Description

【発明の詳細な説明】 体内血管の再構築後に発生する再狭窄を予防する方法令までに、部分的に閉塞し た血管を再疎通すべく、多くの方法が開発され、それらは、血管の拡張や、閉塞 物質の除去によるものであった。これらの方法は単独でも、あるいは組み合わせ ても利用できる。これらの方法としては、バルーン法、レーザー法、温度法、超 高波法、非バルーン法(例えば、atherectory)やステントを用いた 方法などがある。しかしながら、これらの方法によると、血管壁の機械的な障害 が起こり、それにより遅かれ早かれその損傷部位にあるいはその近(に望ましく ない再狭窄を高率で発生する。この状況は血管形成(狭窄の減少)のための技術 とその適用において示された高度な知識にもかかわらず広まっている。
多くの研究が損傷した動脈壁中の平滑筋細胞の異常な増殖に関して行なわれてき た。この異常な増殖は、初期のアテローム性動脈硬化症の損傷の形成における場 合と同様に再狭窄においても、重要な因子としての血管形成によって引き起こさ れる。初期の閉塞の問題に対するアプローチとしては、ある程度の毒性反応を示 す光活性化される化学物質を投与することによって平滑筋細胞の増殖を阻害する という試みがあった。この研究は主にヘマトポルフィンとその誘導体について行 なわれた。例えば、ジヘマトボルフィンのエステルとエーテルは、非アテローム 性動脈硬化症の動脈由来の平滑筋細胞に比べ、プラク(plaque)細胞の増 殖をより大きく阻害した。このことは、プラク細胞に対してより大きい阻害活性 を有しているということを示唆している。(Dartschら、非アテローム性 動脈硬化症の動脈由来及びアテローム性動脈硬化症斑由来のヒト平滑筋細胞の増 殖に対するホトフィリンIIのインビトロでの影響“Arterioscler osis vollo、pp616.1990)。又ヘマトポルフィンのエステ ルやエーテルへの紫外線照射は、非アテローム性動脈硬化症の動脈細胞に比べ、 アテローム性動脈硬化症斑由来の平滑筋細胞をよく強く破壊することが解かって いる(Dartschら、ヒトの非アテローム性動脈硬化症の動脈と光照射後の 初期の狭窄領域からの平滑筋培養細胞の反応:血管狭窄に対する光化学治療との 関係、J、Am、coil、Cardiol、vol15.pp1545.19 90)。
又、他の研究により、ヘマトポルフィンがアテローム性動脈硬化症斑に選択的に 取り込まれていることが、光照射により蛍光を発したその斑の像が確認されるこ とにより示されている(米国特許第4577636号)。更に動物を用いた実験 により、光活性化されたヘマトポルフィンによってアテローム性動脈硬化症斑を 破壊するという試みが示されている(米国特許第4512762号及び第477 3899号)。
アテローム性動脈硬化症斑の選択的阻害や、破壊というこれらの結果にもがかわ らず、ヘマトポルフィン固有の性質は人への使用を躊躇させてきた。種々の癌に 対する処置の多くの報告によると、ヘマトポルフィンの著しい毒性が示されてき ておりヘマトポルフィンを全身的に投与する場合には、皮膚の光毒性を避ける注 意が必要であるということが示されている。ある研究によると、広がった転移性 膀胱癌の患者らにヘマトポルフィン誘導体を全身投与した場合は、その患者らは “処置後数週間”光にさらされないよう、保護する必要がある(Benson、 全膀胱へのへマドポルフィン誘導体の光力学治療による広がった転移性癌への処 置、J、Urol、vol 134.pp675.pp678..1985)。
癌治療における患者の光感受性の持続という点については同様の影響を以下の文 献に見ることができる。[)oughertyら、“転移性癌の処置における光 照射治療”、Cancer Res、vo138.pp2628..1,978 .(30日持続する)、Lamら、“低投与量のホトフィリンIIとを用いた初 期肺癌の検出、” Chest vo197.pp333,1990.(最近で も30日持続)、Proutら、“高転移性膀胱癌の処置におけるヘマトポルフ ィン誘導体による光力学的治療”、Now Eng、J、Med、vo1317 .pp1251.1987. (4から6週間持続)、Razumら、“皮膚の 光感受性・静脈中のへマドポルフィン誘導体、HpDとDHHの持続と強さ、“ Phochem、and Photobiol、、vo146.pp925.1 987 (7週間持続)。
