【発明の詳細な説明】 骨用セメントの自動混合装置 発明の分野 本発明は、股関節交換などの外科手術に使用される骨用セメントを混合する方 法及び装置に関し、より詳細には、斯かる装置及び方法において使用されるイン ペラに関する。 発明の背景 関節炎、他の退行性の疾病や外傷により人体関節に対して損傷が生ずると、多 くの場合、関節を人工器官装置で置換えることになる。股関節交換手術の場合、 斯かる装置は、股関節窩に嵌合すると共に髀臼(ひきゅう)に置き代わるカップ と、ステムに取付けられると共に大腿骨に嵌合して大腿骨の頂部に置き代わるボ ールとを含んでいる。又、人工器官装置を骨に固定する為に、セメントが使用さ れる。 現在では、斯かる置換手術に使用されるセメントはポリメタクリル酸メチル(P MMA)であるが、セメント硬化用の触媒を使用する必要がある。この触媒は液体形 態で供給されるが、セメントの完全かつ均等な硬化を確かなものとする為に、触 媒は、メタクリル酸メチルのモノマと、セメントの他の乾燥要素とに対して完全 に混合されねばならない。好適には、上記セメントは真空下で混合され、セメン ト内に気泡が入り込むのが防止される。この点、最終的なセメント混合物内に気 泡が在ると、引張り強さが不都合に低いセメントになる可能性がある。通常、所 望の稠度(consistency)を有するセメント混合物を得る為には、0.5 分間〜2分 間だけセメント成分を混合 する必要がある。 現在、骨用セメントを混合すべく入手し得る装置では手動で混合する必要があ る。この処理は非常に手数が掛かると共に、混合処理を終了するまで継続的に身 体を動かし続ける必要がある。もし混合処理が適切でなく、又は、セメントが供 給前に硬化したとすれば、そのセメントを廃棄するとともに、別の回分量を調製 せねばならない。また、その処理が反復されるべきであれば、セメント混合が開 始される時点まで進行してきた手術処理は遅延される。 一方、骨用セメントは殺菌されており、この状態を維持すべきことから、セメ ント調製の為に全ての殺菌成分を混合かつ処理する上では「滅菌」人員が必要で ある。この点、現在入手可能なセメント混合機では、「滅菌」人員に加えて「非 滅菌」人員も必要とされるが、これは、セメント混合装置の設置(set-up)を行う と共に、混合処理に関与する非滅菌成分の取扱いを補助する為である。また、混 合処理の終了時には、更なる「滅菌」人員が必要とされることも多いが、これは 、患者に対して骨用セメントを供給すべくカートリッジ中にセメントを移動する のを補助する為である。従って、セメントの調製には少なくとも2人の、かつ、 多くの場合には3人もの人員が必要とされる。 ところで、予硬化されたセメント成分、すなわち、メタクリル酸メチルモノマ 及び触媒は、刺激性、及びおそらくは毒性、を有する。従って、セメント成分、 特に上記モノマの吸入を防止するのが好適である。然るに、現在入手し得る骨用 セメント混合機は混合処理を通して密閉シールされてはおらず、また、成分をセ メント混合機内に手動で投入する必要があることから、操作者をそれらの煙霧(f ume)に曝してしまう。すると、セメント調製箇所の近傍の全ての人員により長時 間に渡りこれらの煙霧が吸入され、セメント調製によ る炎症を起こすと共に健康までも損なう可能性がある。 セメントは混合された後、カートリッジ内に注入されて接合箇所に供給される 。このとき、セメントと接触した全ての器具は徹底的に洗浄するか、廃棄せねば ならない。セメントの移動に伴う更なる問題とは、セメントが気泡を取込んでし まうことであり、これは、セメント/骨の界面に良くない影響を与えると共に、 セメントの接合強度を低下させることもある。 従って、当該装置の設置及びセメントの混合に必要な段階の全てを完結する上 で必要な人員がひとりだけの装置を提供することが望まれる。また、その装置は 、セメント調製に伴う煙霧の全てを遮断することにより、上記毒性化合物に対す る院内作業員の露出を減少するのが望ましい。同様に、斯かる装置は、セメント 混合に要する人的作業を回避すべくモータにより駆動されると共に、不適当なセ メントバッチの廃棄の必要性を減少する混合割合を確実に再現し得るのが望まし い。