使用する新規方法並びにリユーマチの長期治療及
び/又は予防法に関する。 リユーマチは広く蔓延した病気である。米国に
おいてだけでも、約3千万人の患者が関節炎で苦
しんでいる。雑誌「タイム」第33号、第41頁、
1982年8月16日発行参照。関節炎協会(3400ピー
チトリー・ロード、N.E.アトランタ、ジヨージ
ア30326)発行の“リユーマチ会報”第32巻、第
1号(1982年)、第1頁にリユーマチ様関節炎に
関する周知の治療法が記載されている。 ビタミン類なる集合概念で要約される薬剤は周
知である。ヘルマン・レンプ教授の著書“ヘミ・
レキシコン”フランクヘツシエ書店、スタワトガ
ルトを参照。 本書にはビタミン類の短期投与によつてもまた
高単位投与及び/又はいわゆるビタミン類の大量
投与によつても、リユーマチ様関節炎の病状を改
善することは出来ないと記載されている。“関節
炎と関連する症状、リユーマチ学の教科書”著者
ジヨゼフ・リー・ホーランダー、M.D.、第7版、
(欧州では)ヘンリー・キンプトン発行、ロンド
ン、1966年、特に主章37,“リユーマチ様関節炎
における食事とビタミン類”ロナルドW.ラモン
ト・ハーバース、M.D.参照。 ところが驚くべきことに、特定のビタミン群を
長期投与することで、リユーマチ性疾病を長期に
にわたつて、予防、回復及び/又は治癒すること
で極めて効果的に治療できることが判明した。 すなわち、本発明は、 (a)ビオチン、(b)ビタミンB1、(c)ビタミンB2、
(d)ニコチンアミド、(e)ビタミンB6、(f)ビタミン
B12、及び(g)パントテン酸又はパントテニルアル
コール又はそれらの塩からなるビタミン群を有効
成分とするリユウマチ性疾病治療剤である。 本発明はリユーマチ性疾病の長期治療及び/又
は予防に際して、特にこれが炎症性と変質性の種
類であつて、関節、筋肉、腱及び運動器官のその
他の部分に現われた場合に、各種ビタミンを個々
に又は組み合わせて使用する新規方法に関する。
かかる疾病を例示すれば、関節炎、慢性の多発関
節炎、初期の多発関節炎及び一般的な関節炎、と
くに慢性及び/又は急性の関節炎である“多発関
節炎”なる用語には、これらの疾病(複数)とそ
の病原学上全ての可能な発生形式の集合的概念が
相当するのであるが、ここではリユーマチ様関節
炎、初期の慢性多発関節炎、骨関節炎又はこれら
疾病のそのほかの種類と変形、症状及び症候群
100種のうちいずれか1種を対象とする。以下に
おいては、多発関節炎の上位概念及び集合概念を
用いることにする。 長期治療は次の何れかの方法で実施することが
できる。そのうち主たるものは系統的な方法、注
射、及び局所治療であつて、これらのうち最もよ
く使用されているのは経口投与である。リユーマ
チ治療剤として作用する薬剤は経口及び/又は注
射によつて、一日当り約0.25〜2mg/Kg・体重、
好ましくは0.5〜1mg/Kg/日を調剤して投与す
るが、この際これより多量又は少量投与すること
も可能であつて、その量は医師等の専門家によつ
て容易に決定できる。ビタミンの組み合わせにお
いては、個々のビタミン量を加減することができ
る。すなわち薬剤は個々の用量として、又は数種
類の分包にして投与することもできる。好適に経
口投与するには投与形態として、通常の使用形
態、例えばカプセル、糖衣錠又はタブレツト等が
考えられる。このような投与形態の製造は通常の
方法で実施可能である。 本発明に則つた使用を行なうに際しては、患者
を長期にわたつて本発明に則つて治療する必要が
ある。ここで“長期”なる意味は、一般に治療す
べき疾病の程度、患者の一般状態及び患者の体重
と年齢に依存する。専門家なら病状が回復するま
