Claims (1)
- 【特許請求の範囲】 1. (a)光吸収の1つまたは複数の特性波長または特性波長帯について選 択される光反応剤を、内部、すなわち、生体内処置場所に加える過程と、 (b)光反応剤の吸収波長または波長帯にほぼ等しい1つまたは複数の放出波 長または放出波長帯を有する光源を、患者の体内に配置する過程と、 (c)その光源から放出された光を内部、すなわち、生体内処置場所に供給す る過程と、 を備え、前記光は光反応剤によって吸収され、それによりその光反応剤は処置場 所において希望の治療的変化を行わせられる、内部、すなわち、生体内の治療理 場所での、希望の治療的変化を行わせる、フォトダイナミック治療方法。 2. 光源を配置する過程は、 (a)末端部と近端部を有するカテーテルを提供し、前記光源をカテーテルの 末端部に配置する過程と、 (b)カテーテルおよび光源を患者の体内で動かす過程と、 (c)カテーテル末端部と光源が内部、すなわち、生体内処置場所に近接して 配置されるように、カテーテルを配置する過程と、 を備える請求の範囲1に記載の方法。 3. 前記カテーテルは、カテーテルの末端部と近端部の間を全体として延長 する少なくとも1つのルーメンを含み、光反応剤供給源を前記少なくとも1つの ルーメンに流体が通じるようにして結合する過程を更に備え、加える過程は、カ テーテルの末端部において内部、すなわち、生体内処置場所に光反応剤を灌流す るように、前記少なくとも1つのルーメンを通じてその光反応剤を流す過程を備 える請求の範囲2に記載の方法。 4. 光源を配置する過程は、 (a)内部、すなわち、患者の体内の生体内処置場所の近くに光源を侵襲的に 配置する過程と、 (b)希望の治療的変化が起きるまで、光を処置場所に照射しながら患者の体 内に光源を残しておく過程と、備える請求の範囲1に記載の方法。 5. 患者の体内に前記光源を侵襲的に配置する過程は、 (a)内部、すなわち、生体内処置場所に接近するために患者の体を貫かせる 過程と、 (b)前記貫きを止めて前記光源を患者の体内に埋め込んだままにする過程と 、 を備える請求の範囲4に記載の方法。 6. 光源を設ける過程は、少なくとも1つの発光ダイオードを設ける過程を 備え、光源を侵襲的に配置する過程は、前記少なくとも1つの発光ダイオードに よって 放出された光で処置場所を照明するために、前記少なくとも1つの発光ダイオー ドを内部、すなわち、生体内処置場所に近接して配置する過程を備える請求の範 囲4に記載の方法。 7. 光源を設ける過程は、少なくとも1つのレーザダイオードを設ける過程 を備え、光源を侵襲的に配置する過程は、前記少なくとも1つのレーザダイオー ドによって放出された光で処置場所を照明するために、少なくとも1つのレーザ ダイオードを内部、すなわち、生体内処置場所に近接して配置する過程を備える 請求の範囲4に記載の方法。 8. 前記カテーテルはカテーテルの末端部と近端部の間を全体として延長す る少なくとも1つのルーメンを含み、 (a)外部電源と、この電源に接続される導電体とを設ける過程と、 (b)外部電源からルメンを通じて光源まで延長する導電体を通じて外部電源 から電流を供給して光源を付勢する過程と、 をさらに備える請求の範囲2に記載の方法。 9. 前記光源は少なくとも1つの発光ダイオードを備え、前記少なくとも1 つの発光ダイオードの電圧−電流特性を決定することによって内部、すなわち、 生体内処置場所の温度を周期的に監視する過程を更に備える請 求の範囲1に記載の方法。 10. 前記光源は少なくとも1つのレーザダイオードを備え、前記少なくと も1つのレーザダイオードの電圧−電流特性を決定することによって内部、すな わち、生体内処置場所野温度を周期的に監視する過程を更に備える請求の範囲1 に記載の方法。 11. (a)外部電源を設ける過程と、 (b)外部電源を光源に電磁的に結合して、光源を付勢するための電流を供給 する過程と、 を更に備える請求の範囲1に記載の方法。 12. 光源を有するすべてを完備している電源を患者の体内に配置して前記 光源を付勢するための電流を供給する過程を更に備える請求の範囲1に記載の方 法。 13. フォトダイナミック治療の効率を改善するために処置場所を加熱する 過程を更に備える請求の範囲1に記載の方法。 