Claims (1)
- 【特許請求の範囲】 1.アクセスチューブ集成体であって: 近位端、遠位端及びこの近位端から遠位端に及ぶ中央ルーメンを有する弾性ア クセスチューブ、ここでこのチューブは軸張力又は放射状圧縮に委ねられていな いときは固有の長さ及び固有の外径を有している; 前記ルーメン内に収納可能であり、且つ前記弾性アクセスチューブの遠位端に 結合しているスタイレット; 前記弾性チューブの少なくとも遠位部分を被覆しており、且つ前記弾性チュー ブの固有の外径よりも小さい内径を有する取外可能なさや; を含んで成る集成体。 2.前記弾性アクセスチューブがその遠位端付近に膨張式アンカーを有する、 請求項1記載のアクセスチューブ。 3.前記膨張式アンカーが Malecot構造をとっており、ここでこの構造は前記 スタイレットが前記弾性アクセスチューブの遠位端と結合しているときに伸長、 そして崩壊できるものである、請求項2記載のアクセスチューブ集成体。 4.前記膨張式アンカーが膨張式バルーンである、請求項2記載のアクセスチ ューブ集成体。 5.弾性アクセスチューブが密閉された遠位端を有し、その遠位端は前記スタ イレットと、及びその遠位端の近傍に位置する少なくとも一側面ポートと結合し ている、請求項1記載のアクセスチューブ集成体。 6.前記側面ポートが、前記アクセスチューブが軸張力下にないときに開く1 又は複数の軸状スリットより成る、請求項5記載のア クセスチューブ集成体。 7.前記アクセスチューブの近位端を着脱式に固定するための前記スタイレッ ト上のラッチ機構を更に含んで成り、ここでこのラッチ機構は前記スタイレット の遠位端から前記弾性アクセスチューブの固有の長さよりも20%以上長い距離離 れて位置している、請求項1記載のアクセスチューブ集成体。 8.前記弾性アクセスチューブが2cm〜20cmの範囲の固有の長さ及び2mm〜8 mmの範囲の固有の外径を有する、請求項7記載のアクセスチューブ集成体。 9.前記弾性アクセスチューブの近位端上に固定具を更に含んで成り、ここで この固定具は前記ラッチ機構によって固定されており、そしてこのラッチ機構は 前記スタイレットの遠位端から3cm〜40cmの範囲の距離離れて位置している、請 求項8記載のアクセスチューブ集成体。 10.前記取外式さやが2mm〜8mmの範囲の内径を有するポリマーチューブであ る、請求項1記載のアクセスチューブ集成体。 11.前記ポリマーチューブが、前記さやの分割及び除去を促すために少なくと も片側上に軸状に筋が付けられているか又は分割されているものである、請求項 10記載のアクセスチューブ集成体。 12.アクセスチューブ集成体であって: 近位端、遠位端及びこの近位端から遠位端に及ぶ中央ルーメンを有する弾性ア クセスチューブ、ここでこのチューブは軸張力又は放射状圧縮に委ねられていな いときは固有の長さ及び固有の外径を有している;並びに 近位端及び遠位端を有し、且つ前記弾性アクセスチューブの前記中央ルーメン 内に収納可能であるスタイレット、ここでこのスタイレットの遠位端は前記アク セスチューブの遠位端に着脱式に固定す ることが可能であり、そしてこのスタイレットは経皮導入を促すためにその遠位 端に貫入要素を含む; 近位端、遠位端及びこの近位端から遠位端に及ぶ中央ルーメンを有する弾性ア クセスチューブ、ここでこのチューブは軸張力又は放射状圧縮に委ねられていな いときは固有の長さ及び固有の外径を有する;並びに 前記中央ルーメン内に収納可能であり、且つ前記弾性アクセスチューブの遠位 端に対して結合可能であるスタイレット、ここでこの弾性アクセスチューブ上に はこのスタイレット由来の張力下に委ねられているときにテーパー状の遠位端が 形成され、これによりこのテーパー状の遠位端は前記長い拡張部材の軸状ルーメ ンを介する前記アクセスチューブ及びスタイレットの導入を促す; を含んで成るアクセスチューブ集成体。 22.前記長い拡張部材が前記放射状膨張式管状本体の上に形成された取外式さ やを更に含み、ここでこのさやは、前記長い拡張部材が経皮的に進行する際に前 記管状本体を覆っており、それ故前記弾性アクセスチューブのテーパー状の先端 が前記管状本体を通って進行する際に前記管状本体の放射状膨張を可能にするよ うにその後取外されることのできるものである、請求項21記載のアクセスチュー ブ集成体。 