【発明の詳細な説明】
【０００１】
【産業上の利用分野】本発明は多層移植片に関する。
【０００２】
【従来の技術】手術を行っている間に通常の均質移植片
が体の中に移植されている。もしこれらが多孔質材料か
或は再吸収性を示す材料を含んでいる場合、組織が成長
してそれらの孔の中に入るか或は治癒過程中の移植片の
場所に入り込むことで、新しい組織構造体を生じる。空
洞またはガイディング（guiding）構造を有する非再吸
収性移植片を用いると、この非再吸収性移植片は完全な
再生を可能にしないことから、特定点における生理学的
な細胞の増加がちょうど良く生じない可能性がある。従
って、非晶質で無秩序な新しい組織が生じる傾向を示
し、瘢痕化と収縮が生じる。その結果として、組織の強
度が不適当になる可能性があり、そしてこれは更に、も
し移植片が再吸収性を示さないことからある程度の機械
的安定性を保証している材料で出来ている場合、合併症
がもたらされ得る。従って、その移植片と体自身の組織
との組み合わせが示す自然発生的な生物学的機能は、そ
の元の健康な組織が示すそれに相当しなくなる。

【発明が解決しようとする課題】本発明の目的は、組織
再生の異なる段階における組織の成長値および速度に選
択的および差別的影響を与える移植片を提供することに
ある。
【０００４】
【課題を解決するための手段】この目的は、請求項１の
特徴を示す多層移植片を用いることによって達成され
る。
【０００５】すなわち、本発明に従う多層移植片には少
なくとも２層備わっており、これらの少なくとも２層は
異なる間隙率を有している。従って、この移植片の構造
は、この移植片が移植されている体内の組織再生過程に
特異的に適合することが可能であり、そして望まれてい
る、組織が成長して中に入り込むこと（growing-in）が
調節され得る。例えば、急成長する細胞単位は、その移
植片の一方の側から、大きな孔を有する層の中に速く進
行して入って行くことができる一方、もう一方の側で
は、有意な間隙率を有していない層、例えばフィルムに
よって、細胞の入り込みが完全に防止され得る。本発明
に従う移植片に備わっているいくつかの層は、種々の再
吸収性を示す材料および再吸収性を示さない材料を含ん
でいてもよく、これらは今度は、異なる速度で劣化し得
る。この移植片に備わっている個々の層の材料、間隙
率、厚さおよび寸法、並びにこの層の順序および全体的
な形状を適切に選択したならば、この移植片が新しい組
織を生じさせる刺激は、阻害影響を示さず、治癒過程に
選択的に適合し得るものであり、その結果として、促進
された秩序ある組織増殖がもたらされる。
【０００６】与えられた手術中に移植される本発明に従
う移植片の構成および外部形状は、従って、置き代える
べき元の組織構造体の種類および形状を基としており、
従って場合場合で異なっている。個々の層が有する材料
および構造に関する適切な選択を通して、本発明に従う
移植片をその与えられた生物学的条件に適合させること
ができ、その結果として、その新しく生じてくる組織は
再び分化し（高い値）、そして好適な場合として、元の
健康な組織と同じ秩序ある構造を示すようになる。
【０００７】本発明に従う多層移植片の好適な変形にお
いて、織ったか或は編んだメッシュ（格子）の一つの側
に、多孔質フリース（fleece）層を完全にか或は部分的
に与える。このフリース層が示す間隙率は、２００Ｐａ
の入り口圧力、５０ｃｍ2の試験表面積および１ｍｍの
試験厚を用いて測定して１００から１０００Ｌ／（ｍ2
秒）気体流量の範囲にある。このような構成を用いる場
合、このメッシュは、粗い間隙率を有する層を形成して
おり、これを貫通して組織が実質的に妨げられないで成
長する。他方、この組織の増殖速度は、該フリース層が
示す間隙率によって調節され得る。従って、この組織
は、小さい間隙率を有する材料よりも大きな間隙率を有
する材料の中により迅速に成長して入り込む傾向を示
す。このメッシュおよびフリース層は、再吸収性を示す
材料か或は再吸収性を示さない材料を含んでいてもよ
い。例えば、このフリース層が再吸収性を示すがそのメ
ッシュが再吸収性を示さない場合、このメッシュは、あ
る期間経った後、その体内で単にその移植片の構成要素
のままであり、必要に応じて、永久的な追加的物理安定
性を保証し得る。
【０００８】別の好適な変形において、織ったか或は編
んだメッシュの両側に、多孔質フリース層を完全にか或
は部分的に与え、そしてこれらの２つのフリース層は異
なる間隙率を有していてもよい。これらのフリース層は
また、異なる形状または輪郭を有していてもよい。この
間隙率はまた、１つのフリース層内で変化していてもよ

