
図３２に最も良く示されているように、多角形クランク軸５００がアクチュエータ４１８を横切るように延在し、その両端がクランク４２６及び４２６’に係合している。クランク軸５００は、アクチュエータ４１８の両側の円柱構造５０２及び５０４によって囲まれている。この実施形態の第２の態様に従えば、一方の円柱構造５０４が複数のラチェット歯５０６を備え、クランク４２６’が、図３３に示されているように、ラチェット爪５１０が取り付けられた周辺ポスト５０８を有する。当業者であれば、ラチェット及び爪により、両方のクランク４２６及び４２６’が逆回転すなわち例示されている実施形態では時計回りに回転するのが防止されていることを理解できよう。
この実施形態の第３の態様に従えば、図３４及び図３５に最も良く示されているように、トランスミッション４４６がカウンタ歯車５１２に結合されている。この実施形態では、トランスミッション歯車が、図１‐図６の実施形態とは僅かに異なって配置されている。具体的には、クランク軸５００が図３５に示されているように、カウンタ歯車５１２に噛合する小さなハブ歯車５１４に結合している。クランク軸はまた、図４に示されている歯車４９に比べて反転したステップアップ平歯車４４９を駆動する入力平歯車４４７にも結合している。平歯車４４９は、ピニオン４５２に結合されたクラウン歯車４５０を駆動する。ピニオン４５２は、フライホイール４５６に結合されたシリンダ４５４に結合されている。
カウンタ歯車５１２は、好ましくは両側に設けられた表示５１６及び直立リブ５１８を含む。アクチュエータ４１８の本体は、少なくとも１つ、好ましくは２つの窓５２０及び５２２（図３１‐図３３、図３６、及び図３７を参照）を含む。これらの窓を介して、カウンタ歯車の表示５１６を読むことができる（１つの表示だけを読める）。例示されたカウンタは、一連の５個のクリップに対して使用される。ガレージが一杯の場合、直立リブ５１８と数字４との間のカウンタ部分を窓を通して見ることができる。直立リブの空間が確保され、そしてハブ歯車の１回転によりカウンタ歯車が５分の１回転弱回転するように、カウンタ歯車の歯の数が選択される。クリップが排出されると、カウンタ歯車が時計回りに回動して残りのクリップの数が小さくなる。クリップの残りが０の場合、０の表示を窓を介して見ることができる。加えて、この位置にある場合、直立リブ５１８が、窓の壁部などのアクチュエータの内側の構造に当接する。これにより、カウンタ歯車及びクランクの更なる前進が防止される。カウンタ歯車５１２は、組み立て中に歯車の向きを正確に合わせるために用いられるインデックス孔５２４を含むのが好ましい。
当業者であれば、直立リブ５１８以外の手段を用いて回動を停止できることを理解できよう。例えば、最後のクリップが排出されると、制御部材４８のねじがなくなるように構成することができる。別法では、ランプを、最後のクリップが排出された時に剛性部材６０が係合して止まる位置で制御部材４８に設けることができる。更なる別法では、最後のクリップが発射された後に、プッシャー７０が戻り止めフィンガー８０ｍ及び８０ｎに係合して固定されるようにプッシャー７０を配置することができる。停止させる目的は、プッシャーがジョーに進入するのを防止すること、及び全てのクリップが使用されたことを表示することである。
図３６及び図３７に例示されているこの実施形態の第４の態様に従えば、ジョーが閉じている場合にのみクランクが回動できるようにクランク戻り止めロック４５８がレバー４２２に係合している。具体的には、ステップ５２８まで上昇するランプ５２６を前記した円柱構造５０２のフロアーに備えている。クランク４２６及び／またはクランク軸５００が、クランク軸の軸から径方向外向きに上向きリップ５３２まで延びた板ばね５３０に結合されている。図３６及び図３７に例示されているように、クランク／クランク軸がロック位置にあり、板ばね５３０がステップ５２８に近接している。クリップを排出するべくクランクを作動させようとしても、板ばね５３０がステップ５２８に衝当してクランクの回動が止まる。
