Description
この特許出願では、２００４年２月５日に出願され「Method and apparatus for prediction and monitoring of respiration manifested clinical episodes」と題する出願番号第６０／５４１，７７９号の米国仮特許出願に基づく優先権を主張している。なお、この米国仮特許出願はこの特許出願の被譲渡人に譲渡されており、この特許出願において援用する。
本発明は、広くは、異常な生理的状態を予測しモニタすることに関し、より詳しくは、呼吸の特性を測定し分析することにより異常な生理的状態を予測しモニタする方法及び装置に関する。
慢性疾患は、多くの場合、臨床的徴候の発症的な悪化により現れる。慢性疾患の予防的治療は、必要となる薬剤の全体的用量と付随する副作用とを減少させる。一般的に、予防的治療は、最も早い臨床的徴候が検出されたときに直ちに開始される又は強化されるべきであり、それによって、臨床的発症の進行及び悪化を防止し、病態を停止し逆転させることができる。従って、発症前の指標を正確にモニタすることができればね慢性疾患の予防的治療の有効性を増すことが可能になる。
多くの慢性疾患は、様々な病理学的機序を通じて正常な呼吸パターンを妨げる。喘息、慢性閉塞性肺疾患（ＣＯＰＤ）、嚢胞性繊維症（ＣＦ）などの一般的な呼吸不全は、呼吸パターンに直接的に影響を与える。糖尿病、癲癇、ある種の心疾患などそれ以外の慢性疾患も、交感神経系や副交感神経系の異常に至る病態のために、異常な呼吸活動に影響することが知られている。
喘息は、治療法が知られていない慢性疾患である。喘息症状の実質的な緩和は、気管支拡張薬や抗炎症性薬剤の使用など、予防的な療法により可能である。喘息の管理は、喘息患者の生活の質（ＱＯＬ）の改善に狙いを定めている。喘息管理は、患者と医師との両者にとって深刻な挑戦を提供するのであるが、その理由は、予防的な治療法のためには、肺機能の定期的なモニタリングとそれに対応する医薬品のタイプ及び用量の調整とが必要であるからである。しかし、肺機能のモニタリングは単純ではなく、複雑な器具と専門知識を必要とし、それは、医療機関の外部すなわち一般家庭の環境では利用できない。
肺機能のモニタリングは、患者の追跡においてだけでなく、適切な治療を決定する際の主要なファクタと見なされている。好ましい治療法は、全身的な副作用を最小化するエアロゾル型の薬剤に基づくものである。エアロゾル型の治療法の有効性は、患者の遵守に大きく依存するが、これは評価が困難であり、また、肺機能のモニタリングの重要性にも影響する。
喘息の発症は、通常、数日間の期間にわたって進展するが、ときには予期せずに現れることもありうる。喘息発症が漸進的に開始する場合には、炎症プロセスを停止させ逆転させるための防止策を開始する機会を提供してくれる。発症前の段階での早期治療は、臨床的発症の発現を著しく減少させ、臨床前段階から臨床的発症段階への移動をすべて防止する場合さえある。
喘息のモニタリングには、２つの技術が用いられるのが一般的である。第１の技術である肺活量測定法は、肺が吸い込み吐き出す空気の体積を測定する装置である肺活量測定計を用いて肺機能を評価する。管を経由して肺活量測定計に接続されたマウスピースの中への患者による強制的に調整された吸気及び呼気の間に、気流の力学的性質が測定される。ピーク・フロー・メータは、肺活量測定計に似ていて、より単純な装置であり、同じような態様で用いられる。第２の技術は、専用の酸化窒素モニタを用いて酸化窒素の濃度を測定することにより肺機能を評価する方法である。患者は、管を経由してモニタに接続されたマウスピースの中に息を吹き込む。
効果的な喘息の管理には、呼吸機能の日常的なモニタリングが必要であるが、これは、特に医療機関ではない一般家庭の環境においては、ほぼ実際的でない。ピーク・フロー・メータや酸化窒素モニタは、肺機能の状態の一般的な指標を提供する。しかし、これらのモニタリング装置は予測値を有しておらず、発症期間中のマーカとして用いられる。更に、ピーク・フロー・メータや酸化窒素モニタは、患者の能動的な参加を必要とするが、これは、多くの子供の場合には得ることが困難であるし、幼児の場合にはほぼ不可能である。
