本願は、概して、医療用デバイスに関し、より具体的には、上室性不整脈を治療するためのシステム、デバイス、および方法に関する。

Claims (16)
- 制御装置およびメモリと、
該制御装置と協働して、上室性不整脈事象を検出するように適合された感知器であって、該上室性不整脈事象は、上室性不整脈発現または上室性不整脈発現に対する前兆を含む、感知器と、
神経標的における交感神経活性を刺激すること、および/または、副交感神経活性を抑制することにより、交感神経反応を誘導するように適合された神経刺激装置と
を含むデバイスであって、
該感知器は、生理学的パラメータを検出することにより、神経刺激制御フィードバックを提供するように適合されており、
該制御装置は、数秒から数分までの間、一時的神経刺激を送達することにより、該検出された上室性不整脈事象に応じて、交感神経反応を誘導するように適合されており、
該制御装置は、
該検出された上室性不整脈事象に応じて、該神経刺激装置を制御して、該交感神経反応を誘導することにより、上室性不整脈治療のために該検出された上室性不整脈発現を抑制または転換するか、または、予防的治療のために該上室性不整脈発現を回避することと、
該神経刺激制御フィードバックを用いて、該神経刺激および/または抑制の強度を調節することであって、該制御装置は、該メモリ内に格納された該交感神経反応に対する標的範囲と、該検出された生理学的パラメータとを比較することにより、該神経刺激の強度を調節することと
を行うように適合されている、デバイス。 - 前記感知器は、心房活性を反映する電気図を感知するように適合された感知器を含む、請求項1に記載のデバイス。
- 前記感知器は、心臓活性を反映する電気図を感知するように適合された感知器を含み、判別アルゴリズムを実行して、前記上室性不整脈事象を検出するように適合されたプロセッサをさらに含む、請求項1に記載のデバイス。
- 前記上室性不整脈事象は、前記上室性不整脈発現に対する前兆を含み、前記制御装置は、前記神経刺激装置を制御して、予防的神経刺激を送達することにより、該上室性不整脈発現を回避するように適合されている、請求項1に記載のデバイス。
- 前記上室性不整脈事象は、前記上室性不整脈発現を含み、前記制御装置は、前記神経刺激装置を制御して、該上室性不整脈発現に対する治療的神経刺激を送達するように適合されている、請求項1に記載のデバイス。
- 前記神経刺激装置は、前記神経標的の近傍に経脈管的に配置され、該神経標的を経血管的に刺激するように適合されたリード線を含む、請求項1に記載のデバイス。
- 前記神経刺激装置は、鎖骨下静脈、無名静脈、または内頸静脈内に経脈管的に配置されるように適合されたリード線を含む、請求項6に記載のデバイス。
- 前記神経刺激装置は、前記神経標的に皮下的に供給されることにより、神経カフを介して、前記神経刺激を該神経標的に送達するように適合されたリード線を含む、請求項1に記載のデバイス。
- 前記神経刺激装置は、前記神経標的における交感神経活性を刺激して、交感神経反応を誘導するように適合されている、請求項1に記載のデバイス。
- 前記神経刺激装置は、前記神経標的における副交感神経活性を抑制して、交感神経反応を誘導するように適合されている、請求項1に記載のデバイス。
- 前記神経標的は、星状神経節を含む、請求項1に記載のデバイス。
- 前記神経標的は、心臓交感神経を含む、請求項1に記載のデバイス。
- 前記神経標的は、迷走神経を含む、請求項1に記載のデバイス。
- 前記制御装置と協働して、抗頻脈ペーシング、抗頻脈ショック、または抗不整脈薬物を送達するように適合されたパルス発生器をさらに備える、請求項1に記載のデバイス。
