【発明の詳細な説明】
発明の背景
技術分野
本発明は、中空繊維膜型人工肺およびその製造
方法に関する。
先行技術
一般に心臓手術等において、患者の血液を体外
に導き、これに酸素を添加するために、体外循環
回路内に中空繊維膜型人工肺が用いられている。
このような人工肺において使用される中空繊維
膜としては、均質膜と多孔質膜の2種類がある。
均質膜はシリコーン膜を用いるので、強度的に
膜厚を100μm以下にすることができず、このため
ガス透過に限界があり、特に炭酸ガスの透過が悪
い。また数万本束ねたときに装置が大型化しプラ
イング量の増大をきたし、また、加工性が悪くコ
ストが高いという欠点がある。
一方、多孔質膜は、膜の有する微細孔が透過す
べき気体分子に比べて著しく大きいため、体積流
として細孔を通過する。そして、例えば、マイク
ロポーラスポリプロピレン膜等の多孔質膜を使用
した人工肺が種々提案されている。
しかしながら、多孔質膜は水蒸気の透過性が高
いので結露水によつて性能が低下するだけでな
く、長期間血液を循環させて使用すると、血漿が
漏出する場合があつた。
このような多孔質膜の諸欠点を解消するため
に、直径10ミクロン以下の貫通した微細孔を有す
る側壁をもつ中空繊維基体の側壁に、メチルハイ
ドロジエンポリシロキサンの非通気性の薄膜を形
成させてなる中空繊維膜が提案されている(特公
昭54−17052号)。
しかしながら、このような中空繊維膜は、中空
繊維基体の微細孔内だけでなく、中空繊維基体の
内外両表面にもメチルハイドロジエンポリシロキ
サンの被膜が形成されるために、その分だけ中空
繊維基体の中空内径が小さくなるので交換能力が
低下する。また、その小さくなる分だけ微細孔内
に充填されるメチルハイドロジエンポリシロキサ
ンの量(充填厚み)が増大するので、酸素、炭酸
ガス等のガス透過率が低いという欠点があつた。
また、前記中空繊維はアクアラング等には使用
し得ても、人工肺として長時間使用すると、血漿
が漏出し始めるという欠点があつた。
このような欠点をさらに改良するために、本出
願人は、中空繊維膜の微細孔を有する側壁にはシ
リコーンオイル層を形成させることなく、微細孔
内のみをシリコーンオイルで閉塞してなる中空繊
維−シリコーン膜複合人工肺を提案している(特
願昭58−92325号)。
この提案の場合は、人工肺のモジユールを組立
てたのちシリコーンオイルの溶液を中空繊維に含
浸させ、ついでこのシリコーンオイルを除去し、
シリコーンオイルの溶媒と非溶媒との混合物を流
して中空繊維基体壁面に付着したシリコーンオイ
ルを除去し、微細孔内のみをシリコーンオイルで
閉塞しようとするものである。
これによれば、血漿漏出量は改善されるが、シ
リコーンオイルが血液中に流出することがあつ
た。
発明の目的
本発明の目的は、前述のような欠点を改良して
長時間体外循環を行つても血漿の漏出がなく、コ
ンパクトで炭酸ガスの除去性能が良く、さらに血
液適合性に優れる中空繊維膜型人工肺と、その製
造方法を提供することにある。
このような目的は下記の本発明によつて達成さ
れる。
すなわち第1の発明は、
ハウジングと、該ハウジング内に挿入された多
数のガス交換用中空繊維膜からなる中空繊維束
と、該中空繊維膜の外表面と前記ハウジング内面
とにより形成される第1の流体室と、該第1の流
体室に連通する第1の流体流入口および流出口
と、前記中空繊維膜の各端部をそれぞれ支持し、
かつ前記第1の流体室から隔離する隔壁と、前記
ハウジングの端部に取付けられたヘツダーにより
形成される前記中空繊維膜の内部空間に連通する
第2の流体流入口および流出口とよりなる人工肺
において、
前記中空繊維膜が貫通した微細孔を有し、該微
細孔はシリコーンオイルと20〜80重量%のシリコ
ーンゴムとを含むシリコーン混合物によつて閉塞
されており、中空繊維膜の側壁には、実質的に前
記シリコーン混合物からなる層が形成されていな
いことを特徴とする中空繊維膜型人工肺である。
またその実施態様は下記のとおりである。
) 第1の発明において、
前記シリコーンゴムがシリカを含まない室温
硬化性シリコーンゴムであること。
) )において、
前記シリカを含まない室温硬化性シリコーン
ゴムがビニルメチルシロキサンとメチルハイド
ロジエンシロキサンの重合物であり前記シリコ
ーンオイルがジメチルまたはメチルフエニルシ
リコーンオイルであること。
また、第2の発明は、
ハウジングと、該ハウジング内に挿入された多
数のガス交換用中空繊維膜基体からなる中空繊維
束と、該中空繊維の外表面と前記ハウジング内面
とにより形成される第1の流体室と、該第1の流
体室に連通する第1の流体流入口および流出口
と、前記中空繊維膜基体の各端部をそれぞれ支持
しかつ前記第1の流体室から隔離する隔室と、前
記中空繊維膜基体の内部空間に連通する第2の流
体流入口および流出口とよりなる人工肺のモジユ
ールを組立てたのち、
前記中空繊維膜基体の内部にシリコーンオイル