鬱血性心不全（ＣＨＦ）は、心臓が脆弱化し血液を身体の必要に見合うように循環させることができない状態である。それに続く脚部、腎臓及び肺における血液の集積によって、その状態が鬱血性として特徴付けられる。脆弱化は、心臓の左、右又は両側のいずれかと関連し、それぞれのタイプに対して異なる病因論及び治療が関連付けられる。ほとんどの場合には、故障を生じるのは心臓の左側であり、従って、血液を体系的な循環に効果的に送り出すことができない。結果として肺において生じる鬱血により、速度及び／又はパターンの変化を含めて呼吸が変化し、呼吸困難及び頻呼吸の増加がそれに続く。
そのような異常呼吸の数量化は、ＣＨＦの進行を評価する基礎を提供する。例えば、チェーン・ストローク呼吸（ＣＳＲ）は、無呼吸と過呼吸とが交互に生じる規則的な循環を伴う１回の呼吸量の周期的な振動によって特徴付けられる呼吸パターンである。ＣＳＲは、多くの異なる病状（脳炎、脳の循環障害、呼吸中枢の障害など）において観測されうるが、心不全を悪化させる独立の危険因子として認識され、ＣＨＦを有する患者の生存率を低下させてきた。ＣＨＦにおいては、ＣＳＲは、睡眠を断片化する頻繁な覚醒と関連し、付随的に交感神経の活動と関連するが、これらは両方ともにＣＨＦを悪化させる可能性がある。これ以外の異常な呼吸パターンは、延長された呼気又は吸気と関連し、又は、呼吸速度の漸進的な変化は、頻呼吸に至るのが通常である。
Scanlonへの米国特許第５，８５３，００５号は、この特許出願において援用するが、この米国特許にはパッドの中の流体と連絡するトランスデューサが記載されている。パッドは音又は運動源と近接して接触するように保持され、流体の中に送られる音響信号をモニタする。信号パターンは、聴覚的にモニタされ、及び／又は、所定の基準パターンと比較され、この比較の結果に応答して光学的制御及びシミュレーション手段を付勢することができる。感知された音響信号は、遠隔の受信機に伝送することができるし、ローカルに処理することも可能である。典型的には、音響信号は、生物の心音又は呼吸を表す。このモニタリング・システムは、ＳＩＤＳ、無呼吸、家庭での新生児のモニタリング、医療輸送装置、血圧カフス、シート、戦闘における負傷兵の治療及びハンドヘルド型装置などを含む様々な状況に応用可能である。このシステムが健康状態のモニタリング、回復、研究又は存在検出のために家庭又は施設のマットレスに付着されるという実施例が記載されている。
Lehman他への米国特許第６，６６６，８３０号は、この特許出願において援用するが、この米国特許には、閉塞型睡眠時無呼吸事象が完全に生じる前に、そして、呼吸停止が生じる前に、閉塞型睡眠時無呼吸事象の開始を検出するシステムが記載されている。このシステムは、人の気道の中における呼吸音を検出することができる１又は複数のマイクロフォンを含む。このマイクロフォンは、呼吸音を表す信号を発生し、その信号をコントローラに送る。コントローラは、閉塞型睡眠時無呼吸事象の開始時において生じるその人間の呼吸パターンと関連する少なくとも１つの信号パターンを識別する。また、コントローラは、その人間の部分的に閉塞した呼吸パターンと関連する少なくとも１つの信号パターンを識別する。コントローラは、デジタル信号処理技術を用いて呼吸音を表す信号を分析することにより、信号パターンを識別する。この方法は、人の気道の中における呼吸音を検出し、呼吸音を表す信号を発生し、閉塞型睡眠時無呼吸事象の開始時において生じるその人の呼吸パターンと関連する少なくとも１つの信号パターンを識別することを含む。