で、又は完全治癒するまでにどれ位の期間治療を
続ける必要があるか容易に判る。場合によつては
薬剤又は薬剤の組み合わせの日々の投与を1週間
又は1ケ月、あるいはそれ以上中断し、次に少量
の投与を継続することができる。治療すべき個人
の特殊性に応じて、好ましい長期治療期間は少な
くとも5ケ月、時に少なくとも6ケ月、好ましく
は1ケ年、更に好ましくは少なくとも2ケ年であ
ることが発見されている。またある場合には治療
すべき患者に対して長期治療を生涯継続すること
の極めて有利なことが示されている。 一般的にリユーマチ性疾病、特に多発関節炎、
更に初期の慢性多発関節炎患者の治癒及び/又は
抑制及び/又は防止(予防)を行なうには、本発
明に則つて使用する薬剤又は本発明に則つて使用
する薬剤の組み合わせを、長期にわたつて規則的
に、中断せずに連続的に、毎日投与することが極
めて有利である。 ここで言うビタミン又はビタミン類とは主とし
てビタミン−B−コンプレツクスに属し、本概念
の定義については、例えば冒頭に記載した“ヘ
ミ・レキシコン”を参照されたい。 本発明に則つて使用するには、ビタミン又はビ
タミン類の組み合わせに少なくとも1種のミネラ
ル及び/又は少なくとも1種の微量元素及び/又
は少なくとも1種の添加剤、特に担体物質、酸化
防止剤、抗細菌剤を組み合わせる。 以下に本発明に則つて新たに使用可能な個々の
化合物を示すと共に、その同義語、生物学的な前

グ(現在レニングラード)、ソ連領で生れ、ロシ
ア・フインライド人の混血であり、1924年来チユ
ーリツヒ(スイス)に居住している。 彼女の両親と兄弟姉妹は、1934年来レニングラ
ードとの接触が許可にならないので不明である。
彼女の家族に今まで如何なる病気が発生したの
か、しなかつたのかについても、Aは1982年現在
何も承知していない。 Aは高年になるまで心底から丈夫であつた。
1964年になつて74才の時に盲腸手術を受けたが、
驚くべき軽さで切り抜けたので、担当の主治医は
1週間たつまでは退院を許さなかつたが、病院か
ら直接休暇旅行に出かけることをはつきり許可し
たほどであつた。1982年の初めまでAは全部自分
の歯をもつていた。 Aは身長約153cm、愛らしい容姿の持主であつ
た。彼女は生涯喫煙しなかつたが、いつも“美食
を愛し”て肉食を好み、アルコール類はグラス一
杯のワインを食事時にだけとる程度の適度に飲酒
するのを常とした。1972年の彼女の体重は約47Kg
であつたと推定される。今から10年前の1972年の
ことであつたが、右肩と右膝関節に“リユーマチ
性の痛み”を訴え始めた。1948年来Aは寡婦にな
つたが、自分の町に対する愛着にふけるくせがあ
つて、その間彼女は毎日相当の距離(約10〜12
Km)を徒歩で歩いた。右膝に始まつた痛みは、町
での毎日のこの大変延びた散歩をおのずと短縮す
る結果になつた。疾病は徐々にしのびより、数年
以上にも亘つて緩慢にではあるが昂進した。 長い期間(多分1973〜1975年)の間に彼女は家
庭医師に診察を頼んで、そのあと各種のNSAID
−リユーマチ治療剤を服用した。例えばアスピリ
ン、イルガピリン、ブタゾリジン、ボルタレンそ
のほか多くの薬であつて、これらは医師によつて
処方されたものであつたが、陽性の結果は表われ
なかつた。それから1975年頃、医者が患者に一連
の多数の注射(余り多すぎて思い出させない程の
数)を直接膝に注射(ある種の黄金製剤)するよ
うになつたとき、彼女はまだ大変なしつかりもの
だつたので、自分にとつて苦痛のあるこの治療
が、ある期間たつたあとで(どの位の期間か、ま
たどれ位注射したあとかは判らないが)我慢でき
なくなつて、多発関節炎の治療をこれ以上受ける