と20〜80重量%のシリコーンゴムとを含むシリコ
ーン混合物の20〜80重量%の原料シリコーン溶液
を導入して含浸し、
次いで、前記シリコーン混合物を溶解しうる溶
媒と、前記シリコーン混合物が不溶な液体とを含
む洗浄液を流通させ、
前記中空繊維膜基体の微細孔にシリコーンゴム
またはシリコーン混合物を充填することを特徴と
する中空繊維膜人工肺の製造方法である。
そしてその実施態様は、第2の発明において、
洗浄液が10センチポイズ以上の粘度を有すること
である。
発明の具体的構成
以下、本発明の具体的構成について詳細に説明
する。
第1図は、本発明の中空繊維膜型人工肺の全体
図を示す。
すなわち、第1図に示すように、本発明による
人工肺10は、人工肺10を構成する筒状ハウジ
ング11の内部空間には、中空繊維膜12の繊維
束13が収納されている。
中空繊維膜12の両端部は、隔壁14,15を
介してハウジング11に液密に保持されている。
ハウジング11の両端部には、ヘツダー16,
17がハウジング11に螺合されるカバー18に
よつて固着されている。
ヘツダー16の内面と隔壁14とは、中空繊維
膜12の内部空間に連通する第2の流体流入室と
しての血液流入室19を画成し、ヘツダー16に
は、第2の流体流入口としての血液流入口20が
形成されている。
ヘツダー17の内面と隔壁15とは、中空繊維
膜12の内部空間に連通する第2の流体流出室と
しての血液流出室21を画成し、ヘツダー17に
は、第2の流体流出口としての血液流出口22が
形成されている。
また、隔壁14,15、ハウジング11の内壁
および中空繊維膜12の外壁とは、第1の流体室
としてのガス室23が形成され、ハウジング11
の両端側には、それぞれガス室23に連通する第
1の流体流入口としてのガス流入口24および第
1の流体流出口としてのガス流出口25が形成さ
れている。
なお、ハウジング11の内壁中央部には、繊維
束13の外形を縮径する絞り用拘束部26を設け
られることが好ましい。その結果、第1図に示す
ように軸方向の中央において絞り込まれ、絞り部
が形成される。
したがつて中空繊維膜12の充填率は、軸方向
に沿う各部において異なり、中央部分において最
も高くなつている。
隔壁14,15は、中空繊維膜12の内部と外
部とを隔離するという重要な機能をはたすもので
ある。
通常、この隔壁14,15は、極性の高い高分
子ポツテイング剤、例えばポリウレタン、シリコ
ーン、エポキシ樹脂等をハウジング11の両端内
壁面に遠心注入法を利用して流し込み、硬化させ
ることにより作られる。
さらに詳述すれば、まずハウジング11の長さ
り長い多数の中空繊維膜12を用意し、この両開
口端を粘度の高い樹脂によつて目止めした後、ハ
ウジング11内に並べて位置せしめる。
この後、カバーで各両端を完全に覆つて、ハウ
ジング11の中心軸を中心にそのハウジング11
を回転させながら両端部から高分子ポツテイング
剤を流入したのち硬化し、さらにカバーを外した
のち硬化したポツテイング剤の外側面部を鋭利な
刃物で切断して、中空繊維膜12の両開口端を表
面に露出させることにより形成される。
しかして、前記人工肺に使用される中空繊維膜
は、第2図に示すように、貫通した微細孔31を
有する側壁32をもつ多孔性中空繊維基体33の
壁面34に、実質的にシリコーン層を形成させる
ことなく、側壁32の微細孔31内をシリコーン
ゴム、またはシリコーンゴムとシリコーンオイル
とを含むシリコーン混合物35で閉塞してなる中
空繊維膜型ガス交換膜である。
この中空繊維膜に使用される多孔性中空繊維基
体としては、ポリプロピレン、ポリエチレン、ポ
リテトラフロロエチレン、ポリスルホン、ポリア
クリロニトリル、ポリエチレンテレフタレート、
ポリブチレンテレフタレート、ポリカーボネー
ト、ポリウレタン、ナイロン−6,6、ナイロン
−6、セルロースアセテート等があり、好ましく
はポリオレフインであり、特に好ましくはポリプ
ロピレンである。
この中空繊維基体を用いて製造されたガス交換
膜を使用する人工肺としての実用可能な性能を得
るためには、膜厚および空孔率に自ずと制限を生
じる。一般に膜を介しての基体の透過量qは、次
式で表わされる。
q=P×△p×A/l
(ただし、式中、Pはガス透過係数、△pは透
過気体の圧力差、Aは膜面積、lは膜厚である。)
本発明の中空繊維膜は、気体の透過する部分が
シリコーンゴム、またはシリコーン混合物で閉塞
された微細孔であるため、実質膜面積は多孔質膜
と比較して非常に小さくなる。
これを補うためには、前記式から明らかなよう
に膜厚を薄くする必要がある。
このため本発明において中空繊維膜の膜厚lの
範囲は5〜200μm、好ましくは10〜50μmである。
中空繊維の内径は100〜1000μm、好ましくは
100〜300μmであり、空孔率の範囲は20〜80%、
好ましくは40〜80%である。微細孔の直径は0.01
〜5μ、好ましくは0.01〜1μmである。
