となって挿入される。内部2のテーパー部分1によりス
クリューは骨孔6への侵入を開始するが、ねじ山部7
で、靭帯4を固定した後、内部2をスクリューの挿入方
向と逆方向に外す。外部3はねじ山部7を持っている
が、先端8は、靭帯4へのストレスを減らすため、鈍な
構造になってもよい。内部2にはガイドワイヤ9が通る
穴10があり、ガイドワイヤ9下に、スクリューを通す
ことでスクリューの方向性を安定させる。外部3の後端

13が、穴10より大きいとき、ガイドワイヤ9を挿入
方向と逆に引き抜くことにより、内部2が抜ける。これ
により靭帯4が動いても、従来に比べスクリュー先端で
靭帯4を傷付ける可能性が減る。図11の様に、ブロッ
カー18を挿入すれば、外部3がつくる死腔を減らすこ
