(57)【要約】 【目的】 ヘッド部分と柄部分との間の距離を調整する
ことができる肩甲関節用の体内プロステーシスの提供。 【構成】 体内プロステーシスは、球形の関節面を含む
ヘッド部分と、近位上腕部に挿入され、前記ヘッド部分
に接続されるようになっている細長い柄部分とを有し、
ヘッド部分（１０）は、ヘッド部分と柄部分の間の距離
を調整することができるように、柄部分（１２）の近位
端に設けられた第２のねじ部分（３４）と共に作用する
第１のねじ部分（１８）を有する。

Description

【発明の詳細な説明】
【０００１】
【産業上の利用分野】本発明は、肩甲関節用の体内プロ
ステーシス（ｅｎｄｏｐｒｏｔｈｅｓｉｓ）に関する。
【０００２】
【従来の技術】このタイプの体内プロステーシスは、球
形の関節面を有するヘッド部分と細長い柄部分との間に
傾斜プラグイン接続を含む。プラグイン接続の軸線は、
柄部分の軸線に一致し、関節面の湾曲部の中心は前記軸
線から間隔を隔てている。
【０００３】この構造において、関節面、すなわちその
湾曲部の中心と柄部分との間の距離は、ヘッド部分の形
状に依存し、ヘッド部分を製造した後は、もはや変更す
ることはできない。特に、関節から上腕骨の側方の距離
を調整することができない。
【０００４】
【発明が解決しようとする課題】本発明の目的は、ヘッ
ド部分と柄部分との間の距離を調整することができる肩
甲関節用の体内プロステーシスを提供することである。
【０００５】
【課題を解決するための手段】本発明によれば、体内プ
ロステーシスのヘッド部分は、ヘッド部分と柄部分との
間の距離を変化させることができるように、柄部分の近
位端にねじによって接続されている。作用において、ヘ
ッド部分は、柄部分に所望の距離が得られるまでねじ込
まれる。
【０００６】本発明の実施例によれば、間接面の曲率の
中心は、第１のねじ部分の軸線に配置されている。これ
は、柄部分にねじ込まれるときに有利なようにヘッド部
分を対照的に形成することができる。
【０００７】本発明の他の実施例によれば、第２のねじ
部分の軸線は、柄部分の長手方向の軸線に関して傾斜し
ている。これは、関節と上腕骨との間の横方向の距離を
調整することを可能にする。傾斜角は、実際の関節に適
応するように約５０°であることが好ましい。
【０００８】本発明の他の実施例によれば、ねじ部分
は、雄ねじを有するねじピンとして形成され、第２のね
じ部分はねじ穴として形成される。これは、一方の側
で、ヘッド部分をねじ込むときに取り扱いを容易にし、
他方でヘッド部分を柄部分に対して回転しないように固
定する。
【０００９】本発明の他の実施例において、ねじピン
は、軸線に平行に伸びると共に円周方向に間隔を置いた
長手方向の溝を有し、前記溝は、ピンを回転しないよう
に固定するために丸い断面輪郭を有することが好まし
い。
【００１０】本発明の他の実施例によれば、ヘッド部分
は、関節面の縁部または下側に設けられたマーキングを
含み、このマーキングは、前記長手方向の溝に整合して
いる。外科医は、ねじが見えないときであっても長手方
向の溝の向きを観察することができる。
【００１１】本発明の他の実施例において、ヘッド部分
は、大きなトルクを与えるために回転工具に係合する溝
を有し、この溝は、関節面の縁部またはその縁部に接近
した場所、または間接面の下側に配置される。
【００１２】さらに、長手方向の溝は、前記約９０°の
各々であり、各マーキングは、係合溝として形成され
る。
【００１３】本発明の他の実施例によれば、第２のねじ
は、骨材料が穴に成長することができないようにめくら
穴に形成される。
【００１４】本発明の他の実施例によれば、固定ねじ
は、第２のねじ部分に関して横方向に伸びるねじ穴の中
に回転される。固定ねじは、ピンが回転しないように固
定するためにねじピンの長手方向の溝に係合することが
できる。固定ねじは、ピンタイプのねじによって形成す
ることが好ましく、丸い先端部と、回転工具と係合する
ように形成されたヘッドとを有することが好ましい。本
発明の好ましい実施例によれば、固定ねじの軸は、横断
方向に伸びるプラスティック挿入体を有する。プラステ
ィック挿入体は、回転ロックが突然はずれることを防止
する。
【００１５】さらに本発明は、ピーン部が備えられてお
り、このピーン部は、上腕骨の破断部分を取り付ける、
柄部分の近位部分に設けられた開口を有する。
【００１６】本発明の他の実施例によれば、柄部分は、
開口部、好ましくは軸線方向に伸びる細長い開口部と、
円滑な遠位部分とを有することが好ましい。上腕骨に入
る柄部分は、円滑面を有し、近位の柄部分の開口部は、
中に成長する骨部分によって固定を容易にする。
【００１７】本発明の他の実施例によれば、ねじ部分の
近位端に接近した柄部分は、柄に関して垂直方向に伸び
るカラーを有し、柄部分は、柄部分の長手方向の軸線に
関してねじ部分の遠位端の高さにおいて、楕円形の断面
を有する。骨、柄及びヘッドの間の関節領域は、適性な
形状に形成することができる。
【００１８】本発明の他の実施例において、ヘッド部
分、柄部分及び／または固定ねじは金属で製造され、好
ましくは、チタン、またはコバルトクローム合金から製
造される。

