Abstract

(57)【要約】 【目的】 寛骨臼ソケット支持リングを接合剤を用いる
ことなしに移植可能にし、ソケットを骨盤内と骨幹内と
に固定し、より簡単に整列させ得るようにする。 【構成】 ソケット支持リングが、モジュール構成にさ
れ、対応骨部分の上と内部とに接合剤を用いずに取付け
るための２つの固定用ストラップ７，８を有する支持リ
ング１と、骨接合剤を使用せずに固定部材４により支持
リング１に取付け可能で、支持リング１内で旋回調節可
能なソケットカップ２と、関節頭を受容するそう入体３
とから成るようにする。

Description

【発明の詳細な説明】
【０００１】
【産業上の利用分野】本発明は、寛骨臼ソケット支持リ
ング、それも、対応骨部分内又は対応骨部分上に、接合
剤無しで取付けるための少なくとも１つの取付ストラッ
プを有する支持リングと、関節球を受容する部分を備え
たソケットカップとを有しており、前記ソケットカップ
が、前記支持リング内で旋回調節可能であり、かつま
た、骨接合剤を使用することなしに、固定部材を介して
前記支持リングに取付可能な形式のものに関する。
【０００２】
【従来の技術】この種の寛骨臼ソケットは、ヨーロッパ
特許出願第０１２３５１４号により公知である。この出
願にはソケットカップを調節する着想が開示されている
が、固定手段が比較的複雑かつ不十分である。
【０００３】寛骨臼ソケット支持リングは、更に、種々
の実施態様のものが公知であり、問題のある股関節の矯
正手術時に主として利用される。特にその問題点は、大
ていの場合、骨の大幅な欠損により骨接合剤が骨に吸収
されることにある。そうした場合、接合剤不使用の、標
準化された寛骨臼移植材の固定は、一般に極めて難し
い。形成異常の寛骨の場合も、股関節ソケットの固定
は、きわめて難しい問題をともなう。
【０００４】この先行技術の支持移植材は、骨用ねじに
より寛骨臼内に固定され、股関節ソケットは骨接合剤に
よって接合される。しかしながら、この措置にも、骨接
合剤を使用した場合の周知の欠点、詳言すれば骨による
吸収作用や長期的な耐振性の不足がともない、そうした
欠点のため、移植材の弛みが生じる。公知のソケット支
持リングには、ソケット固定に骨接合剤を使用する必要
があることのほかに、更に、恥骨内の、正確に適正な位
置にそう入することが、限られた程度でしか可能でない
欠点がある。
【０００５】
【発明が解決しようとする課題】前記先行技術にもとづ
き、本発明が課題とするところは、寛骨臼ソケット支持
リングを接合剤を用いることなしに移植可能にするこ
と、より正確に言えば、ソケットを、一方では骨盤内
に、他方では骨幹内に固定し、より簡単に整列させ得る
ようにすることにある。
【０００６】
【課題を解決するための手段】この課題は、次のような
寛骨臼ソケット支持リングによって解決された。対応骨
部分内又は対応骨部分上に接合剤なしで取付けるための
少なくとも１つの取付ストラップと、関節球を受容する
ための部分を有するソケットカップとを備えたソケット
支持リングである。この場合、前記ソケットカップは、
前記支持リング内で旋回調節可能、かつ骨接合剤を用い
ることなしに固定部材を介して前記支持リングに取付け
ることができる。更に、前記支持リング内部には、自縛
式外ねじ山付き前記固定部材を受容する内ねじ山付き円
筒部分と、これに続く、前記ソケットカップを受容する
ための球形部分とが設けられている。更にまた、関節球
を受容する前記部分は、一般にはプラスチック材料製の
そう入体である。
【０００７】先ず第一に、前記ソケット支持リングの固
定には骨接合剤を使用しない。また、モジュール構成の
ため、前記ソケットカップのきわめて精密な調節が可能
である。
【０００８】
【実施例】次に本発明の実施例を図面につき詳説する。