レバー４２２は、先端部にリフティングランプ５３６を備えたタング５３４を有する。開口５３８が、ステップ５２８に近接した円柱構造５０２に通じている。タング５３４は、レバー４２２が閉じた位置に移動してジョーを閉じる際に開口５３８内に進入するように構成されている。タング５３４が開口５３８に進入すると、リフティングランプ５３６が上向きリップ５３２に係合して板ばね５３０をステップ５２８の上に持ち上げる。この位置では、板ばねとステップによりクランクの回転が妨げられないため、クリップを排出することができる。クランクの回動により、板ばねが円柱構造のフロアーのランプ５２６を下って１回転することができる。タング５３４は下側に凹部５４０を備えている。この凹部５４０により、板ばね５３０がクランクの１回転の最後で凹部５４０の下側を通過することができ、この時点で板ばねが再びステップ５２８に当接する。タング５３４は、レバー４２２が戻されてジョーが開いた時に凹部５４０が上向きリップ５３２の上を通過できる十分な弾力性を有する。従って、ジョーが閉じた後、クランクが正確に１回転することができ、ジョーが再び開いて閉じるまで回転することができない。
力制限ばねの代替の実施形態
図３８‐図４０に、力制限ばね以外は上記したアクチュエータと実質的に同一の手動アクチュエータ６００の力制限ばねの第１の代替の実施形態が例示されている。アクチュエータ６００は、リンケージ６０４に結合されたサムレバー６０２を有する。リンケージ６０４の先端部が、ばね６０６の先端部に係合している。ワッシャー６０８が、ばね６０６の基端部近傍に配置され、シャトル６１０がワッシャー６０８の基端側に配置されてリンケージ６０４のストッパー壁６１１に当接している。制御部材６１２が、リンケージ６０４、ばね６０６、及びワッシャー６０８を通過してシャトル６１０に結合されている。シャトル６１０は、図４０に例示されている２つの同一部品から構成されている。それぞれの部品６１０は、概ね円柱状であって、一対の固定ナブ６１０ａ及び６１０ｂと、一対のナブ受容ソケット６１０ｃ及び６１０ｄを有する。部品６１０の内部には、中心をずれたワイヤ係合タング６１０ｅが形成され、両端には軸方向の半孔６１０ｆ及び６１０ｇが形成されている。上記説明から、当業者であれば、制御ワイヤをシャトルの２つの部品間に配置し、これらの部品を互いに押圧して制御ワイヤを２つのタング６１０ｅによってＳ字状に曲げ、シャトルを制御ワイヤに対して固定できることを理解できよう。
上記説明から、レバー６０２が開位置（図３９）から図３８に示されている閉位置に向かって移動すると、リンケージ６０４によりばね６０６及びワッシャー６０８がシャトル６１０に対して基端側に移動し、これにより制御ワイヤ６１２が、制御ワイヤをこれ以上引張ることができなくなるすなわち所定の張力がワイヤにかかるまで基端側に引張られる。更にレバー６０２の閉止を続けて、リンケージ６０４及びばね６０６を更に基端側に移動させる。しかしながら、ワイヤ６１２、ワッシャー６０８、及びシャトル６１０が移動しないため（すなわち、ばね定数がワイヤにかかる張力よりも小さいため）、ばね６０６がリンケージ６０４とワッシャー６０８との間で圧縮し始め、レバーが固定されると圧縮した状態に維持される。この圧縮の程度は、制御ワイヤの通路の曲がり具合による。現在好適な実施形態に従えば、図３８に示されているようにレバーが固定されると必ず、ばねがある程度圧縮される。レバー６０２が解除されると、まずリンケージ６０４がワッシャー及びシャトルに対して先端側に移動してばねが伸長する。最終的に、リンケージ６０４のストッパー壁６１１がシャトル６１０に到達してそのシャトルを先端側に押し、これにより制御ワイヤ６１２が先端側に移動する。
当業者であれば、ワッシャー６０８の役割が、ばね６０６とシャトル６１０との正の係合を提供することであることを理解できよう。正の係合を提供する１つの方法は、ばね６０６の端部を「ｅ」形に曲げてばね６０６の端部を部分的に閉じることである。別法では、ばねのワイヤの端部を平坦かつ幅広にしてシャトルに係合させることができる。また、当業者であれば、シャトル６１０のためにリンケージ６０４にストッパー壁６１１を設ける代わりに、レバーが閉じた位置から解放された時にレバー６０２がシャトル（従ってワイヤ６１２）を直接先端側に押すようにレバー６０２を構成することができることを理解できよう。
図４１‐図４３に、力制限ばね以外は上記したアクチュエータと実質的に同一の手動アクチュエータ７００の力制限ばねの第２の代替の実施形態が例示されている。アクチュエータ７００は、捩りばね７０６によって互いに結合された２つの部品７０２ａ及び７０２ｂから形成されたサムレバー７０２を有する。図４２に最も良く示されているように、レバー７０２の上側部品７０２ｂは、制御ワイヤ（不図示）に結合したボール７１０を受容するためのソケット７０２ｃ及び２つの離間した脚７０２ｄ及び７０２ｅを有する。レバーの下側部分７０２ａは、脚７０２ｄと脚７０２ｅとの間に延在する上部フィンガー７０２ｆを有する。捩りばね７０６は、上部フィンガー７０２ｆ及び脚７０２ｄ及び７０２ｅを貫通した回動ポスト７０３に取り付けられている。