本発明による人工肺を製造するには、まず、第
3図a,bに示されるように、中空繊維膜を人工
肺のモジユールに組み込んでからシリコーンゴム
とシリコーンオイルの混合物の溶液(シリコーン
溶液)を流通させて充分含浸させる。
ついでガスを流通させ、第4図a,bに示され
るように、所定の洗浄液を中空繊維の少なくとも
内面に流通させ、さらに必要に応じ加熱架橋させ
る。
使用されるシリコーンゴムは室温硬化型
(RTV)であり、1液型、2液型、いずれのもの
を用いてもよい。
2液型のものとしては、原料モノマーないしオ
イルにビニル基および/または水素を含み、混合
後C−H間で架橋してなる2次元ポリマーの固形
状のゴムが一般的である。
例えば、2液型のRTVシリコーンゴムとして
は、ビニルメチルシロキサンとメチルハイドロジ
エンポリシロキサンの重合体が好ましい。
なお、これらの硬化架橋に際しては、白金族金
属の単体、酸化物、化合物等、例えば塩化白金酸
などが用いられている。
また、その硬化温度は20℃〜30℃以上とする。
また、ここで用いるシリコーンゴムは抗血栓の
点からシリカと含まないRTVゴムが好ましい。
しかし、シリコーンゴムのみでは、液状態での
粘度が高く、中空繊維膜内部への流入が困難にな
るなどの問題がある。
このため、シリコーンゴムにシリコーンオイル
を併用する。
本発明において用いられるシリコーンオイル
は、シロキサン結合を持つている液状物質であ
り、例えば、ジメチルシリコーンオイル、メチル
フエニルシリコーンオイル、メチルクロロフエニ
ルシリコーンオイル、分岐状ジメチルシリコーン
オイル、メチルハイドロジエンシリコーンオイル
等があり、好ましくはジメチルシリコーンオイル
およびメチルフエニルシリコーンオイル、最も好
ましくはジメチルシリコーンオイルである。
シリコーンゴムとシリコーンオイルとからなる
シリコーン混合物中でのシリコーンゴム(固形
分):シリコーンオイル(液状分)の比は、重量
比で2:8〜8:2が好ましく、より好ましくは
4:6程度である。
シリコーンゴムが8より大となると、溶液の粘
度が上昇して、中空繊維膜基体壁面に付着するシ
リコーンの除去が困難となる。また、シリコーン
ゴムが2未満となると、混合されたシリコーンオ
イルが血液中に流出する可能性がある。
このシリコーンゴム、またはシリコーン混合物
は、通常20〜80重量%、好ましくは30〜60重量%
の溶液として使用される。
また、その溶媒としては、ベンゼン、トルエ
ン、キシレン、ヘキサン、ジクロルメタン、メチ
ルエチルケトン、ジフロルエタン、酢酸エチル、
トリフロルトリクロルエタン(フレオン)等があ
る。
中空繊維基体壁面に付着したシリコーンゴムま
たはシリコーン混合物を実質的に除去する液体
(洗浄液)は、前記溶媒では含浸させたシリコー
ンが溶出してしまうため、シリコーン混合物が溶
解しない有機液体、特にアルコール系液体を用い
る。
そして、その粘度は、室温にて10センチポイズ
以上、特に30〜80センチポイズ以上とすることが
好ましい。
これにより、余剰のシリコーンゴムまたはシリ
コーン混合物の除去が良好に行われ、これらが均
質化し、性能が安定化する。
また、洗浄液には、このような粘度を保持した
上で、10vol%以下、特に0.5〜10vol%の前記シ
リコーン溶媒を混合することが好ましい。
これにより、余剰のシリコーンが排出しやすく
なり、性能の向上がはかれる。ただ、10vol%を
こえると、シリコーンが溶出してしまうので好ま
しくない。
特に好ましい範囲は2〜6vol%である。
このような洗浄液組成としては、
トルエンとプロピレングリコール
トルエンとジプロピレングリコール
ジクロルメタンとジエチレングリコール
ジクロルエタンとエチレングリコール
メチルエチルケトンとエチレングリコール
等がある。
なお本発明で、シリコーンゴムやシリコーン混
合物を実質的に除去するとは、中空繊維膜内壁に
付着したシリコーンゴムまたはシリコーン混合物
の層を500Å以下の厚みとするという意味である。
500Å以下にすれば気体透過には実質的に影響
を与えず、後に述べるようにCO2の充分な透過が
確保されるからである。
発明の具体的実施例
以下、本発明の実施例を示し、本発明をさらに
詳細に説明する。
実施例 1
延伸法により軸方向に延伸されて形成された内
径200μm、肉厚25μmで、平均孔径700Åの貫通し
た微細孔を有するポリプロピレン製の中空繊維
(空孔率50%)を用いて、第1図に示されるよう
な人工肺(モジユールA)を作製した。
このモジユールAの血液の流路に、第3図a,
bに示すように、ビニルメチルシロキサンとメチ
ルハイドロジエンシロキサンとの2液型で、塩化
白金酸触媒を添加したシリコーンゴム(シリカは
含有せず)と、ジメチルシリコーンオイルとのシ
リコーン混合物(シリコーンゴム40wt%)の60
%フレオン溶液(シリコーン溶液)を流入させ、
3分間浸した。
こののち、空気を流通させ、さらに3%トルエ