Murphyへの米国特許第６，７９０，１８３号は、この特許出願において援用するが、この米国特許には、患者の吸気及び呼気と関連する肺の音を収集し組織化し分析する際に用いるための肺音診断システムが記載されている。このシステムは、患者の胸部付近の様々な場所に設置することができる複数のトランスデューサを含む。マイクロフォンが信号処理回路とデータをデジタル化し好ましくはデジタル・データをコンピュータ・ステーションに提供するＡ／Ｄコンバータとに結合されている。このシステムは、異常音を検出し分類するアプリケーション・プログラムを含むこともある。結果的に得られる情報は、様々なフォーマットで表示され診断を容易にすることができる。更に、このシステムは、検出された異常音に対応する選択された基準を所定のスレショルドと比較して、おおよその診断を提供する分析プログラムを含む場合がある。また、特発性肺線維症（ＩＰＦ）を有する患者が生じるクラックル音とＣＨＦ患者が生じるクラックル音とを区別するシステム及び方法も記載されている。

本発明の実施例では、慢性の症状をモニタする方法は、典型的には夜間の睡眠中における被験者の少なくとも１つの呼吸パターンを非侵襲的にモニタするステップを含む。このパターンは、（ａ）喘息発作などの近接しつつある臨床的発症を予測し、（ｂ）臨床的発症が生じたときにはその深刻度及び進行をモニタするために分析される。パターンの分析は、そのパターンを基準パターンと比較するステップを含む。近接しつつある臨床的発症を予測することによって早期の予防的治療が容易になり、必要となる医薬品の量を低減することができる。

更にまた、本発明の実施例によると、臨床的発症の間の被験者の呼吸を感知し感知された呼吸に応答して信号を発生する呼吸センサと、前記信号を受け取り、前記信号に応答して前記被験者の少なくとも１つの呼吸パターンを決定し、前記呼吸パターンを基準呼吸パターンと比較するして、前記比較に少なくとも部分的に応答して慢性閉塞性肺疾患（ＣＯＰＤ）、嚢胞性繊維症（ＣＦ）、糖尿病及び癲癇から構成されるリストから選択された状態と関連する異常呼吸パターンを検出する制御ユニットと、を含む装置が提供される。
本発明の実施例によると、臨床的発症の予測及び評価方法であって、夜間睡眠の間の呼吸速度変動パターンを測定するステップと、前記呼吸速度変動パターンを正常の呼吸速度変動パターンと比較するステップと、近接しつつある臨床的発症の蓋然性又は既に生じている発症の進行若しくは深刻度を決定するステップを含む方法が提供される。
ある応用例では、呼吸速度変動パターンの測定は、複合身体運動信号を測定し前記複合身体運動信号から周期的で呼吸に関連する運動信号を抽出する手段によって実行される。あるいは、呼吸速度変動パターンの測定は、口及び／又は鼻からの呼吸気流を測定する手段によって実行される。また、あるいは、呼吸速度変動パターンの測定は、胸部、背中、首及び／又は顔からの気流及び肺音を測定する音響測定手段によって実行される。
ある応用例では、前記正常な呼吸速度パターンは、非徴候的な期間の間に患者から抽出される。ある応用例では、前記正常な呼吸速度パターンは、年齢、身長、体重及び／又は性別などの特性が類似する正常で健康な対象からの平均化されたパターンから抽出される。
ある応用例では、前記呼吸速度変動パターンには、（１）典型的な継続時間が数秒から数分までの範囲にある循環的なパターンと、（２）通常は数時間継続する分節化された単調に減少する呼吸速度の低速傾向との少なくとも１つが含まれる。
ある応用例では、前記比較は、前記正常な呼吸速度変動パターンからの前記呼吸速度変動パターンの逸脱の度合いの計算に基づく。
ある実施例では、前記臨床的発症は臨床的な喘息発症である。
ある応用例では、前記臨床的発症は、糖尿病、心臓状態、神経的な不調、癲癇など呼吸速度パターンに影響を与える任意の慢性疾患に関係する。
更に、本発明の実施例によると、臨床的発症を評価及び予測する装置であって、呼吸を測定する呼吸センサと、前記呼吸センサの出力信号を増幅する増幅器と、前記増幅器の出力をデジタル化するＡ／Ｄカードと、呼吸速度パターンを抽出して抽出されたパターンを正常なパターンと比較するプロセッサと、結果を数値、テキスト、グラフィカルなディスプレイに提供する、又は、結果を臨床追試センターに送信する出力装置とを含む装置が提供される。