このようにヘマトポルフィン類は、平滑筋細胞の増殖を阻害し、アテローム性動 脈硬化症に対して有用であるという性質を有している。けれども、ヘマトポルフ ィン類の光毒性は、現在のこの分野での知識によれば人へのその使用は薦められ ない。しかしながら、現在のところ、再狭窄の進行を減少させ、予防する有用で 効果的な方法はない。
本発明の目的は血管の拡張や閉塞物質の除去などの血管形成による、末梢のある いは心臓の血管系の再疎通後に発生する再狭窄の予防に対し新しいアプローチを 提供するものである。
更には、本発明の目的は末梢のあるいは心臓の血管系の再疎通後に発生する再狭 窄の予防として、紫外線照射によってわずかな毒性しか示さずかつ平滑筋細胞の 増殖を阻害する光活性化される薬剤を全身投与するという光化学治療法を提供す るものである。
更に詳しくは、本発明の目的は拡張した血管の部位で、異常増殖している平滑筋 細胞中に全身投与したソラレンを取り込ませ、それにカテーテルを用いて紫外線 を照射することにより、末梢のあるいは心臓の血管系の再疎通後に発生する再狭 窄を予防する方法を提供することである。
又、更には、本発明の目的は、バルーン血管形成による冠状動脈の再構築後に発 生する再狭窄を予防する方法として、光活性化されるソラレンを紫外線照射しな がら、平滑筋細胞に投与し、平滑筋細胞の増殖を阻害するという方法を提供する ことである。
今、光活性化されるソラレンの著しい平滑筋細胞増殖の阻害効果は、長波長紫外 線(PUVA)の照射によって起こることが解かった0本発明は、血管の再疎通 によって発生する再狭窄を予防する方法であって、光活性化されるソラレンを投 与し、少なくとも部分的に閉塞した血管を再構築し、再疎通した部位に紫外線を 照射するものである。ここで再疎通とは、血管の拡張、切除、閉塞物質の除去な どの血管形成をすることにより、閉塞した血管の血流量を増大させる方法を意味 する。
本発明による新しい方法は、単独でもあるいは末梢のあるいは心臓の血管系の再 疎通のための他の方法、バルーン法、レーザー法、温度法、超音波法、非バルー ン法(例えばatherectory)、ステントを用いた方法との組み合わせ で用いることもできる。本発明の方法による処置は、冠状動脈閉塞、末梢動脈閉 塞、動脈あるいは静脈移植閉塞、合成血管移植閉塞のような状況の場合に遺して いる。
光活性化されるソラレンが、紫外線の照射によって、平滑筋細胞の増殖を著しく 阻害するという発見は、ヒトにおいて再狭窄の進行を減少させ予防するための方 法として有用で効果的なものが現在ないという状況においては、非常に大きい発 展である。乾躯の治療法として、ソラレンの全身投与が用いられてきたという歴 史的な事実は、平滑筋細胞の増殖が原因である再狭窄の予防にソラレンな用いる ことができるということを確信させるものである。
ソラレンは、約320から約400nmのAバンドの範囲(PUVA)での紫外 線照射によって、安全に作用するということが知られている。ソラレンを投与し 、人工的なPUVAを照射した場合は比較的短期間の光感受性である(これは比 較論ではあるが、例えば、ヘストポルフィン類の感受性はもっと長い)というこ とが、乾徊の処置における多(の研究により示されており、このことは、更にや っかいな問題を起こすことな(、再狭窄の予防のために血管の再疎通の方法に付 随的にソラレンの投与を組み合わせることができるということを示している。
ソラレンは投与の8時間後からは光感受性の増加はなく、約24時間で体内より 消失すると言われている。(Honigsmann、光感受性を与える化合物; その化学的、生物学的、臨床上の使用、p143,1989.wi 1ey)。
光毒性の現象としては48から72時間に炎症が報告されている(Ha r b  erら、”光感受性疾患、1989.vo169 (Decker)、5te rnら”乾銅”、Roenigk、Ed、1991.vo1604.(Deck er))。
