Description
本特許出願は、参照により本明細書に組み込まれている、2014年8月14日に出願したインド仮出願第3988/CHE/2014号、名称「DEVICE AND METHOD FOR REMOVAL OF SECRETIONS TO PREVENT VENTILATORASSOCIATED PNEUMONIA」の優先権を主張するものである。
本明細書で言及されているすべての刊行物および特許出願は、それぞれの個別の刊行物または特許出願が、本明細書に組み込まれることが特に、また個別に指示されている場合と同じ範囲にわたってそっくりそのまま本明細書に組み込まれる。
一般に、流体管理システムは、流体管路、センサー、コントローラおよびコントローラ用回路、および洗浄コンポーネントを備え得る。コントローラは、典型的には、流体管理システムコンポーネントの動作を監視する流体管理システムの一部分である。コントローラは、気管チューブに沿った領域から流体を吸引して除去する場合には、流体取り出し、または気管チューブが挿入されている口腔または呼吸管の特定の領域の洗浄の場合には、流体送達を調節するための回路およびマイクロコントロールを含むことができる。コントローラは、呼吸挿入デバイスを流体管理システムの感知、吸引、およびポンプ動作コンポーネントに連結する流体管路を接続し、維持する弁を含み得る。使用時に、コントローラは、感知、吸引、およびポンプ動作サイクルを調整するための回路を含むマイクロコントロールも備え得る。コントローラは、定期的に、自動的に、および独立して、圧力、吸引、または感知を各流体管路に施す。
述べたように、上で説明されている流体管理システムは、呼吸挿入デバイスに結合され得る。図17〜図30は、流体管理システムに接続されている呼吸挿入デバイスさらには企図されている呼吸挿入デバイスの異なる実施形態を説明している。
次の段落では、流体管理システムを呼吸挿入デバイスとともに使用する方法について説明する。使用者は、複数の開口部を有する呼吸挿入デバイスを被験者の呼吸管内に挿入する作業を、第1の開口部が口腔(たとえば、被験者の舌根の近く)に位置決めされ、第2の開口部が被験者の中咽頭に位置決めされ、第3の開口部が被験者の声門下部に位置決めされるように行うことができる。ある場合には、呼吸挿入本体部は、既存の気管チューブに取り付けられ、他の場合には、気管チューブを組み込んだ呼吸挿入本体部は、患者の気管に新たに挿入される。

Claims (11)
- 呼吸器の複数の領域から流体を自動的に取り出し、前記呼吸器の口腔部分を洗浄するためのシステムであって、
複数の吸引弁を備えるコントローラであって、前記複数の吸引弁は、吸引機構に流体的に連通している、コントローラと、
複数の流体管路であって、各前記流体管路は、気管チューブに沿って配置された複数のポートのうちの1つのポートに関連付けられ、前記気管チューブを前記呼吸管に挿入すると、前記呼吸管の複数の領域からの流体を前記複数の流体管路を通して除去するために、前記複数のポートは前記呼吸管の複数の領域と接触するようになっており、各前記流体管路は、前記複数の吸引弁のうちの1つの吸引弁と連通し、前記コントローラは、前記複数の吸引弁を制御して、前記複数の流体管路のうちの各流体管路を通して前記吸引機構から陰圧を印加するように構成される、複数の流体管路と、
複数の流量センサーおよび圧力センサーであって、前記流体管路は各々、前記流体管路内の流体流量および圧力を前記コントローラに報告するように構成されている流量センサーおよび圧力センサーに結合されている、複数の流量センサーおよび圧力センサーと、
洗浄液体源およびポンプであって、前記コントローラは、前記呼吸器の口腔部分を洗浄するために、前記ポンプを用いて、前記洗浄液体源から、前記複数の流体管路以外の洗浄送達流体管路へ洗浄液体を送達するように構成されている、洗浄液体源およびポンプと、
それぞれが複数の前記流体管路の1つの流体管路に結合され、前記流体管路から流体を回収するように構成されている複数の回収容器とを備え、
前記コントローラは、
前記吸引機構を制御して、定期的に、自動的に、および独立して陰圧を前記流体管路の各々に印加し、前記流体管路内の流体の流れがなくなるまで陰圧を印加し続け、前記流体管路内の前記流量センサーが、流体が存在しないことを示す場合に、前記複数の流体管路の各流体管路内で陰圧を印加するのを停止するように構成され、かつ、
前記流体管路における流体流量および圧力に基づいて、前記流体管路内で蓄積した流体による閉塞を検出するように構成され、
さらに、前記コントローラは、前記流体管路の閉塞を除去するために、前記吸引機構を制御して、陽圧を次いで陰圧を印加するよう構成されるシステム。 - 前記洗浄液体が口腔に送達される頻度である洗浄送達頻度、前記口腔の洗浄が実行される持続時間である洗浄持続時間、前記洗浄液体が送達される圧力である洗浄圧力、吸引印加頻度、および吸引圧力のうちの1つまたは複数を含むユーザによって選択された制御情報を受け取るように構成されている入力部をさらに備え、前記ユーザによって選択された制御情報に基づいて、前記コントローラは、前記ポンプを用いて洗浄液体を送達し、流体を除去するために前記吸引機構から陰圧を印加するよう構成されている、請求項1に記載のシステム。
- 前記コントローラは、ディスプレイをさらに備え、前記コントローラは、前記流体管路内の前記流量センサーによって検出された流体の流量を含むデータを第1、第2、および第3の流体管路のうちの1つまたは複数について表示するように構成される、請求項1に記載のシステム。
- 前記コントローラは、前記ポンプを用いて陽圧を印加して前記洗浄送達流体管路を通して前記洗浄液体を送達し、前記複数の流体管路のうちの1つまたは複数の流体管路に陰圧を印加して前記洗浄液体を取り除くように構成される、請求項1に記載のシステム。
- 前記複数の流体管路は、第1、第2、および第3の流体管路を備え、前記コントローラは、前記第1、第2、および第3の流体管路の各々を通して陰圧を独立して印加するように構成される、請求項1に記載のシステム。
- 前記複数の流体管路は、第1、第2、第3、および第4の流体管路を備え、前記コントローラは、前記第1、第2、第3、および第4の流体管路の各々を通して陰圧を独立して印加するように構成される、請求項1に記載のシステム。
- 前記洗浄送達流体管路は、前記洗浄液体源に接続されている、請求項1に記載のシステム。
- 前記洗浄液体源は、洗浄液体を保持するように構成されている貯蔵容器を備える、請求項1に記載のシステム。
- 前記複数の流量センサーは、前記複数の流体管路の外側に配置された非接触型センサーである、請求項1に記載のシステム。
- 前記コントローラは、閉塞を除去するために、閉塞が検出された流体管路を通して洗浄液体を送達するために前記ポンプを用いて陽圧を印加するように構成される、請求項1に記載のシステム。
- 前記回収容器は、前記複数の吸引弁と前記吸引機構との間で前記流体管路に接続される、請求項1に記載のシステム。