ある応用例では、呼吸センサは、ベッド・マットレスの下に設置される運動感知性のセンサとして実現される。あるいは、呼吸センサは、被験者の顔に向けられた気流検出器として実現される。更にあるいは、呼吸センサは、被験者の顔、胸部又は背中に向けられた又は付着された音響検出器として実現される。
本発明は、以下の詳細な説明を添付の図面とともに読むことによって、より完全に理解されるはずである。
図１は、本発明の実施例による慢性の健康状態をモニタするシステム１０を図解するブロック図である。システム１０は、典型的には、呼吸関連データ取得モジュール２０と、呼吸パターン分析モジュール２２と、出力モジュール２４とを含む。応用例によっては、モジュール２０、２２及び２４の中の２つ又はそれ以上が、単一のハウジングの中にパッケージ化される。それ以外の応用例では、これらのモジュールは別個にパッケージ化され、例えば、呼吸関連データ取得モジュール２０によって局所的に取得された呼吸信号のパターン分析モジュール２２による遠隔的な分析が可能になる。
本発明の実施例では、データ取得モジュール２０は、被験者の呼吸パターンを非侵襲的にモニタするように構成されている。パターン分析モジュール２２は、（ａ）喘息発作などの近接しつつある臨床的発症を予言し、及び／又は、（ｂ）それが生じた場合には臨床的発症の深刻度及び進行を予測するために、パターンを分析するように構成されている。出力モジュール２４は、被験者及び／又は医療従事者に、予測された又は発生している発症について告知するように構成されている。近接しつつある臨床的発症を予測することにより、早期の予防的な治療が容易になり、それにより、必要になる医薬品の量を減少させることができるのが一般的である。喘息の治療の際には、いったん発症が生じてしまった場合の炎症状態を緩和するのに必要となるのが典型的である大量の医薬品使用に伴う副作用を最小化することができるのが一般的である。
システム１０は任意の時刻に呼吸パターンをモニタすることができるが、何らかの条件のために、夜間の睡眠中に呼吸パターンをモニタするのが一般的に最も効果的である。被験者が目覚めているときは、モニタされている条件とは関係のない身体的及び精神的活動が呼吸パターンに影響を与える場合が多い。そのような関係のない活動は、ほとんどの夜間の睡眠中には影響が少ないのが一般的である。応用例によっては、システム１０は、一晩のすべて又は大半を通じてパターンをモニタし記録する。結果的に得られるデータ・セットは、一般的に、典型的な長期の呼吸パターンを含み、総合的な分析が容易である。更に、データ・セットが非常に大きいために、統計的に有意な分析に十分なデータを維持しながら、運動やそれ以外の人為的な要素によって汚染された部分を排除することが可能である。
次に、図２を参照するが、これは、本発明の実施例によるデータ取得モジュール２０を図解しているブロック図である。データ取得モジュール２０は、典型的には、呼吸センサ３０と、少なくとも１つの前値増幅器３２、少なくとも１つのバンドパス・フィルタ３４、アナログ・デジタル（Ａ／Ｄ）コンバータ３６などの適当なそれ以外の回路とを含む。
本発明の実施例では、呼吸センサ３０は、圧力ゲージを含み、この圧力ゲージは典型的には被験者がその上で眠るマットレスの中、上又は下に取り付けられて、被験者の呼吸に関連する運動を感知する。応用例によっては、呼吸センサ３０は、被験者がその上で眠る、シーツ、マットレス・パッド、マットレス・カバーなどのマットレス被覆物の下に取り付けられる場合もある。パターン分析モジュール２２は、図３を参照して後述するように、呼吸パターンを運動データから抽出するように構成されている。これに代わって、又は、更に加えて、呼吸センサ３０は、被験者の顔、首、胸及び／又は背中に付着される又は向けられる音響又は気流センサなどの別のタイプのセンサを含む。
