Abstract

PROBLEM TO BE SOLVED: To suture at health tendon part even if the length till a health tendon tissue is long by forming a medical suturing needle which is a straight needle having a sharp top edge and fixing a thick nylon filament as a loop shape at another end. SOLUTION: The medical suturing needle is formed by using a straight needle having a sharp edge at one end and a thick nylon monofilament fixed as loop shape at another end thereof. For example, a needle with a loop filament is formed by fixing 3-0 or 2-0 nylon filament of about 30cm or 40cm length making a loop shape at the not sharp end of a straight needle of 3cm or 5cm length with a sharp edge. Using the straight needle with loop filament, the filament is penetrated into periphery stump from a peritenon A3 to a periphery by TSUGE method. Then, the needle is penetrated through A3 peritenon, A2 peritenon and A1 peritenon in the order and the side of the perphery is pulled into the center of the peritenon by drawing the filament. Then the filament is penetrated into the central stump of the tendon using TSUGE method and suture is conducted at the center of a given position from the A1 peritenon.

かしこの縫合針は弯曲しているために、長い腱内を真っ
直ぐに貫通させるためには不自由である。例えば陳旧性
の腱断裂で縫合部に強い緊張が加わったり、腱移植術を
必要とする時、さらには腱を骨内のトンネル中に埋入さ
せる時など不自由である。そこで、これらの問題点を改
善すべく弯曲した針ではなく3cm、5cm、7cmお
よび10cmの長さの直針で先端を角状に鋭利にした針
を作成し、この針の反対側(鈍側)にループ状にしたナ
イロン糸を固着させた(図2)。 [構成]長さ30cmあるいは40cmの3−0ナイロ
ン糸、2−0ナイロン糸あるいは1−0ナイロン糸をル
ープ状として、3cm、5cm、7cm、10cmの先
端を先鋭とした直針の鈍側にそれぞれ固着させた11種
類のループ糸付き直針を作成する(表1)。

腱鞘より末梢でまず津下法により腱の末梢断端に糸を通
し、次にこの針をA3腱鞘、次いてA2腱鞘さらにA1
腱鞘内を通過させたのち糸をひっぱり、腱の末梢側を腱
鞘内中枢に引き込む。しかる後、腱の中枢断端にも図6
のごとく、津下法により糸を通し、A1腱鞘より2cm
から3cm中枢で縫合する。これにより、”no ma
n’s land”での屈筋腱においては、ナイロン糸
は腱内をのみ通過するため外見上ナイロン糸は見えず
(図7)、従来の腱縫合部が存在する場合に比べて著し
く癒着の改善に寄与することがわかった。
