本発明は半月板断裂を修復する分野に関する。
対象の関節(例えば膝)における半月板断裂はよく起こる損傷である。以前は、断裂した半月板が、しばしば部分的にまたは完全に切除されていた。近年に至って、半月板断裂を修復するための技法、例えば関節鏡を使って設置されるタック(tack)の使用または断裂端の縫合などが開発された。
本発明によれば、対象の半月板断裂を修復する方法は、コラーゲン膜材料のシートを用意することを含み、シートは、その一側に、その上での細胞接着を阻害しかつそこを通る細胞の通過を阻害する平滑障壁面を持ち、シートは平滑障壁面の反対側に線維面を持ち、線維面はその上での細胞成長を許容し、前記シートのコラーゲンは主としてI型コラーゲンである。コラーゲン膜材料のシートは、線維面が半月板断裂の方を向くように、半月板断裂上に固定される。
断裂した半月板の修復過程の一因となる細胞は隣接する滑膜組織に由来する。滑膜細胞は、断裂した半月板における修復過程の一因となるだけでなく、結合組織を分解する能力および収縮する能力も持っている。例えば滑膜組織は典型的なI型コラーゲン足場を分解することができる。
重量%を超えるか、80重量%を超えるか、または90重量%を超えることができる。1実施形態によれば、本発明に従って利用される膜シートのコラーゲンは、約95重量%のI型コラーゲンである。そのような膜のコラーゲンは約5重量%のIII型コラーゲンを含んでよい。
実施例1
半月板断裂の処置を目的とするCHONDROGIDE(登録商標)膜の応用:インビトロ実験
ChondroGide(登録商標)膜は、断裂した半月板に、その修復を促進する目的で適用することができる。修復過程の一因となる細胞は隣接する滑膜組織に由来する。これらの滑膜細胞は、断裂した半月板における修復過程の一因となるだけでなく、結合組織を分解する能力および収縮する能力も持っている。CondroGide(登録商標)膜は、(1)滑膜細胞がその上を移動することができる足場として役立つことによって、滑膜細胞を半月板中の断裂に誘導し、(2)修復過程中にそれらの細胞を欠損部に含むだろう。ChondroGide(登録商標)膜は、滑膜組織と接触してもその完全性を維持することができ、滑膜細胞が移入することができる足場として役立つこともできる。滑膜組織はウシI型コラーゲン足場を分解することができるが、ChondroGide(登録商標)を分解せず、ChondroGide(登録商標)膜は、滑膜の収縮にもかかわらず、そのサイズおよび形状を保つことを、インビトロデータは実証している。さらにまた、滑膜由来の細胞はChondroGide(登録商標)中に移入することができる。
Claims (18)
- 対象の半月板断裂を修復するためのコラーゲン膜材料のシートであって、
前記シートは、滑膜組織と接触してもその完全性を維持することができ、滑膜細胞が移入することができる足場として役立つことができ、
前記シートは、自身の一側に、その上での細胞接着を阻害し、かつ、そこを通る細胞の通過を阻害する平滑障壁面を有し、
前記シートは前記平滑障壁面の反対側に線維面を有し、
前記線維面はその上での細胞成長を許容し、
前記コラーゲンは主としてI型コラーゲンであり、

- 修復は、前記コラーゲン膜材料を前記半月板断裂の上に、縫合糸、生理学的に許容できる接着剤またはそれらの組み合わせを使って固定することによって行われる、請求項1に記載のコラーゲン膜材料のシート。
- 前記接着剤はフィブリン糊である、請求項4に記載のコラーゲン膜材料のシート。
- 前記コラーゲン膜材料は60重量%を超えるI型コラーゲンを含む、請求項1に記載のコラーゲン膜材料のシート。
- 前記コラーゲン膜材料は70重量%を超えるI型コラーゲンを含む、請求項1に記載のコラーゲン膜材料のシート。
- 前記コラーゲン膜材料は80重量%を超えるI型コラーゲンを含む、請求項1に記載のコラーゲン膜材料のシート。
- 前記コラーゲン膜材料は90重量%を超えるI型コラーゲンを含む、請求項1に記載のコラーゲン膜材料のシート。
- 前記コラーゲン膜材料はブタ材料またはウシ材料である、請求項1に記載のコラーゲン膜材料のシート。
- 前記コラーゲン膜材料は腹膜に由来する、請求項10に記載のコラーゲン膜材料のシート。
- 前記腹膜はブタの腹膜である、請求項11に記載のコラーゲン膜材料のシート。
- 前記シートの前記コラーゲンは95重量%のI型コラーゲンを含む、請求項12に記載のコラーゲン膜材料のシート。
- 前記シートの前記コラーゲンは5重量%のIII型コラーゲンを含む、請求項13に記載のコラーゲン膜材料のシート。
- 前記シートは0.1〜1mmの範囲内の乾燥厚を有する、請求項1に記載のコラーゲン膜材料のシート。
- 前記平滑障壁面、前記線維面、または両者に、少なくとも1つの軟骨細胞、線維芽細胞様細胞、間葉系幹細胞、少なくとも1つのグリコサミノグリカン、少なくとも1つの成長因子またはその混合物を含浸させる、請求項1に記載のコラーゲン膜材料のシート。
- 前記グリコサミノグリカンはヒアルロン酸、コンドロイチン6−硫酸、ケラチン硫酸またはデルマタン硫酸である、請求項16に記載のコラーゲン膜材料のシート。
- 前記少なくとも1つの成長因子はトランスフォーミング成長因子ベータ、ヒト血小板由来成長因子、肝細胞成長因子、骨形成タンパク質−2、インスリン様成長因子−1、上皮成長因子、骨原性タンパク質−1、またはそれらの混合物である、請求項16に記載のコラーゲン膜材料のシート。