図３は、本発明の実施例によるパターン分析モジュール２２を図解しているブロック図である。パターン分析モジュール２２は、典型的には、デジタル信号プロセッサ（ＤＳＰ）４０と、デュアル・ポートＲＡＭ（ＤＰＲ）４２と、ＥＥＰＲＯＭ４４と、Ｉ／Ｏポート４６とを含む。パターン分析モジュール２２は、データ取得モジュール２０によって生成された生のデータから呼吸パターンを抽出し、呼吸パターンの処理及び分類を実行するように構成されている。パターン分析モジュール２２は、典型的には睡眠中の呼吸パターンにおける変化を分析する。この分析に応答して、パターン分析モジュール２２は、（ａ）近接しつつある臨床的発症を予測し、及び／又は、（ｂ）発症の深刻度及び進行をモニタする。
上述したように、本発明の実施例では、呼吸センサ３０は、マットレスの下に備え付けられた圧力ゲージを含み、被験者の呼吸に関連する運動を感知する。睡眠中の被験者の運動は、規則的な呼吸運動に加えて、それ以外の関係のない身体運動も含む。一般的に、呼吸に関連する運動が、睡眠中の身体運動への主要な寄与要素である。パターン分析モジュール２２は、圧力ゲージから受け取られる呼吸に関連しない運動を表す運動信号の部分を実質的に排除するように構成される。例えば、パターン分析モジュール２２は、呼吸とは関連のない運動によって汚染された信号の部分を除去することができる。呼吸に関連する運動は周期的であるが、呼吸とは関連のない運動は一般的にランダムであり非周期的である。応用例によっては、パターン分析モジュール２２は、周波数領域のスペクトル分析や時間領域の回帰分析を用いて、呼吸とは関連のない運動を排除する。これらの分析手法を適用する方法は、この明細書を読んだこの技術分野の当業者には明らかなはずである。応用例によっては、パターン分析モジュール２２は、線形予測やアウトライヤ分析などの統計的方法を用いて、呼吸とは関連しない運動を信号から排除する。パターン分析モジュール２２は、典型的には、少なくとも１０Ｈｚのサンプリング・レートで運動データをデジタル化する。ただし、応用例によっては、これよりも低い周波数が適している場合がある。
パターン分析モジュール２２は、典型的には、それぞれのパルスが１つの吸気−呼気のサイクルを含む一連の過渡的な呼吸パルスから呼吸パターンを抽出するように構成されている。夜間の睡眠中の呼吸パターンは、一般的に、以下のものを含む複数のカテゴリの中の１つとなる。
比較的高速で変化し、ランダムな呼吸パターンであり、主にＲＥＭ睡眠の間に生じるもの。
循環的な呼吸速度変動パターンであって、その典型的な継続時間が数秒から数分までの範囲にあるもの。
呼吸速度の低速傾向（典型的には、健康な被験者の正常な睡眠の間に見られるものであって、この場合の低速傾向には、通常は数時間継続する部分的には実質的に単調減少する呼吸速度が含まれる。喘息など特定の条件を慢性的に有している被験者の場合には、例えば図４を参照して後述するように、この単調減少はそれほどはっきりしないか、又は、存在しない）。
咳やそれ以外の睡眠妨害要素など、呼吸パターンの中断。
瞬間的な覚醒によって生じる呼吸パターンの中断。
これらの呼吸パターンは、睡眠段階、心配、体温などの様々な生理学的パラメータと関連する。例えば、ＲＥＭ睡眠にはランダムに変動する呼吸パターンを伴うのが通常であるが、深い睡眠段階にはより規則的で安定したパターンが伴う。以上に高い体温は呼吸速度を加速させることがあるが、通常は、正常な循環的な呼吸速度変動パターンを維持する。心配などの心理的な変数も睡眠中の呼吸パターンを変動させる要素であるが、その影響は、睡眠が進行するに連れて減少するのが通常である。咳や瞬間的な覚醒によって生じる呼吸パターンの中断は、正常である場合、喘息と関連する場合、それ以外の関連のない病理的なものと関連する場合があり、コンテキストの中で評価される。
