Description
Claims (13)
- 脊椎ロッドをネジのヘッド部分に形成されたU字形チャンネル内に埋め込むための低侵襲的なシステムであって、
ガイドと、スリーブと、を有し、
前記ガイドは、本体と、近位端部と、遠位端部と、第1の長手方向スロットと、前記第1の長手方向スロットと直径方向反対側に位置する第2の長手方向スロットと、前記遠位端部から前記近位端部まで延びる内腔とを有し、前記第1の長手方向スロット及び前記第2の長手方向スロットは、前記本体の前記遠位端部から前記近位端部に向って延び、前記遠位端部は、ネジのヘッド部分に係合するように寸法決めされ且つ構成され、
前記スリーブは、細長い本体と、近位端部と、遠位端部と、第1の開口と、前記スリーブの遠位端部から当該スリーブにおける前記第1の開口に対して傾斜して交差する位置まで延びる第1の長手方向スロットと、第2の開口と、前記スリーブの遠位端部から当該スリーブにおける前記第2の開口に対して傾斜して交差する位置まで延びる第2の長手方向スロットと、前記遠位端部から前記近位端部まで延びる内腔とを有し、且つ、前記ガイドの内腔内に受入れられるように寸法決めされ且つ構成され、
前記スリーブを前記ガイドに形成された内腔に挿入したとき、前記スリーブに形成された第1の開口は、前記ガイドに形成された第1の長手方向スロットと整列して第1の窓を形成し、前記スリーブに形成された第2の開口は、前記ガイドに形成された第2の長手方向スロットと整列して第2の窓を形成し、前記第1の窓及び前記第2の窓は、その中を通るロッドを受入れるように寸法決めされ且つ構成され、
前記スリーブは、前記ガイド内において第1の位置と第2の位置との間を移動可能であり、
前記スリーブが第1の位置にあるとき、前記第1の窓及び前記第2の窓に受入れられたロッドは、前記ガイドに形成された第1の長手方向スロット及び第2の長手方向スロット内で移動することができないように、前記スリーブに形成された第1の長手方向スロット及び第2の長手方向スロットはそれぞれ、前記ガイドに形成された第1の長手方向スロット及び第2の長手方向スロットと整列せず、
前記スリーブが第2の位置にあるとき、前記スリーブが第1の位置にある間に前記第1の窓及び前記第2の窓に受入れられたロッドは、前記ガイドに形成された第1の長手方向スロット及び第2の長手方向スロット内で移動可能となるように、前記スリーブに形成された第1の長手方向スロットは、前記ガイドに形成された第1の長手方向スロットと整列し、且つ前記スリーブに形成された第2の長手方向スロットは、前記ガイドに形成された第2の長手方向スロットと整列する、システム。 - 前記第1の窓は、前記ガイドの遠位端部から第1の距離のところに形成され、前記第2の窓は、前記ガイドの遠位端部から前記第1の距離と異なる第2の距離のところに形成される、請求項1に記載のシステム。
- 前記スリーブに形成された第1の長手方向スロットは、前記第1の開口に対して実質的に垂直であり、前記スリーブに形成された第2の長手方向スロットは、前記第2の開口に対して実質的に垂直である、請求項1に記載のシステム。
- 前記スリーブに形成された第1の長手方向スロット及び第2の長手方向スロットは、前記スリーブの長手方向軸線に対して実質的に平行であり、前記第1の開口及び前記第2の開口はそれぞれ、前記スリーブに形成された第1の長手方向スロット及び第2の長手方向スロットに対して実質的に垂直であり、その結果、前記スリーブに形成された第1の開口及び第1の長手方向スロットと、前記スリーブに形成された第2の開口及び第2の長手方向スロットとは、実質的にL字形の1対の開口を前記スリーブに形成する、請求項1に記載のシステム。
- 前記スリーブに形成された第1の開口及び第2の開口はそれぞれ、前記スリーブの遠位端部から延びる第1の螺旋開口及び第2の螺旋開口の形態である、請求項1に記載のシステム。
- 前記ネジが患者内に埋め込まれ且つ前記システムが前記ネジに連結されているとき、前記第1の窓は、患者の皮膚レベルの上方に位置するように構成され、前記第2の窓は、患者の皮膚レベルの下方に位置するように構成され、その結果、前記第1の窓及び前記第2の窓の中を通して埋め込まれた脊椎ロッドは、この脊椎ロッドが前記ガイドの内腔内に配置されている間、患者の筋肉層の中を通って移動することが可能である、請求項1〜5の何れか1項に記載のシステム。
- 前記ガイドは、更に、外側カラーを有し、前記外側カラーは、前記ガイドの近位端部から第1の距離のところに位置する第1の位置から、前記ガイドの近位端部から前記第1の距離よりも長い第2の距離のところに位置する第2の位置まで移動可能であり、前記ネジのヘッド部分は、前記外側カラーが第1の位置にあるときに前記ガイドの内腔内に受入れられるように寸法決めされ且つ構成され、前記ネジのヘッド部分は、前記外側カラーが第2の位置にあるときに前記ガイドの内腔内に固定される、請求項1〜6の何れか1項に記載のシステム。
- 前記ガイドの遠位端部は、前記ネジに対する前記ガイドの回転を防止するために、前記ネジのヘッド部分に形成された溝に受入れ可能な少なくとも1つの突出部を有する、請求項7に記載のシステム。
- 前記突出部は、前記ガイドに形成された第1の長手方向スロット及び第2の長手方向スロットが前記ネジのヘッド部分に形成されたU字形チャンネルと整列するように構成される、請求項8に記載のシステム。
- 前記突出部は、前記ガイドに形成された第1の長手方向スロット及び第2の長手方向スロット内に延びる、請求項9に記載のシステム。
- 前記ガイドは、更に、前記外側カラーを第1の位置から第2の位置へ移動させるために前記本体に沿って位置決めされた作動機構を有する、請求項7〜9の何れか1項に記載のシステム。
- 更に、長手方向軸線を有するロッドを有し、前記ロッドは、低侵襲的な前記システムに形成された第1の窓及び第2の窓を通して、前記ネジのヘッド部分に形成されたU字形チャンネルの中へ挿入されるように寸法決めされ且つ構成され、
更に、長手方向軸線を有する関節連結式のロッド挿入器具を有し、前記ロッド挿入器具は、低侵襲的な前記システムに形成された第1の窓及び第2の窓を通して前記ロッドに係合し且つ前記ロッドを前記ネジのヘッド部分に形成されたU字形チャンネルの中に埋め込むように寸法決めされ且つ構成され、前記ロッド挿入器具により、前記ロッド挿入器具の長手方向軸線に対する前記ロッドの長手方向軸線の角度が調節可能である、請求項1〜11の何れか1項に記載のシステム。 - 前記ガイドに形成された第1の長手方向スロット及び第2の長手方向スロットの幅は、約1mm〜約5mmである、請求項1〜12の何れか1項に記載のシステム。
