Description

Claims (21)
- 動静脈(AV)瘻孔を形成するための装置であって、
長尺状部材と、
前記長尺状部材に対して移動可能に前記長尺状部材に接続され、テーパ遠位端を有する遠位部材と、
前記遠位部材を前記長尺状部材に連結するためのシャフトと、
前記シャフトが前記長尺状部材に対して前記遠位部材を延長及び後退させるために延長及び後退可能であることと、
前記長尺状部材及び前記移動可能な遠位部材のうち一つの平滑テーパ面上に配置される能動的要素と、
前記長尺状部材及び前記移動可能な遠位部材のうちの他の平滑テーパ面上に配置された受動要素と、
前記能動的要素及び前記受動要素のうちの一つの外周において周方向に配置された、物理的に分離された複数の突起要素と、を含み、
前記能動的要素は、熱エネルギを組織に印加するために、能動的に通電され、前記受動要素は、前記能動的要素から前記受動要素への癒合組織境界面を通じた熱伝達を促進するために受動的非通電熱伝導性表面であって、
前記能動的要素及び前記受動要素は、前記瘻孔を形成するために前記組織を切断するように構成されている装置。 - 請求項1に記載の装置において、
前記長尺状部材は、長尺状外管を含む装置。 - 請求項1に記載の装置において、
前記シャフト及び前記遠位部材の一つは、可撓性材料で形成される装置。 - 請求項1に記載の装置において、
前記近位の長尺状部材上の前記平滑テーパ面は、遠位テーパ面を含み、
前記遠位部材上の前記平滑テーパ面は、近位テーパ面を含み、
さらに、前記近位テーパ面及び前記遠位テーパ面は互いに整合する装置。 - 請求項4に記載の装置において、
前記能動的要素は、前記近位テーパ面上に配置される装置。 - 請求項5に記載の装置において、
前記受動要素は、前記遠位テーパ面上に配置される装置。 - 請求項1に記載の装置において、
前記能動的要素はDC抵抗エネルギによって通電される装置。 - 請求項1に記載の装置において、
前記受動要素は、受動的な熱伝導性表面を含む装置。 - 請求項1に記載の装置において、
前記能動的要素は、楕円形の形状を有する装置。 - 請求項1に記載の装置において、
前記遠位部材はテーパ状で柔軟である装置。 - 請求項4に記載の装置において、
組織接着を最小限にするための前記近位及び遠位テーパ面のそれぞれの上に配置されるコーティングを含む装置。 - 請求項11に記載の装置において、
前記コーティングはPTFEを含む装置。 - 請求項4に記載の装置において、
前記近位及び遠位テーパ面のそれぞれは、25〜100マイクロインチ(0.6〜2.5マイクロメートル)の滑らかな表面仕上げを有するように構成されている装置。 - 請求項1に記載の装置において、
前記遠位部材は、前記装置の長さ方向に沿う軸の周りを前記長尺状部材に対して回転可能である装置。 - 請求項1に記載の装置において、
さらに、前記能動的要素により生成された熱を拡散し、前記能動的要素から外側に放射される温度勾配を形成するために、前記能動的要素及び前記受動要素の少なくとも一つより上、下又は内に配置された伝導材料を含む装置。 - 請求項3に記載の装置において、
前記シャフトは可撓性材料で形成される装置。 - 請求項1に記載の装置において、前記長尺状部材及び前記遠位部材の少なくとも一つは、複数の前記突起要素によって保持された組織を受容するためのキャビティを含む装置。
- 請求項17に記載の装置において、前記キャビティは、前記受動要素及び前記能動的要素のうちの一つ内に配置される装置。
- 請求項1に記載の装置において、複数の前記突起要素は、前記受動要素及び前記能動的要素のうちの一つの外周においてリングを形成する装置。
- 請求項19に記載の装置において、複数の前記突起要素はいずれも、前記受動要素及び前記能動的要素の一部を形成しない装置。
- 請求項1に記載の装置において、前記受動要素及び前記能動的要素はそれぞれ平面を定義しており、複数の前記突起要素は、複数の前記突起要素が配置された前記受動要素及び前記能動的要素のうちの一つの前記平面を越えて延出する装置。
