Abstract
るために配置された2つの埋め込み部材末端部分(1)
と、埋め込み部材の該末端部分の間にある1つの埋め込
み部材中央部分(2)とから成っており、該中央部分
は、末端部分(1)とそれぞれ1つのネジ山(3,4)
によって接続されている。ここで、両ネジ山(3,4)
は脊柱の縦方向において共軸に配設されていて、互いに
逆のネジ山方向を有している。また、3つの部分(1,
2)全てが、少なくともネジ山(3,4)の領域におい
て円形の横断面を有する管状のスリーブとして構成され
ていて、互いに共軸にネジ締めされている。さらに、ス
リーブ壁は開口で打ち抜かれている。 【効果】埋め込み部材中央部分(2)の回転によって、
生体埋め込み部材全体の長さが変更可能である。また、
生体埋め込み部材を迅速に、かつ、確実に生体に根付か
せる。
Description
しくは椎部分のためのスペース保持部材として脊柱の椎
体間に装入するための生体埋め込み部材で、隣接する椎
骨に据え付けるために配置される2つの埋め込み部材末
端部分と、該部分の間にある埋め込み部材中央部分とよ
り成り、該中央部分は、末端部分にそれぞれ1つのネジ
山によって接続されてるものに関する。ここで、両ネジ
山は脊柱の縦方向において共軸に配設されていて、互い
に逆方向のネジ山方向を有していて、その結果、埋め込
み部材中央部分の回転によって、生体埋め込み部材の長
さ全体が、変更可能である。
明細書第4657550号より公知であり、埋め込み部
材中央部分の回転によって、生体埋め込み部材を含む脊
柱領域の伸延を可能にする。
込み部材の両方の末端部分がそれぞれ2部分に構成され
ていて、それぞれは、隣接する椎骨に据え付けられ、椎
骨へと向かって針部を有する押圧プレート、および、ネ
ジ山ボルトから成っている。該ボルトは、多角頭部でも
って、ボルト軸周りの回転を防ぎ、押圧プレート中の対
応する受入部中に差し込まれている。埋め込み部材中央
部分は、ネジ山ボルトの受け入れのためのネジ山穴ぐり
を備えて構成されている。前記の公知の生体埋め込み部
材は、生体埋め込み部材の装入後の骨形成及び骨再成を
望まれるというままに残しておくという欠点を有してい
る。ドイツ特許DE3435771A1号からは、前記
欠点を回避する生体埋め込み部材が公知であり、該生体
埋め込み部材は、多くの穴を備えているが、前記の如き
生体埋め込み部材は、1つの椎骨もしくは椎部分のため
のスペース保持部材としては不適である。
において、1つの椎骨のためのスペース保持部材として
働く生体埋め込み部材が記されていて、該生体埋め込み
部材は、壁部中に穴を備えたカバー形成部材として構成
されていて、その際、上方及び下方のカバー縁部は、接
続されるべき椎骨間の回転不能の接続を生じるために、
少なくとも部分的に歯状に構成されている。前記部材
は、穴を備えた金属シートより、一層もしくは二層に巻
いて作られている。穴を通って、所望の場合、骨セメン
トもしくは骨断片の如き、カバー内部に装入されるべき
物質が充填されることができ、このことによって、生体
埋め込み部材との、より迅速な血管接続が達成される。
前記の如き生体埋め込み部材において不都合であるの
は、該部材が、隣接する椎骨の伸延を全く可能としない
点である。
刺激にとって特に適していて、従って埋め込み後に特に
容易に、かつ、迅速に根付くことができるように冒頭に
述べた如き生体埋め込み部材を構成するという課題が本
発明の根底を成している。
は、冒頭に述べた如き構成要件を備えた生体埋め込み部
材において、埋め込み部材の3つの部分全てが、少なく
ともネジ山の領域が円形断面の管状スリーブとして構成
されていて、互いに共軸にネジ締めされていること、及
び、スリーブ壁が開口によって穴をあけられていること
