チタンが、骨内に移植された後に人間の骨組織がそれに接着する生体親和性材料 であることが発見されて以来、例えば５チタン製の足首の補綴が開発されている 。導入部で説明された既知の足首の補綴においては、チタンプレートの固定手段 が使用され、最初の手術で脛骨と距骨に挿入されて、確実にネジ止めされる。一 定時間経過後に固定手段がその骨と一体となると、骨の端部が切り取られて、そ の後ボール部とソケット部が骨と一体となる固定手段に確実にネジ止めされる第 二の手術が実行される。固定手段の正確な位置を前もって決定することは不可能 であるので、固定手段が骨と一体となった後に外科医は、二回目の手術で、固定 手段から見て互いに正しく一直線となるボール部とソケット部を確実に見つける ために色々の関節構成要素類別を当たって見なければならない、或いは、手術中 に正しく適合させるために関節補綴の構成要素を加工しなければならない、これ は時間のかかることであり、しかも理想的整列から少し逸脱することが避けられ ない。
本発明の目的は５二回目の手術時において構成要素の研究、又は種々の興なる補 綴構成要素を持たせる必要性を排除する導入部で説明されたタイプの関節補綴を 提供することである。同時に、ボール部とソケット部間の正確な嵌合がなされる ことを意図したものである。
これは、本発明によれば導入部で説明されたタイプの関節補綴において、接合要 素が、関連接合要素に関してボール部とソケット部の相対的横方向転置を可能に する手段、ボール部とソケット部の相互間のｊｌｌ！！後にボール部とソケット 部を各々の接合要素内にロックするための手段、そして少なくとも一つ以上の固 定手段と関連接合要素との間の距離を垂直方向に調整できるように配置された手 段とから構成されると言う事実により達成される。
完全な相対的調整能力は、互いに垂直に延び、そして各々の要素と一体のダブテ ール溝を設けるすることにより、そしてこれらのダブテール溝に適合されたガイ ド手段と共にボール部とソケット部を設けることにより簡単に与えられる。これ はＸ−Ｙ軸に沿ったｍｌ！Ｉを可能にし、しかもＺ軸に沿った調整も接合要素の 一つ、或は両方における構成要素の高さ、または厚みをＨＷｉすることにより達 成可能である。
本発明は、添付の図に示された例を参考にして更に詳細に説明するが、ここで図 １は本発明による足首補綴の部分断面側面図を示し、そして図２は図１の足首補 綴の前面図である。
本発明による足首補綴はプレートｌと外部及び内部ネジ付スリーブ２のペアーの 上部と下部固定手段から構成される。
プレートｌにはその下部端において、円形状溝４に垂直に延長するダブテールを 有するブリッジ５の形の接合要素内の円形状溝４内に配置される円形断面のビー ド３が設けられている。ダブテール溝６内には５溝６と同じ輪郭を有し、且つプ レート７と一体のスライド本体８が受けられる。プレート７は、足首関節ソケッ ト部１０が、例えば接着により固定される。ここに説明した構成では固定プレー トｌに関してソケット部ｌＯを横方向に転置出来るようにする。ビード３と溝４ を介して接続はプレートｌに関するブリッジ５の角度微調整を可能にする。
ダブテール溝１３を有する第ニブリッジ１２の接合要素は４つのリンク、或は＄ ５１４の働きによりスリーブ２に接続されている。リンク１４は、ブリッジのネ ジ付ボアー１６内にネジ込まれる上部ネジ１５と、スリーブ２内にネジ込まれる 下部ネジ１７の支援でブリッジ１２とスリーブ２に確実にネジ止めされている。
関節ボール部として働く半球状本体１９上のスライド本体１８は溝１３内で受け られ、そしてスリーブ２の軸方向へのボール部の転置を許す、スライド本体８と １８は、ネジ付貫通ボアー２３と２４に各々ネジ込まれ、それぞれのダブテール 溝６と１３の各々に固定される固定ネジ、例えばソケット付きネジ２１．２２の 働きによりそれらの溝６と１３内にロック可能である。
説明された足首補綴は２回の手術で移植される。最初の手術で、プレートｌが脛 骨内に移植され、そしてスリーブ２は距骨に移植される。プレートｌにはその全 表面上に均等に間隔が設けられた穴２０が設けられ、そしてその表面には又溝が 彫られている。その穴と溝は最大の骨の統合を促進する。
スリーブ２には望ましくは穴、又はせん孔（図示されない）が設けられる。