ことをあつさりあきらめて、むしろ“リユーマ
チ”の激しい痛みと共に余生を送るか、又はその
ために死ぬことまで無造作に決心した。このこと
はAの生涯において、彼女が常に極めてきわだつ
た生活力を所有して、少なくとも100才まで生き
ることを念頭においている限り極めて重要な決心
であつたと言える。 すでに述べた通り、彼女の多発関節炎の進行は
徐々にであつたけれども、進行性であつて、Aが
規則的な間隔(週に1回)をおいて診察を受けて
いたために、このことはよく観察されていた。 1976年頃以来患者に衰弱の状態が認められ、こ
れは初め簡単に老齢のためであつて多発関節炎の
ためではないと解釈されたが、彼女が更に詳しく
朝に指が硬直することを初めて訴えるに及んで、
多発関節炎が初めと考慮された。この“朝の指の
硬直”が始まると起床直後の衣服の着用、朝の洗
顔と朝食の準備ができなくなつた。Aは一部屋の
住居に一人暮しで、こまごました家政は自分で始
末した。彼女はまる一時間と言わぬまでも、少な
くとも半時間は、訓練によつて指をやわらかくす
る努力をしなければならないと思つていると話し
た。そこへ更に手の関節と指先の関節(親指の関
節は例外)にすぐに疲労と痛みが発生する傾向が
著しくなつてきた。痛みのある指の肢節(最先
端)の皮膚はピンとはれあがつて赤〜紫色にな
り、炎症をおこしたようにふくれて、全体がひど
く痛んだので、誰もみなAの指に触れなければよ
いがと考えた。 親指も含めて指の爪はだんだんもろく、またな
んとなく“うろこ状”になつて、爪床から爪が容
易にとれ、いつもと異なる形が現われはじめた。
特に両方の親指の爪はまくれあがる傾向(硬皮
症)を示した。右の手首関節のすぐそばで、親指
肢節の最上位に大きくて痛みのはげしい、余り硬
くない腫瘍(動くのか)が生成し始めた。発熱し
なかつた。真の体温を測定するのに患者を静養さ
せることもしなかつた。 右肩と右膝は彼女を疑いもなくひどく痛めつけ
たので、例えば彼女は右肩を動かさなくてすむよ
うに左手で食事をとり始めた。これ以後数年にわ
たる病気にも拘らず、この老女はがんこであつ
て、如何なる薬ものむことを徹底的に断り、“軽
い”鎮痛剤だけを服用した。 この状態に対して、段々頻度をますようになつ
た衰弱状態と、大きな疲労徴候に対して彼女は強
壮薬(例えば老人保険薬)をのむ以外何もしなか
つた。このことは彼女にも判つたらしく、上に挙
げた“組み合わせ”の摂取を指示されたがそれ
を我慢して受けた。1976/77年頃からAは中断す
ることなく、規則的に“組み合わせ”のカプセ
ルを毎日摂取した。 “組み合わせ”の投与開始後すぐに最初の陽
性徴候として、衰弱状態が徐々にではあるが、消
失し始めるのが明瞭に認められた。一方関節炎に
よる痛みはつづいて、天候と気分によつて激烈か
ら我慢できない程度まで変動した。痛みは軽減せ
ず、両手、指、右膝及び右肩を動かすときの苦労
はもとのままであつたが、そのほかの一般的な容
態は、患者のこの状態から言う限り、年をとつて
老齢にも拘らず良好に推移しているように見え
た。そして何とかして“リユーマチ性”の苦痛に
対するよい薬をのむように彼女にしむける企て
を、彼女はがんとしてこばみつづけた。 “組み合わせ”の投与(患者Aは少なくとも
“組み合わせ”を8〜12ケ月毎日摂取しなけれ
ばならなかつた)数ケ月後になつて、だいぶ調子
がよくなつたと感じていること、肩と膝の痛みが
もはや間歇的になつたこと、両手と指の痛みがへ
つたこと等のかすかな徴候を示しはじめた。また
上に記載した症状の多くがこの患者ではしばしば
消失し始めたかに観察された。指の爪で最も注目
すべきは指の最先端の肢節であり、指の炎症は
