Description

【発明の詳細な説明】 産業上の利用分野 本発明は、骨接合（ｏｓｔｅｏｓｙｎｔｈｅｓｉｓ）　
に用いられるインプラント、特にを柱接合具用インプラ
ントに関するものである。
上記のインプラントは、骨に固定（投錨）するための部
分と、ロッドを取りつけるための本体とで構成され、こ
の本体は２つの分岐部に分かれ且つ後部に向かって開い
た溝が形成されており、この溝は、さらに、上記ロッド
が収容できるように本体の両側に向かって開いている。
上記の骨に固定（投錨）するための部分は、例えば、適
当な形のネジまたはブレードで構成することができる。
従来の技術 公知のインプラントの上記本体は、閉じているか、後側
が開いているか、両側方が開いていた。
これらのインプラントは、以下の欠点がある。
（１）本体の形式として３つの異なる型があるために、
取付けに必要な部材の数が非常に多くなる。
（２）例えばフランス国特許第２．５４５．３５０号（
８３７０７，４５０）に記載のような、後側が開いた本
体の場合には、追加の取付は要素によって構成された溝
内固定具を使用する必要があり、この追加の取付は要素
を予めロッドに取り付けておかなければならない。
（３）本体が閉じているインプラントの場合には、ロッ
ドを差し込むのはかなり難しい。
（４）本体が開いたインプラントの場合には、正確に位
置決めしなければならず、ネジを使用する際に医師に不
便を強要する。
（５）使用する部材が多様で複雑であるため、かなりの
補助具を使用することが必要である。
（６）大きくて不便である。
（７）インプラント要素の製作は難しく、コストがかか
り、特に、外す場合には切断加工が必要なため、取外し
が困難である。
発明が解決しようとする課題 本発明の目的は、これらの欠点のないインプラントを提
供することにある。
課題を解決するための手段 本発明に従うインプラントは、本体の後端を構成してい
る２つに分かれた分岐部の内壁に形成された雌ネジにネ
ジ込まれるように構成されたプラグを具備し、このプラ
グがネジ込まれた側で上記の溝が閉じられ、２つの直径
方向反対側のるエツジでロッドと当接している。ロッド
の方を向いたプラグの面には、プラグをロッドに対して
並進運動および回転運動しないように係止・固着する手
段が設けられている。
上記分岐部の内壁に形成されたネジによって本体の分岐
部の甲にプラグがネジ込まれるので、インプラントは非
常に小さい寸法となる。
可能な実施例の１つでは、上記係止・固着手段は、プラ
グの他の部分と一体に形成された中心尖端を具備してい
る。
変形として、この係止・固着手段は、プラグの面から突
出して周囲に形成された外周リングで構成するか、また
は、この外周リングと中心尖端との組合せで構成するの
が好ましい。
本発明の他の特徴および利点は添付図面を参照した、以
下の説明により理解されるであろう。しかし、本発明は
これらに実施例に制限されるものではない。
実施例 第１図から第６図に示されたインプラントは、骨接合用
器具（ｏｓｔｅｏｓｙｎｔｈｅｓｉｓ　ｄｅｖｉｃｅ）
、特に脊椎骨接合用器具の構成の一部である。
このインプラントは、骨に固定（投錨）するための部分
（図示した例では湾曲ブレード１で構成されている−こ
れ自体は公知であるので説明は省略する）と、ロッド３
に取付けるための本体２とで構成されている。このロッ
ド３の表面は平滑であるか、ダイアモンド状の切り込み
（ローレフト）を付けた粗面になっている。
本体２は、ブレード１の終端部分から始まる２つの側方
分岐部４で構成され、この分岐部４は、円錐形部分５を
介してブレード１に結合されている。２つの分岐部分４
の間には溝６が形成されており、上記ロッド３はこの溝
６の丸い底部７に収容される。この溝６の丸い底部７と
反対の側は開いている。また、ロッド３を上記分岐部４
の間に収容するために、溝６は分岐部４の両側でも開い
ている。
上記インプラントはプラグ８を具備している。
このプラグ８には、上記２つの分岐部４の内壁に形成さ
れた雌ネジ１１と螺合できるようなネジ９が形成されて
いて、溝６のプラグ８側がこのプラグ８によって閉じる
ことができるようになっている。
また、このプラグ８には、プラグ８が完全に雌ネジ９に
