ーシスを提供する。 【解決手段】 取付部材（２）と、この取付部材をロッ
クするネジ（５）とを備えた関節エンドプロステーシス
である。取付部材は、エンドプロステーシス上に設けら

有する。縮径部分は、ガイドの方向に延在するスリット
（６）を有する第２の表面部分（８）の領域に置かれ、
スリットは縮径部分を受け入れ、且つ拡径端部の径寸法
よりも小さな幅寸法を有する。

Description

【発明の詳細な説明】
【０００１】
【発明の属する技術分野】本発明は、関節エンドプロス
テーシスに係り、特にその取付部材の改良に関する。
【０００２】
【従来の技術】挿入術中に取り付けられたり、あるいは
その後の再手術で取り外したりされるエンドプロステー
シス（体内プロステーシス）の取付部材は、その処置に
必要とされる注意や手先の器用さが可能な限り少なくて
すむように、最も単純で可能な方法で取付られ且つ固定
されるべきである。エンドプロステーシスを取付部材に
接続することは、プロステーシスおよび取付部材に互い
に協働するガイド手段を設けることとして知られてお
り、このガイド手段は、一方を他方の中へ簡単に押し込
むことができ、プロステーシスの使用中にあっては力伝
達のかなりの部分を受け持つことができるものである。
この場合、取付部材の固定とは、取付部材がガイドに沿
って相対的に移動するのを防止することに限られる。こ
の目的のためにネジを用いることが知られており、この
ネジは、プロステーシスと取付部材との重なり合う表面
部分の領域に設けられる。これらの表面部分の一方は、
そのネジを受け入れるネジ穴を含んでおり、他方の表面
部分は、ネジがその固定終端位置にきたときにネジの一
部を受け入れる凹部を含んでいる。その一例が、周知の
股関節エンドプロステーシス（ヴァルデマール・リンク
からのパンフレット「リッペンシステム」）であり、大
腿骨内に挿入されるそのステムは、該ステム上に設けら
れた摺動ガイドに沿って主要部分の取り付け及び取り外
しができるネックベアリングをその端部に備えている。
そのステム上に設けられて且つネックベアリングに平行
に延びた取付部材は、ネックベアリングに設けられたネ
ジ穴内にネジを導入するための穴を備えている。これ
は、操作の煩わしさを生じ、技術を必要としてしまう。

【発明が解決しようとする課題】したがって本発明は、
請求項１の前段部分に特定されるタイプの関節エンドプ
ロステーシスにおいて、取付部材の固定に必要とされる
注意や技術を最小限にしてその操作を容易にする関節エ
ンドプロステーシスを提供することを目的とするもので
ある。その解決手段は、請求項１に規定される特徴にあ
り、さらに好ましくは、請求項１に従属する各請求項に
規定される特徴にある。
【０００４】
【課題を解決するための手段】本発明に係る関節エンド
プロステーシスは、上述のごとき従来技術の課題を解決
し、その目的を達成するために以下のような構成を備え
ている。即ち、取付部材と、該取付部材をロックするた
めのネジとを備えた関節エンドプロステーシスであっ
て、上記取付部材は、該エンドプロステーシス上に設け
られたガイドに沿って着脱可能であり、上記ネジは、該
プロステーシスと取付部材との表面部分の重なり合う領
域に配置され、その第１の表面部分が、該ネジを受け入
れるネジ穴を含んでいる関節エンドプロステーシスにお
いて、上記ネジのシャンクが、該ネジの頭部に隣接して
ネジ部分と、該ネジ部分に隣接して縮径部分と、該縮径
部分に隣接して拡径端部とを有しており、上記縮径部分
が、上記ガイドの方向に延在するスリットを有する第２
の表面部分の領域に置かれ、該スリットは該縮径部分を
受け入れ、且つ上記拡径端部の径寸法よりも小さな幅寸
法を有する。
【０００５】上記ネジ穴を含んでいる上記第１の表面部
分は、より簡単にアクセス可能であるのが好ましい。
【０００６】上記第２の表面部分は、少なくとも部分的
にでも上記拡径端部を受け入れる凹部を、上記スリット
の閉じられた端部に有しているのが好ましい。
【０００７】上記拡径端部は、上記スリットの方に向け
られた円錐面を有しているのが好ましい。
【０００８】取付小片を取り付ける前に、ネジが、その
目的の位置に達するまで、第１の表面部分のネジ穴内へ
ねじ込まれるのを確実にするように注意が払われる。こ
のようにして固定準備の整った位置の状態となり、規定
の位置に緩く保持される必要はなくなる。また、操作中
にその軸方向の位置決めにも注意を払う必要がなくな
る。何故ならば、ネジは限界のストッパに達するまで回
せばよいからである。ここには失敗の生じる余地はな
い。ネジの螺刻された部分に隣接する縮径部分は、取付
部材の第２の表面部分が来る箇所で待機する。第２表面
部分は、適切な位置でスリットを有しており、このスリ
ットは、取付部材がガイド内へ前進させられるときに縮
径部分を受け入れるような位置に形成されている。そし
てネジの拡径端部は、第１の表面部分から見たとき第２
表面部分のもう一方の側に位置し、且つ取付部材の位置
変えを許容するように、第２表面部分のもう一方の側か
ら距離を隔てている。取付部材がその目的の位置へ達す
ると、ネジはその拡径端部が第２表面部分に噛み合うま
で、すなわちスリットのエッジに噛み合うまで締め付け
られ、このことによって取付部材が固定される。したが
って手術中の外科医は、ネジを一端の位置から他端の位
置まで単純に回すだけでよい。このことは、何ら特別な
注意を必要としない単純作業である。取り決められた一
方端位置から他方端位置までの距離だけネジを回せばよ
いので、ただ単に僅かだけネジを回すことが必要とされ
る。そしてこの取り決められた距離は、非常に小さくす
ることができ、ネジの１回転の数分の１で取り決めるこ
とができる。ねじ回しを繰り返して噛み合わせることは
殆ど必要なくなる。
【０００９】ネジの拡径端部が第２表面部分に設けられ
たスリットのエッジと共に機能することは、固定機能の
ための如何なる積極的なロック手段をも必要としない。
ネジをその終端位置まで締め付けることによって得られ
る押圧力は十分なものである。しかし、積極的なロック
が必要であれば、例えば、少なくとも部分的にでもその
拡径している部分を受け入れる凹部と共にスリットの閉
じられた端部を設けることによって、その手段を設ける
こともできる。これに加えて、あるいはその代りとし
て、スリットに面しているネジの拡径端部の側面は、円
錐面に形成することができ、また、ネジの僅かな締め付
けによってでも塑性変形をもたらすようにできる。この
塑性変形は、取付部材を固定するだけでなく、ネジがそ
の終端位置から弛んでしまわないようにネジに大きな摩
擦力を与えもする。
【００１０】ネジ穴は、より簡単にアクセスできるよう
に表面部分に設けられており、この場合のネジ頭は、ね
じ回しと噛み合う例えば６角穴を有しており、手術中の
外科医の方向に向けられる。しかし、その反転配置もま
た考え得る。そしてそのネジの拡径端部は、例えば回転
工具のための６角穴のような取付構造が設けられる。