約４カ月かかると言われる、プレートｌとスリーブ２が骨と一体となる時、Ｘ線 で構成要素の正確な位置を決定し、そしてボール部１９とソケット部ｌＯとの間 の修正垂直距離を得るためにリンク１４の長さと角度位置を決定する。二回目の 手術中、下部ブリッジ１２はネジ１５と１７、そしてリンク１４の支援で脛骨に 固定される。ブリッジ１２はその底面において骨との一体化のための溝、或は歯 ２５が提供されている。もしスリーブ２に穴が開けられるならば、下部ネジ１７ は又最終的に骨と一体化される。
その脛骨は、円筒状端面３が露出され、そしてブリッジ５内の円筒状溝４内に滑 入出来るように短くされる。この接続は、プレートを移植する時の僅かな角度的 エラーの補正を可能にする。ブリッジ５はその上部表面に骨との一体化のための 溝、或は歯２６有している。ブリッジ５．１２を搭１１ｆｌｌ、ボール部とソケ ット部の各々に対する各々のスライド本体８と１８はそれらのダブテール溝内に 滑入され、そして互いに関して横方向に調整されて、関連ネジ２１．２２でロッ クされる。もし必要なら、リンクにより与えられた垂直調整可能性に加えて、追 加的垂直ｍＷ！が上部ブリッジ５と、又はスライド本体８の厚みを変更すること により達成可能である。
上記発明は、特に足首関節を意図した形態で説明されている。固定手段、例えば プレートｌやスリーブ２の適切な修正により関節を肘、肩などのために使用出来 るようにすることが可能である。
ソケット部を除く関節構成要素の全ての材料はチタンである。ボール部は、焼き 戻され、そしておそらくはイオン窒化５または他の表面硬化処理されるチタニュ ーム６ＡＬ４Ｖｍから適切に製造されよう、他のチタン構成要素の全てはチタン の、グレードｌ製のものである。ソケット部はポリエチレン製であり、そして多 孔性で、ＨＥＡＬＯＮをしみ込ませることも可能である。

Claims (1)

  1. 【特許請求の範囲】 １．ボール部とソケット部、そのボール部とソケット部のための外科的に移植可 能な固定手段、そしてボール部とソケット部がそれらの各々の固定手段に固定出 来るようにするための接合要素から構成される関節補綴において、その接合要素 （５，１２）は、その関連接合要素に関してボール部（１９）とソケット部（１ ０）の相対的横方向の転置を可能にする手段（６，８，１３，１８）、相互に関 するボール部とソケット部の調整後にボール部とソケット部を各々の接合要素上 に固定するための手段（２１，２２）、そして少なくとも一つ以上の固定手段と 関連要素（１２）との間の距離の垂直方向の調整が出来るように配置された手段 （１４）から構成されることを特徴とする関節補綴器。 ２．接合要素（５，１２）はダブテール溝（６，１３）を有し、そしてボール部 （１９）とソケット部（１０）は溝に適合するスライド部分（８，１８）を有す ることを特徴とする請求の範囲第１項に記載の関節補綴器。 ３．ダブテール溝（６，１３）は、構成要素がそれらの移植状態にある時その溝 が互いに垂直に広がることが出来るようにそれらの各々の接合要素（５，１２） 上に配備されることを特徴とする請求の範囲第２項に記載の関節補綴器。 ４．固定手段（１，２）、接合要素（５，１２）、そしてホール部（１９）はチ タン製で、そしてソケット部（１０）はポリエチレンであることを特徴とする請 求の範囲第１，２，３項のいずれかに記載の関節補綴器。 ５．固定手段（１）の一つは脛骨内の垂直スロット内に挿入されるよになってい るプレートで形成され、そして脛骨との一体化の後、関連接合要素（５）内の溝 （４）の中に挿入されるように形造されているエッジ部分（３）を有し、前記形 状加工されたエッジ部分と溝はプレートと接合要素間での一定の角度的運動が出 来るように配置されていることを特徴とする請求の範囲第１，２，３，４項のい ずれかに記載の関節補綴器。 ６．他の固定手段（２）は脛骨内に移植されるようになっている内部ネジ付スリ ーブのペアーから形成され、そして関連接合要素（１２）は、骨とスリーブの一 体化後にスリーブに確実にネジ止めされるようになっているリンク（１４）から 構成されることを特徴とする請求の範囲第５項に記載の関節補綴器。 ７．プレート（１）とスリーブ（２）にはせん孔が開けられていることを特徴と する請求の範囲第６項に記載の関節補綴器。 ８．ソケット部（１０）とボール部（１９）はガイド部材（８，１８）を有し、 構成要素が移植される時、各々の接合要素（５，１２）内のタブテール溝（６， １３）内に受けられるその溝は互いに垂直となるように配置されていることを特 徴とする請求の範囲第５，６項のいずれかに記載の関節補綴器。