23.前記放射状に膨張式管状本体が弾性チューブ又は管状ブレー ドである、請求項21記載のアクセスチューブ集成体。 24.前記貫入部材が鋭利な遠位端を有する長い貫入要素を含んで成り、この要 素は前記放射状膨張式管状本体の軸状ルーメン内に取外式に収納されており、そ の鋭利な遠位端はそのルーメンから遠位方向に突き出ている、請求項21記載のア クセスチューブ集成体。 25.前記長い拡張部材がその遠位端付近に膨張式アンカーを更に含んでいる、 請求項21記載のアクセスチューブ集成体。 26.前記弾性アクセスチューブがその遠位端付近に膨張式アンカーを有する、 請求項1記載のアクセスチューブ。 27.前記膨張式アンカーが Malecot構造をとっており、ここでこの構造は前記 スタイレットが前記弾性アクセスチューブの遠位端と結合しているときに伸長、 そして崩壊できるものである、請求項26記載のアクセスチューブ集成体。 28.前記膨張式アンカーが膨張式バルーンである、請求項26記載のアクセスチ ューブ集成体。 29.前記弾性アクセスチューブが密閉された遠位端を有し、その遠位端は前記 スタイレットと、及びその遠位端の近傍に位置する少なくとも一側面ポートと結 合している、請求項21記載のアクセスチューブ集成体。 30.前記側面ポートが、前記アクセスチューブが軸張力下にないときに開く1 又は複数の軸状スリットより成る、請求項29記載のアクセスチューブ集成体。 31.前記アクセスチューブの近位端を着脱式に固定するための前記スタイレッ ト上のラッチ機構を更に含んで成り、ここでこのラッチ機構は前記スタイレット の遠位端から前記弾性アクセスチューブの固有の長さよりも20%以上長い距離離 れて位置している、請求項21記載のアクセスチューブ集成体。 32.前記弾性アクセスチューブが2cm〜20cmの範囲の固有の長さ及び2mm〜8 mmの範囲の固有の外径を有する、請求項31記載のアクセスチューブ集成体。 33.前記弾性アクセスチューブの近位端上に固定具を更に含んで成り、ここで この固定具は前記ラッチ機構によって固定されており、そしてこのラッチ機構は 前記スタイレットの遠位端から3cm〜40cmの範囲の距離離れて位置している、請 求項32記載のアクセスチューブ集成体。 34.前記取外式さやが2mm〜8mmの範囲の内径を有するポリマーチューブであ る、請求項21記載のアクセスチューブ集成体。 35.前記ポリマーチューブが、前記さやの分割及び除去を促すために少なくと も片側上に軸状に筋が付けられているか又は分割されているものである、請求項 34記載のアクセスチューブ集成体。 36.組織貫入部の中にアクセスチューブを導入及び定着するための方法であっ て: 前記アクセスチューブをこの貫入部を介して標的箇所に至るまで導入し、同時 に前記アクセスチューブをその外径が軸張力に委ねられていないときの固有の直 径よりも10%以上小さくなるのに足りる軸張力に委ねる;そして 前記軸張力を解放せしめ、前記チューブの直径が膨張し、且つ前記貫入部の周 囲組織をシールするようにさせる; ことを含んで成る方法。 37.前記標的箇所が胃、腸、腎臓、胆嚢、胸部、肺、腹膜、血管及び膀胱より 成る群から選ばれる空洞身体器官であるか、又は嚢胞である、請求項36記載の方 法。 38.前記アクセスチューブを予め形成しておいた組織貫入部の中に導入する、 請求項36記載の方法。 39.前記アクセスチューブを組織貫入部の形成と同時に導入する、請求項36記 載の方法。 40.前記組織貫入部が、軸張力に委ねられたアクセスチューブの遠位端に固定 された貫入要素を組織を通じて標的箇所に至るまで進行させることにより形成さ れるものである、請求項39記載の方法。 41.前記組織貫入部が前記アクセスチューブの固有の直径よりも小さい有効な 直径を有し、そしてここで前記アクセスチューブ上にはその外径を前記有効な直 径よりも小さくするのに十分な軸張力がかかっている、請求項36記載の方法。 42.前記組織貫入部の有効な直径が1mm〜6mmの範囲にあり、前記アクセスチ ューブの固有の直径が2mm〜8mmの範囲にあり、そして前記アクセスチューブの 外径が軸張力下で導入されたときに2mm〜8mmの範囲にある、請求項41記載の方 法。 43.