乾廁の治療においての、紫外線によって活性化されたソラレンの作用機構につい ては推論がある。それは、光活性化されたソラレンがDNAに結合し細胞の増殖 に影響を及ぼすというものである。又、上皮増殖因子受容体結合におけるUVA 光の影響もソラレンによって引き起こされる。(Laskin、“紫外線によっ て引き起こされるソラレンの上皮増殖因子結合の阻害”Proc、Natl。
Acad、Sci、vo183.pp8211.1986)、 しかしながら、 紫外線照射されたソラニンが、どのような機構によって平滑筋細胞の増殖を阻害 するかは今だに明らかでなく、上皮増殖因子の仲介によって起こるのでもない。
ソラレンは多くの位置において置換が可能な分子構造を有している。本発明の実 施例では8−メトキシソラレンが好ましいが、置換のないソラレンや他の置換体 であっても光活性化されつるソラレンであって、細胞内巨大分子(例えばDNA )に結合し、平滑筋細胞の増殖を阻害するのであれば、本明細書で述べる使用に 適する例えば3,4.4’ 、5.5’又は8位に1つあるいはそれ以上置換し たものを挙げることができる。具体的には、5′−メトキシソラレン、4−メチ ルソラレン、4.4′−ジメチルソラレン、4,5−ジメチルソラレン、4′。
8−ジメチルソラレン、4.5′、 8−トリメチルソラレンを挙げることがで きる。又、他にはビリドソラレン、3−カルベトキシソラレン、4′−低級アル コキシソラレン、4−低級アルキルー8−アミノ−低級アルキルソラレン、5′ −アミ2ノーアルキルソラレン、イソーソラレン(アンジェリシン)を挙げるこ とができ、これらは、Ga5parro、“ソラレンとDNAの光生物学(CR CPress、1988)、米国特許第4321919号、4465691号、 4269851号、4328239号、4269852号、4464354号、 4294822号、4169204号に記載されている。
前記した活性を有しているソラレンの基本的構造は、置換により、光活性化や光 感受性が増加したり、減少したりする。特有の置換のために、種々の特有な反応 があることを考えると、この群の化合物の選択の幅は広い。ソラレンの化学的性 質あるいは光化学療法についてや、この目的ために使用する他のソラニンの構造 についてはHa r b e rらの文献(“光感受性疾患”、1989.第6 章)とA n d e r s o nらの文献(皮膚疾患に対するソラレンの 光化学療法、Ann。
Rev、Phcmicol、Toxicol、vo120.pp235.198 0)を参考にすることができる。
ソラレンの投与において重要なことは光依存的に平滑筋細胞の増殖を阻害するの に適当なソラレンの血中濃度を素速く達成することである。再疎通はカテーテル 法においてはその方法を行っている間直接必要である。一方、ソラレンの投与に おいては、ソラレンの濃度がピークに達する以前に再疎通を行い、ソラレンのa 度がピークの時は再狭窄を防止する別の方法も行なうことができる。
経口による全身投与における好ましいソラレンの血中濃度は、紫外線照射時にお いて約0.1から約10LLM、更に好ましくは約0.5から約2μMである。
個々の投与量は、単投5量が約10から約100mgの場合は、動脈の拡張前3 0分から3時間、好ましくは1時間から2時間前に投与するのがよい、単投5量 が約20から約70mgの場合、動脈拡張前30分から1時間が好ましい、又、 同様の血中濃度を提供できる量の静脈内注射も、経口投与が不可能な場合には行 λ、これは動脈拡張前30分がよい。更にはヘパリンの投与に用いられているカ テーテル(米国特許第4824436号)を用いて局所動脈内注射をすることも 可能である。
カテーテルは血管再疎通の為使用でき又焦点をしぼった紫外線照射のためにも使 用できる。このようなカテーテルとしてはレーザーバルーン血管形成法に用いら れているカテーテルを例示することができ、これは、拡張部位を加熱するためレ ーザー光の照射を行っている。このようなカテーテルは、米国特許第47994 79に記載がある。この中では光の伝達のためファイバーオプティックスのカテ ーテルを好適に用いているが、このカテーテルは紫外線を動脈の内表面に360 度にわたって照射できる。レーザー血管形成法に用いられている典型的なファイ バーオプティックは、その表面を部分的に削ることによって再構築のための要素 (例えばバルーン)の中で使うことができる。(米国特許第4799479号及 び4878492号、Fuj i iら、”粗面末端オプティカルファイバーの 光散乱の性質”0ptics and La5er Technol、vo14 0(2月)1984)。このものは末端から単に照射するのではなく、オプティ カルファイバーに沿って光の拡散が起こるので、目的の血管表面に光照射できる 。
