Description

【発明の詳細な説明】[Detailed description of the invention]

本発明は、血液、血漿、その他の体液より、抗
DNA抗体、抗ENA抗体、抗核抗体などの自己流
抗体および免疫複合体を除去する吸着材に関す
る。さらに詳しくは、全身性紅斑性ループス(シ
ステミツク・ループス・エリテマトーデス;以下
SLEと略記する)をはじめとする膠原病患者の体
液中に存在するこれらの自己抗体、免疫複合体を
特異的に吸着除去することにより、体液を浄化再
生し、疾患の進行を防止すると共に、症状を軽減
せしめ、さらに治癒を早める簡便な吸着材を提供
することを目的とする。 SLEは若年者とくに若い女性に多く、全身臓器
なかでも血管系、結合織がおかされるという難治
性疾患であり、自己免疫病変群の代表的疾患であ
る。その原因については不明であつたが、近年、
SLE患者血液中の抗核抗体等の存在が、各種の症
状の発生に重要な関連性があると考えられ、ま
た、血管炎や腎病変の病因としては、抗原−抗体
免疫複合体が組織に沈着するというメカニズムが
重視されてきている。最近、患者血漿中の抗原、
抗体、免疫複合体といつた悪性物質を除去する上
から、患者血漿と他の新鮮凍結血漿ないしは、ア
ルブミン製剤と入れ換える血漿交換療法が施行さ
れ、かなりの進行防止、症状軽減あるいは治癒効
果が確認されるようになつてきた。 しかしながら、この血漿交換療法には、(1)除去
した血漿を補充するための新鮮凍結血漿ないしは
血漿成分の大量かつ持続的な入手が困難なこと、
(2)他人の血漿を利用するため、肝炎ウイルス等の
感染の危険が高いといつた欠点があり、一般に普
及できるものではない。 また、患者血漿中の悪性物質の除去には、限外
過膜を用いる方法があり、血漿成分の補給を要
しないという長所を有しながらも、(1)分子量によ
り、きれいに分離することができないこと、(2)除
去に分子量以外の選択性がないため、血漿中の有
用な物質も除去してしまうこと、(3)膜の目づまり
による過速度の低下、カツトオフ分子量の変動
などの問題点を有している。 さらに、担体にDNA、合成核酸ポリマー等の
生体(類似)高分子を固定し、自己抗体を除去す
る方法も提案されている。しかし、この方法は、
(1)固定化物質が高価であること、(2)抗原である固
定化物質の遊離が起こり、患者体内への流入の恐
れがあることから、実用上、治癒に用いるには難
点がある。また、特開昭53−136380号公報や特開
昭55−38141号公報にも、免疫複合体や抗核酸抗
体の吸着材が提案されているが、いずれも実用
上、満足のいくものとはいえない。 本発明者らは、これらの問題点を克服すべく、
簡便かつ安全で、患者血漿中から悪性物質のみを
特異的に除去するための吸着材を鋭意研究した。
その結果、従来、生体(類似)高分子において吸
着能を有するものが知られていたのに対し、驚く
べきことに、核酸塩基を構成する低分子物質にお
いても、高分子と同等以上の吸着能を有すること
を見い出し、先に特許出願した。 本発明者らは、さらに詳細に研究した結果、驚
くべきことには、糖のモノまたはジ正リン酸エス
テル、モノまたはジピロリン酸エステル、トリフ
オスフエート、およびこれらの重合度10以下のホ
モまたはヘテロ重合体、さらに重合度10以下のホ
モまたはヘテロオリゴ糖に正リン酸、ピロリン
酸、トリフオスフエート基が結合したものにおい
ても、高分子量と同等以上の吸着能を有すること
を見い出し、本発明を完成するに至つた。 これにより、固定化物質の脱離流出の不安は解
消され、実際の臨床応用上の困難を打破すること
ができた。すなわち、本発明は、糖のモノまたは
ジ正リン酸エステル、モノまたはジピロリン酸エ
ステル、トリフオスフエート、およびこれらの重
合度10以下のホモまたはヘテロ重合体、さらに重
合度10以下のホモまたはヘテロオリゴ糖に正リン
酸、ピロリン酸、トリフオスフエート基が結合し
たものから選ばれた少なくとも1種が不溶性担体
