Claims (1)
- 【特許請求の範囲】 1.手術に使用するアクセスポート装置であって、患者の体に作られた切開部へ 挿入するために、遠位端に出口開口部を有するスリーブを包含し、前記出口開口 部が患者の体腔へのアクセスを可能とし、前記装置はさらに切開部への挿入のた めの出口開口部密封手段を含み、それにより患者の体腔がガスで膨らまされた時 、前記出口密封手段が外科医の手または手術器具のアクセスを提供する一方、患 者の体腔からのガスの実質的な漏洩を防止するアクセスポート装置。 2.請求項1に記載のアクセスポートであって、スリーブの近位端に入口開口部 、および前記入口開口部の領域に前記装置を密封するための入口密封手段を有し 、それにより患者の体腔がガスで膨らまされた時、入口密封手段が外科医の手の ためのアクセスを提供し、かつ前記アクセスポート装置の外側に残っている腕の 周りを密封する一方、患者の体腔からのガスの実質的な漏洩を防止するアクセス ポート。 3.請求項2に記載のアクセスポートであって、前記入口密封手段がスリーブの 近位端に備えられた膨張可能チャンバを包含するアクセスポート。 4.請求項2または3に記載のアクセスポートであって、前記スリーブにアクセ スポートを患者の体外に固定するための粘着剤を有するフランジを備えたもので あるアクセスポート。 5.請求項4に記載のアクセスポートであって、前記フランジがスリーブの近位 端および遠位端の間にあり、そのため使用時にフランジが患者の体に接着された 時に、スリープの遠位端が切開部から挿入されて、患者の体腔内に入り、かつア クセスポートが患者の体から僅かな距離突き出るものであるアクセスポート 6.前記請求項のいずれかに記載のアクセスポートであって、スリーブの遠位端 が切開部を通して挿入された時、切開部の周りの患者の筋肉組織が前記スリーブ の遠位端および近位端の間の中間部分の密封を助けるための密封手段として機能 するアクセスポート。 7.前記請求項のいずれかに記載のアクセスポートであって、入口密封手段が、 スリーブを取り巻く形に配置され、かつスリーブに圧力をかけることができる膨 張可能チャンバを包含し、少なくともスリーブの一部分を押し潰すことによって 前記入口開口部を密封するアクセスポート。 8.前記請求項のいずれかに記載のアクセスポートであって、膨張可能チャンバ がスリーブと流体連通しておらず、そのため使用時に前記チャンバが患者の体腔 と流体連通せず、従って膨張可能チャンバ内の圧力が患者の体腔内圧力と異なる ものであるアクセスポート。 9.請求項3〜7のいずれかに記載のアクセスポートであって、前記膨張可能チ ャンバがスリーブと流体連通するアクセスポート。 10.請求項7または8のいずれかに記載のアクセスポートであって、前記膨張 可能チャンバが上部チャンバと下部チャンバを規定する一般的な“砂時計”型の 形状であり、前記下部チャンバが患者の体腔に作られた切開部に挿入可能なもの であるアクセスポート。 11.請求項3に記載のアクセスポートであって、前記スリーブおよび膨張可能 チャンバが共軸であり、かつそれらの共通近位端と側端部に結合されたガス透過 性の可撓性材料のシートを含み、前記スリーブが膨張可能チャンバの遠位領域内 にあるアクセスポート。 12.請求項11に記載のアクセスポートであって、チャンバが装置の近位領域 にある内側スリーブの周りにある外側スリーブで規定され、また内側スリーブ内 にあって、さらに可撓性材料の2つのシートの間に形成されて近位端から遠位端 へ伸びるのがフラップバルブであるアクセスポート。 13.請求項12に記載のアクセスポートであって、フラップバルブの遠位端部 がフェザー構造(そぎ端)であり、かつチャンバが加圧された時にフラップバル ブのシートおよび端部が互いに内向きに押し潰されてシールを形成するアクセス ポート。 14.請求項12または13に記載のアクセスポートであって、フラップバルブ が内側および外側スリーブに、それぞれの側面に沿った2つの場所で結合され、 それによってその2つの場所の間のフラップバルブの領域が外科医の手または腕 の部分の周りに沿うことができるアクセスポート。 15.請求項11〜14に記載のアクセスポートであって、内側および外側スリ ーブが特定の場所で接合部(join)により互いに連結され、それによって膨張可能 チャンバを流体連通する上部および下部サブチャンバに区分し、そのため加圧さ れた時に前記サブチャンバが入口密封手段またはその一部を形成する少なくとも 1つの辺縁表面シールを規定するものであるアクセスポート。 16.請求項15に記載のアクセスポートであって、接合部(join)が複数の対向 する接合(weld)を含むものであるアクセスポート。 17.前記請求項のいずれかに記載のアクセスポートであって、出口密封手段が スリーブの遠位端部またはその近接部に、患者の腹腔内のガス圧力により押し潰 されるスリーブのシートにより与えられるアクセスポート。 18.請求項17に記載のアクセスポートであって、シートに通常横方向の力を かけ、出口密封手段の一部分としてさらなるシールの作用を有するぴんと張った 領域をスリーブに作り出すために、スリーブの遠位端から離れた遠位領域に別個 の緊縮装置を備えたアクセスポート。 19.請求項18に記載のアクセスポートであって、前記緊縮装置が、腹腔から のスリーブの後退を防止するように働く一対の対向弓型バンドを包含するアクセ スポート。 20.請求項19に記載のアクセスポートであって、対向弓型バンドの固定点を 提供するために、スリーブの側端部にウイングが備えられたアクセスポート。 21.トロカール・スリーブを通して挿入および後退できるような適当なサイズ を有する延長シャフト、並びに脱着可能な器具頭部および前記器具頭部をシャフ トに脱着可能に結合するための手段を包含する手術用器具であって、それにより 器具頭部が結合されない状態でシャフトをトロカール・スリーブを通すことがで き、かつ器具頭部を患者の腹腔内でシャフトに脱着可能に取り付けできるもので ある手術用器具。 22.請求項21に記載の手術用具であって、前記脱着可能頭部がトロカール・ スリーブを通過するのに必要なサイズであることを要しない手術用器具。 23.低剥離強度の接着剤を有する手術用ドレープと組み合わせて使用できるよ うに強力接着剤を備えた少なくとも1つの表面を有する手術用具であって、使用 時に前記用具がその強力接着剤によりドレープの非接着側に固定されるものであ る手術用具。 24.請求項23に記載の手術用具であって、前記強力接着剤が前記手術用具の 一部領域に適用される手術用具。