ばね後部７０６ａがフィンガー７０２ｆの後部に係合し、ばね前部７０６ｂがレバーの上部７０２ｂに係合している。ボールは、図４３に例示されている２つの同一部品から形成されている。この部品は、概ね半球状であるが、一対の固定ナブ７１０ａ及び７１０ｂ並びに一対のナブ受容ソケット７１０ｃ及び７１０ｄを有するという点で上記したシャトルに類似している。部品７１０の内部には、中心をずれたワイヤ係合タング７１０ｅが形成され、径方向正反対に位置する一対の半孔７１０ｆ及び７１０ｇがタング７１０ｅと同軸上に形成されている。上記説明から、当業者であれば、制御ワイヤをボールの２つの部品間に配置し、これらの部品を互いに押圧して制御ワイヤを２つのタング７１０ｅによってＳ字状に曲げ、ボールを制御ワイヤに対して固定できることを理解できよう。
上記説明から、レバー７０２が開位置から図４１に示されている閉位置に向かって移動すると、レバーの上側部品７０２ｂ及び制御ワイヤ（不図示）が、制御ワイヤを引張ることができなくなるまで基端側に移動する。この時、レバーの上側部品７０２ｂが静止状態に維持される。しかしながら、レバーを完全に閉じた位置まで移動させるには、レバーの下側部品７０２ａを回動軸７０３を中心に回動させ続けて、フィンガー７０２ｆがばね７０６の後部７０６ａに力を加えるようにし、ばねフィンバー７０６ｂが、ばねフィンガー７０６ａ及びレバーの上側部品７０２ｂから離れるようにする。レバーが完全に閉じた位置から解放されると、最初にフィンガー７０２ｆの力がばねに対してかからなくなるがボール７１０またはワイヤは移動しない。最終的に、ばねフィンガー７０６ｂがレバーの上側部品７０２ｂに衝当すると、レバー全体、ボール７１０、及び制御ワイヤが先端側に移動する。
予荷重クリップエジェクタ
現在好適な実施形態では、十分な円柱の剛性が得られるように可撓性コイルの少なくとも一部（この場合は先端側コイル１２ｂ）が予荷重される。ジョー８４及び８６が閉じると、プルワイヤによりクレビスとナットの間の荷重が大きくなる。クリップが取り付けられると、プルワイヤが先端側に作動してジョーが開く。圧迫されたクリップ列の力、そして必要に応じてプルワイヤからの力が、先端側コイルの予荷重に十分に打ち勝ち、ジョー及びクレビスが、ジョーの成形アンビルに対して圧迫されたクリップの尖叉から離れる。この概念の代替の実施形態では、クレビスのボスに取り付けるための楕円形の孔をジョーに設ける。ジョーが閉じる際に、プルワイヤによりジョーがクレビスに対して基端側に移動する。プルワイヤの張力が解放されて先端側に作動すると、ジョーもまた、クレビスに対して先端側に移動してクリップの尖叉に対する圧縮荷重が解除される。
代替の実施形態に従えば、先端コイル１２ｂが先端側にばね付勢されるように予荷重される。ジョー８４及び８６が閉じる時に、プルワイヤ６２が先端コイルに対して引っ張られ、先端コイルが予荷重に対して短くなる。クリップが排出された後、ジョーは開いている。ジョーが開いている場合、先端コイルに対する予荷重により、クレビス８２、ジョー８４及び８６、及びガレージ８０の全てが先端側に僅かな距離移動する。この時、クリップがガレージに対して自由に移動できるようにそのガレージ内に配置されているためクリップは移動しない。この動作は、成形されたクリップの尖叉からジョーの先端側アンビルを分離する効果があり、ジョーが開くのが容易になる。
先端コイルの予荷重により、コイルに十分な円柱の強度が付与され、タンジェンシャルバイト（tangential bites）が可能となり、ジョー閉止中の座屈が防止され、ジョーが開く時に摩擦力に打ち勝つ反力が得られる。
代替のジョーの実施形態
図４４及び図４５に、取付け孔１８４ｄ及び１８６ｄを除いてジョー８４及び８６に実質的に同一であるジョー１８４及び１８６の代替の実施形態が例示されている。この実施形態では、取り付け孔１８４ｄ及び１８６ｄは円形ではない。これらの取付け孔は、偏円すなわちスロットである。これにより、ジョーが開閉する時に先端側及び基端側にスライドすることができる。従って、ジョーは、図４４に示されているように、閉じる時に基端側に移動する。この位置で、クリップが、上記したように成形アンビルとして作用する閉じたジョーの中に発射される。図４５に示されているように、ジョーは、開く時に、成形されたクリップから離れるように先端側にスライドし、これによりジョーの先端アンビルが成形クリップの尖叉から離れるため、ジョーが容易に開くことができる。