図１には、ソケット支持リングの４つの部材が示されて
いる。すなわち、下から上へ支持リング１、ソケットカ
ップ２、そう入体３、固定リング４である。前記そう入
体３は、プラスチック材料製、好ましくはポリエチレン
製であるか、又は少なくとも部分的に金属製である。他
の部材は、金属製、たとえばチタン、チタン合金、クロ
ム／コバルト／モリブデン合金のいずれか、又はプラス
チック材料製である。図１と図３とに最もよく示されて
いるように、前記支持リングには、前記固定リングを受
容する内ねじ山を有する円筒形部分５が設けられ、この
部分５には、ソケットカップ２を受容する同心的な球形
部分６が続いている。前記支持リング１は、外側に短い
ストラップ７と長いストラップ８とを有している。骨と
接触するすべての金属部分の表面には、前記支持リング
の表面９をも含めて、骨の増殖を促進する措置が施され
ている。たとえば、表面にはガラスを吹付けたり、水酸
化燐灰石又は類似の生活性材料でコーティングされてい
る。あるいは又、燐酸カルシウム・ガラスのスパッタリ
ングが施されている。こうすることによって、骨が移植
材内へ増殖又は食い込んで長期的な安定性が得られる。
【０００９】基本的な安定性は２つのストラップ７，８
によって得られる。短いほうのストラップ７は、解剖学
的に適切な形状を有している。すなわち、先端へ向って
先細にされ、穴１０を有している。穴１０内へは骨が増
殖して入り込み、その結果、保持安定性が増大する。短
いほうのストラップ７は、ふつうは恥骨内へ取付けられ
る。これが不可能な場合、つまり、恥骨区域内の骨の堅
さが不十分なため、リングの固定に支障がある場合に
は、支持リングは穴１０にねじを通して固定する。ねじ
穴の配置形式によって、前記ねじは、穴の軸線に沿って
ねじ込み得るだけでなく、ある程度まで傾けてねじ込む
こともできる。短いストラップ７の縁部は、骨に打ち込
んだ場合に骨を割ることのないような形状にされてい
る。この短いストラップは、全体が骨のなかに固定され
るので、全表面に表面処理を加えておくのが好ましい。
【００１０】解剖学的に適切な形状の長いほうのストラ
ップ８は、短いストラップ７の反対側に設けられ、図２
及び図３から最もよく分かるように、軸方向に１５°〜
３０°だけずらされて配置されている。これら２つのス
トラップは解剖学的な条件に適合するようにされてい
る。短いストラップと異なり、長いストラップは骨内へ
打ち込むのではなく、骨盤に骨用ねじ２７、たとえば海
綿質ねじ又は皮質ねじによって固定される。手術中の状
態に応じて、長いストラップを解剖学的条件に合わせる
ことができる。この長いストラップは、一方では、移植
材の十分な基本的安定性を保証し、他方では、発生する
応力を寛骨臼に対し長期にわたって生理的に円滑に伝達
するのに役立つ。この場合も、ねじは、一定範囲内で穴
内で傾けることができる。穴が球形の皿穴として構成さ
れているからである。
【００１１】ソケットカップ２は、外側が球面１２をな
している（特に図５参照）。この球面は、解剖学的な条
件にきわめてよく合致し、広い面に一様に、したがって
生理的に円滑に力を伝達する。囲まれた区域外の前記ソ
ケットカップ外表面にも骨の増殖を促進させる措置が施
されている。こうして前記外表面には、表面を厚くする
小球状のコーティング又は生活性的な材料、たとえば水
酸化燐灰石などのコーティングが施されるか、もしくは
外表面が粗面化される。
【００１２】前記ソケットカップの内面１３も、大部分
が球形であるが、外側リムの近くでは円筒面１４を有し
ている。この円筒面には、円形みぞ１５と、縁部近くの
凹所１６とが形成されている。円形みぞ１５と凹所１６
とは、いずれもそう入体３の固定に役立っているが、こ
