Description

【発明の詳細な説明】 ラミネートされたパッチ組織修復シート材料 発明の分野 本発明は、生体組織の修復、特にはヘルニア修復に有用なインプラント用シー ト材料の分野に関する。 背景技術 組織修復のためのインプラント用シート材料は周知である。これらの材料で通 例修復される組織欠損には、腹部の創傷、特にはヘルニアの修復がある。胸壁や 横隔膜の創傷、及びこの他の筋及び腱膜組織の弱部もまたこれらの材料で修復さ れる。 最も支配的に使用されるシート材料には二つの基本的タイプがある。第一のタ イプは、例えばC.R.Bard社(Billerica、マサチューセッツ州 )から入手できるMarlex(商標)メッシュのような、材料を貫く多数の開 口部を有するメッシュ材料である。これは、直径約0.17mmのポリプロピレ ンモノフィラメントから編まれ、直径約0.54mmの開口部を有する目の荒い メッシュである。これらのメッシュ材料は、良好な縫合保持性、良好な機械的強 度を提供し、またメッシュ開口部を通って組織が成長することを許容する。 組織修復のためのインプラント用シート材料のもう1つのタイプは、W.L. Gore & Associates社(Flagstaff、アリゾナ州)か ら入手できる厚さ1.0と2.0mmの多孔質延伸ポリテトラフルオロエチレン から作成されたGORE−TEX(商標)ソフトティッシュパッチである。これ は、メッシュ の仕立ての中に大きなマクロ多孔質的孔を含まない多孔質シート材料である。組 織がその中又は周りに成長する大きな孔がないため、この材料は組織接着の危険 性を最少限にすることが望まれる場合に有用である。 発明の要旨 本発明は、メッシュタイプのシート材料を貫く多数のマクロ多孔質的開口部を 有するメッシュタイプのシート材料の層にラミネートされた多孔質ポリテトラフ ルオロエチレンシート材料の層を含んでなり、その開口部は約0.1mmの平均 最小直径を有するインプラント用パッチ組織修復材料のシートである。本発明の パッチ組織修復材料は、良好な強度特性、良好な生体適合性、縫合保持性、及び 柔軟性を提供する。 本願における用語「ラミネートされた」は、普通の使用の際に層の後発的分離 を防ぐ任意の仕方で、材料の2枚の層が一緒に結合されたことを表現するために 使用される。本願での「マクロ多孔質的開口部」は、拡大せずに肉眼で見え、そ の開口部が形成されている層の厚さを貫いて見た目に開口している開口部である とみなされる。ミクロ多孔質的開口部は、それを肉眼で見えるようにするには拡 大の利用を必要とする。 図面の簡単な説明 図1は、本発明のラミネートされたパッチ組織修復シート材料の透視図を示す 。 図2は、本発明のラミネートされたパッチ組織修復シート材料の横断面を示す 。 発明の詳細な説明 図1は、メッシュタイプのシート材料を貫く多数の開口部を有するメッシュタ イプのシート材料の層13にラミネートされた多孔質PTFEの層11を有する本発 明のラミネートされたパッチ組織修復シート材料の透視図を示す。図2は、その ラミネートされたパッチ組織修復シート材料の横断面を示す。 メッシュタイプのシート材料を貫く開口部は、直径が少なくとも約0.1mm でなければならない。開口部の全体的な形状は、生物学的に特に重要であるとは 考えられず、このため円、楕円、三角形、正方形、長方形、六角形等でよい。長 さ若しくは長径及び幅若しくは短径を有する非円形の開口部についての用語「最 小直径」は、本願では、シート材料の表面に実質的に平行に測定された、非円形 開口部の幅若しくは短径を表す最大直径と定義される。この最小直径は、変形さ せる力を与えていない緩んだ状態のサンプルで測定されるべきである。平均最小 直径は、少なくとも10個の巨視的開口部を含むサンプル領域をランダムに選択 し、その領域内の最も大きい10個の開口部の最小直径を位置決めして測定し、 それらの10個の開口部の平均直径を計算することによって求められる。少なく とも10個の開口部を含むサンプルを得ることができない場合、入手できる最も 大きいサンプルの領域内の全ての開口部が、平均値の計算に含められるべきであ る。 メッシュタイプのシート材料の層13は、ポリプロピレン、ポリエチレンテレフ タレート、PTFE、又はミクロ多孔質PTFEを含む任意の適切な生体適合性 材料でよい。メッシュタイプのシート材料は、それから開口部15が切り込まれた 又は別な仕方で形成されたシートの形態でよく、あるいは布帛の広い間隔のスト ランドで形成された多数の開口部を有する編物又は織物のような目の荒い布帛で もよい。 ミクロ多孔質PTFEの層11は、好ましくは米国特許第3953566号、同 4187390号の教示にしたがって作成されたミクロ多孔質延伸PTFEであ る。この材料は、フィブリルによって相互に接続されたノードの微細構造を有す る。また、荒い微細構造を有する高強度材料が望まれる場合、米国特許第448 2516号、同4598011号にしたがって作成されることもできる。所望に より、層11は、米国特許第4385093号、同4478665号による教示の ように多数の層のラミネートとして作成されることもできる。 層11と13は、少なくとも2つの異なる方法によってラミネートされることがで きる。第一に、2枚の層を接着するために接着剤が使用されることができる。こ のような適当な接着剤の1つは、ダウコーニング社(ミッドランド、ミシガン州 )から入手できるSilastic(商標)医療用接着剤シリコーンタイプA( カタログ番号891)である。E.I.DuPont de Nemours( ウィルミントン、デラウェア州)から入手できるテフロンFEP120水系ディ スパージョンのような熱可塑性接着剤も使用可能である。多孔質PTFE層11に 接着しようとするメッシュタイプの層13の面に、FEPディスパージョンのコー ティングが施される。次いで、約290℃の、テフロンFEP120ディスパー ジョンの溶融を生じさせるに充分な熱を与え、熱と圧力の適用(例えば、向かい 合ったローラーによって適用)によってその2枚の層を接着させる。 あるいは、付加的な接着剤を使用せず、熱と圧力を使用して層11と13をラミネ ートすることもできる。これは、材料のメッシュタイプの層13が、多孔質PTF E層11よりも低い融点を有する場合に好 ましい方法であることが経験されている。例えば、メッシュタイプのポリプロピ レン層13が使用された場合、層11と13の平面の垂直方向に圧力を同時に加えなが ら、ポリプロピレンの融点を超える約180℃までポリプロピレン層を加熱する と、2枚の層の間に良好な接着が得られることが経験されている。 また、多孔質延伸PTFEのシート材料から、層11と13の両方を形成できるこ とが経験されている。直径約3mmの丸いポンチを用いて層13を貫く孔15を作成 することにより、約6cm×6cmで厚さ1mmのGORE−TEXソフトティ ッシュパッチ材料がメッシュ層13に使用された。その孔は約1.2cmの間隔で 離れていた。次いで熱と圧力を使用して、層13が、GORE−TEXソフトティ ッシュパッチ材料の第二の層11にラミネートされ、ここで層11は孔を含まなかっ た。ラミネートは、380℃に加熱された2つのプレートの間に層11と13を配置 し、その2枚の層に約1.3kg/cm2の圧力を加えることによって行われた 。圧力を維持されながら、加熱されたプレートを約300℃まで放冷した。この 終了時に圧力を開放し、多孔質延伸PTFEの2枚のラミネートされた層11と13 を取り出した。ラミネートされた層11と13は、ラミネートされた層の繰り返しの 屈曲が層の分離の傾向を全く示さなかった点で、良好に接着されたことが分かっ た。