14. 加熱する過程は、内部、すなわち、生体内処置場所の近くに配置され ている光源からの廃熱を使用する過程を更に備える請求の範囲13に記載の方法 。 15. 内部、すなわち、生体内処置場所における生理学的パラメータを監視 する過程を更に備える請求の範囲1に記載の方法。 16. 光反応剤を内部、すなわち、生体内処置場所に周期的に注入する過程 を更に備える請求の範囲1に記 載の方法。 17. 周期的に注入する過程は少なくとも1つの外部溜めから光反応剤を注 入する過程を備える請求の範囲16に記載の方法。 18. 周期的に注入する過程は、患者の体内に光源とともに配置されている 少なくとも1つの外部溜めから光反応剤を注入する過程,を備える請求の範囲1 6に記載の方法。 19. 光源は複数の光源を備え、光を供給する過程は、複数の光源のうちの 選択した1つを順次付勢して、複数の光源のうちの前記選択した1つが光を放出 したときに、内部、すなわち、生体内処置場所の種々の部分を照明する過程を備 える請求の範囲1に記載の方法。 20. (a)フォトダイナミック治療に適当な少なくとも1つの特性波長ま たは特性波長帯を有する光源と、 (b)、前記光源のための支持構造体であって、この構造体は患者の体内に侵 襲的に配置されるようにされて、光源を内部、すなわち、生体内処置場所の近く に支持し、かつ光を光源から直接前記処置場所に送るように形成され、更にその 場所に放置されるようにされて、前記光源を選択的に励起できるようにして、前 記光によって前記内部、すなわち、生体内処置場所を照射して、処置場所におい て希望の治療変化を行わせる前記指示構造体と、 (c)光源を励起する電流を供給する電源と、 を備える内部、すなわち、生体内処置場所においてフォトダイナミック処置治療 を行って、希望の治療変化を行わせる装置。 21. 光源は少なくとも1つの発光ダイオードを含む請求の範囲20に記載 の装置。 22. 光源は少なくとも1つのレーザダイオードを含む請求の範囲20に記 載の装置。 23. 電源は赤外線源と、赤外線を光源の付勢に用いる赤外線検出器とを含 む請求の範囲20に記載の装置。 24. 前記赤外線源は患者の体に向けられる赤外線を発生するようにされ、 前記赤外線は.患者の体を貫いて赤外線検出器に到達し、その赤外線検出器は患 者の体内に配置されるようにされる請求の範囲23に記載の装置。 25. 支持構造体はカテーテルを備え、このカテーテルは内部、すなわち、 生体内処置場所に配置されるようにされた末端部と、患者の体外に止まる近端部 とを有する請求の範囲20に記載の装置。 26. カテーテルは、それの近端部からそれの末端部までカテーテルを全体 として延長する少なくとも1つのルーメンを含み、光反応剤供給源を更に備え、 この光反応剤供給源は前記少なくとも1つのルーメンに流体が通じるようにして 連結されて内部、すなわち、生体内処置場所に灌流し、かつ、患者の体外に配置 され、前記光 反応剤は供給源から前記少なくとも1つのルーメンを通ってカテーテルの末端部 において前記少なくとも1つのルーメンを出る請求の範囲25に記載の装置。 27. カテーテルは、それの近端部からそれの末端部までカテーテルを全体 として延長する少なくとも1つのルーメンを含み、カテーテルの近端部は患者の 体外にとどまり、電源は患者の外部にあり、電源に電気的に結合される導電体を 更に備え、この導電体はカテーテル内部の前記少なくとも1つのルーメンを通っ て光源まで延長し、導電体は電源から電流を伝えて光源を付勢する請求の範囲2 5に記載の装置。 28. カテーテルの末端部に配置されるセンサと、対応する監視器とをさら に備え、前記センサは監視器に結合され、それは処置場所においてフォトダイナ ミック処置の有効性を判定するために、その有効性を示す生理学的パラメータを 監視することによってその有効性を判定するために用いられる請求の範囲25に 記載の装置。 29. 支持構造体は、カテーテルの末端部に配置されるプローブをさらに備 え、このプローブは基板を含み、この基板の上に光源が取り付けられる請求の範 囲25に記載の装置。 