Abstract

(57)【要約】 【課題】 延びたベアリング面を有するセラミックス物
質からなるカップ状挿入体を含むプロテーゼ組立体を提
供する。 【解決手段】 股臼シェル２の形状をなす固定要素と、
ベアリングカップ部４からなるプロテーゼ組立体。ベア
リングカップ部４は、拡大部１０がある縁８を有するの
で、ベアリング面での動きの範囲が増し、位置のずれの
危険が減少する。縁８の拡大部１０はカップ部挿入体４
からシェル２の平らな縁２の上をほぼ放射状に延びる。
拡大縁部１０の強度が増強されているので、拡大縁部１
０が破裂する危険なく挿入体４はセラミック物質から形
成できる。

【発明の詳細な説明】
【０００１】
【発明の属する技術分野】この発明は、プロテーゼ組立
体、特に、患者の股臼に挿入するシェルと股関節に対す
るベアリング面を形成する挿入体に関する。
【０００２】
【従来の技術及び発明が解決しようとする課題】セメン
トを用いてシェル部を患者の股臼に取り付ける股関節プ
ロテーゼ又は、ネジ止具によって又はネジ止具なしで直
接骨にシェル部を患者の股臼に接着する股関節プロテー
ゼが知られている。一般にカップ状の挿入体をシェル内
に配置する。カップの内面が大腿部頭に対してベアリン
グ面となる。
【０００３】ヨーロッパ特許第０４３６３１７号公報に
は、カップ形状から上方向に挿入体の縁が延びて形成さ
れる延びたベアリング面を有する挿入体が開示されてい
る。この文献に示される挿入体は金属又はポリエチレン
物質からなる。破裂強度が低いので非常に脆いが耐久性
と生物学的適合性が他の物質より良好であるのでカップ
状挿入体をセラミックスで形成することが好ましい。セ
ラミックス物質が脆いということは、セラミックス物質
からなるカップ状挿入体では、シェルにより支持されな
いカップの部分が破裂しやすいので、延びたベアリング
面を有することはできないことを意味する。

