【発明の詳細な説明】[Detailed description of the invention]
(a) 産業上の利用分野
本発明は、皮膚又は粘膜より薬物を連続的に投
与させるための外用医薬部材に関するものであ
る。
(b) 従来の技術
人体又は動物体に薬物を投与するに際し、薬物
の放出速度を制限することによつて薬物を長時間
有効血中濃度内に維持する方法が種々提案されて
いる。
第一の方法は、マイクロカプセル化した薬物を
経口投与し、その徐放性によつて薬物の放出をコ
ントロールするものである。
第二の方法は、感圧性接着剤の如き高分子物質
に薬物を混合して成形し、これを皮膚又は粘膜に
適用して濃度勾配により薬物を吸収させるもので
ある。
第三の方法は、上記第二の方法において、経皮
吸収促進剤を薬物含有高分子物質中に分散させる
ものである。
第四の方法は、上記第三の方法を改良したもの
であつて、経皮吸収促進剤を尿素に付加させて包
接化合物となし、この包接化合物を薬物含有高分
子物質に分散配合するものである。
(c) 発明が解決しようとする問題点
しかしながら、上記第一の方法による経口投与
では、肝臓での代謝が避けられず、又十分なマイ
クロカプセル化剤が見つかつてないため、徐放性
も完全なものとはいい難い。
又上記第二の方法においては、皮膚表面の角質
層が経皮吸収を妨害するバリアーとして作用する
ため、十分な血中濃度を得られない場合がある。
そこで、上記第三の方法においては、経皮吸収
促進剤を薬物含有高分子物質中に分散させ、薬物
の経皮吸収を促進するようにしている。ところが
このように経皮吸収促進剤を高分子物質中に分散
させると、高分子物質層が可塑化されて該層の凝
集力が低下し、特に高分子物質として感圧性接着
剤となるものを用いた場合には、皮膚に適用後剥
離除去するときに、高分子物質が適用皮膚面に残
存したり、貼付使用時に外用医薬部材側面部より
高分子物質がはみ出すなどの問題が生ずる。
この問題を解決するために上記第四の方法にお
いては、経皮吸収促進剤を尿素に付加させて包接
化合物となしているが、尿素の結晶格子は包接さ
れた分子(ゲスト分子)がなくなることによつて
崩れ、皮膚面又は粘膜面に尿素が放出される。そ
のため尿素の持つ副作用として、患者によつては
刺激症状、炎症等が見られることがある。又尿素
と経皮吸収促進剤の解離は水分の多少に敏感に感
応するため、発汗量の個人差、季節等の影響を受
けて薬効に差が出たり、発汗量の多い場合には必
要以上に経皮吸収促進剤が解離されて凝集力が低
下するなどの問題がある。
(d) 問題点を解決するための手段
本発明は、上述のような従来の問題点を解決す
べく鋭意研究を重ねた結果なされたものである。
即ち本発明の外用医薬部材は、シクロデキスト
リン又はその誘導体に液状の経皮吸収促進剤を包
接した包接化合物を、薬物含有高分子化合物中に
分散させた組成物からなることを特徴とするもの
である。
以下本発明を詳細に説明する。
本発明において包接化合物のホスト分子となる
シクロデキストリン又はその誘導体(以下、シク
ロデキストリン類という)としては、α−シクロ
デキストリン、β−シクロデキストリン、γ−シ
クロデキストリン、或はこれらにアルキル化等の
処理をした誘導体が挙げられる。
又包接化合物のゲスト分子となる液状の経皮吸
収促進剤としては、シクロデキストリン類に包接
されうるものであることはもとより、皮膚又は粘
膜に貼り付けた後放出されるものでなければなら
ず、後述の高分子化合物中に含有されている薬物
の溶解能を高める機能、薬物が物質中を移動する
のを促進する機能、或は薬物が皮膚又は粘膜から
吸収されるのを促進する機能(例えば角質軟化機
能)などを有するものが用いられる。
このような機能を有する液状の経皮吸収促進剤