前記アクセスチューブが前記貫入部を通じて導入される際にさやで覆われ ており、前記アクセスチューブの放射状膨張を可能とするように前記さやを取外 すことを更に含んで成る方法。 44.前記さやが取外しを促すために少なくとも一側面に軸状に筋が付けられて いるか又は分割されている、請求項43記載の方法。 45.貫入すべき組織を拡張することを更に含んで成る請求項36記載の方法。 46.前記組織貫入部を前記アクセスチューブの導入前に拡張させる、請求項45 記載の方法。 47.前記組織貫入部を前記アクセスチューブの導入の際に拡張させる、請求項 46記載の方法。 48.前記組織貫入部の後部上で前記アクセスチューブの遠位端付近のアンカー を膨張させることを更に含んで成る、請求項36記載の方法。 49.前記アンカーが Malecot構造であり、前記アクセスチューブ上の軸張力を 解放せしめたときに膨張するものである、請求項48記載の方法。 50.前記アンカーを、前記アクセスチューブを導入した後にふくらます、請求 項48記載の方法。 51.十分な軸張力を供するように内部スタイレットの上で前記アクセスチュー ブを軸状に伸長させることを更に含んで成る、請求項36記載の方法。 52.前記アクセスチューブを導入する際に前記軸張力を維持するよう、前記ア クセスチューブの近位端に前記スタイレットを取外式に固定することを更に含ん で成る、請求項51記載の方法。 53.軸張力の解放が前記スタイレットからのアクセスチューブの近位端の取外 しを含み、これにより前記アクセスチューブの直径は大きくなる、請求項52記載 の方法。 54.組織貫入部の中にアクセスチューブを導入及び定着するための方法であっ て: 組織を通じて標的箇所に至るまで長い拡張部材を貫入する; 前記拡張部材のルーメンの間に前記アクセスチューブを導入し、同時に前記ア クセスチューブをその外径が軸張力に委ねられていないときの固有の直径よりも 10%以上小さくなるのに足りる軸張力に委ねる; 前記長い拡張部材を取外し、前記アクセスチューブを前記組織貫入部に残す; そして 前記軸張力を解放せしめ、前記チューブの直径が膨張し、且つ前記貫入部の周 囲組織をシールするようにさせる; ことを含んで成る方法。 55.前記標的箇所が胃、腸、腎臓、胆嚢、胸部、肺、腹膜、血管 及び膀胱より成る群から選ばれる空洞身体器官であるか、又は嚢胞である、請求 項54記載の方法。 56.前記アクセスチューブを、組織貫入部が形成された実質的に直後に前記拡 張部材の間に導入する、請求項54記載の方法。 57.張力下で導入したときの前記アクセスチューブの外径が前記長い拡張部材 のルーメンの直径より大きく、これにより前記拡張部材の直径は前記アクセスチ ューブを前記部材の近位端から前記部材の遠位端を通じて進行させる際に放射状 に膨張する、請求項54記載の方法。 58.前記拡張部材のルーメンの直径が1mm〜6mmの範囲にあり、そして軸張力 下で導入したときの前記アクセスチューブの外径が2mm〜8mmの範囲にあり、そ して前記アクセスチューブの固有の直径が5mm〜8mmの範囲にある、請求項57記 載の方法。 59.前記アクセスチューブが前記拡張部材のルーメンの間に導入される際にさ やで覆われており、前記アクセスチューブの放射状膨張を可能とするように前記 さやを取外すことを更に含んで成る方法。 60.前記さやが取外しを促すために少なくとも一側面に軸状に筋が付けられて いる請求項59記載の方法。 61.前記組織貫入部の後部上で前記アクセスチューブの遠位端付近のアンカー を膨張させることを更に含んで成る、請求項54記載の方法。 62.前記アンカーが Malecot構造であり、前記アクセスチューブ上の軸張力を 解放せしめたときに膨張するものである、請求項61記載の方法。 63.前記アンカーを、前記アクセスチューブを導入した後にふくらます、請求 項61記載の方法。 64.十分な軸張力を供するように内部スタイレットの上で前記アクセスチュー ブを軸状に伸長させることを更に含んで成る、請求項54記載の方法。 65.前記アクセスチューブを導入する際に前記軸張力を維持するよう、前記ア クセスチューブの近位端に前記スタイレットを取外式に固定することを更に含ん で成る、請求項64記載の方法。 66.軸張力の解放が前記スタイレットからのアクセスチューブの近位端の取外 しを含む、請求項65記載の方法。