の側からは、この移植片の中に入り込むことができない
からである。このフィルムが再吸収性を示す場合、この
効果が継続するのは一定期間のみである。しかしなが
ら、これは、特定の環境下では、急速に成長する細胞単
位がその移植片の中に入り込むのを防止するに充分であ
り、その間に、もう一方の側から、ゆっくりと成長する
細胞単位がその多孔質フリース層の中に成長して入り込
むが、これにそのフィルムが備わっていない場合、それ
らがそうするのを、その進んで来る急成長する細胞単位
が妨げる。
【００１０】フィルムが備わっている本発明に従う他の
多層移植片構造もまた可能である。従って、織ったか或
は編んだメッシュの片側または両側にフィルムを完全に
か或は部分的に与えることができる。少なくとも１つの
フリース層が備わっている移植片を用いる場合、これら
のフリース層の１つをフィルムで完全にか或は部分的に
コートすることができる。さもなければ、上記移植片
は、上に挙げた構造の１つを有していてもよい、即ちメ
ッシュを含んでいてもよい。二者択一的に、単に２つの
層、即ちフィルム、即ち小さいか或は有意でない間隙率
を有する層とフリース層とが含まれている変形もまた考
えられ得る。
【００１１】本発明に従う多層移植片が有する個々の層
は、各々、再吸収性を示すか或は再吸収性を示さない材
料で出来ていてもよい。これは、メッシュ、フリース層
またはフィルムに適用される。再吸収性を示す材料とし
て適切なものは、例えば好適にはポリグラクチン（Poly
glactin）、ポリジオキサノン（Polydioxanone）または
ポリグレカプロン２５（Poliglecaprone 25）であるか
或はこれらの材料の組み合わせである。
【００１２】ポリプロピレンまたはポリエステルが非再
吸収性材料として用いられ得る。

明に従う多層移植片の種々の可能な構造を示すものであ

【００１５】図１は、本発明に従う多層移植片の第一変
形を示している。織ったか或は編んだメッシュ（格子）
１２の片側に、１０００Ｌ／ｍ2秒の間隙率を有するフ
リース１１を与える。このメッシュ１２のもう一方の側
に、間隙率が１００Ｌ／ｍ2秒のフリース１３を存在さ
せる。５０ｃｍ2の試験表面積と１ｍｍの試験厚を有す
るフリース層を２００Ｐａの入り口圧力で気体に暴露し
たとき生じる気体通過を、間隙率の尺度として与える。
【００１６】図２に示す具体例の場合、その上方のフリ
ース層は、異なる間隙率を有する２つのゾーンを有して
いる。ゾーン２１は、１０００Ｌ／ｍ2秒の間隙率を有
するフリースで出来ており、一方ゾーン２２は、より低
い間隙率である２００Ｌ／ｍ2秒を有するフリースを含
んでいる。２つのゾーン２４（８００Ｌ／ｍ2秒の間隙
率）および２５（３００Ｌ／ｍ2秒の間隙率）を有する
フリース層を、メッシュ２３のもう一方の側に配置す
る。この移植片をその与えられた条件に適合させるのを
最適にする目的で、ゾーン２２と２５の輪郭が異なって
いる点を注意する必要がある。