としては、例えばメチルオクチルスルホキサイ
ド、ジメチルラウリルアミド、ジメチルスルホキ
サイド、ジメチルアセトアミド、ジメチルホルム
アミド、ドデシルピロリドンの如きスルホキサイ
ド系、ホスホキサイド系、ジメチルアミド系、ア
ルキルピロリドン系、イソソルビトール系の溶剤
類、エチルアルコール、イソプロピルアルコール
等の低級アルコール、エチレングリコール、ポリ
エチレングリコール、グリセリンの如き多価アル
コール類、その他動植物油類、油脂類、ノナン酸
の如き高級脂肪酸類、パラフイン類などが挙げら
れる。
シクロデキストリン類に液状の経皮吸収促進剤
を包接させた包接化合物は、薬物含有高分子化合
物中に分散される。
ここで高分子化合物としては、包接化合物を分
散状態で保持するものであつて、常態で感圧接着
性を有さない高分子化合物(A)、常態で感圧接着性
を有する高分子化合物(B)が好ましいものとして挙
げられる。
(A)に属する高分子化合物としては、軟質ポリ塩
化ビニル、軟質ポリアミド、ポリオレフイン、ポ
リ酢酸ビニル、ポリビニルアルコール、ポリアク
リル系樹脂、熱硬化又は室温加硫タイプのシリコ
ーンゴム、エチレン−酢酸ビニル共重合体などが
例示される。
(B)に属する高分子化合物としては、(メタ)ア
クリル酸−n−ブチル、(メタ)アクリル酸ヘキ
シル、(メタ)アクリル酸−2−エチルブチル、
(メタ)アクリル酸イソオクチル、(メタ)アクリ
ル酸−2−エチルヘキシル、(メタ)アクリル酸
デシル、(メタ)アクリル酸ドデシル、(メタ)ア
クリル酸トリデシルの如き(メタ)アクリル酸エ
ステルと、該エステル類と共重合可能な(メタ)
アクリル酸、(無水)マレイン酸、(無水)イタコ
ン酸、フマール酸、(メタ)アクリル酸ヒドロキ
シエチル、(メタ)アクリル酸ヒドロキシプロピ
ル、(メタ)アクリルアミド、ジメチル(メタ)
アクリルアミド、(メタ)アクリル酸メチルアミ
ノエチル、(メタ)アクリル酸メトキシエチルの
如き官能性モノマー及び/又は(メタ)アクリロ
ニトリル、酢酸ビニル、プロピオン酸ビニル、バ
ーサチツク酸ビニル、スチレン、ビニルピロリド
ン、塩化ビニルの如きビニルモノマーとの共重合
体が挙げられる。その他シリコーンゴム、ポリイ
ソプレンゴム、ポリイソブチレンゴム、スチレン
−ブタジエン(又はイソプレン)−スチレンブロ
ツク共重合体ゴム、アクリルゴム、天然ゴムなど
を主成分とするゴム系粘着性物質、ポリビニルエ
ーテル、ポリビニルアルコール、ポリ酢酸ビニ
ル、ポリビニルピロリドンなどを主成分とするビ
ニル系粘着性物質なども使用できる。
又これらの高分子化合物に溶解及び/又は結晶
状態で分散される薬物としては、経皮吸収性があ
り、しかも上記の経皮吸収促進剤と共に放出しう
る性質を有するものが用いられる。
このような薬物としては、例えば以下に列挙す
るものが挙げられ、これらの薬物は2種類以上を
併用することもできる。
(イ) コルチコステロイド類:例えばハイドロコー
チゾン、プレドニゾロン、パラメタゾン、ベク
ロメタゾンプロピオネート、フルメタゾン、ベ
タメタゾン、ベタメタゾンバレレート、プロピ
オン酸ベクロメタゾン、デキサメタゾン、トリ
アムシノロン、トリアムシノロンアセトニド、
フルオシノロン、フルオシノロンアセトニド、
フルオシノロンアセトニドアセテート、プロピ
オン酸クロベタゾールなど、
(ロ) 鎮痛消炎剤:例えばアセトアミノフエノン、
メフエナム酸、フルフエナム酸、インドメタシ
ン、ジクロフエナツク、ジクロフエナツクナト
リウム、アルクロフエナツク、オキシフエンブ
タゾン、フエニルブタゾン、イブプロフエン、
フルルビプロフエン、ケトプロフエン、サリチ
ル酸、サリチル酸メチル、l−メントール、カ
ンフアー、スリンダツク、トルメチンナトリウ