に結合している抗ネイテイブDNA抗体(抗n−
DNA抗体)、抗ダブルストランドDNA抗体(抗
ds−DNA抗体)、抗核抗体等の自己抗体および免
疫複合体の吸着材である。 本発明の吸着材の除去対象とする疾患は、抗n
−DNA抗体、抗ds−DNA抗体、抗核抗体および
その免疫複合体の出現するものであれば、いずれ
にも適用できる。具体的には、SLE、混合性結合
織病(MCTD)、進行性全身性硬化症、シエーグ
レン症候群、皮膚筋炎(多発性筋炎)、ケイ肺症、
慢性関筋リウマチ、橋本病、慢性肝炎、糖尿病、
気官支拡張症などがある。 抗原−抗体反応を利用し、抗原を担体に固定化
して、抗体を除去しようとする試みが知られてお
り、抗DNA抗体等においても、DNAを担体に固
定して除去が可能である。しかし、すでに述べた
ように、実用には不適であつた。本発明により、
体内に万一はいつても安全な物質を用いて、
DNAと同等以上の吸着性能を有する吸着材が開
発され、これにより広範囲な臨床応用が可能にな
り、今後、自己免疫疾患に治療に大きな進歩をも
たらすものと思われる。 本発明において、不溶性担体に結合させる物質
は、エリトロース、トレオース等のテトロース
類、アラビノース、キシロース、リキソース、リ
ボース等のアルドペントース類、キシルロース、
ペンツロース、リグロース等のケトペントース
類、ガラクトース、グルコース、タロース、マン
ノース等のアルドヘキソース類、ソルボース、タ
ガトース、プシコース、フルクトース等のケトヘ
キソース類、N−アセチル−グルコサミン等ヘキ
ソサミン類、アルドヘプトース、ケトヘプトー
ス、ケトオクトース、ケトノノース等の多単糖
類、2−デオキシペントース、6−デオキシヘキ
ソース、2−デオキシヘキソース、2,6−ジデ
オキシヘキソース、3,6−ジデオキシヘキソー
ス等のデオキシ糖類、糖アルコール無水物類、ウ
ロン酸、ケトアルトン酸、アスコルビン酸等の酸
性糖類、糖メチルエーテル類、分岐糖類、アミノ
糖類、のモノまたはジ正リン酸エステル、モノま
たはジピロリン酸エステル、トリフオスフエー
ト、およびこれらの重合度10以下のホモまたはヘ
テロ重合体、さらに重合度10以下のホモまたはヘ
テロオリゴ糖に正リン酸、ピロリン酸、トリフオ
スフエート基が結合したものである。上記した糖
は、D、L体、スレオ、エリスロ体にかかわりな
く用いることができる。また、上記の不溶性担体
に結合させる物質のうち特に、リボース、デオキ
シリボース等のペントースのリン酸エステルが好
ましく、さらに、デオキシリボースの3,5ジ正
リン酸およびそのオリゴマーがより好ましい。 これらを単に一つだけ固定するのではなく、複
数の種類を担体に固定してもよい。 本発明に用いる担体は、水に不溶性のものであ
れば、いずれも用いることができるが、非特異的
吸着を避ける点で、親水性担体を用いるのが望ま
しい。不溶性担体の形状は、粒子状、繊維状、中
空糸状、膜状、ガーゼ状、不織布等いずれのもの
も用いることができるが、実施上の取り扱い、リ
ガンドの保持量、血漿との接触有効面積等の点
で、粒子状、繊維状のものが好ましい。 粒子状担体としては、平均粒径50μから2500μ
の範囲にあることが好ましい。平均粒径はJIS−
Z−8801に規定されるフルイを用いて流水中で分
級した後、各級の上限粒径と下限粒径の中間値を
各級の粒径とし、その重量平均として平均粒径を
算出する。また、粒子形状は球形が好ましいが、
特に限定されるものではない。平均粒径が2500μ
を超えると、悪性物質の吸着量および吸着速度が
低下し、50μ未満では、体外循環に用いると圧損
が大きく目づまりが起こりやすく、また凝固系の
活性化、血球粘着をおこしやすい。 使用できる粒子状担体としては、アガロース
系、架橋デキストラン系、セルロース系、架橋ポ
リアクリルアミド系、多孔性シリカビーズ、活性
炭等の担体を用いることができるが、アガロース
系、架橋デキストラン系、セルロース系等の多糖
類ゲルが良い結果を与える。また、通常、固定化