徐々に減少した。Aは自分の身動きをひどく制限
した朝方に生ずる指の硬直性は段々短かくなつた
と話した。当時2時間つづいた硬直性は、今では
30分で消えるようになつた。指の爪は徐々にでは
あるが多少健康的な外観を再びとりもどし始めた
(硬皮症)。しかも最も病状がひどかつた両方の親
指の爪も、1982年11月には病気の回復時にみられ
るはつきりした変化の痕跡を示している。 数年前及びその後の数年間、Aの両手は痛みと
圧迫にひどく鋭敏だつたので、高い感性をもつて
いたが、代表的な老人の手であつて、多発関節炎
の徴候は全く認められかつた。今日患者のどこを
圧迫しても何の痛みもなく、患者は平気である。 右肩及び右膝に常に感じられた本来の痛み(A
はこの痛みを、目前にせまつた天候の急変のせい
にした)は、多分消耗性現象と思われるが、しか
し炎症をおこしてはいない。 さらに注目すべき事実が次の事柄に認められ
る。すなわち“組み合わせ”のお陰で多発関節
炎を克服したように見えてからは、高齢であるに
も拘らず1981年には僅かずつではあるが、明らか
に体重の増加が認められ、しかも食物摂取量が比
較的に見て段々少なくなりつつある場合におきた
ことである。増加は多分2.5〜3Kg程度であろう。 以上多発関節炎の経過と、“組み合わせ”に
よる克服の経過をAについて説明した。 Bの場合 男性Bは1920年5月24日にソ連領アトカルスク
(ボルガ河畔)で貧困の家庭に生れた。Bの家族
は、1924年(スイス人なので)好ましからざる外
国人として、ソ連から追放された。それ以来Bは
スイスに居住している。 Bの父親は59才(1948年)で肺ガンで死亡し
た。年上の姉2人は夫々45,58才で乳ガンで死亡
している。 多発関節炎は父と姉2人にはあらわれなかつ
た。Bの母は若いとは言えない中年になつて、初
めて多発関節炎にかかつた。Bの場合約9年前の
1973年に多発関節炎を示唆する最初の徴候が現わ
れた。すなわち右人指し指の中間関節に生じた特
異な、中程度の硬さをもつひどく痛みのある腫脹
が全く直りにくかつた(直りにくさは今日“組み
合わせ”による治療で僅かな部分だけになり、
しかも無痛である)。この現象に対しては誰も注
目しなかつた。ところが約3年前の1979年に、B
は毎朝目を覚すと指が硬直しているのに気づき始
めた。数ケ月後には全ての最先端の指関節(親指
を除く)に赤色で痛みを伴う、ぱんぱんにふくら
んだ腫脹が現われ、1981年の初頭には極めて不快
な障害になつたので、Bは手書き又はタイプの操
作能力が全く失われてしまうのではないかと恐れ
る程であつた。最先端の指関節はひどく肉厚にな
り、指の先端に蟻走感をひどく感じるようになつ
た。両手は感じが鋭敏になつてひどく痛んだの
で、Bは挨拶のために相手に手を差出すのを遠慮
した。Bは手と指の痛みにひどく悩み、何を企て
ても決然として反対したが、最後には“組み合わ
せ”を処方される結果になつた。 “組み合わせ”の最初のカプセル30錠ではま
だ期待通りの結果が現われず、その上本調剤に特
有な特異物が発生した。すなわち顔面の一定部分
が斑点状に不健康な赤色に変色して、ちようど
“花が咲いたよう”になりはじめたので、“組み合
わせ”の投与を中止せざるを得なくなり、その
結果、このアレルギーは速やかに消失した。また
“組み合わせ”と“組み合わせ”の使用を交
互に避けた。1975年7月に右小指の骨を折つて骨
折の手術を受けた。“ねじ”をとり去つたあと、
傷を受けた関節には暫くの間なお軽い痛みを感じ
た。約3年後になつて、第一の指関節、すなわち
手術をうけ関節が炎症と硬化の徴候を示し始め
た。右小指のこの指関節の炎症は、純粋に外傷的
な原因をもつていたが、長い間継続してほかの7