【実施例】図１を参照すると、肩甲関節用の体内プロス
テーシスは、柄部分１２にねじ込まれると共に固定ねじ
１４によって固定されたヘッド部分１０を有する。回転
可能なヘッドとして形成されたヘッド部分１０は、実際
の肩甲骨（図示せず）の人工的な間接ソケット（同様に
図示せず）と共に作用して肩甲関節を形成する。
【００２０】ヘッド部分１０は、球形の関節面１６と、
雄ねじ２０を含むねじピン１８とを有する。関節面１６
は、ほぼ半球形をしており、曲率の中心は、ねじピンの
軸線２２に配置されている。関節面は、縁部２４で終わ
っている。図２及び図３は、ねじピン１８の外側面２０
が、ピンの自由端に設けられた凹部２６まで伸びている
ことをさらに詳細に示している。
【００２１】ねじピン１８は、軸線に平行に伸びかつ丸
い輪郭を有する４つの周縁方向に間隔を置いた長手方向
の溝を有する。係合溝として形成された４つのマーキン
グ３０は、関節面１６の縁部２４に沿って設けられ、そ
のマーカーは、ねじピン１８の長手方向溝２８に整合し
ている。
【００２２】柄部分１２の近位端は、穴３２を有するね
じ部分３２を有し、この穴３２は、ねじの軸線２２に沿
って雄ねじ２０と協働するめねじを有する。この軸線２
２は、柄部分の長手方向の軸線３８に関して所定の角度
で傾斜している。さらに、柄部分１２の近位端は、固定
ねじ１４を受けるために軸線２２に関して垂直方向に伸
びるねじ穴４０を含み、前記柄部分は、同様に軸線２２
に関して垂直方向に伸びる円周方向のカラー４２を有す
る。さらに３つの開口部４６を有するピーン部４４が柄
部分の近位端の近くに設けられている。
【００２３】柄部分１２は、近位部分４８を含み、近位
部分４８は、軸線方向に細長い開口部５０と円滑に形成
された遠位部分５２とを有する。固定ねじ１４は、雄ね
じ５４と、回転工具と係合するようになっている形状の
ヘッド部５６並びに丸い先端部５８とを含む。さらにピ
ンタイプのねじは、プラスティック製の固定具を包囲す
る穴６０を有する。
【００２４】第４図乃至第６図は、本発明による体内プ
ロステーシスの他の実施例をさらに詳細に示す。ねじ穴
３４′は、めくら穴と同じような形状であり、ヘッド部
分のねじピンと共に作用する。図示した他のすべての特
徴は、図１を参照しながら説明した実施例に対応する。
軸線２２に関するカラー４２の同心円的な形状が明瞭に
見ることができる。図６は、ねじ部分３２の遠位部分の
高さの部分Ａ−Ａにおいて軸線３８に関する柄部分の楕
円形の断面を示す。
【００２５】操作において、円滑な遠位部分５２を有す
る柄部分は、前方を向けて上腕骨に導入される。ヘッド
部分１０は、軸線２２に沿って柄部分１２に向かって案
内され、例えば、軸線３８に関して関節面１６と柄部分
１２との間の所望の距離が得られるまでねじ穴３４にね
じピン１８によってねじ込まれる。次のマーク、すなわ
ち、係合溝３０は、穴４０に整合し、プラスティック挿
入体を含む固定ねじ１４は、ねじ穴４０に固定される。
ピンタイプのねじ１４は、その丸い先端部５８がねじピ
ン１８の長手方向溝２８に係合するまで締め付けられ
る。ヘッド１０は、軸線２２の周りで回転運動を行わな
いように固定され、ねじのプラスティック挿入体が緩み
を防止する。関節面１６を含む体内プロステーシスは、
骨（図示せず）に挿入され、もしあれば、骨折部分をピ
ーン部４４に固定する。プロステーシスを挿入した後、
骨セメント材料は、近位柄部分４８の細長い開口部５０
に成長して入る。

Claims (23)