47. (a)内部、すなわち、生体内処置場所を患者の体外からアクセスで きるようにその内部、すなわち、生体内処置場所を露出させる過程と、 (b)光吸収の特性波長または特性波長帯について選択される光反応剤を内部 、すなわち、生体内処置場所に加える過程と、 (c)光反応剤の光吸収の波長または波長帯にほぼ等しい放出波長または放出 波長帯を有する光源を処置場所の近くに埋め込む過程と、 (d)患者の体内で光源を閉じる過程と、 (e)光源から光を処置場所に送る過程と、 を備え、前記光は光反応剤によって吸収され、それからその光は希望の治療変化 を行わせる、患者の体内の内部、すなわち、生体内処置場所においてフォトダイ ナミック処置を行って、希望の治療変化を行わせる方法。 48. (a)フォトダイナミック処置のために求められる所定の光吸収波長 または光吸収波長帯にほぼ等しい少なくとも1つの放出波長または放出波長帯を 有する光源と、 (b)侵襲的に埋め込まれるようにされ、前記内部、すなわち、生体内処置場 所の近くで、患者の体内に閉じ込められたままにされ、かつ、光を内部、すなわ ち、生体内処置場所に直接供給するようにされた、前記光源の支持構造体と、 (c)光源が埋め込まれている患者の体の外を延長する導体を用いることなし に光源を付勢させる電流を供給する電源と、 を備え、光源によって放出された光は希望の治療変化を行わせる、内部、すなわ ち、生体内処置場所においてフォトダイナミック処置を行って、希望の治療変化 を行わせる装置。 49. (a)光吸収の少なくとも1つの特性波長または特性波長帯について 選択された光反応剤を、内部、すなわち、生体内処置場所に加える過程と、 (b)フォトダイナミック処置を行うために求められる少なくとも1つの所定 の放出波長または放出波長帯を有し、内部、すなわち、生体内処置場所から離隔 されている、個々にアドレスできる発光装置のアレイを設ける過程と、 (c)発光装置の特定の1つを選択的に付勢することによって発光装置のアレ イから内部、すなわち、生体内処置場所に光を供給する過程と、 を備え、前記光は希望の治療的変化を起こさせる、内部、すなわち、生体内の治 療場所で希望の治療的変化を行わせるフォトダイナミック処置方法。 54. (a)フォトダイナミック処置のために求められる少なくとも1つの 所定の放出波長または放出波長帯をおのおの有し、内部、すなわち、生体内処置 場所から離隔されている、個々にアドレスできる発光装置のアレイと、 (b)希望の治療変化を行わせるために、内部、すな わち、生体内処置場所を照明する光を放出する時に、特定の1つの発光装置を選 択的に付勢する手段と、 (c)選択的に付勢する手段に結合され、電流を供給してアレイ中の特定の1 つの発光装置を付勢する電源と、を備え、内部、すなわち、生体内の治療場所で 希望の治療的変化を行わせるフォトダイナミック処置のための装置。 60. (a)フォトダイナミック処置のために求められる少なくとも1つの 放出波長または放出波長帯びを有する光源と、 (b)光源に電気的に結合され、光源を付勢する電流を供給する電源と、 (c)近端部と末端部を有するカテーテルであって、それに光の伝導を可能に させるそれの光学的特性に対して選択された材料を含むカテーテルと、 を備え、前記カテーテルの近端部は光源に光学的に結合され、したがって、カテ ーテルは光をそれの末端部まで伝え、前記カテーテルの末端部は患者の体内に挿 入されるようにされて、内部、すなわち、生体内処置場所に配置され、カテーテ ルによって伝えられた光は希望の治療変化を行わせる、内部、すなわち、生体内 の処置場所で希望の治療的変化を行わせるフォトダイナミック処置のための装置 。 63. (a)光吸収の少なくとも1つの特性波長ま たは特性波長帯に対して選択された光反応剤を、内部、すなわち、生体内処置場 所に加える過程と、 (b)光反応剤の吸収の波長または波長帯にほぼ等しい少なくとも1つの放出 波長または放出波長帯を有する光源を、処置場所の近くに配置する過程と、 (c)その光源から放出された光を処置場所に供給する過程と、 を備え、前記光は光反応剤によって吸収され、それによりその光反応剤は希望の 治療的変化を行わせられる、患者の内部の、洞空所、鼻咽頭表面、口表面、のど 表面、および奥地内部表面のうちの少なくとも1つを含む処置場所においてフォ トダイナミック処置を行う方法。