によって解決される。
の、本発明に則した発展は、生体埋め込み部材内部に、
通り抜ける中空空間を生じさせ、該空間は、スリーブ壁
中にある開口を通り抜けて至る所で生体埋め込み部材外
側につながっている。このことは、骨セメントまたは、
患者自身もしくは他人の骨くずを前記中空空間に満たす
可能性と併せて、生体埋め込み部材を迅速に、かつ、確
実に、根付かせる。それにもかかわらず、手術の間の生
体埋め込み部材の伸延の可能性は充分に維持される。
実施態様は、埋め込み部材末端部分が埋め込み部材中央
部分に外側から軸方向に重ねられ、その際、ネジ山がそ
れぞれ、埋め込み部材中央部分上では雄ネジとして埋め
込み部材末端部分中では雌ネジとして構成されているこ
とを特徴としている。前記配設の結果、ネジ山の外側は
十分に埋め込み部材末端部分によって覆われている。埋
め込まれた状態において埋め込み部材の部分の相互の回
転を阻止するために、埋め込み部材末端部分の少なくと
も1つがネジ山領域において、半径方向のネジ穴を有し
ていてよく、該ネジ穴中を、埋め込み部材中央部分に対
して締付可能な締付ネジが案内されていて、該締付ネジ
は締め付けられた状態において埋め込み部材の両方の部
分を互いに固定する。締付ネジの領域においてスリーブ
壁を固定させるためには、締付ネジを備えた埋め込み部
分が軸方向において内側に位置するスリーブ縁に、残り
のスリーブ部分に比べるとより大きな壁厚のリングフラ
ンジを有していて、該リングフランジ中に締付ネジ用ネ
ジ穴が配設されていることが好ましい。ネジ穴中の締付
ネジの十分に大きな案内長を得るために、リングフラン
ジがネジ穴の領域において、半径方向に外側に突出する
突起を備えているのが合目的である。
ジ山の間に、ネジ山のないスリーブ部分を有していて、
該スリーブ部分中で開口がスリーブ周囲に渡って均一に
分配されていて、埋め込み部材中央部分の回転に役立つ
スパナの差し込みのためのスパナ開口として構成されて
いる。
ーブ部分の外側での開口の構成及び配設に関して、本発
明の枠内で、ある選択の自由がある。一方で、開口全体
の横断面形状ができる限り大であり、他方では、開口
は、埋め込み部分の間のネジ山接続の確実性が損われな
いよう、ネジ山領域を許容できないくらいに小さくして
はならない。前記全ての要求を特に好都合に満たしてい
て従って本発明の枠内で好ましい実施態様は、開口が埋
め込み部分中該部分のネジ山領域中に備えられていて、
スリーブ周囲方向に向けられた開口の長軸をもつ長穴と
して構成されていることを特徴としている。合目的に
は、開口は、自身の開口の長軸の方向に、スリーブ軸に
垂直な平面に並べられている。たいていの場合、各ネジ
山領域毎に、1つより多くの穴の列が備えられる。そこ
で、同一のネジ山領域中の幾つかの穴の列において、隣
接する開口が周方向において合目的には互いに位置をず
らされている。前記の如くに位置をずらされた開口にお
いては、一つの列の開口は自身の開口の中心が、主に、
隣接する列のそれぞれ連続する2つの開口の間の中央に
位置している。更に、開口の長軸が、それぞれの列にお
いて連続する開口の間の縁の距離よりも長く、特に2倍
より長いように、かつ、隣接する列に配設された開口の
間の軸方向の縁の距離が、開口の短軸の長さより小さ
く、特に約半分であるように、配設がなされていること
が好都合である。前記の如き開口配設の場合、隣接する
開口の間に、十分に大きくて、しっかりとつながってい
るネジ山領域が残っており、該領域は、埋め込み部分間
の荷重伝達を確実に保証する。
骨との間の力の伝達は、埋め込み部材端部の、椎骨に据
え付けるスリーブ縁を介してのみ可能である。しかしな
がらその際、椎骨における応力が余りに大きくなるなら