ン/ジプロピレングリコール溶液(洗浄液)を内
外面に流通させた。
洗浄液の粘度は100センチポイズである。
これにより、実質的に微細孔内にのみシリコー
ンゴムとシリコーンオイルとのシリコーン混合物
を充填した中空繊維膜型人工肺(モジユールC)
を得た。
この人工肺モジユールCについて新鮮ヘパリン
加牛血を用い、酸素飽和度65%、炭酸ガス分圧45
mmHgとなる静脈血を作製し、これを被検人工肺
に流通させて性能評価を行なつた。ベモグロビン
含量は12g/dl、温度は37℃であつた。
酸素流量1/min/m2のときの血液流量と酸
素添加能との関係を示すと、第5図の曲線Cのと
おりである。
また血液流量1000ml/min/m2のときの酸素流
量と炭酸ガス除去能との関係を示すと、第6図の
曲線Cのとおりである。
さらに、雑犬を用いて静脈−動脈の部分体外循
環試験を行つた。
循環時間と血漿漏出量との関係は、それぞれ第
7図における曲線Cのとおりであつた。
比較例 1
実施例1のモジユールAの中空繊維の微細孔は
貫通したままで、シリコーン膜等を形成させない
人工肺を用い、実施例1と同様な測定を行い、第
5図、第6図、第7図における曲線Aを得た。
比較例 2
シリコーン中空繊維膜を用いて、実施例1と同
様な人工肺(モジユールB)を製作した。
このモジユールBについて実施例1と同様な測
定を行い、第5図、第6図、第7図において曲線
Bを得た。
第5図に示すように、O2添加能は多孔質膜
(モジユールA A曲線)、シリコーン膜(モジユ
ールB B曲線)、本発明(モジユールC C曲
線)間に有意差はなかつた。
CO2除去能は、第6図に示すように、シリコー
ン膜(モジユールB B曲線)が非常に低値を示
した。
長時間動物実験による第7図では、多孔質膜
(モジユールA A曲線)は、経時的に血漿の漏
出量が増加したのに対し、シリコーン混合物で閉
塞した本発明(曲線C)では血漿が漏出しなかつ
た。
これらから、本発明のモジユールCは、炭酸ガ
ス除去能、酸素添加能および血漿漏出防止能の総
合特性で最良の結果を与えることがわかる。
比較例 3
中空繊維内外面に3%トルエン/ジプロピレン
グリコール溶液の洗浄液を流通させなかつた以外
は、実施例1と全く同一の条件で中空繊維膜人工
肺モジユールEを得た。
モジユールCおよびEの人工肺を切断し、内部
の中空繊維を取り出し、中空繊維膜を軸方向に切
断し、電子顕微鏡にて、10000倍で観察した。
モジユールCでは、内外面に、シリコーン混合
物の連続した層は存在していなかつたが、モジユ
ールEでは、内外面にシリコーン混合物の連続し
た層が存在し、内面での厚さは約2μm、外面での
厚さは約1μmであつた。
比較例 4
実施例1において、シリコーン溶液をジメチル
ポリシロキサン(シリコーンオイル)の50%フレ
オン溶液にかえ、洗浄液の流通を行なわなかつた
他は実施例1と全く同一の条件で中空繊維膜モジ
ユールFを得た。
内外面にはシリコーンオイルの連続した層が存
在し、内面での厚さは約3μm、外面での厚さは約
1μmであつた。
比較例 5
実施例1において、シリコーン溶液を実施例1
のシリコーンゴムの50%フレオン溶液にかえ、洗
浄液の流通を行なわなかつた他は実施例1と全く
同一の条件でモジユールGを得た。
内外面にはシリコーンゴムの連続した層が存在
し、内面での厚さは約8μm、外面での厚さは約
1μmであつた。
これら各モジユールC、E、F、Gにつき、前
記と同様に酸素添加能、炭酸ガス除去能および血
漿漏出量を測定した。血液流量は1000ml/min/
mm2、酸素流量は3000ml/min/mm2とし、24時間循
環後の血漿漏出量を測定した。

Furthermore, when tens of thousands of pieces are bundled, the size of the device increases, resulting in an increase in the amount of plying, and there are also disadvantages in that processability is poor and costs are high. On the other hand, in a porous membrane, the fine pores of the membrane are significantly larger than the gas molecules to be passed through, so that the gas passes through the pores as a volumetric flow. For example, various oxygenators using porous membranes such as microporous polypropylene membranes have been proposed. However, since