ネジ込まれたときに、ロッド３に当接してロッド３中に
食い込む（第３図）直径方向両側の２つのエツジが設け
られている。
本発明の付加的な特徴によれば、プラグ８のロッド３側
を向いた面８ａには、プラグ８をロッド３に係止し且つ
固着させて並進運動と回転運動を阻止する手段が設けら
れている。
図示した実施例では、この係止手段が、プラグ８の他の
部分と一体に形成された中心尖端１２と、面８ａから突
出した外周リング１３とで構成されている。この外周リ
ング１３は、断面が三角形で、好ましくは頂点１３ａが
丸くなっており、プラグ８の他の部分と一体に形成され
ている。
外周リング１３および中心尖端１２と反対側のプラグ８
後部には一定の輪郭の穴１４が形成されていて、適当な
工具を用いて雌ネジ１１にプラグ８をネジ込むごとがで
きるようになっている。この穴１４は例えば六角形にす
ることができる。インプラントの本体の２つの側面すな
わち分岐部４の雌ネジ１１とプラグ８の雄ネジ９は、鋸
歯ピッチとすることができる。こうすることによって、
プラグ８をネジ込んだ場合の荷重の半径方向成分が完全
に除去され、２つの分岐部４が開かないようにすること
ができる。
プラグ８のロッド３の方を向いた面８ａには、粗面部分
１５（第６図参照）が設けられている。この粗面部分は
、斜線部分１５ａが、プラグ８の締まる方向に向いた鋸
歯状断面形状を有しているのが有利である。プラグ８は
、冷間加工、例えば冷間鍛造で製作できる。これらの加
工方法で作られたプラグ８は、切削加工で製作されたロ
ッド３よりも大きい硬度を有している。
固定ブレード１と、本体２と、プラグ８は生体適合性の
良い同じ材料、例えばフランス規格協会（ＡＦＮＯＲ）
規格ＡＳ　　Ｉ　　Ｚ２　　ＣＮＤ　１７．１３に相当
するアメリカ規格協会（ＡＮＳＩ＞規格３１６Ｌ鋼、で
作られている。
インプラントの組立は、上述のように非常に簡単である
。最初にロッド３を分岐部４の間から溝６中にロッド３
が底部７に当接するまで挿入する。
次に、プラグ８を雌ネジ１１に途中までネジ込む。
インプラントには収縮力、圧縮力、ねじれ応力が加わる
。次いで、穴１４に合った工具でプラグ８を締めてイン
プラントを固定する。最後に、プラグ８を中心尖端１２
と外周リング１３がロッド３に食い込むまで締め込んで
最終固定する。このように、プラグ８の中心尖端１２と
外周リング１３がロッド３に食い込むのは、プラグ８を
構成している材料の硬度が、ロッド３の材料の硬度より
も大きいことによる。この結果、尖端１２および外周リ
ング１３によって全体がゆるむのが防止される。尖端１
２と外周リング１３の直径方向両端部は上記粗面部分１
５とともにロッド３中に埋め込まれる。
これらの作業の結果、ロッド３は回転運動と並進運動に
対して固定され、尖端１２と外周リング１３がナツト保
持として働いて固定の安全性を確保する。
第８図および第９図は、脊椎骨の一つ２５（第７図）が
骨折しているを椎１０に上記の骨接合器具を使用した場
合の取付は状態を示している。この骨接合装置は、横断
ロッド２２と、各ロッド３に取付けられたフック２３と
を有する２個の安定兼横方向牽引用器具２０　（ＤＴＴ
＞を具備している。この場合の骨接合は、第１図から第
４図を参照して説明したものと類似のインプラント１８
および１９によって完成されている。インプラント１９
は、骨に固定する部分が前記の湾曲ブレード１ではなく
、ネジ付きロッド２１で構成されているところが異なっ
ている。各インプラント１８および１９は、ロッド３に
沿って、適切に配置されている。
発明の効果 本発明によるインプラントは、以下の利点がある。
（１）溝内固定具のような複雑な部材を必要とせずにロ
ッド３をインプラントの本体後部に直接挿入できる。
（２）ネジ込みインプラントの場合には、圧縮および収
縮させる（すなわち、を椎または骨の一部を伸張させる
）際に、従来方式の円錐形インプラントでは、正確な方
向付けが必要であったが、本発明では、頭部の方向を決
める必要がない。
（３）従来は、複数タイプの本体が使用されていたが、
本発明では後部が開いた１種類の本体に減るので、骨接
合具の取付けが大幅に簡便になり、それと同時に補助具
も簡単になる。
（４）プラグ８が分岐部４の内側の雌ネジ１１にネジ込
