Description

【発明の詳細な説明】 檄泉上Δ扶且立野 本発明は全身性カンジダ感染の治療剤に関する。
従来技術と 明が解決しようとする課題アンホテリシン
Bは広いスペクトルのポリエン抗真菌剤であって、はと
んどの真菌感染の治療に有効であることが認められてい
る。この抗生物質の完全な記載は、U、S、特許第29
08611号に見られる。しかしながらアンホテリシン
Bの有用性はその毒性によって制限される。長期の治療
には、腎毒性および貧血を伴うことが少なくなく、その
急性毒性には熱病、さむけ、吐気および嘔吐が含まれる
。この毒性を解消するため、多数のアプローチが開発さ
れている。これらのアプローチとしては、一般に段階的
用量のアンホテリシンBを用いて治療を開始して、耐性
を決定し、かつ腎機能を密接にモニターする。アンホテ
リシンBは通常、注入投与のためコロイド懸濁液で使用
することができる。アンホテリシンBのコロイド懸濁液
を得るには、ナトリウム・デソキシコレート(deso
xycholate)の使用が必要で、さもなければ、
アンホテリシンBは水溶液に不溶または貧溶解となる。
アンホテリシンBは、これをインビボ投与すると、急速
にリボ蛋白類や赤血球膜と結合すると思われる。
最近、ロペズーベレステインらは、アンホテリシンBの
配合物に関し新しいアプローチを報告しているが、この
配合物はアンホテリシンBの薬効を保持あるいは改善し
つつ、低毒性をもたらすものである。このアプローチに
は、アンホテリシンBをリポソーム(l iposom
e)類に封入する必要がある。これらの結果は、下記論
文に報告されている。

頁、1987年、“リポソーム性アンホテリシンBによ
る肝脛真菌感染の治療“) 最近に特許されたU、S 、特許第4663167号に
、シミリストイルホスファチジルコリンとシミリストイ
ルホスファチジルグリセロールを約7:3のモル比で必
須成分とするリポソームにアンホテリシンBを封入した
組成物が記載されている。
また、ヨーロッパ特許出願第884005208には、
リビド類とアンホテリシンBを含育し、リピド類に対し
アンホテリシンBの割合を高くした組成物が記載されて
いる。
今日に至るまで、ヒトの全身性真菌感染には、適切な抗
カビ剤による長期にわたる多重治療が必要であった。ま
た、臓器移植における免疫抑制剤の使用や癌叡者におけ
る攻撃的化学療法の増大に伴って、カンジダのような時
宜を得た病原体によって起る感染がますます流行する。
最近、イギリスにおいて、カンジダ膣炎の治療のため1
日療法用のフルコナゾール(r 1uconazol−
e)が承認されている。
本発明によれば、1回用量投与による全身性真菌感染(
カンジダ感染)の治療に、アンホテリシンB・リビド錯
体(ABLC)を使用しうろことがわかった。これらの
真菌感染には、インラインカテーテル関連カンジダ血症
、非侵襲性のカンジダ食道炎および粘膜カンジダ症が含
まれる。アンホテリシンB・リビド錯体は、ヨーロッパ
特許出願第88400520.0に記載の如く、リビド
類に対するアンホテリシンBの割合が比較的高い。
1回用量投与は、多重投与の必要を除去するため、AB
LCの摂取を極めて簡単にする。
カンジダ感染を治療する1回用量投与のABLCは、多
重用量のアンホテリシンB使用によって得られる効果と
比較して、アンホテリシンBの毒性を考慮すると、少な
くとも同等らしくはそれ以上の効果が達成される。
1回用量投与は、多重用量による長期治療の場合に高く
なる治療コストを低減する。
1回用量投与は、カンジダ感染の治療に必要なアンホテ
リシンBffiの低減によって起る副作用の可能性を低
下せしめる。
ヒト全般の真菌感染やカンジダ感染には特に、適当な抗
真菌剤を用いる長期の多重治療が要求される。これに対
し、本発明では1回用量のみしか要しない哺乳動物のカ
ンジダ感染の治療法が付与される。このことは、アンホ
テリシンB・リビド錯体の特異性によって可能である。
本発明のアンホテリシンB・リピド錯体は、以下の手順
で製造される。すなわち、アンホテリシンBをDMSO
に溶解し、濾過し、次いでさらに濾過で殺菌する。この
濾過および殺菌した溶液を、殺菌混合容器に入った塩化
メチレン中のシミリストイルホスファチジルコリンおよ
びシミリストイルホスファチジルグリセロールのフィル
ター殺菌溶液と無菌的に混合する。この混合物に、当該
錯体を水和するため、フィルター殺菌した塩化ナトリウ
ム注射剤(標準食塩水)を加える。混合物の塩化メチレ
ン部を蒸発除去する。残った食塩水中のABLC錯体の
殺菌@濁液を、残りのDMSOと共に、さらに標準食塩
水で希釈して、取扱い易くする。かかる懸濁液を殺菌コ
ロイド微粉砕機にて、生成物の大部分の粒径が5ミクロ
ン以下となるまで無菌的に微粉砕する。微粉砕した懸濁
液を、1〜5ミクロン部分のアンホテリシンB・リピド
錯体を回収するため、孔径1.2〜5ミクロンのセラミ
ック接線(tangential)フィルターで無菌的
に濾過する。
本発明のABLC配合物は、1回用量投与により、イン
ラインカテーテル関連カンジダ血症、非侵襲性のカンジ
ダ食道炎および粘膜カンジダ症などの哺乳動物における
各種真菌関連疾患の治療剤として使用することができる
。哺乳動物の真菌感染に対抗するため、ABLC配合物
を1回用量として、約0.5〜LOxg/kHの量で投
与することができる。好ましい投与法は、静脈内投与で
ある。
次に挙げる実施例には、マウスの全身性カンジダ症の治
療に1回用量投与の有効性が示されている。
実施例1 前記詳細な説明で記載した方法に従い、下記組成のAB
LC治療剤を製造する。

トイルホスフアチジルグリセロール(DMPG)   
          −−−2,0R9塩化ナトリウム
       −−−9.OB注射用水       
    全体を1m12実施例2 雌のスイス・ウェブスターマウスに、カンジダ・アルビ
カンス5C5314の5X10”個の公葬胞子を静脈注
射して感染せしめ、5時間後に、ABLCまたはアンホ
テリシンBのいずれかで1回の治療を行う。アンホテリ
シンBは、実験マウスへの静脈注射のため5%ブドウ糖
に希釈した。
ABLC治療剤も静脈注射で投与した。結果を表1に示
す。
表1 表1は実験3回の範囲を示す。表1に示されるように、
ABLCの薬効は生存E D s。が1.I6*v/k
gのABLCで、実験3回の範囲は1.07〜!、28
である。これに対して、アンホテリシンBの場合の生存
E D s。は0.46*9/kgで、実験3回の範囲
は0.36〜0.54である。薬効を腎クリアランスに
基づくと、ABLCはなお、アンホテリシンBのE D
 s。が0 、7 R9/kgと比較して、E D s
o 1 、9319/ kgを有し育効である。

  1. 【特許請求の範囲】 1、アンホテリシンB・リピド錯体から成ることを特徴
    とする哺乳動物の全身性真菌感染の治療剤。 2、真菌感染がインラインカテーテル関連カンジダ血症
    である請求項第1項記載の治療剤。 3、真菌感染がカンジダ食道炎である請求項第1項記載
    の治療剤。 4、真菌感染が粘膜カンジダ症である請求項第1項記載
    の治療剤。