ば、スリーブ縁を拡大するか、もしくは、開口によって
できるだけ穴をあけられている横板で全体的に覆う可能
性が存在する。更に、椎骨に据え付けるためのスリーブ
縁は基本的には一平面中に配置され、その平面は個々の
場合の必要に応じてスリーブ軸にほぼ垂直に、もしく
は、スリーブ軸に傾斜した方向に配されていてよい。し
かしながらもちろん、スリーブ縁が、1つの平面からそ
れた面に配置されることも可能である。
の少なくとも1方の軸方向に外側の縁が、1つもしくは
幾つかの、隣接する椎骨中に入り込むために配置された
刃部もしくは尖端を備えていることによって、隣接する
椎骨における埋め込み部材末端部分の望ましくない移動
を防ぐ可能性が存在する。個々において、種々の構成が
可能である。そこで、刃部は、周方向において、縁に沿
って、連続してもしくは断続して配置されることができ
る。しかしながら、刃部は、半径方向においても、縁に
沿って次々に設けられた突起の端部として構成されてい
てよい。最後に、尖端は、縁に沿って配設され軸方向に
整列されたピンとして形成されていてよい。刃部もしく
は尖端が、隣接する椎骨中に深く入り込みすぎることを
回避するために、埋め込み部分に、刃部もしくは尖端の
入り込む深さを制限し、半径方向に内側もしくは外側へ
と突出するリングウエブが備えられていることが好まし
く、該ウエブは、隣接する椎体に対するストッパーとし
て働くことができる。該リングウエブの半径方向におい
て外側の縁部面は、ほぼ軸方向に向けられていて凸面の
稜部を備えていてよく、該縁部は生体埋め込み部材の確
実なセッティングに寄与する。

それ自体は描かれていない脊柱の椎体の間に装入するた
めに、脊柱から離れた椎骨もしくは椎部分のためのスペ
ース保持部材として働く。生体埋め込み部材は、隣接す
る椎骨に設置するために配置される2つの埋め込み部材
末端部分1と、該部分の間にある埋め込み部材中央部分
2とより成っている。該埋め込み部材中央部分はそれぞ
れ1つのネジ山3,4によって埋め込み部材末端部分に
接続されていて、その際、両ネジ山は脊柱の縦方向にお
いて共軸に配設されていて、特に図2より明らかなよう
に互いに反対のネジ方向を有している。このことによっ
て、両方の埋め込み部材末端部分1に対する埋め込み部
材中央部分2の相対的な回転の際に、生体埋め込み部材
全体の長さが変えられることができ、その際、特に、生
体埋め込み部材を備えた脊柱領域が埋め込みの際に伸延
されることができることが達成される。
て埋め込み部材中央部分2の外側上を覆い、その際、ネ
ジ山3,4はそれぞれ、埋め込み部材中央部分2上で雄
ネジとして、埋め込み部材末端部分1中で雌ネジとして
構成されている。埋め込み部材の両方の末端部分1は更
に、ネジ山領域に、半径方向のネジ穴5を有していて、
該ネジ穴中を、埋め込み部材中央部分に対して締付可能
で、図面においては見通しを良くするためにそれ自体は
不図示の締付ネジが案内されている。該締付ネジのそれ
ぞれの埋め込み部材中央部分2に対する締付によって埋
め込み部材の部分1,2は相互に回転しないよう固定さ
れる。その際、埋め込み部材末端部分1が、それぞれ軸
方向において内側に位置するスリーブ縁に、残りのスリ
ーブ部分に比べて大きな壁厚のリングフランジ6を有し
ていてよい。そこで締付ネジのためのネジ穴5が前記リ
ングフランジ6中に配設されていて、該リングフランジ
は、埋め込み部材端部を、締付ネジの締付の際に生じる
力に対して安定なものとする。又、前記リングフランジ
6は、ネジ穴5の領域において、半径方向において外へ
と突出する突起7を与えられてもよく、その結果、ネジ
穴5中において、締付ネジのための十分な大きさの案内
長が使われる。