本発明の実施例では、パターン分析モジュール２２は、喘息発作の開始を予測する、及び／又は、その深刻度及び進行をモニタするように構成される。パターン分析モジュール２２は、典型的には、呼吸速度と呼吸速度変動パターンとの組合せの変化を分析して喘息発作の開始を予測する。呼吸速度は典型的には発作開始の前に上昇するが、この速度の上昇だけでは、常に発作開始の特定のマーカになるとは限らない。従って、発作開始をより正確に予測し、発作の深刻度及び進行をモニタするために、パターン分析モジュール２２は、呼吸速度変動パターンの変化を分析するのが典型的である。応用例によっては、パターン分析モジュール２２は、以下のパターンの中の１又は複数をそれぞれの基準パターンと比較して、基準からの逸脱を、（ａ）発作の開始、及び／又は、（ｂ）進行中の発作の深刻度を示すものと解釈する。
低速傾向呼吸速度パターン。パターン分析モジュール２２は、発作が近接しつつある又は進行していることを示すものとして、基準に対する上昇、例えば、ほぼ健康な被験者の場合には、少なくとも１時間、例えば少なくとも２、３又は４時間にわたる呼吸速度の典型的な分節化された単調減少の減衰や、この減少が発作の深刻度に応じて呼吸速度パターンの上昇に転じた場合を解釈する。
呼吸速度変動パターン。パターン分析モジュール２２は、呼吸速度変動の減少を、発作の近接又は進行を示すものと解釈する。この減少は、発症の開始が近接しつつあると生じるのが一般的であり、発作中の呼吸の短さの進行と共に強まる。
呼吸デューティサイクル・パターン。パターン分析モジュール２２は、呼吸デューティサイクルの実質的な上昇を、発作の近接又は進行を示すものと解釈する。呼吸デューティサイクル・パターンには、限定的であることを意味しないが、吸気時間／全体の呼吸サイクル時間、呼気時間／全体の呼吸サイクル時間、（呼気＋吸気時間）／全体の呼吸サイクル時間が含まれる。
咳、睡眠妨害要素、覚醒などによって生じる呼吸パターンの中断。パターン分析モジュール２２は、これらの事象を数量化して、潜在的な喘息発作の予測との関連性を決定する。
パターン分析モジュール２２は、典型的には、非徴候的な期間の間に被験者の呼吸パターンを分析することによって基準パターンを決定する。これに代わり、又は、これに加えて、パターン分析モジュール２２は、人口平均に基づく基準パターンを用いてプログラムされている。応用例によっては、この人口平均は、年齢、身長、体重、性別などの特徴により分節化される。
本発明の実施例では、呼吸サイクルは、吸気及び呼気の連続的な部分に分割される。パターン分析モジュール２２は、睡眠中（典型的には夜間の睡眠中）の吸気及び／又は呼気部分のより長い継続時間への傾向を、発作の近接又は進行を示すものと解釈する。
本発明の実施例では、呼吸センサ３０は、更に、喘鳴又は咳嗽によって生じるような呼吸関連音の測定のための音響センサを含む。（呼吸センサ３０が圧力ゲージを含むような応用例では、音響センサは圧力ゲージと一体化されている。あるいは、音響センサは別個のコンポーネントである。）パターン分析モジュール２２は、これらの呼吸音を独立に処理するか、又は、例えば、スペクトル平均化を用いて喘鳴音の信号対雑音比を向上させることにより呼気及び／又は吸気に時間固定して処理する。応用例によっては、喘鳴のレベルと吸気及び呼気のタイミングに対するそのタイミングとにより、近接しつつある喘息発作を予測する及び／又は発作の深刻度及び進行をモニタする追加的な情報が得られる。
喘鳴及び咳嗽は、呼吸サイクルの特定の部分（主に、吸気及び呼気）に帰着することができるから、近接しつつある又は進行中の呼吸困難のタイプに関する有益な示唆を与える。更に、喘鳴は、呼吸サイクルの周期性に従ってフィルタリングすることが可能であり、従って、妨害されている気道の呼吸関連音の識別を向上させる。周期的で呼吸サイクルに関連する喘鳴は、近接しつつある又は進行中の呼吸困難のタイプに関する追加的な示唆を提供することができる。
本発明の実施例では、パターン分析モジュール２２は、典型的には夜間の睡眠中に、頻呼吸やチェーン・ストローク呼吸（ＣＳＲ）などの鬱血性心不全（ＣＨＦ）と関連する異常な呼吸パターンを検出するように構成されている。ＣＨＦの治療は有効であるから、その早期の検出は重要である。