【課題を解決するための手段】本発明の目的は、延びた
ベアリング面を有するセラミックス物質からなるカップ
状挿入体を含むプロテーゼ組立体を提供することであ
る。本発明は、患者の骨に取り付けるための固定要素
と、縁のあるベアリングカップ部とからなり、縁の一部
は拡大していて、拡大部はカップ部からほぼ放射方向に
延びる肩の形であり、縁の一部の幅が増しているプロテ
ーゼ組立体を提供する。
【０００５】好ましくは、カップ部はセラミック物質か
ら作られている。セラミック物質はアルミナまたはジル
コニアでよい。好ましくは、固定要素は合成プラスチッ
ク物質又は金属で作られている。好ましくは、固定要素
とベアリングカップ部の間にテーパ接触がある。ベアリ
ングカップ部は、雄部を形成するようにテーパされてい
る外面を有し、固定要素は、ベアリングカップ部の雄部
と対応する雌部を形成するようにテーパされている内面
を有してもよい。
【０００６】好ましくは、ベアリングカップ部の縁は実
質的に楕円である。好ましくは、カップ部のベアリング
内面はカップ部の楕円内で中心からずれている。好まし
くは、プロテーゼ組立体は股臼カップ組立体である。
【０００７】
【発明の実施の形態】本発明によるプロテーゼ組立体の
実施例について図面を参照して説明する。図に示すよう
に、プロテーゼ組立体１は、シェル２の形状をなす固定
要素と、挿入体４の形状をなすベアリングカップ部から
なる。シェル２は、患者の骨に取り付ける階段状の外面
６を有する。この実施例では、プロテーゼ組立体１は股
関節部取替用股臼カップ組立体であり、シェル２は患者
の股臼に取り付けられる。
【０００８】シェル２は圧入により股臼に取り付けられ
る。そのとき、ネジ止具を用いてもよい。ネジ止具はシ
ェル２の孔２４を通して挿入される。ネジ止具の頭はシ
ェル２に埋まり、挿入体４との相互作用を避ける。別の
方法では、シェル２は患者の股臼にセメント付けするこ
ともできる。
【０００９】シェル２の形状はほぼ半球であり、平らな
縁２６がある。シェル２には、シェル２の縁２６の隣り
にテーパ部１４がある。挿入体４はカップ状であり、そ
の内面１２は関節部、この実施例では大腿部頭に対する
ベアリング面となる。挿入体４の外面２８にはテーパ部
１６があり、シェル２の雌テーパ部１４内にフィットす
る雄テーパとなる。挿入体４の外面２８の残りの部分
は、シェル６の内面を圧迫しない大きさとする。挿入体
４の外面２８とシェル２の内面２２の間の空間は、シェ
ル２と挿入体４の間のテーパ６を操作するために必要で
ある。
【００１０】挿入体４には縁８があり、縁８には挿入体
４のカップ形状から放射方向に延びる拡大部１０があ
る。縁の拡大部１０は、シェル２の平らな縁２６の上に
外方向に延びる肩を形作る。シェル２の平らな縁２６と
縁の拡大部１０の間に、ギャップ１８が形成される。図
３に示すように、挿入体４の縁８は楕円形であり、楕円
形の中心からはずれて半球ベアリング面１２がある。図
１及び図２に示すように、挿入体４の縁８を急に縁の拡
大部１０に延ばすことができる。
【００１１】現在２つのセラミック物質、即ちジルコニ
アとアルミナが整形外科器具に使用できる。ジルコニア
は軟らかい物質、例えば超高分子密度ポリエチレンと使
用できる。ジルコニアを股関節部取替プロテーゼに使用
するとき、大腿部頭をジルコニアで作り、挿入体を超高
分子密度ポリエチレンで作る。
【００１２】アルミナはジルコニアよりかなり堅い物質
であり、最も良い結果を得るためには、大腿部頭とベア
リング挿入体に同じ物質を使用することが勧められる。
しかしながら、セラミックヘッドをコバルトークロミウ
ムヘッドで置き換えることもできる。アルミナ／アルミ
ナ結合が最も良好な特性を有するが非常に脆い。コバル
トークロミウム／ポリエチレン結合の摩耗速度は年当た
り０．２ｍｍと０．５ｍｍの間であることが知られてい
る。アルミナ／アルミナ結合の摩耗速度は年当たり０．
０００１ｍｍと０．００５ｍｍの間である。
【００１３】シェルを股臼の正確な位置に取り付けるの
がしばしば困難であるため、拡大挿入体４が必要とな
る。これは患者の解剖学的構造等に起因すると考えられ
る。従って、シェル２の不正確な位置付けの問題を克服
するために、ずれたベアリング面１２を有する挿入体４
を使用することが必要である。関節の動きの範囲が増
し、ずれの危険が減少する。
【００１４】セラミック挿入体が知られており、もし挿
入体がシェルの面でおおわれていれば、挿入体が壊れる
危険はほとんどない。もし挿入体のベアリング面が拡大
されていれば、縁の一部は支持されず、従ってセラミッ
ク挿入体が壊れる危険は非常に高い。外科医が手術する
とき、初めにシェルを股臼に固定し、次に、生物学的適
合性プラスチックで作られたトライアル挿入体４を用い
て拡大挿入体４に最適な位置を見い出だすよう努力す
る。外科医が挿入体４に必要な正確な位置を決めたら、
セラミック挿入体４をシェル２の中に押し込む。挿入体
４とシェル２におけるテーパ部１４と１６の相互作用に
より、挿入体４はシェル２内でロックされる。テーパロ
ック機構は挿入体がシェル２の内側で３６０度回転でき
るようにし、完全な位置自由性を付与する。その後、外
科医は動く範囲をためし、シェル２内の拡大挿入体４の
位置を少し変えることができる。満足したら、外科医は
挿入体４をシェル２に密着させ、２つのテーパ部１４と
１６を互いにしっかりとロックさせる。
【００１５】本発明では、縁８の支持されない最も高い
部分における縁の厚み１０を増すことにより、拡大挿入
体４の強度を変えた。拡大挿入体４の強度が増したこと
により、拡大挿入体をアルミナ、ジルコニウム等のセラ
ミック物質で作ることが可能となった。縁の拡大部１０
とシェル２の平らな縁２６の間には、小さなギャップ２
０がなければならない。これにより、テーパロックの操
作が可能となる。
【００１６】従来の技術によるセラミック物質で作られ
た拡大挿入体と、本発明の拡大挿入体４について、限定
要素分析がなされた。本発明の拡大挿入体４では応力が
減り、従って挿入体４が破壊される全体的な危険も減る
という結果が示された。本発明の範囲を離れることな
く、前述したものに対し変更や改良を加えることはでき
る。
【００１７】好適な実施態様を以下に示す。 （１）前記カップ部がセラミック物質から作られている
請求項１記載のプロテーゼ組立体。 （２）前記セラミック物質がアルミナまたはジルコニア
である実施態様（１）記載のプロテーゼ組立体。 （３）前記固定要素が合成プラスチック物質又は金属で
作られている請求項１及び実施態様（１）ないし（２）
のいずれかに記載のプロテーゼ組立体。 （４）前記固定要素と前記ベアリングカップ部の間にテ
ーパ接触がある請求項１及び実施態様（１）ないし
（３）のいずれかに記載のプロテーゼ組立体。 （５）前記ベアリングカップ部が、雄部を形成するよう
にテーパされている外面を有する実施態様（４）記載の
プロテーゼ組立体。
【００１８】（６）前記固定要素が、前記ベアリングカ
ップ部の雄部と対応する雌部を形成するようにテーパさ
れている内面を有する実施態様（５）記載のプロテーゼ
組立体。 （７）前記ベアリングカップ部の縁が実質的に楕円であ
る請求項１及び実施態様（１）ないし（６）のいずれか
に記載のプロテーゼ組立体。 （８）前記カップ部のベアリング内面が前記カップ部の
楕円内で中心からずれている実施態様（７）記載のプロ
テーゼ組立体。 （９）前記プロテーゼ組立体が股臼カップ組立体である
請求項１及び実施態様（１）ないし（８）のいずれかに
記載のプロテーゼ組立体。 （１０）図面に記載されているようなプロテーゼ組立
体。

【発明の効果】以上述べたように、本発明によれば、延
びたベアリング面を有するセラミックス物質からなるカ
ップ状挿入体を含むプロテーゼ組立体を提供できる効果
がある。