【発明の実施形態】股関節プロステーシスは、そのネッ
ク部１が断面で示されているが、取り外し可能なネック
ベアリング２を有している。このネックベアリングは、
プロステーシスのカラー部分３に押し付けられる。その
目的のために有効なのは、それ自体は知られているので
詳細は説明しないが、ガイドの構成（guide arrangemen
ts）であり、その形状は破線によって参照符号４で示さ
れている。前進させられた後のネックベアリングの位置
は一点鎖線によって左に示されている。前進した位置で
ネックベアリングを固定するために、ネジ５が用いられ
ており、そのシャンクがネックベアリング内のスリット
６によって受け入れられる。ネジ５は、カラー３の表面
部分７に着座する。このネジと共に機能するスリット６
は、ネックベアリングの表面部分８に配置されている。
【００１２】これらの表面部分は、ネックベアリングの
前進が完了する前の状態で図２に示されている。スリッ
ト６は、ネジ５までに未だ達していない。
【００１３】ネジ５は、ネジ回しが噛み合うための６角
穴１１を内側に有する頭部１０と、表面部分７のネジ穴
１３に螺合するネジ１２であって、その軸方向長さが表
面部分７の厚さよりも長くないネジ１２と、縮径部分１
４と、ネジの部分に向かって縮径されていく円錐面１６
を有する拡径端部１５とを備えている。ネジの締め込み
前の状態では、ネジは完全に表面部分７の中へねじ込ま
れており、その頭部は表面部分７の上面に着座してい
る。この状態において、縮径部分１４の自由な部分の長
さは、少なくとも表面部分８の厚さ寸法よりは長くされ
ている。
【００１４】表面部分８がその目的の位置へ達した後、
その位置では縮径部分１４の回りをスリット６が取り囲
むことになるが、ネジ５が回されてその頭部１０が表面
部分７の上面から離れる。この操作によって、ネジの拡
径端部１５は表面部分８へ接近し、その円錐面１６がス
リット６のエッジを最終的にグリップするまで進む。ネ
ジが図３に示す位置まで十分に回されると、ネジは、表
面部分８またはネックベアリング２をその機能する位置
に固定する。

【発明の効果】本発明に係る関節エンドプロステーシス
では、取付部材の固定に必要とされる注意や技術を最小
限にでき、その操作は極めて容易になって手術中の外科
医の負担を軽減できる。

    【特許請求の範囲】
  1. 【請求項１】 取付部材（２）と、該取付部材をロック
    するためのネジ（５）とを備えた関節エンドプロステー
    シスであって、 上記取付部材は、該エンドプロステーシス上に設けられ
    たガイド（３，４）に沿って着脱可能であり、 上記ネジは、該プロステーシスと取付部材との表面部分
    （７，８）の重なり合う領域に配置され、その第１の表
    面部分（７）が、該ネジ（５）を受け入れるネジ穴（１
    ３）を含んでいる関節エンドプロステーシスにおいて、 上記ネジのシャンクが、該ネジの頭部（１０）に隣接し
    てネジ部分（１２）と、該ネジ部分に隣接して縮径部分
    （１４）と、該縮径部分に隣接して拡径端部（１５）と
    を有しており、 上記縮径部分（１４）が、上記ガイド（３，４）の方向
    に延在するスリット（６）を有する第２の表面部分
    （８）の領域に置かれ、該スリット（６）は該縮径部分
    （１４）を受け入れ、且つ上記拡径端部（１５）の径寸
    法よりも小さな幅寸法を有することを特徴とする関節エ
    ンドプロステーシス。
  2. 【請求項２】 上記ネジ穴（１３）を含んでいる上記第
    １の表面部分（７）は、より簡単にアクセス可能である
    ことを特徴とする請求項１記載のプロステーシス。
  3. 【請求項３】 上記第２の表面部分（８）は、少なくと
    も部分的にでも上記拡径端部（１５）を受け入れる凹部
    を、上記スリット（６）の閉じられた端部に有している
    ことを特徴とする請求項１または２記載のプロステーシ
    ス。
  4. 【請求項４】 上記拡径端部（１５）は、上記スリット
    （６）の方に向けられた円錐面（１６）を有しているこ
    とを特徴とする請求項１ないし３のいずれかに記載のプ
    ロステーシス。
