Abstract
Description
Claims (1)
- 【特許請求の範囲】 1.姿勢に依存するペーシング拍数で患者の心臓をペーシングする装置において 、 患者が前方に傾斜する程度を表わす身体姿勢傾斜信号を取得する手段と、 ペーシング拍数を確立するように逸脱間隔を規定し、患者が前方に傾斜するこ とを示す前記身体姿勢傾斜信号に応答して前記逸脱間隔を変更する手段を含む逸 脱間隔規定手段と、 ペーシング拍数でペーシング・パルスを生成する手段と、 ペーシング・パルスを患者の心臓に印加する手段と を備える患者の心臓をペーシングする装置。 2.患者の活動を検知し、患者の活動に依存する信号レベルを有する患者活動信 号を取得する手段を更に備え、前記逸脱間隔手段が、前記身体姿勢傾斜信号と前 記患者活動信号の双方に応答する逸脱間隔を規定する手段を含む請求項1記載の 装置。 3.身体姿勢傾斜信号を取得する前記手段が更に、第1の予め定めた低周波レン ジで反復し測定された傾斜信号を取得する患者の身体の前後軸における加速度の 作用力を測定する手段を含む請求項1または2に記載の装置。 4.患者活動を検知する前記手段が更に、前記第1の予め定めた低周波レンジよ りも周波数が高い第2の予め定めた周波数レンジで反復し患者の活動信号を取得 する患者の身体の前後軸における加速度の作用力を測定する手段を含む請求項2 記載の装置。 5.身体姿勢傾斜信号を取得する前記手段と患者の活動を検知する前記手段とが 更に、 患者の身体の略々前後の軸に沿って患者の身体に対する重力の作用力に応答し 、該作用力から患者が直立スタンスから前方に傾斜する程度を表わす極性と大き さの前方傾斜信号を取得する手段と、 患者の足取りによる患者の身体における加速度の作用力を測定し、該作用力か ら患者の足取りを表わす患者活動信号を取得する手段とを含む請求項2記載の装 置。 6.逸脱間隔を規定する前記手段が更に、 前記患者活動信号の関数として患者の心臓をペーシングする目標拍数制御信号 を生じる手段と、 階段登り拍数制御信号を生じる手段と、 傾斜ウインドウを規定する手段と、 前記身体姿勢傾斜信号を前記傾斜ウインドウと比較する手段と、 前記身体姿勢傾斜信号が前記傾斜ウインドウ内にある時、前記階段登り拍数制 御信号を用いて逸脱間隔を規定する手段と、 前記身体姿勢傾斜信号が前記傾斜ウインドウ外にある時、前記目標拍数制御信 号を用いて逸脱間隔を規定する手段とを含む請求項2記載の装置。 7.前記目標拍数制御信号を前記階段登り拍数制御信号と比較し、ペーシング拍 数制御信号としてより大きなものを用いる手段を更に備える請求項6記載の装置 。 8.逸脱間隔を規定する前記手段が更に、 前記患者活動信号の関数として患者の心臓をペーシングする目標拍数制御信号 を生じる手段と、 階段登り拍数制御信号を生じる手段と、 内側および外側傾斜ウインドウを規定する手段と、 前記身体姿勢傾斜信号を前記傾斜ウインドウと比較する手段と、 前記身体姿勢傾斜信号が前記内側傾斜ウインドウ内にある時、前記階段登り拍 数制御信号を用いて逸脱間隔を規定する手段と、 前記身体姿勢傾斜信号が前記外側傾斜ウインドウ外にある時、前記目標拍数制 御信号を用いて逸脱間隔を規定する手段と、 前記身体姿勢傾斜信号が前記外側傾斜ウインドウ内にありかつ前記内側傾斜ウ インドウ外にある時、中間的拍数制御信号を取得し、該中間的拍数制御信号を用 いて逸脱間隔を規定する手段とを含む請求項2記載の装置。 9.前記目標拍数制御信号を前記階段登り拍数制御信号と比較し、より大きいも のを用いて前記身体姿勢傾斜信号が内側または外側傾斜ウインドウ内にあるか否 かに従って逸脱間隔を規定する手段を更に備える請求項8記載の装置。 10.傾斜信号を取得する前記手段がDC加速度計を含む請求項1または2に記 載の装置。