まれ、更にインプラントの本体２が円筒形であるため、
インプラントの寸法が相当小さくなる。
（５）分岐部４の雌ネジ１１が、分岐部４の内側に形成
されているので、インプラントの作業中に雌ネジ１１を
傷める危険がまったくない。
（６）尖端１２と外周リング１３がプラグ８と一体に形
成されているので、プラグと、それと別部品の固定ネジ
とを用いたインプラントに比較して、必要部品数が減少
し、必要な作業が減少し、組立も外科医にとって簡単に
なる。
（７）器具を再組立する（若い患者の成長によってこの
作業が必要となる）または単純に取り外すことができる
。取り外しは、特に外傷の治療において必要である。実
際、本発明のインプラントはプラグ８をゆるめることで
非常に簡単に分解できる。
（８）従来の閉じた本体にロッドを挿入することは難し
いが、本発明ではインプラントにロッドを挿入すること
が容易である。
変形としては、プラグに尖端１２のみ、または外周リン
グ１３のみを設けることができる。

Claims (9)

    【特許請求の範囲】
  1. （１）骨に固定される部分（１）と、ロッド（３）に取
    り付けられる本体（２）とで構成され、この本体には本
    体後側に向かって開いた溝（６）が形成されており、こ
    の溝によって本体は２つの分岐部（４）に分けられ、さ
    らに、ロッド（３）を収容するためにこの溝（６）は本
    体の両側で開いているような骨接合具用、特に脊椎骨接
    合具用のインプラントにおいて、 本体（２）の後端を構成する上記２つの分岐部（４）の
    内壁に形成された雌ネジ（１１）にネジ込み可能なプラ
    グ（８）を具備し、このプラグ（８）はその直径方向反
    対側の２つのエッジがロッド（３）と当接するように溝
    （６）のネジ込まれた側を閉じ、このプラグ（８）のロ
    ッド（３）の方向を向いた面（８ａ）にはプラグ（８）
    をロッド（３）に係止して固着させる手段が設けられて
    おり、この係止・固着手段によってロッドの並進運動と
    回転運動とが固定されることを特徴とするインプラント
    。
  2. （２）前記係止・固着手段がプラグ（８）の他の部分と
    一体に形成された中心尖端（１２）を含むことを特徴と
    する請求項１に記載のインプラント。
  3. （３）前記係止・固着手段がプラグ（８）の面（８ａ）
    から突出した外周リング（１３）を含み、この外周リン
    グ（１３）は断面が三角形で、好ましくは頂点（１３ａ
    ）が丸くなっており、プラグ（８）の他の部分と一体に
    形成されていることを特徴とする請求項１に記載のイン
    プラント。
  4. （４）前記係止・固着手段が中心尖端（１２）とプラグ
    （８）の面（８ａ）から突出した外周リング（１３）を
    含み、該尖端と外周リングがプラグの他の部分と一体に
    形成されていることを特徴とする請求項１に記載のイン
    プラント。
  5. （５）固定部（１）、本体（２）およびプラグ（８）が
    生体適合性の良い同じ材料で作られていることを特徴と
    する請求項１〜４のいずれか一項に記載のインプラント
    。
  6. （６）プラグ（８）をネジ込んだ場合の荷重の半径方向
    成分を除去するために、本体（２）の２つに分かれた分
    岐部（４）の雌ネジ（１１）とプラグ（８）の雄ネジ（
    ９）とが、鋸歯ピッチであることを特徴とする請求項１
    〜５のいずれか一項に記載のインプラント。
  7. （７）プラグ（８）がロッド（３）の方を向く面（８ａ
    ）に、粗面部分（１５）を有することを特徴とする請求
    項１〜６のいずれか一項に記載のインプラント。
  8. （８）前記粗面部分（１５）が、斜辺部分（１５ａ）が
    締まる方向に向いた鋸歯状に形成されていることを特徴
    とする請求項７に記載のインプラント。
  9. （９）プラグ（８）が、冷間加工、例えば冷間鍛造で、
    製作され、切削加工で製作されたロッド（３）よりもか
    なり大きい硬度を付与されていることを特徴とする請求
    項１〜８のいずれか一項に記載のインプラント。