再疎通及び平滑筋細胞の阻害の進行中に人工的な閉塞を避けるため動脈血の流れ を提供するためカテーテルを用いるのが望ましい。このような場合は米国特許第 4771777号、4790315号及び4892519号に記載されている潅 流カテーテルをUVA光の伝達のためにファイバーオプティックスに改良したも のを用いることができる。
カテーテルの先端でのエネルギー放射は約0.1からIOJ/cm”である。
しかし経験的にはもっと明確な程度に限定できる0例えば大きな効果を望むのな ら、減衰の結果起こるUVのエネルギーの表面吸収を補うために高いエネルギー レベルが必要である。他の変数によってもエネルギー放射の率は変わる。エネル ギー放射の間の血管の閉塞を最小とするため放射時間は患者と他の要因の状況に 応じて変えるのが好ましい。
入力する仕事率W/cm”は、必要なJ / c m ”を秒で割って算出でき る0例えば5秒後に5J/cm”必要ならIW/cm”の仕事量の出力が要求さ れる。
カテーテルの基部端において入力する仕事率は、cm”で表わされる蛍光領域、 カテーテルの長さによる減衰定数、光学的結合部における焦点損失と膨張のため に用いたバルーンや他の物質を通過することによって起こる他の減衰によって決 定できる。一般的に言って、約1.5cm2の血管壁の円周の領域への照射は、 オプティカルファイバーの動作領域から照射されるとすると、時間と仕事率の組 み合わせにおいて約0.15から約15J必要である。仕事率と時間の相互関係 から考えると10から100秒の照射においてはIWからO,IWの仕事率が要 求される。
ソラレンと平滑筋細胞の間の反応を光活性化することによって、平滑筋細胞の増 殖が阻害されるという発見は牛大動脈平滑筋細胞の増殖におけるPUVA照射の 効果の研究において示された。細胞の増殖と生存を確認するためトリバンブルー を用いた測定法により14日以上にわたって観察を続けた。細胞周期への影響は 無血清の状態にて細胞を同調させ、それに血清を加え、そしてチミジンの取込み とフローサイトメトリーによるDNAの定量を行うことにより測定した。
細胞を10μMの濃度の8−メトキシソラレン単独で処理した場合や、2J/c m”以下のエネルギーでUVA照射を単独でした場合は効果は観察されなかった 。長波長のUVの照射と8−メトキシソラレンは協同的に作用し、7μM、0. 22J/m2から0.2μM、1.4J/cm”の組み合わせの範囲において平 滑筋細胞のDNA合成を阻害する能力を有しており、その範囲においてE Cs 。
を示した。同様な増殖阻害の効果が投与量とエネルギーの組み合わせを逆にした 場合でも認められた。
血清による刺激を伴った場合は、DNA合成の阻害はPUVA照射の直後あるい は遅れて(16時間)観察された。これは8−メトキシソラレンを洗い流しても 影響されなかった。フローサイトメトリーの結果によると、この効果は同調した 細胞の8期の前で阻害されることによって起こることがわかった +tsI標識 した血漿板由来成長因子(PDGF)結合実験によると、PLIVAによる影響 はその結合部位の数や親和性には現われなかった。
8−メトキシソラレン処置において、IJ/cm”のPUVAの単回照射は、更 に照射を(り返すことがなくても、14日間にわたって細胞増殖を完全に止めた 。この間トリパンブルー、ボジイテブな細胞の増加は観察されなかった。このこ とは未処置の細胞に比べ細胞の致死を増加することな(培養した血管の平滑筋細 胞の局所的な阻害を示している。
血管壁の構成物質の損傷は結果的に再狭窄を引き起こす平滑筋細胞の増殖を誘発 する中心的な役割を演じているとして知られている。それ故、他の細胞を破壊す ることなく平滑筋細胞の増殖を阻害する増殖阻害剤の利用が重要である。