ム、ナプロキセン、フエンブフエンなど、
(ハ) 催眠鎮静剤:例えばフエノバルビタール、ア
モバルビタール、シクロバルビタール、トリア
ゾラム、ニトラゼパム、フルニトラゼパム、ロ
ラゼパム、ハロペリドールなど、
(ニ) 精神安定剤:例えばフルフエナジン、チオリ
ダジン、ジアゼパム、フルジアゼパム、フルニ
トラゼパム、クロルプロマジンなど、
(ホ) 抗高血圧剤:例えばクロニジン、塩酸クロニ
ジン、ピンドロール、プロプラノロール、塩酸
プロプラノロール、ブフラノール、インデノロ
ール、ニバジビン、ロフエジキシン、ニプラジ
ロール、ブクモロール、ニフエジピンなど、
(ヘ) 降圧利尿剤:例えばハイドロサイアザイド、
ベンドロフルナサイアザイド、シクロベンチア
ザイドなど、
(ト) 抗生物質:例えばペニシリン、テトラサイク
リン、オキシテトラサイクリン、硫酸フラジオ
マイシン、エリスロマイシン、クロラムフエニ
コールなど。
(チ) 局所麻酔剤:例えばリドカイン、ベンゾカイ
ン、アミノ安息香酸エチルなど、
(リ) 抗菌性物質:例えば塩化ベンザルコニウム、
ニトロフラゾン、ナイスタチン、アセトスルフ
アミン、クロトリマゾールなど、
(ヌ) 抗真菌物質:例えばペンタマイシン、アムホ
テリシンB、ピロールニトリン、クロトリマゾ
ールなど、
(ル) 自律神経用剤:例えばスコポラミン類、ア
トロピン、臭化メチルアトロピンなど。
(ヲ) ビタミン剤:例えばビタミンA、ビタミン
E、ビタミンK、エルゴカルシフエロール、コ
レカルシフエロール、オクトチアミン、リボフ
ラビン酪酸エステルなど、
(ワ) 抗てんかん剤:例えばニトロゼパム、クロ
ナゼパム、メプロバメートなど、
(カ) 冠血管拡張剤:例えばニトログリセリン、ニ
トログリコール、イソソルビドジナイトレー
ト、エリスリトールテトラニトレート、ペンタ
エリスリトールテトラニトレート、プロパチル
ニトレート、ジピリダモール、モリシドミンな
ど、
(ヨ) 抗ヒスタミン剤:例えば塩酸ジフエンヒド
ラミン、クロルフエニラミン、ジフエニルイミ
ダゾールなど、
(タ) 鎮咳剤:例えばデキストロメトルフアン、
テルブタリン、エフエドリン、塩酸エフエドリ
ン、サルブタモール、イソプロテノールなど、
(レ) 性ホルモン:例えばプロゲステロン、エス
トラジオールなど、
(ソ) 抗鬱剤:例えばドキセピンなど、
(ツ) 脳循環改善剤:例えばビデルギン、エルゴ
ツトアルカロイド、イフエンプロジルなど、
(ネ) 制吐剤、抗潰瘍剤:例えばメトクロプラミ
ド、クレボプライド、ドンペリド、スコポラミ
ン、臭化水素酸スコポラミン、5−フルオロラ
ウシル、メルカプトプリンなど、
(ナ) 生体医薬:例えばポリペプチド類(TRH、
LHRHの誘導体)、プロスタグランジン類な
ど、
(ラ) その他:例えばフエンタニール、デスモブ
レシン、ジゴキシンなど。
高分子化合物と薬物との混合割合は、用いる高
分子化合物、薬物、包接化合物中の経皮吸収促進
剤の種類又は量によつても異なるが、例えば0.05
〜50mg/24hrsの薬物が皮膚又は粘膜に供給され
るような量とすることが望ましく、通常は両者の
混合物中に占める薬物の割合が0.01〜30重量%と
なるようにすることが好ましい。
この薬物含有高分子化合物に分散配合する上述
の包接化合物の割合は、配合物全量に対し1〜50
重量%の範囲から選択することが望ましい。包接
化合物の割合がこの範囲より少ないときは所望の
経皮吸収促進効果が得られず、一方この範囲より
多いときは薬物の吸収が加多となつて、持続性に
欠けるようになる場合がある。
本発明の外用医薬部材は、この薬物含有高分子
化合物中に上記包接化合物を分散させた組成物か
らなるが、皮膚又は粘膜への適用を容易にするた
め、下記、のような工夫を施すことが望まし
い。