ここまで、可撓性内視鏡クリップアプライヤーの複数の実施形態を説明及び例示してきた。本発明の特定の実施形態を説明したが、本発明は当分野で許容される最大の範囲を含み、本明細書も同様に最大の範囲を含むため、本発明がそのような特定の実施形態に限定されるものではない。従って、本発明のコイルが平坦なストックから形成されるとして記載したが、このようなストックを円形または他の形状の断面にすることもできることを理解されたい。また、本発明の様々な要素の形成に特定の材料が好適であると記載したが、他の材料も利用できることを理解されたい。更に、クリップを前進させるためにワイヤ制御要素を所定数回転させる歯車機構を本発明に利用すると記載したが、ワイヤ制御要素を同数または異なった回数回転させる他の歯車機構も利用できることを理解されたい。更に、本発明はジョーとクリッププッシャーの２種類のエンドエフェクタを用いて説明したが、本発明の他の構成要素を別のエンドエフェクタに利用できることを理解されたい。従って、当業者であれば、特許請求の範囲に記載した範囲及び概念から逸脱することなく本発明の他の様々な変更形態が可能であることを理解できよう。
本発明の実施態様は以下の通りである。
（１）内視鏡クリップアプライヤーであって、
基端部及び先端部を有する可撓性チューブと、
前記可撓性チューブ内に延在する、基端部及び先端部を有する制御部材と、

（４）前記ジョーが閉じると、前記ジョーの内部が前記クリップを成形するためのアンビルを成すことを特徴とする実施態様（１）に記載の内視鏡クリップアプライヤー。
（５）前記クリップが２つの対抗した尖叉を有し、前記アンビルにより前記各尖叉が概ね半円状に曲げられることを特徴とする実施態様（４）に記載の内視鏡クリップアプライヤー。
（６）内視鏡クリップアプライヤーであって、
基端部及び先端部を有する可撓性チューブと、
前記可撓性チューブ内に延在する、基端部及び先端部を有する制御部材と、
前記チューブの前記先端部及び前記制御部材の前記先端部に結合された一対の同一の無性ジョーと、
前記チューブの前記基端部及び前記制御部材の前記基端部に結合されたアクチュエータとを含み、
前記ジョーの内部がアンビルを成すことを特徴とする内視鏡クリップアプライヤー。
（７）内視鏡クリップを排出する方法であって、
内視鏡の内腔を介して、一対の同一の無性ジョーを有する器具を送るステップと、
前記ジョーを組織に対して閉じるステップと、
前記閉じたジョーの内部に前記クリップを押し込んで前記クリップを排出するステップとを含むことを特徴とする方法。

  1. 内視鏡クリップアプライヤーであって、
    基端部及び先端部を有する可撓性チューブと、
    前記可撓性チューブ内に延在する、基端部及び先端部を有する制御部材と、
    前記チューブの前記先端部及び前記制御部材の前記先端部に結合された一対の同一の無性ジョーと、
    前記ジョーに近接した複数のクリップと、
    前記チューブの前記基端部及び前記制御部材の前記基端部に結合されたアクチュエータとを含み、
    前記各ジョーが非同一線上の異なる軸にそれぞれ回動可能に取り付けられていることを特徴とする内視鏡クリップアプライヤー。

  4. 前記クリップが２つの対抗した尖叉を有し、前記アンビルにより前記各尖叉が半円状に曲げられることを特徴とする請求項３に記載の内視鏡クリップアプライヤー。
  5. 内視鏡クリップアプライヤーであって、
    基端部及び先端部を有する可撓性チューブと、
    前記可撓性チューブ内に延在する、基端部及び先端部を有する制御部材と、
    前記チューブの前記先端部及び前記制御部材の前記先端部に結合された一対の同一の無性ジョーと、
    前記チューブの前記基端部及び前記制御部材の前記基端部に結合されたアクチュエータとを含み、
    前記ジョーの内部がアンビルを成し、
    前記各ジョーが非同一線上の異なる軸にそれぞれ回動可能に取り付けられていることを特徴とする内視鏡クリップアプライヤー。
  6. 内視鏡クリップアプライヤーであって、
    基端部及び先端部を有する可撓性チューブと、
    前記可撓性チューブ内に延在する、基端部及び先端部を有する制御部材と、
    前記チューブの前記先端部及び前記制御部材の前記先端部に結合された一対のジョーと、
    前記ジョーに近接した複数のクリップと、
    前記チューブの前記基端部及び前記制御部材の前記基端部に結合されたアクチュエータとを含み、
    前記各ジョーが非同一線上の異なる軸にそれぞれ回動可能に取り付けられていることを特徴とする内視鏡クリップアプライヤー。