    【特許請求の範囲】
  1. 【請求項１】 球形の関節面を含むヘッド部分と、近位
    上腕部に挿入され、前記ヘッド部分に接続されるように
    なっている細長い柄部分とを有する体内プロステーシス
    において、ヘッド部分（１０）は、ヘッド部分と柄部分
    の間の距離を調整することができるように、柄部分（１
    ２）の近位端に設けられた第２のねじ部分（３４）と共
    に作用する第１のねじ部分（１８）を有することを特徴
    とする体内プロステーシス。
  2. 【請求項２】 第１のねじ部分（１８）の軸線（２２）
    は、関節面（１６）の曲率のほぼ中心を通って伸びてい
    る請求項１に記載の体内プロステーシス。
  3. 【請求項３】 第２のねじ部分（３４）の軸線（２２）
    は、柄部分（１２）の長手方向の軸線（３８）に関して
    傾斜している請求項１または２に記載の体内プロステー
    シス。
  4. 【請求項４】 第２のねじ部分（３４）の軸線（２２）
    は、ほぼ５０°の角度で柄部分（１２）の長手方向の軸
    線（３８）に関して傾斜している請求項３に記載の体内
    プロステーシス。
  5. 【請求項５】 第１のねじ部分（１８）は、雄ねじ（２
    ０）を有するねじピン（１８）によって形成され、第２
    のねじピン（３４）は、ねじ穴（３４）によって形成さ
    れる請求項１乃至４に記載の体内プロステーシス。
  6. 【請求項６】 ねじピン（１８）は、その軸線に平行に
    伸びると共に円周方向に間隔を置いた長手方向の溝を含
    み、前記溝は、固定手段（１４）と係合して前記ピンを
    回転しないように固定する請求項５に記載の体内プロス
    テーシス。
  7. 【請求項７】 長手方向の溝（２８）は、丸い断面輪郭
    を有する請求項６に記載の体内プロステーシス。
  8. 【請求項８】 ヘッド部分は、基準マーク（３０）を有
    し、この基準マークは関節面の縁部に、またはそれに接
    近して、またはその下側に設けられており、また前記基
    準マークは前記長手方向の溝に整合している請求項６ま
    たは７に記載の体内プロステーシス。
  9. 【請求項９】 ヘッド部分は、回転工具に係合する溝を
    含み、この溝は、関節面の縁部に、またはそれに接近し
    て、またはその下側に設けられている請求項５乃至９の
    いずれか１項に記載の体内プロステーシス。
  10. 【請求項１０】 ４つの長手方向の溝が９０°の間隔を
    置いて円周方向に設けられており、各基準マークが工具
    係合溝として形成されている請求項６乃至９のいずれか
    １項に記載の体内プロステーシス。
  11. 【請求項１１】 第２のねじは、めくら穴に形成されて
    いる請求項５乃至１０のいずれか１項に記載の体内プロ
    ステーシス。
  12. 【請求項１２】 固定ねじ（１４）は、第２のねじ部分
    を横断するように伸びているねじ穴にねじ込まれている
    請求項５乃至１１のいずれか１項に記載の体内プロステ
    ーシス。
  13. 【請求項１３】 固定ねじ（１４）は、ピンタイプのね
    じである請求項１２に記載の体内プロステーシス。
  14. 【請求項１４】 固定ねじ（１４）は、丸い先端部を含
    む請求項１２または１３に記載の体内プロステーシス。
  15. 【請求項１５】 固定ねじは、回転工具と係合するよう
    に形成されたヘッド（５６）を含む請求項１２乃至１４
    のいずれか１項に記載の体内プロステーシス。
  16. 【請求項１６】 固定ねじの軸は、横断する方向に伸び
    るプラスティック挿入体である請求項１２乃至１５のい
    ずれか１項に記載の体内プロステーシス。
  17. 【請求項１７】 骨の破片を取り付けるための開口部
    （４６）を含むピーン（４４）は、柄部分にほぼ固定さ
    れている請求項１乃至１６のいずれか１項に記載の体内
    プロステーシス。
  18. 【請求項１８】 柄部分は、開口部（５０）含む近位部
    分と、円滑な遠位部分（５２）とを含む請求項１乃至１
    ７のいずれか１項に記載の体内プロステーシス。
  19. 【請求項１９】 有孔部分（４８）は、軸線方向に伸び
    る細長い開口部を含む請求項１８に記載の体内プロステ
    ーシス。
  20. 【請求項２０】 柄部分は、長手方向の柄の軸線（３
    ８）に関してねじ部分（３４）の遠位端の高さで楕円形
    の断面を含む請求項１乃至１９のいずれか１項に記載の
    体内プロステーシス。
  21. 【請求項２１】 ねじ軸（２２）に関して垂直方向に伸
    びるカラー（４２）は、ねじ部分の近位端に接近した柄
    部分に設けられている請求項１乃至２０のいずれか１項
    に記載の体内プロステーシス。
  22. 【請求項２２】 ヘッド部分、柄部分及び／または固定
    ねじは、金属で作られている請求項１乃至２１項のいず
