【目的】 前立腺肥大症等の凍結手術により、組織の大
量凍結壊死を防止し、麻酔の簡易化と、更に、術後の逆
行性射精をなくすか、部分的におさえ、凍結壊死組織の
剥離による大量出血の防止を目的とする。 【構成】 凍結手術用プローベ１の全周に亘る凍結部２
を一方の側面のみの、細長の金属凍結プレートとし、更
に、患部えの作用部位を正確に行うため、フレキシブル
の液体窒素輪送チューブ９を、取り出し部分で回転でき
るようにした。

Description
【０００１】
本考案は、前立腺肥大症の凍結手術を、より容易に、安全に行うための考案で ある。
【０００２】
従来の、凍結による前立線肥大症の手術は、全身麻酔、脊椎麻酔、仙骨麻酔、 又は、硬骨膜麻酔の下で行い、膀胱の出口を腫大占拠している腺腫を、その全周 に亘り、凍結壊死させて、除去するものである。
【０００３】
従来の凍結手術には、次の解決すべき欠点がある。即ち、 （イ）広範囲な凍結により、大量の凍結壊死組織を生ずる。 （ロ）この為に必発する、炎症性腫脹に対して、術後の尿道の確保と、止血のた めに、尿道カテーテルの留置を必要とする。 （ハ）１〜２週間後に、凍結壊死組織の剥離による、大量出血の可能性がある。 （ニ）大がゝりな麻酔が必要である。 （ホ）内尿道口部全周の凍結壊死であるため、術後、逆行性射精となる。

（イ）従来のプローベ 図２ の使用による、凍結部位全周に亘る、広範囲な、 腺腫の凍結壊死を防ぐため、排尿困難や、残尿をなくすために必要な、最低限度 の側溝を作る程度に 凍結部位を制限し、周辺組織を保存すべく配慮した。 （ロ）更に、従来のプローベは、回転不能であったが、必要な選定部位に、正確 に、凍結プレートが作用できるように、回転可能とした。

凍結部位を限定することにより、内尿道口の損傷が 部分的となり、併せて、 簡単な麻酔ですむことになる。例えば、冷却による麻酔効果も見られるので、前 以て、基礎麻酔としての 鎮痛剤の投与（経口、座薬、注射など）を行い、表面 麻酔剤入りの粘滑剤で尿道を、そして、膀胱内えの局所麻酔の注入などで充分で あり、大がゝりな麻酔の必要がなくなる。

凍結手術のための局所麻酔後、内視鏡検査により、腺腫の状態を確認して、尿 道の長さを計測し、この長さに合せてプローベを挿入し、ガイドボタンの触診に より、更に正確な位置に保持して、要部のみに、凍結部（Ｆｒｅｅｚｉｎｇ Ｐ ｌａｔｅ）を当てて 凍結手術を行う。図４は前立腺肥大腺腫に、本プローベを 挿入した状態を示す。図５の（ａ）は腫大・閉塞した尿道前立腺部を示し、（ｂ ）は手術によって作られた側溝（５）を示したものである。これで、排尿困難や 残尿をなくす意味からでは、充分と思われるが、不充分な場合は、この溝を拡大 するか、（ｃ）のように両側に溝を作ってもよい。

本考案のプローベによる手術の利点は、（イ）前立腺肥大腺腫の凍結壊死部を 、必要個所のみに限定し、広範囲な凍結壊死組織の剥離による、大量出血を防止 することができる。更に、（ロ）麻酔の容易さに加えて、内尿道口の損傷を減ら すことにより、従来の各手術法による、腺腫の全部、もしくは大部分を除去する ためにみられた、（ハ）精丘よりの逆行性射精をなくすか、又は、部分的におさ えることができ、充実した性生活、ＱＯＬの維持が可能である。なお、（ニ）本 プローベは、前立腺肥大腺腫に限らず、従来の周囲組織を保護するための誘導筒 などを用いなくても、痔核、食道静脈瘤、その他の疾患にも適用できる。
