Description
本出願は、米国仮特許出願第60/782,911号(2006年3月16日出願)、および米国仮特許出願第60/852,932号(2006年10月19日出願)からの優先権を主張し、該仮出願の両方は、その全体が明確に参考として援用される。
本発明は、概して、脱出症治療分野のシステムおよび方法に関する。より具体的には、本発明は、複数の構成部品を有する装置、および同装置を使用して組織壁脱出症を外科的に矯正するための方法に関する。特に、本発明の一実施形態は、予めカットされた形状のメッシュグラフトおよびグラフト送達デバイスを有するキットである。
簡略的に上述された構成部品を使用して、設置方法は、次のステップを含む。膣壁に切開口を設ける。膣内および骨盤底へのアクセスを得るために切開口を開く。グラフト設置デバイスなどの縫合糸捕捉デバイスを手に取る。グラフト設置デバイスなどの縫合糸捕捉デバイスに、弾丸針を備えたメッシュウイングを取り付ける。切開口を通して膣内にウイング、針および縫合糸デバイスを挿入する。靭帯または筋肉を通してウイングおよび弾丸針を押入する。縫合糸デバイスを用いて切開口からウイングを引き出す。縫合糸デバイスから弾丸針およびウイングを取り外す。縫合糸デバイスに別のウイングとその弾丸針とを取り付け、膣内の別の位置でそのプロセスを反復する。全てのウイングが一般的に容認されている尖端および側部の支持構造のような一部の内部構造に取り付けられるまで、他のウイングおよび弾丸針を用いてプロセスを反復する。これらの支持構造は、一般的に仙棘靭帯、近位腱弓、および肛門挙筋である。このウイング固定は、患者ごとの個別の調節を可能にするが、これは従来の縫合固定では行われないことである。グラフト調節本体が膣の内部頂部壁を覆うように、次にウイングが必要に応じて締められる。過剰のメッシュウイング材料が次いで切除され、破棄される。残りのメッシュが固定された後に、膣切開口が閉口される。
Claims (28)
- 膣壁に切開口を設けるための手段と、
中央本体部分とそこから延在する少なくとも1つのウイングを有するグラフトであって、該少なくとも1つのウイングがメッシュ材料から形成され、該中央本体部分が長手軸を規定し、該少なくとも1つのウイングが該中央本体部分から該長手軸に対して鋭角で延在し、該少なくとも1つのウイングは、定着組織に固定可能であり、かつ端部を有する、グラフトと、
該端部に取り付けられる弾丸針であって、該グラフトが、該切開口を通して挿入されるように構成されている、弾丸針と、
該ウイングおよび弾丸針を体内組織を通して押入するためのグラフト送達デバイスであって、該弾丸針が該体内組織を通って留置され、そして該体内組織を通って留置された後、該送達デバイスのキャッチ部分内に固定されるように構成されている、グラフト送達デバイスと、を備える、骨盤症状を治療するためのシステム。 - 前記グラフト送達デバイスが、切開口から前記ウイングを引き出すようにさらに構成されている、請求項1に記載のシステム。
- 前記グラフト送達デバイスから、前記弾丸針とウイングとを取り外すための手段をさらに備える、請求項1に記載のシステム。
- 前記切開口を縫合して閉じるための手段をさらに備える、請求項1に記載のシステム。
- 前記切開口を設けるための手段は、筋肉および筋肉組織を後方に引くことにより膣傍組織を切開するように構成されている、請求項1に記載のシステム。
- グラフト挿入デバイスをさらに備え、前記弾丸針の1つが該グラフト挿入デバイスに装填され、その結果として、前記グラフトの残部が該グラフト挿入デバイスから垂下し、該弾丸針の該1つが前記ウイングの1つの終端に取り付けられる、請求項1に記載のシステム。
- 前記グラフト送達デバイスが、一般的に受け入れられているあらゆる尖端および側方の支持構造を通して該ウイングと弾丸針とを押入するようにさらに構成されている、請求項1に記載のシステム。
- 過剰のウイング材料を切除するための手段をさらに備える、請求項1に記載のシステム。
- 前記グラフトを引き、そのそれぞれの区画において該グラフトを実質的に平坦に配置させることによって調節されるように、該グラフトがさらに構成されている、請求項1に記載のシステム。