【００２１】図７に示す変形には安定化用メッシュが含
まれていない。この移植片は、単に、１０００Ｌ／ｍ2
秒の間隙率を有するフリース７１とフィルム７２とで出
来ている。上記変形は、例えば歯周症の治療を行う時、
その歯近くの骨領域のための再吸収性移植片として適切
である。フィルム７２をその歯肉側に置くようにその移
植片を位置させると、これによって、急成長する歯肉が
その骨領域に入り込むのが有効に防止される。従って、
この骨物質は、成長してそのフリース７１の中に入り込
むに充分な時間を有している。このようにして、健全な
骨領域が再び作られる。
【００２２】図８に示す変形は、図７のそれに類似して
いる。ここでは、フリース層に２つのゾーンが備わって
いる、即ち１０００Ｌ／ｍ2秒の間隙率を有するフリー
スで出来ているゾーン８１と、５００Ｌ／ｍ2秒の間隙
率を有するフリースで出来ているゾーン８２が含まれて
いる。このフリース層の１つの側はフィルム８３で覆わ
れている。
【００２３】図９は、中間層として与えられているメッ
シュ９３が再び含まれている変形を示している。このメ
ッシュ９３の１つの側は、１００Ｌ／ｍ2秒の間隙率を
有するフリース９２であり、これはフィルム９１で覆わ
れている。メッシュ９３のもう一方の側には、５００Ｌ
／ｍ2秒の間隙率を有するフリース９４が配置されてお
り、これには最終フィルム９５が備わっている。
【００２４】図１０は、本発明に従う複雑な構造を有す
る多層移植片を示している。メッシュ１０４が支持体と
して働いている。このメッシュ１０４の上側に、間隙率
が２００Ｌ／ｍ2秒のフリースで出来ている外部ゾーン
１０２と間隙率が１００Ｌ／ｍ2秒のフリースが備わっ
ている内部ゾーン１０３とで出来ている、フリース層が
存在している。この内部ゾーン１０３の一部はフィルム
１０１で覆われている。このメッシュ１０４の下側に
は、ゾーン１０５（２５０Ｌ／ｍ2秒の間隙率）と１０
６（１５０Ｌ／ｍ2秒の間隙率）が備わっているフリー
ス層が配置されている。このゾーン１０６の一部はフィ
ルム１０７で覆われている。
【００２５】簡潔さの目的で、これらの図に示す多層移
植片は、長方形の基本的形状を有していると共に、それ
らのフリース層は一定した厚さを有している。しかしな
がら、与えられた用途では、この移植片の基本的構成、
個々の層の材料および間隙率、並びに層の厚さおよび移
植片の全体的形状などを、その条件に適合させる。
【００２６】本発明に従う多層移植片が有する個々の層
を互いに結合させる目的で、例えば、異なる融点を有す
る材料を隣接層として用いることが可能であり、この移
植片の製造中に、これを、より低い融点を有する材料の
融点よりも高い温度であるが融点が高い方の材料の融点
よりも低い温度に加熱すると、これらの層の１つが溶融
してその隣接層と結合する。再吸収性を示す材料、好適
にはポリジオキサノンおよびポリグラクチンで作成した
移植片の成分は、ドイツ特許第38 30 005 C1号に記述さ
れている。
【００２７】異なる融点を有する２つの層の間に、より
低い融点を有する材料から作成したフィルムを位置させ
て加熱する結果として、このフィルムを溶融させて多孔
性を示すようにさせると共に上記２つの層を互いに結合
させることも可能である。このようにして、例えば、再
吸収性を示さない材料から作成したメッシュと、より低
い融点を有する再吸収性を示す材料から作成した多孔質
フリース層とを互いに連結させることができる。
【００２８】本発明の特徴および態様は以下のとうりで
ある。
【００２９】１． 少なくとも２つの層が備わってお
り、これらの少なくとも２つが異なる間隙率を有するこ
とを特徴とする多層移植片。
【００３０】２． ２００Ｐａの入り口圧力、５０ｃｍ
2の試験表面積および１ｍｍの試験厚で測定して１００
から１０００Ｌ／（ｍ2秒）気体流量の範囲にある間隙
率を有する多孔質フリース層を、織ったか或は編んだメ
ッシュの１つの側に完全にか或は部分的に与えることを
特徴とする第１項記載の多層移植片。
【００３１】３． ２００Ｐａの入り口圧力、５０ｃｍ
2の試験表面積および１ｍｍの試験厚で測定して１００
から１０００Ｌ／（ｍ2秒）気体流量の範囲にある間隙
率を有する多孔質フリース層を、織ったか或は編んだメ
ッシュの両側に完全にか或は部分的に与えることを特徴
とする第１項記載の多層移植片。
【００３２】４． 該２つのフリース層が異なる間隙率
を有していることを特徴とする第３項記載の多層移植
片。
【００３３】５． ２００Ｐａの入り口圧力、５０ｃｍ
2の試験表面積および１ｍｍの試験厚で測定して１００
から１０００Ｌ／（ｍ2秒）気体流量の範囲にある間隙
率を有する多孔質フリース層を、織ったか或は編んだメ
ッシュの１つの側に完全にか或は部分的に与え、そして
もう一方の側に、１０から５０μｍの範囲の厚さを有す
るフィルムを完全にか或は部分的に与えることを特徴と
する第１項記載の多層移植片。
【００３４】６． 該フリース層の少なくとも１層内で
該間隙率が変化していることを特徴とする第２から５項
の１項記載多層移植片。
【００３５】７． １０から５０μｍの範囲の厚さを有
するフィルムを、織ったか或は編んだメッシュの片側ま
たは両側に完全にか或は部分的に与えることを特徴とす
る第１項記載の多層移植片。
【００３６】８． フリース層を少なくとも１層含んで
おりそして該フリース層の少なくとも１層がフィルムで
完全にか或は部分的にコートされていることを特徴とす
る第１から５項の１項記載多層移植片。
【００３７】９． 該層の少なくとも１層が再吸収性を
示すことを特徴とする第１から８項の１項記載多層移植
片。
【００３８】１０． 該層の少なくとも１層が再吸収性
を示さないことを特徴とする第１から８項の１項記載多
層移植片。
【００３９】１１． 再吸収性を示す材料としてポリグ
ラクチン９１０、ポリジオキサノン、ポリグレカプロン
２５またはそれらの組み合わせを用いることを特徴とす
る第９項記載の多層移植片。
【００４０】１２． 再吸収性を示さない材料としてポ
リプロピレンまたはポリエステルを用いることを特徴と
する第１０項記載の多層移植片。

    【特許請求の範囲】
  1. 【請求項１】 少なくとも２つの層が備わっており、こ
    れらの少なくとも２つが異なる間隙率を有することを特
    徴とする多層移植片。
  2. 【請求項２】 ２００Ｐａの入り口圧力、５０ｃｍ2の
    試験表面積および１ｍｍの試験厚で測定して１００から
    １０００Ｌ／（ｍ2秒）気体流量の範囲にある間隙率を
    有する多孔質フリース層を、織ったか或は編んだメッシ
    ュの１つの側或は両側に完全にか或は部分的に与えるこ
    とを特徴とする請求項１記載の多層移植片。