この組成物をフイルム又はシート状に成形す
る。
皮膚又は粘膜への適用に際しては、このフイ
ルム又はシートをばんそう膏状のもので皮膚又
は粘膜に固定すればよい。又このフイルム又は
シートの表面に感圧性接着剤を塗布しておい
て、皮膚又は粘膜に貼付するようにすることも
できる。
この組成物を柔軟な平面状の担持体の表面に
層状に設けて積層構造とする。
ここで担持体としては、水分と実質的に透過
させないか、或は皮膚又は粘膜にかぶれ、白化
などの副作用を起こさせない程度に水分の透過
度合を設計したプラスチツクフイルム・シー
ト、金属箔、織布、不織布、或はこれら金属
箔、織布、不織布とプラスチツクフイルムとの
ラミネート物、金属蒸着したプラスチツクフイ
ルムなどが用いられる。
皮膚又は粘膜への適用に際しては、高分子化
合物として常態で感圧接着剤を有するもの、又
は水分によつて感圧接着性を有するものを用
い、上記組成物の層自体が粘着性を有するとき
は、その組成物の側を皮膚又は粘膜に貼付すれ
ばよい。又高分子化合物としては常態で感圧接
着性を有しないものを用い、上記組成物の層が
粘着性を有しないときは、その組成物の側を皮
膚又は粘膜に当てると共に担持体の外側から粘
着性テープ等で皮膚又は粘膜に固定すればよ
い。
なお、上記においては、担持体の表面に形成
した組成物層の上から、剥離処理を施したポリエ
チレンテレフタレートの如き可撓性を有する剥離
フイルムを被覆して該層の表面を保護することが
好ましい。
本発明の外用医薬部材は、水分不透過性に優れ
る包装用資材、例えばアルミニウム箔とプラスチ
ツクフイルムのラミネートフイルムからできた袋
に一個づつ又は一括して収納し、保存・流通に供
するのがよい。
(e) 作 用
次に本発明の作用をその機構も含めて詳細に説
明する。
本発明においては、包接化合物のホスト分子と
してシクロデキストリン類を用いている。シクロ
デキストリン類は、ゲスト分子である液状の経皮
吸収促進剤を解離した後もその構造を崩さないと
いう性質を示す。
シクロデキストリン類をC、経皮吸収促進剤を
A、AがCに包接された包接化合物をA・Cと
し、組成物中におけるこれらの濃度をそれぞれ
[C]、[A]、[A・C]で表すとき、
C+AA・C
の平衡関係が成立し、又A・Cからのゲスト分子
Aの解離は、
Kα=[C]×[A]/[A・C]
で表わされる解離定数Kαにより支配されると考
えられる。
従つて、解離定数Kαはそれぞれの系において
固有の値を持つ定数であるがゆえに、解離したA
が体内に吸収され減少するに伴つて組成物のAの
濃度[A]が減少すると、Kα値を一定に保つた
めCと包接状態となつているAの解離が促進され
解離平衡を保とうと働き、又逆にAが過剰に解離
しようとしても、解離定数Kαを一定に保つため
に包接状態の濃度[A・C]が高くなるように働
き過剰の解離を妨げるものと思われる。
又Cの濃度[C]を高くすることによつて、解
離したAの濃度[A]を低く押さえることが可能
となる。
このようにCの濃度[C]、Aの濃度[A]を
調整することによつて、Aが組成物中より皮膚中
へ放出された量に応じて、Aが必要量のみA・C
より解離される。
本発明の外用医薬部材を皮膚又は粘膜に適用す
ることにより、皮膚又は粘膜から分泌される水分
によつて包接化合物から経皮吸収促進剤が解離す
ると共に、それに応じて薬物含有高分子化合物中
の薬物が皮膚又は粘膜から体内に浸透する。
(f) 実施例
次に実施例を挙げて本発明を更に説明する。以
下「部」とあるのは重量部を示すものとする。
実施例 1
2−エチルヘキシルアクリレート97部とアクリ
ル酸3部よりなるモノマー混合物をアゾイソブチ
ロニトリル0.2部の存在下でトルエン媒体中にて
65℃に昇温して重合させ、アクリル系共重合体を
得た。