光活性化されるソラレンの投与の時期はその手法を施す時期や他の要因に基づい て当業者が任意に変えることができる。例えばソラレンは、再疎通の後に投与し てもよ(、又前に投与してもよい。重要なことはソラレンの投与と再疎通の順序 ではなく、紫外線を照射するときに橿的細胞中にソラレンが存在するか否かであ る。ソラレンの投与系のような場合は標的部位への移行時間が様々であるので、 実用においては、個々の状況において医者がその順序等を正確に制御することが 必要である。この分野の当業者においては本発明の概念や目的から離れることな く付加的な改変を挿入することは容易である。

Claims (20)

    【特許請求の範囲】
  1. 1.(a)光活性化されるソラレンを血管再疎通を施した患者に全身投与し、か つ、該ソラレンは平滑筋細胞の増殖を光量依存的に阻害するのに適したソラレン の血中濃度を達成するベく安全かつ有効な量で投与され、(b)少なくとも部分 的に閉塞した血管を再疎通し、(c)該ソラレンの存在下で該再疎通部位に紫外 線を照射する、ことよりなる血管再疎通後に発生する末梢のあるいは心臓の脈管 構造における再狭窄の予防法。
  2. 2.ソラレンが3,4,4′,5,5′あるいは8の位置において1又はそれ以 上置換されていることを特徴とする請求の範囲1記載の方法。
  3. 3.置換基がメチル又はメトキシである請求の範囲2記載の方法。
  4. 4.ソラレンが8−メトキシソラレンである請求の範囲3記載の方法。
  5. 5.ソラレンが経口投与される請求の範囲1記載の方法。
  6. 6.紫外線照射が約320から約400nmである請求の範囲5記載の方法。
  7. 7.段階(a)と段階(b)が逆の順である請求の範囲1から6のいずれかに記 載の方法。
  8. 8.(a)光活性化されるソラレンを冠動脈血管形成を施した患者に全身投与し かつ該ソラレンは平滑筋細胞の増殖を光量依存的に阻害するのに適したソラレン の血中濃度を達成するベく安全かつ有効な量で投与され、(b)少なくとも部分 的に閉塞した冠動脈を冠動脈血管形成により再構築し、(c)該血管形成領域の 動脈の内表面にファイバーオプティックスを用いて紫外線照射する、 ことよりなる冠動脈の血管形成後に発生する再狭窄の予防法。
  9. 9.ソラレンが、3,4,4′,5,5′あるいは8の位置において1又はそれ 以上置換されていることを特徴とする請求の範囲8記載の方法。
  10. 10.置換基がメチル又はメトキシである請求の範囲9記載の方法。
  11. 11.ソラレンが8−メトキシソラレンである請求の範囲10記載の方法。
  12. 12.ソラレンが経口投与される請求の範囲8記載の方法。
  13. 13.紫外線照射が約320nmから約400nmである請求の範囲12記載の 方法。
  14. 14.段階(a)と段階(b)が逆の順である請求の範囲8から13のいずれか に記載の方法。
  15. 15.(a)光活性化されるソラレンを血管再疎通を施した患者に全身投与し、 かつ該ソラレンは約0.1から約10μMの血中濃度を達成するのに十分な量で 投与され、 (b)少なくとも部分的に閉塞した血管を再疎通し、(c)該ソラレンの存在下 で該再疎通部位に紫外線を照射する、ことよりなる血管再疎通後に発生する末梢 のあるいは心臓の脈管構造における再狭窄の予防法。
  16. 16.ソラレンが3,4,4,5,5′あるいは8の位置において1又はそれ以 上置換されていることを特徴とする請求の範囲15記載の方法。
  17. 17.ソラレンが、8−メトキシソラレン、5−メトキシソラレン、4−メチル ソラレン、4,4′−ジメチルソラレン、4,5′−ジメチルソラレン、4′, 8−ジメチルソラレン、4,5′,8−トリメチルソラレンから成る群より選ば れることを特徴とする請求の範囲16記載の方法。
  18. 18.ソラレンが約0.1から約10μMの血中濃度を達成するのに十分な量で 全身投与される8−メトキシソラレンである請求の範囲15記載の方法。
  19. 19.紫外線照射が約320nmから約400nmである請求の範囲15から1 8のいずれかに記載の方法。
  20. 20.段階(a)と段階(b)が逆の順である請求の範囲15から18のいずれ かに記載の方法。