図１を再び参照する。出力モジュール２４は、典型的には、ＬＣＤやＣＲＴモニタなどの専用表示ユニットを含む。これに代わり、又は、これに加えて、出力モジュールは、取得して処理したデータを更なる分析や解釈のために遠隔サイトに中継するための無線又は有線の通信ポートを含む。
図４を参照すると、この図は、本発明の実施例に従って行われた実験の間に測定された慢性喘息患者の呼吸速度パターンを図解するグラフである。喘息患者の呼吸が、何日かにわたり夜間の睡眠中にモニタされた。この患者の呼吸速度が、睡眠中の１時間単位で平均化された（ただし、レム（rapid eye movement）睡眠の期間は除外されている）。この患者は、モニタされた最初のほぼ２ヶ月の間は、喘息の発症を全く経験しなかった。ライン１００は、この非発症期間の間に記録された典型的な低速傾向呼吸パターンを表しており、従って、この患者についての基準低速傾向呼吸速度パターンを表している。注意すべきであるのは、非喘息患者において典型的に観測される呼吸速度の単調な低下とは異なり、この実験における慢性喘息患者の基準呼吸速度パターンは、最初の数時間の睡眠の間に生じる呼吸速度の初期低下を反映しており、その晩の残りの時間ほとんどを通じた呼吸速度の漸進的な上昇がそれに続いている。
ライン１０２及び１０４は、ほぼ２ヶ月の期間の終了時における連続的な２つの晩に記録された。ライン１０２はこの２つの晩の最初の夜のものであり、ライン１０４はあとの夜のものである。この患者は、２日目の夜に、喘息の発症を経験した。ライン１０２及び１０４は、従って、発症前の低速傾向呼吸速度パターンと発症時の低速傾向呼吸速度パターンとをそれぞれ表している。グラフにおいて見ることができるように、この患者の呼吸速度は、発症前の晩の全時間を通じ基準と比較して実質的に上昇しており、発症した晩には基準と比較して更に上昇している。
ここで説明した方法を用いて、ライン１０２のパターンは、患者が喘息の発症を経験するであろうことを予測するために、ライン１００の基準パターンと比較される。ライン１０４のパターンは、喘息発症の進行を評価するために、ライン住居用配電システム１００の基準パターンと比較される。
ここで説明された複数の実施例は特に喘息の発症又はＣＨＦと関連するものであったが、本発明の原理は、ＣＨＦの他にも、慢性閉塞性肺疾患（ＣＯＰＤ）、嚢胞性繊維症（ＣＦ）、糖尿病、神経的障害（例えば、癲癇）及びある種の心疾患など正常な呼吸パターンに影響を与える呼吸及び非呼吸条件の予測及びモニタリングに準用することができる。
本発明は以上で示し説明した内容に限定されないことはこの技術分野の当業者にとっては明らかである。むしろ、本発明の範囲は、以上の説明を読むことによりこの技術分野の当業者が容易に想到でき従来技術の中に含まれない改変や修正だけでなく、以上で説明された様々な特徴の組合せや下位組合せの両方を含む。
Claims (16)
- 感知された被験者の呼吸を用いることによって前記被験者における臨床的発症の開始を予測する装置の作動方法であって、
データ取得モジュールが、前記被験者の呼吸を感知し、感知された呼吸に応答して信号を発生するステップと、
パターン分析モジュールが、少なくとも１時間にわたって前記信号に応答して前記被験者の少なくとも１つの低速傾向呼吸速度パターンを決定するステップと、
前記パターン分析モジュールが、前記低速傾向呼吸速度パターンを基準低速傾向呼吸速度パターンと比較するステップと、
前記パターン分析モジュールが、前記比較に少なくとも部分的に応答して前記発症の開始を予測するステップと、
を含んでおり、
前記臨床的発症は、喘息、慢性閉塞性肺疾患（ＣＯＰＤ）、嚢胞性繊維症（ＣＦ）、ＣＨＦ、糖尿病及び癲癇から構成されるリストから選択される条件と関連付けられた発症を含むことを特徴とする方法。 - 請求項１に記載の方法において、前記呼吸の感知は、前記被験者の呼吸に関連する身体運動データの取得を含むことを特徴とする方法。