酵素、アフイニテイクロマトグラフイに用いられ
る公知の担体は、特別な限定なく使用できる。 また、本発明で用いられる粒子状担体として
は、多孔性合成重合体粒子を用いることができ
る。該粒子はその表面に、先に記した本発明に用
いる物質を固定できるものであり、平均孔径300
Åないし9000Å、より好ましくは1000Åないし
6000Åの範囲にあるものである。該粒子の重合体
組成は、ポリアミド系、ポリエステル系、ポリウ
レタン系、ビニル化合物の重合体を用いることが
できるが、特に親水性のビニル化合物系多孔性合
成体粒子が好ましい結果を与える。 該多孔性構造は平均孔径300Åないし9000Åの
範囲にあるものが好ましいが、平均孔径が小さす
ぎる場合には、吸着される自己抗体の量が少な
く、大きすぎる場合には、重合体粒子の強度が低
下し、かつ表面積が減少するため実用的ではな
い。 平均孔径の測定は、水銀圧入式ポリシメーター
によつて行うことができる。この方法は多孔性物
質に水銀を圧入してゆき、浸入した水銀量から気
孔量を、圧入に要する圧力から孔径を求める方法
であり、40Å以上の孔を測定することができる。
平均孔径は、孔径をr、ポロシメーターで測定し
た累積気孔量をvとしたとき、dv/dlogrの値が
最大となるときのrの値とする。 繊維状担体を用いる場合は、その繊維径が0.02
デニールないし10デニール、より好ましくは0.1
デニールないし5デニールの範囲にあるものがよ
い。繊維径が大きすぎる場合には、自己抗体の吸
着量および吸着速度が低下し、小さすぎる場合に
は、凝固系の活性化、血球粘着、目づまりをおこ
しやすい。使用できる繊維状担体としては、再生
セルロース系繊維、ナイロン、アクリル、ポリエ
ステル等公知の親水性繊維である。 結合リガンドを親水性担体に固定化する方法
は、共有結合、イオン結合、物理的吸着法、架橋
法、包理あるいは担体表面への沈澱不溶化等あら
ゆる公知の方法を用いることができるが、結合リ
ガンドの溶出を防ぐ上で、共有結合によるものが
よい。 共有結合の方法としては、通常、固定化酵素、
アフイニテイーカラムの製造に用いられる方法が
とられる。担体を活性化する方法として、ハロゲ
ン化シアン、過ヨウ素酸、架橋試薬、エポキシド
等を用いて行なえる。担体とリガンドとの間に、
任意の長さの分子(スペーサー)を挿入して使用
することができる。 本発明で担体に結合させるリガンドの量は、担
体1g(乾燥重量)当り0.1μmolないしは
1000μmolの範囲であり、より好ましくは0.5μmol
ないしは100μmolの範囲である。保持量が
0.1μmol未満の場合は、悪性物質の吸着能の低下
が起こり、200μmolを超える場合は、血漿有用成
分の非特異吸着が起こり好ましくない。担体と結
合させるリガンドの部分は、水酸基、リン酸基、
アミノ基を用いることができる。 該吸着材の作用機構は、抗原抗体反応に近いも
のと考えられるが、本来の抗原である生体高分子
または類似した合成高分子を用いずに、万一体内
にはいつても何ら支障がなく、体内で代謝される
低分子によつて、充分な吸着除去能力を呈するこ
とは驚くべきことといえる。また、免疫複合体の
吸着には、複合体中の抗体の残余Fab部分とリガ
ンドとの相互作用が寄与しているものと考えられ
る。 本発明の吸着材による自己体吸着装置は、上記
説明の自己抗体吸着材を、流体の導出入口を有す
る容器内に充填し、体液が自己抗体吸着材と接触
しつつ通過するようにしたものである。これの具
体例を第1図と第2図に示す。第1図において、
円筒1の両端開口部に、内側にフイルター2を張
つたパツキング3を介して体液導出入口4を有す
るキヤツプ5をネジ6で嵌合し、両端のフイルタ
ーの間隙に吸着材7を充填保持させてなるもので
ある。 吸着材としては、本発明の前記吸着材を単独で
充填してもよく、他の吸着材と混合もしくは積層
してもよい。吸着材層の容積は、体外循環に用い
る場合、50〜400ml程度が適当である。 上記の装置を体外循環で用いる場合には、大略