- 前記グラフトを少なくとも2つの分離したピースに切断するための手段をさらに備える、請求項1に記載のシステム。
- 中央本体部分とそこから延在する少なくとも1つのウイングとを有するグラフトであって、該少なくとも1つのウイングがメッシュ材料から形成され、該中央本体部分が長手軸を規定し、該少なくとも1つのウイングが該中央本体部分から該長手軸に対して鋭角で延在し、該少なくとも1つのウイングは、定着組織に固定可能であり、かつ端部を有する、グラフトと、
該端部に取り付けられる弾丸針であって、該定着組織を通って留置され、そして該定着組織を通って留置された後、送達デバイスのキャッチ部分内に固定されるように構成されている弾丸針と、を備える、骨盤症状の治療のための装置。 - 前記グラフトは、ポリプロピレンメッシュから作製される、請求項11に記載の装置。
- 前記グラフトは、患者の膣円蓋全体を覆うサイズおよび形状を有する、請求項11に記載の装置。
- 前記少なくとも1つのウイングは、患者の仙棘靭帯に対して固定可能なように構成される、請求項11に記載の装置。
- 前記少なくとも1つのウイングは、患者の腱弓に対して固定可能なように構成される、請求項11に記載の装置。
- 前記少なくとも1つのウイングはある材料で形成され、前記弾丸針は該少なくとも1つのウイングの材料から形成される、請求項11に記載の装置。
- 前記グラフトは、不織重合体繊維によって囲まれた複数の開放気孔を有するメッシュを備え、前記中央本体部分の大部分の開放気孔は、前記少なくとも1つのウイングの大部分の開放気孔よりも大きい、請求項11に記載の装置。
- 前記グラフトはある材料から作製され;そして前記弾丸針は該材料から形成され、そして該グラフトの一体部分である、請求項11に記載の装置。
- 前記ウイングは先細り形状であり、前記少なくとも1つのウイングの幅は該少なくとも1つのウイングの中央本体部分から該少なくとも1つのウイングの端部まで単調に減少する、請求項11に記載の装置。
- 請求項11に記載の装置であって:
前記中央本体部分は幅を有し;そして
前記少なくとも1つのウイングは、該中央本体部分の幅よりも狭い幅を有する、装置。 - 請求項11に記載の装置であって:

前記グラフトは前記中央本体部分から延在する第2ウイングを有し、該ウイングは定着組織に固定可能であり、
該グラフトは該中央本体部分から延びる第3ウイングを有し、該第3ウイングは定着組織に固定可能であり、そして
該グラフトは該中央本体部分から延びる第4ウイングを有し、該第4ウイングは定着組織に固定可能であり、
該装置がさらに、
該第2ウイングに連結される第2弾丸針であって、定着組織を通って留置され、そして患者の身体内で送達デバイスのキャッチ部分内に固定されるよう構成される第2弾丸針と、
該第3ウイングに連結される第3弾丸針であって、定着組織を通って留置され、そして患者の身体内で送達デバイスのキャッチ部分内に固定されるよう構成される第3弾丸針と、
該第4ウイングに連結される第4弾丸針であって、定着組織を通って留置され、そして患者の身体内で送達デバイスのキャッチ部分内に固定されるよう構成される第4弾丸針とを備える、装置。 - 前記中央本体部分および前記少なくとも1つのウイングは、メッシュ材料から形成される、請求項11に記載の装置。
- 前記少なくとも1つのウイングと前記弾丸針との間に延在する縫合糸をさらに備える、請求項11に記載の装置。
- 請求項11に記載の装置であって、前記少なくとも1つのウイングは第1ウイングであり、前記装置がさらに、

- 請求項11に記載の装置であって、前記中央本体部分が第1部分および第2部分を含み、該第1部分の少なくとも1つが患者の前膣壁に隣接して配置されるように構成され、そして該第2部分が患者の後膣壁に隣接して配置されるよう構成される装置。
- 請求項1に記載のシステムであって、前記ウイングが先細り形状であり、前記少なくとも1つのウイングの幅が該少なくとも1つのウイングの中央本体部分から該少なくとも1つのウイングの端部まで単調に減少するシステム。