一方、70℃に加熱したα−シクロデキストリン
10-2Mを含む20%(V/V)イソプロピルアルコ
ール水溶液を徐々に冷却することにより、α−シ
クロデキストリン・イソプロピルアルコール1:
1包接化合物を得た。
上記で得たアクリル系共重合体100部にこの包
接化合物50部とイソソルビドジナイトレート5部
とを混合して均一に分散させて組成物とした後、
この組成物をポリエステルフイルム上に乾燥後の
厚みが200μmとなるように塗布、乾燥させ、更
にその組成物層の上から剥離処理を施したポリエ
チレンテレフタレートフイルムを被覆して、外用
医薬部を得た。
実施例 2
2−エチルヘキシルアクリレート55部、2−メ
トキシエチルアクリレート25部及び酢酸ビニル20
部よりなるモノマー混合物を過酸化ベンゾイル
0.3部の存在下で酢酸エチル・トルエンの1:1
(V/V)混合媒体中にて60℃に昇温して重合さ
せ、アクリル系三元共重合体を得た。
一方、β−シクロデキストリン10-1Mを含む水
溶液中に当該シクロデキストリン2倍モル相当の
ノナン酸を徐々に加えて約24時間振とうすること
により、β−シクロデキストリン・ノナン酸
1.5:1包接化合物を得た。
上記で得たアクリル系三元共重合体100部にこ
の包接化合物40部とニトラゼパム0.5部とを混合
して均一に分散させて組成物とした後、この組成
物を軟質ポリ塩化ビニルフイルム上に乾燥後の厚
みが200μmとなるように塗布、乾燥させ、更に
その組成物層の上から剥離処理を施したポリエチ
レンテレフタレートフイルムを被覆して、外用医
薬部材を得た。
実施例 3
メチル化したβ−シクロデキストリン10-1Mを
含む水溶液中にメチルオクチルスルホキサイドを
少量づつ滴下し、24時間振とうすることにより、
メチル化β−シクロデキストリン・メチルオクチ
ルスルホキサイド1:1包接化合物を得た。
上記で得た包接化合物50部、ケン化度98モル%
のポリビニルアルコール100部及び臭化水素酸ス
コポラミン5部を含む水溶液をガラス板上で流延
し、乾燥後剥離して、厚み100μmの外用医薬部
材を得た。
比較例 1〜3
シクロデキストリン類と経皮吸収促進剤とから
なる包接化合物に代えて、同量の経皮吸収促進剤
を用いたほかは実施例1〜3と同様にして外用医
薬部材を作成した。実施例1に対応するものを比
較例1、実施例2に対応するものを比較例2、実
施例3に対応するものを比較例3とする。
実験例
実施例1〜3及び比較例1〜3で得た外用医薬
部材につき。その血中濃度、保持力、糊残り、経
皮吸収促進剤ブルーミングを下記のようにして測
定した。ただし実施例3及び比較例3について
は、外用医薬部材に感圧接着性がないので保持力
及び糊残りの測定は不可能であり、皮膚面への貼
り付けには外科用テープを用いて固定を行つた。
結果を第1表に示す。
「血中濃度」
各外用医薬部材を3cm径に裁断して試験片と
し、予め除毛したラツトの腹部に貼り付け、4時
間、8時間、24時間後のそれぞれにラツトの血液
を抜き取り、ガスクロマトグラフイー、高速液体
クロマトグラフイー、或はガスクロマトグラフイ
ー・マススペクトル装置を用いて薬物血中濃度を
測定した。
「保持力」
外用医薬部材から1cm巾、10cm長さの試験片を
作成し、これをベークライト板端に2cmだけ貼り
付け、試験片の他端に300gの荷重をかけ、40℃
の恒温器化で凝集破壊にて試験片がベークライト
板から落下するまでの時間を測定した。
「糊残り」
各外用医薬部材を上腕側部に貼り付けて、24時
間経過してからはがす際に適用皮膚面に糊残りが
生じるか否かを判定した。
「経皮吸収促進剤ブルーミング」
各外用医薬部材につき、使用時剥離シートを糊
面よりはがす際に剥離シート上に経皮吸収促進剤
がブルーミングしているか否かを目視判定した。