れについては後述の通りである。ソケットカップの円筒
部は、股関節球から前記そう入体へ伝えられる摩擦モー
メントを吸収し、それによって前記そう入体の転位を防
止する。
【００１３】そう入体３は、ポリエチレン製であるのが
好ましく、股関節球を受容する目的を有している。この
そう入体の外表面１７は、前記ソケットカップの内表面
１３，１４に対応している。ビード１８は、みぞ１５に
対応している。ビード１８は、全周にわたって延びてい
るのではなく、扇形に設けられているだけである。回転
防止のため、前記そう入体には凹所１６に対応する突起
１９が設けられている。凹所と突起の数は、それぞれ任
意に選定される。
【００１４】このそう入体は交換する必要が生じるかも
しれないので、その点で、いくつかの措置を取っておく
必要がある。すなわち、前記ビードは、全周にわたって
延びるようにするのではなく、図４に示されているよう
に、たとえば９０°にわたって２個所にだけ設けておく
ようにする。更に、前記ビードは、前記そう入体が取外
されると仮定した場合には、前記みぞから引出さねばな
らない。それには、そう入体を圧縮可能なものにしてお
く必要がある。その目的のため、そう入体３の前記円筒
形部分の内方ほぼ３分の１に凹所２０が設けられ、かつ
また環状面２７に、たとえば４つのみぞ２１が設けられ
ている。これらの措置により、そう入体３の円筒形部分
に一定の可とう性を与えることができ、したがって、ビ
ード１８近くに設けた２つの穴２２に器具を差込むこと
によって、そう入体３の円筒形部分を圧縮することがで
きる。
【００１５】そう入体３の内面２３は、股関節窩の周知
の形状をなし、球面を有しており、この球面に拡大部分
２４が続いていて、自由な動きが十分に出来るようにさ
れている。脱臼防止のための関節頭の機械的クランプ
は、内面２３が赤道部を超えて突出するようにすること
で可能である。
【００１６】固定リング４は、支持リングの内側円筒形
部分５の内ねじ山に対応する自縛式外ねじ山２５を有し
ている。前記固定リングの内径は、ソケットカップ２の
外径より僅かに大きいだけなので、固定リング４のクラ
ンプ作用が十分に発揮される。固定リング４をねじ込
み、かつ固定するため、その前面には切欠き２６又は穴
等が設けられている。
【００１７】前述の固定リングの代りに、別のクランプ
部材を用いて、同じようにソケットカップを整列位置に
固定することもできる。
【００１８】支持リング１を相応の骨部分内に固定した
のち、ソケットカップ２が、はめ込まれ、生体力学的に
最適な位置に調整配置される。そのさい、傾斜範囲は、
図２に示したように、通常は３０°で十分と仮定され
る。次に、ソケットカップ２が固定リング４により不動
に固定される。ポリエチレン製のそう入体３が、次にソ
ケットカップ２内にそう入され、スナップ式にはめ込ま
れる。ソケットカップ２が広い角度範囲で傾斜でき、し
たがって最適状態に位置決めできるため、かつまた、ソ
ケットカップ２が支持リング１内に接合剤なしで固定で
きるため、きわめて長期にわたる安定性と最大限の自由
な動きが得られるのである。

Claims (9)

    【特許請求の範囲】
  1. 【請求項１】 寛骨臼ソケット支持リングにおいて、対
    応骨部分内又は対応骨部分上に接合剤無しで取付けるた
    めの少なくとも１つの取付ストラップ（７，８）と、関
    節球を受容する部分を有するソケットカップ（２）とが
    備えられており、前記ソケットカップが、前記支持リン
    グ（１）内で旋回調節可能であり、かつ骨接合剤を使用
    することなしに固定部材（４）を介して前記支持リング
    に取付けることができ、更に前記支持リング（２）が、
    自縛式外ねじ山（２５）付きの前記固定部材（４）を受