ジ山3,4の間にネジ山のないスリーブ部分8を有して
いる。該ネジ山のないスリーブ部分8中において、開口
9がスリーブ周に渡って均一に分配されていて、かつ、
埋め込み部材中央部分2の回転に役立つ不図示のスパナ
の差し込みのためのスパナ開口として構成されている。
実施例においては、該スパナ開口が、円形シリンダ状の
穴の形を有していて、その結果、スパナは特に簡単な実
施態様において、単に、差込端部において円形シリンダ
状ロッドによって形成されていてよい。
全ての開口10は該埋め込み部材のネジ山3,4の領域
中に備えられていて、スリーブ周方向に向けられた開口
の長軸11を備えた長穴として構成されている。開口1
0はその長軸11の方向において、スリーブ軸に垂直な
平面に並べられている。図1及び図2の実施例において
は、それぞれ同一のネジ山領域3,4中に、2つの前記
の如き穴の列があり、他方、図4〜図12の実施例にお
いては、少なくとも3つの穴の列が備えられている。そ
の際、隣接する列の開口10が周方向において互いに位
置をずらされていて、その際、詳細には、それぞれ1つ
の列の開口は、自身の開口の中央が、隣接する列のそれ
ぞれ2つの連続する開口の間の中央に位置するように配
設がなされている。開口10の開口の長軸11は、それ
ぞれの列中で連続する開口の間の縁の距離の2倍よりも
大である。隣接する列中に配設された開口の間の軸方向
の縁の距離13は、開口の短軸14の長さの約半分であ
る。

るために配置される、軸方向において外側の縁15が、
スリーブ軸に垂直な平面中に配置される。それに対して
図6の実施例においては、前記の縁15は、スリーブ軸
に対して傾斜させられた平面中に配置され、その際、実
施例において傾斜角は10度である。前記の軸方向にお
いて外側の縁15は、図1、図10、及び、図12の実
施例における如く、刃部を備えているか、もしくは、図
4及び図6における如く、縁15に沿って、次々に設け
られた突起16を備えていてよく、その際、刃部ないし
は突起は、該刃部ないしは突起が隣接する椎骨中に程度
の差はあれ深く入り込むことによって、埋め込み部材末
端部分1の、隣接する椎骨に据え付けた状態で横への位
置のずれもしくは移動を防ぐという目的を有している。
突起16も、自身に刃部を備えていてよく、該刃部は、
椎骨中への突起の入り込みを容易にさせるために周方
向、もしくは、周方向に交差した方向に配される。その
際、入り込みの深さを制限するために、図1の実施例に
おける如く、下方の埋め込み部材末端部分1に半径方向
において外側へと突出するリングウエブ17が備えられ
ていてよく、該リングウエブは、隣接する椎骨に対する
ストッパーとして働く。リングウエブ17の外側の縁の
面は軸方向に向けられており、外側へと突出する、即
ち、凸面の稜部18を備えている。

に維持される。

- 【請求項1】 脊柱から離れた椎骨もしくは椎部分のた
めのスペース保持部材として、脊柱の椎体間に装入する
ための生体埋め込み部材で、隣接する椎骨に据え付ける
ために配置される2つの埋め込み部材末端部分(1)
と、該埋め込み部材末端部分の間にある1つの埋め込み
部材中央部分(2)とより成っていて、該埋め込み部材
中央部分は、埋め込み部材末端部分(1)とそれぞれ1
つのネジ山(3,4)で接続されていて、その際、両ネ
ジ山(3,4)は脊柱の縦方向において共軸に配設され
ていて、互いに逆のネジ山方向を有していて、その結
果、埋め込み部材中央部分(2)の回転によって、生体
埋め込み部材全体の長さが変更可能であるというものに
おいて、 埋め込み部材の3つの部分(1,2)全てが少なくとも
ネジ山(3,4)の領域において円形の横断面を有する
管状のスリーブとして構成されていて、互いに共軸にネ
