Description
1.クリップ600のフック601とを挿入具800のフック902に接続する。
2.挿入具の引き金805を引き、解除する。ストッパ機構が働き引き金が確実にロックされ、クリップ600が挿入具の管803の端部を閉成状態に装着される(図10参照)。

3.装着した挿入具を腹壁に位置づけたトロカールを通して腹部に挿入する(図10)。
4.牽引する対象の臓器に届いたら、引き金805を引いてクリップのフラップ607を開く(図12参照)。
[牽引]
5.咬合部606とフラップ607の間の臓器1602を把持して、引き金802を解除して臓器の上でフラップを閉じる。
6.一旦、ストッパ802を押して挿入具800の端部からクリップ600を開成する。クリップと挿入具のフック601、902はそのまま係止状態にある(図9参照)。
7.クリップ600を回転して、フック601を措置対象の内腹壁1601の所定箇所に位置づける。フック601で腹部組織1601を貫通して臓器1602を牽引する(図13参照)。
[挿入具除去]
8.一旦、ストッパ802を押して挿入具のフック902を解除し、挿入具800を回転させてフック902をクリップのフック601から外し、これらを別々に側方に摺動させる。
9.クリップを残したまま、トロカールを通して挿入具を抜くことができる。
[牽引位置の変更または除去]
10.解放された挿入具のストッパ802を1、2回押してフック902を伸長させる。
11.解放された挿入具800をトロカールを通して腹腔に挿入する。
12.側部から接近して挿入具のフック902とクリップのフック601を係止し、ストッパ802が止まるまで引き金805を引き、フック601と902を係着させる(図9参照)。
13.挿入具800を上方に動かして、クリップのフック601を腹壁1601の組織から外す。
14.次の選択肢がある:
(a)クリップ取り外し: 引き金805が止まり、フラップ607が開くまで引き金を引き、組織1602をフラップ607から解放し、引き金805を離してフラップを閉じ、クリップ600を挿入具800に装着し、トロカールを通して取り外す。
(b)腹壁上でのクリップの位置変更: 段階7〜9に従う。
(c)別の臓器または他の位置での牽引の反復措置: 段階4〜9に従う。
Claims (8)
- 主縦軸によって特徴づけられると共に固定咬合部(406、606)及び基端部を有する本体(402、602)と、少なくとも1つの軸(408、605)とを備えた外科手術に用いるクリップであって、
前記軸(408、605)は前記本体(402、602)の前記主縦軸方向に沿って往復動するように構成されており、前記軸(408、605)は少なくとも1つのスリーブ状に囲繞する圧縮バネ(403、603)によって少なくとも一部が囲繞されており、
前記基端部は前記軸(408、605)に連結した少なくとも1つの係止フック(401、601)を備えており、
前記固定咬合部(406、606)は前記軸(408、605)の末端部と機械的に連動する少なくとも1つの可動咬合部(407、607)を備えていると共に、体腔、内臓、組織、血管またはこれらの組み合わせにおける内壁の群から選ばれた少なくとも1つを可逆的に把持するように構成されており、
前記可動咬合部(407、607)は少なくとも1つの開成状態と少なくとも1つの閉成状態とによって特徴づけられており、前記開成状態から前記閉成状態または前記閉成状態から前記開成状態に少なくとも一部が可逆的に遷移するように構成されており、
前記状態遷移は、(i)前記軸(408、605)が前記本体(402、602)に対して線形に進退運動され、且つ、(ii)前記圧縮バネ(403、603)が圧縮または解放されて前記少なくとも1つの可動咬合部(407、607)が再設定されるように、前記係止フック(401、601)を前記クリップの前記主縦軸方向に沿って往復線形運動させることによって実行されることを特徴とする外科手術に用いるクリップ。 - 前記スリーブ状に囲繞する圧縮バネ(403、603)は、前記可動咬合部が初期状態で前記閉成状態に拘束されるべく前記スリーブ状に囲繞する圧縮バネ(403、603)から前記可動咬合部に圧力を印加されるように、前記少なくとも1つの可動咬合部(407、607)の少なくとも一部を囲繞することを特徴とする請求項1に記載のクリップ。
- 前記クリップの前記係止フック(401、601)が、前記体腔において挿入具または他の手術器具に係合するように構成されたフックまたはループ係合手段であることを特徴とする請求項1に記載のクリップ。
- 前記フックまたはループ係合手段は前記体腔の前記内壁に固定するように構成されていることを特徴とする請求項3に記載のクリップ。
- 前記外科手術中における低侵襲手術装置の固定、内臓の牽引、前記体腔内における組織の牽引、または血管の一時保持からなる群から選択される少なくとも処置を行うために用いられることを特徴とする請求項1に記載のクリップ。
- 前記状態遷移が前記少なくとも1つの可動咬合部(407、607)の塑性変形によらずに実行されることを特徴とする請求項1に記載のクリップ。
- 前記体腔の前記内壁に少なくとも部分的に可逆的に取り付けるように構成されており、吸引カップである吸引手段と、体腔内部、外部もしくは両方に位置づけられる磁石または電磁石から選択された磁気手段と、鉤部材、固定ワイヤーまたは自留クランプから選択された機械手段と、圧着ゲル剤である接着手段と、またはこれらを組み合わせた手段とから選ばれる固定装置を更に備えたことを特徴とする請求項1に記載のクリップ。
- 前記作動手段(401、601)が前記体腔の前記内壁に少なくとも部分的に可逆的に取り付けるように構成されていることを特徴とする請求項1に記載のクリップ。