    容する内ねじ山（５）付き円筒部分と、この部分に続い
    て、前記ソケットカップ（２）を受容する球形部分
    （６）とを備えており、更にまた、関節球を受容する前
    記部分がそう入体（３）であることを特徴とする、寛骨
    臼ソケット支持リング。
  2. 【請求項２】 前記支持リング（１）が、恥骨内に取付
    るための穴（１０）を有する、より短い取付ストラップ
    （７）と、骨盤に取付けるための穴（１１）を有する、
    より長い取付ストラップ（８）とを備えており、これら
    のストラップ（７，８）が、それぞれ取付けられる骨部
    分に応じて軸方向にずらされて配置されていることを特
    徴とする、請求項１記載の寛骨臼ソケット支持リング。
  3. 【請求項３】 前記ソケットカップ（２）が球形の外面
    （１２）を有し、かつその内面が円筒形部分（１４）と
    球形部分（１３）とから形成され、前記円筒形部分が前
    記そう入体（３）を固定するための手段（１５，１６）
    を有しており、前記そう入体（３）の外面（１７）が、
    前記ソケットカップ（２）の内面（１３，１４）に対応
    し、対応固定手段（１８，１９）を備えていることを特
    徴とする、請求項１又は２に記載の寛骨臼ソケット支持
    リング。
  4. 【請求項４】 前記そう入体（３）が、プラスチック材
    料製、好ましくはポリエチレン製であるか、又は少なく
    とも部分的に金属製であり、かつ前記ソケットカップか
    ら取外すための手段（２１，２２）を有していることを
    特徴とする、請求項１から３のいずれか１項に記載の寛
    骨臼ソケット支持リング。
  5. 【請求項５】 前記ソケットカップ（２）内に前記そう
    入体（３）を固定する前記手段が、前記ソケットカップ
    （２）内の円形みぞ（１５）及び凹所（１６）と、前記
    そう入体（３）のビード（１８）及び突起（１９）とか
    ら成ることを特徴とする、請求項３記載の寛骨臼ソケッ
    ト支持リング。
  6. 【請求項６】 前記ポリエチレン製そう入体が取外し可
    能であり、前記円筒形部分が外側凹所（２０）を有して
    おり、かつまた前記環状面（２７）が、ビード（１８）
    の設けられている個所にみぞ（２１）と穴（２２）とを
    備えており、これによって前記そう入体を、その前面近
    くで圧縮し、かつ前記穴（２２）に係合する工具を介し
    て前記ソケットカップ（２）から取外すことが可能であ
    ることを特徴とする、請求項１から５のいずれか１項に
    記載の寛骨臼ソケット支持リング。
  7. 【請求項７】 前記支持リング（１）と前記ソケットカ
    ップ（２）との、骨と接触する部分に、骨の増殖を促進
    させる手段が備えられていることを特徴とする、請求項
    １から６のいずれか１項に記載の寛骨臼ソケット支持リ
    ング。
  8. 【請求項８】 前記増殖促進手段が、生体活性材料のコ
    ーティング、より具体的には水酸化燐灰石、燐酸カルシ
    ウム、ガラスのいずれかのコーティングから成るように
    されるか、又は、リング外表面が粗面されることを特徴
    とする、請求項７記載の寛骨臼ソケット支持リング。
  9. 【請求項９】 前記支持リング（１）と、前記ソケット
    カップ（２）と、前記固定リング（４）とが、金属製、
    より具体的にはクロミウム／コバルト／モリブデンの合
    金製、チタン製、チタン合金製のいずれかであるか、又
    はプラスチック材料製であることを特徴とする、請求項
    １から８のいずれか１項に記載の寛骨臼ソケット支持リ
    ング。