the porous membrane has a high permeability to water vapor, its performance not only deteriorates due to condensation water, but also causes plasma leakage when used with blood circulating for a long period of time. In order to eliminate these drawbacks of porous membranes, we formed an impermeable thin film of methylhydrogen polysiloxane on the sidewalls of a hollow fiber substrate that has sidewalls with penetrating micropores of 10 microns or less in diameter. A hollow fiber membrane has been proposed (Special Publication No. 54-17052). However, in such a hollow fiber membrane, a film of methylhydrodiene polysiloxane is formed not only in the micropores of the hollow fiber substrate but also on both the inner and outer surfaces of the hollow fiber substrate. Since the inner diameter of the hollow space becomes smaller, the replacement capacity decreases. Furthermore, the amount of methylhydrodiene polysiloxane filled in the micropores (filling thickness) increases as the size of the pores decreases, resulting in a disadvantage that the permeability of gases such as oxygen and carbon dioxide gas is low. Furthermore, although the hollow fibers can be used in Aqua Lungs and the like, they have the disadvantage that plasma begins to leak out when used as an artificial lung for a long time. In order to further improve these drawbacks, the present applicant has developed a hollow fiber membrane in which only the inside of the micropores are closed with silicone oil, without forming a silicone oil layer on the side wall having micropores of the hollow fiber membrane. -Proposed a silicone membrane composite oxygenator (Japanese Patent Application No. 58-92325). In this proposal, after assembling the oxygenator module, the hollow fibers are impregnated with a solution of silicone oil, and then the silicone oil is removed.

は、6時間後でも血小板数90%を維持していた
が、シリカ含有シリコーン混合物は70%まで低下
した。
発明の具体的効果
本発明は多孔質膜使用の人工肺に比して長時間
使用しても血漿の漏出がなく、シリコーン膜使用
の人工肺に比して加工性がよく、装置が小型化で
き、コストを低くすることができるばかりでな
く、中空繊維膜の壁面に実質的にシリコーン層を
形成させることがないので、ガス交換率が良い。
特にシリコーンゴムをシリコーンオイルに添加
することによつて、CO2除去能がシリコーンオイ
ル単独のものに比べて非常に向上する。
またシリコーンゴムにシリカを含まない室温硬
化型シリコーンゴムを用いることにより、抗血栓