次の二通りの方法がある。一つには、体内から取
り出した血液を遠心分離機もしくは膜型血漿分離
器を使用して、血漿成分と血球成分とに分離した
のち、血漿成分を上記の装置に通過させ、浄化し
た後、血球成分と合わせて体内にもどす方法であ
り、他の一つは体内から取り出した血液を直接上
記の装置に通過させ、浄化する方法である。 また、血液もしくは血漿の通過速度について
は、該吸着材の吸着能率が非常に高いため、吸着
材の粒度を粗くすることができ、また充填度を低
くできるので、吸着材層の形状の如何にかゝわら
ず、高い通過速度を与えることができる。そのた
め多量の体液処理をすることができる。 体液の通液方法としては、臨床上の必要に応
じ、あるいは設備の設置状況に応じて、連属的に
通液してもよいし、また断続的に通液使用しても
よい。 本発明の吸着材は、以上述べてきたように、体
液中の抗n−DNA抗体、抗ds−DNA抗体、抗核
抗体等の自己抗体および免疫複合体を高率かつ特
異的に吸着除去し、非常にコンパクトであると共
に簡便かつ安全である。また、滅菌操作も容易か
つ確実に実施できるという効果を併せもつてい
る。また、本発明の吸着材は、装置に充填して治
療器として用いられるにとどまらず、抗n−
DNA抗体、抗ds−DNA抗体、抗核抗体等の自己
抗体および免疫複合体の分離、精製用吸着材およ
びこれらの測定用基材としても極めて有効に利用
できる。 以下、実施例により、本発明の実施の態様をよ
り詳細に説明する。 実施例 1 CNBr活性化セフアローズ4B(スエーデン・フ
アルマシア社製、平均粒径260μ)に通常の方法
によつてデオキシリボース3,5−ジリン酸を結
合せしめ(“実験と応用アフイニテイクロマトグ
ラフイー”P48、1976講談社サイエンテイフイツ
ク刊)、通常の活性基をトリスハイドロオキシメ
チルアミノメタンでブロツキングした。保持量は
60μmol/gであつた。吸着材は生理食塩水で充
分に洗浄した後、脱水して実験に供した。 吸着実験は、患者血漿3容と吸着材1容を混合
し、37℃、3時間インキユベーシヨンにより行つ
た。また、対照として、未活性化セフアローズ
4Bを用いた。 抗ds−DNA抗体価はクリチデア ルシリア
(crithidia luciliae)を用いる螢光抗体間接法を
用いた。 抗核抗体価は、細胞を塗抹したスライドガラス
に段階希釈した検体(一次抗体)を滴下し、抗原
−抗体反応を行い、ペルオキシダーゼ標識抗ヒト
免疫グロブリン抗体(二次抗体)を滴下し、酵素
の呈色反応を光学顕微鏡で観察した。測定には、
「エンザイムANAテスト」〔(株)医学生物学研究所
製〕のキツトを用いた。陽性を示す最高希釈倍数
をもつて抗体価を表示した。 免疫複合体量の測定には、ポリエチレングリコ
ール(PEG)により免疫複合体を沈降させ、補
体溶血反応と組合わせる方法を用いた。この方法
の操作方法、条件は次のようにして行つた。 (1) 検体0.3mlを分離管に注ぐ。次に、
0.2NEDTA50μを加えて撹拌し、さらにほう
酸緩衝液を50μ加えて撹拌する。12.5%PEG
(分子量6000)を0.1ml加えて撹拌し、4℃、90
分静置する。 (2) 4℃、1700gで10分間遠心し、沈澱を2.5%
PEG1.0mlで洗う。1700gで15分間遠心し、上
清を捨てる。 (3) 37℃のGVB++(2価陽イオンを含むゼラチン
ベロナール緩衝液)30μを加え、沈澱を溶解
する。補体源としてプール健康人血清10μを
加える。37℃、30分間、免疫複合体と補体とを
反応させる。 (4) 1.5×108/mlEA(抗体感作赤血球)1.0mlを加
え、37℃、60分間、振とうさせて、残存補体に
よる溶血反応を促進させる。 (5) 反応後、4℃の生理食塩水6.5mlを加えて遠
心し、上清の吸光度(OD414)を測定する。 (6) 健康人血清を対照とし、これに対する溶血の
阻止率を算出する。単位をPEG−CC%ととす
る。 阻止率=(対照)−(検体吸光度)/(対照吸光度)