本の指まで似たような炎症と痛みを示すようにな
つた。 不愉快な現象を克服するのに、この場合充分に
意識して“組み合わせ”/“組み合わせ”
(毎日タブレツト1錠、Bは腎臓結石の“経験”
があり、発泡タブレツトはカルシウムを含むので
投与を思い留まつた)を与えた時、次のことが観
察された。すなわち約8〜12ケ月後になつて先ず
第一に関節と外傷を受けた右小指の最先端の肢節
が治癒して、残つている関節の結節は強く押して
も全く痛まないようになつたことである。 更に8〜10ケ月経つた後、関節炎と8本の指関
節(最先端)の炎症の徴候も消失した。ただ“組
み合わせ”/“組み合わせ”の投与を暫くの
間(約10〜14日間)中止すると、原発性炎症のプ
ロセスが発生していた7本の指にただちに関節痛
が発生したが、これは激しい急性の症状を伴つて
は現われず、一方外傷のある右小指はこの中止に
は反応しなかつたことを記載しておく必要があ
る。 数回にわたつて実施した本剤の中止テストは10
〜14日後にはいつも中断するはめになつたが、新
しい炎症と痛みに対する危険をまねこうとはしな
かつた。 数年前Bはしばしば多発関節炎性の痛みを左肩
と左足関節に度々感じたが、この痛みは長くは続
かなかつたので、それには余り注目しなかつた。
しかし、この特発的に表われた多発関節炎に起因
する痛み(いわば副作用としてであるが)が完全
に消失したことと、この痛みがBに無痛の、ある
いはこれ以上痛まない多分骨化した腫脹を左脚中
間の骨に残していたことが確かめられた。 更にここでBの指の多発関節炎が消失したこと
と関連して、今1つの現象が認められる。すなわ
ちいく本かの指の最先端の関節に結節が明瞭に残
つていたことで、これらの結節は“組み合わせ
”/“組み合わせ”の摂取を継続する限り、
今日完全に無痛で残つている。これの明確な結節
は徐々に、極めて徐々に生成するものである。比
較用のレントゲン写真が存在しないので、本現象
を暫定的に証明することはできないが、上に述べ
た右人指し指のデレンク上の特異な腫脹は明白に
衰退していたが、ずつと以前から、すなわち“組
み合わせ”/“組み合わせ”の摂取数ケ月後
になつて初めて決定されていたものである。 その他;Bは20年来喫煙せず、“組み合わせ
”/組み合わせ”以外に、やむをえない場合
にだけ特発的に頭痛薬(トノパン)を飲んで、少
なくとも前ぶれのある多発関節炎を克服したあと
では、全く健康であると感じている。 Bの今までの生涯では健康上2つの出来事がき
わだつている。すなわち1949年に閉鎖性TBC(関
節レントゲン撮影)にかかり、これは完全に治つ
た。1968年に大きな交通災害の被害者になり、そ
の後遺症として左腕の複雑な骨の破片を体内にも
つことになつたが、これら撓骨と尺骨の破片は骨
接合術(ねじ13個のついた2枚のチタン板)で人
工的に手際よく補修された。Bは2枚の板を合計
13個のねじで1982年にも骨に止めていたが、これ
で少しの障害も感じなかつた。 これら両場合から判かることは、例示した“組
み合わせ”、“組み合わせ”及び“組み合わせ
”の長期使用によつて、一目でわかる多発関節
炎を苦痛と苦労のない、すなわち潜伏性の状態に
することができるのに対して、いわゆる治療の最
初の月での大量投与によつては奇妙にも陽性、陰
性の両効果とも生じないことが明らかになつた。
短期と中期で本グループ調剤を使用しても、多発
関節炎に関しては陽性の結果は決して現われな
い。 これらを基礎にして、上記調剤と、本調剤の全
グループを用いる数ケ月に及ぶ治療(予防)を特
別に強調することに対して重大なる価値をおくも
のである。 次表では上に記載したA,B両人に関する報告