更に、セメントと接触する装置部分の全てが使い捨て式であり、従って清掃 作業が最小限とされるのが望ましい。 発明の要約 人工器官の取付に使用される骨用セメントを混合する動力式混合機、及び、こ の混合機を使用する方法を記載する。この混合機は、混合チャンバと、混合チャ ンバ内のライナと、ライナ内のインペラと、インペラを駆動するモータと、モー タの作動を調節するプログラマブル手段とを備えて成る。 作動時には、骨用セメントの乾燥成分が混合機内に載置されるとともに、骨用 セメントの触媒を含む小瓶(vial)が混合機内の小瓶用キャビティ内に載置される 。混合機は密閉シールされると共に小瓶が破壊され、骨用セメントの乾燥成分内 に触媒が放出される。つぎ に、混合機が始動される。混合機は、予めプログラムされた混合シーケンスの制 御下で作動する。混合シーケンスの最後において、混合されたセメントは収集さ れる。 本発明の好適実施例においては、予めプログラムされた混合シーケンスは、混 合機の内部に真空を付与する段階と、モータを始動して混合機内のインペラを駆 動する段階と、所定の時間に渡って混合を行う段階と、再び真空を付与して混合 機内部から煙霧を排気する段階と、混合機の内部に圧力を付与して混合機からセ メントを排出する段階とを備えて成る。 本発明の更なる好適実施例においては、混合機の内部への真空付与は、取付可 能な骨用セメントガンカートリッジすなわちシリンジに対して真空を付与するこ とにより行われるが、これは、カートリッジを混合チャンバと同一の真空に曝す インペラの中空軸を介して行われる。混合チャンバ内に正圧が導入されたとき、 このインペラは混合チャンバからカートリッジを密閉遮断してカートリッジの真 空を維持し、混合機からカートリッジへの骨用セメントの移動を助ける。この配 置構造によりカートリッジへのセメント放出が更に容易になる、と言うのも、カ ートリッジ内には空気圧が存在しないので正圧を相殺すべくセメントがカートリ ッジ内に押し込まれるからである。それはまた、混合機からカートリッジ内への セメント放出間におけるセメント内への空気導入の阻止も促進する。 図面の簡単な説明 骨用セメント混合機10の第二実施例は図2に示されている(本明細書を通じ、同 一の部材には同一の参照番号を付している)。本発明の骨用セメント混合機の利 点は、モータが取付けられて自動化され、セメント要素及びそれにより発生され る煙霧に対する操作者及び手術室内の他の人員による接触が最小化され又は排除 される、ということである。 概略的には、本発明の骨用セメント混合機はハウジング12を備えて成り、ハウ ジングは継続使用式(non-disposable)であり、モータ、電池、プログラマブルメ モリ機構、及び、真空圧ポンプ(図示せず)などの他の継続使用式部品を収納し ている。これらの継続使用式要素は、使用に際して圧熱滅菌により消毒される。 また、混合機の継続使用式部品は、セメントと直接的には接触されない。 セメントと直接的に接触する要素は使い捨てであってセメント混合キットとし て供給されるが、それはライナ30を備え、ライナはハウジング内に嵌合されると 共にセメント混合に必要な混合用インペラ90を備えて成る。キットの他の使い捨 て要素にはセメント注入ガン又はアダプタが含まれ、その内部に対しては、混合 処理の終了時に混合セメントが集積される。混合機の使い捨て要素は、予滅菌さ れたキット形態で供給される。骨用セメント混合機の個々の構成要素は、以下に 詳述する。 作動時において、ライナ30(以下に詳述する図3〜図8を参照)は混合機ハウ ジング12内に載置されると共に、注入ガン又はアダプタ(図示せず)は混合機の 中央吐出口上に載置される。次に、メタクリル酸メチルモノマを含む粉体などの セメント成分がライナ内に載置される。ライナ内に位置せしめられた小瓶用キャ ビティ32内には、セメント硬化用の触媒を含む小瓶が載置される。次に、骨用セ メント混合機は所望の混合時間に対してプログラムされる。次にハウジングの蓋 体14が閉じられ、混合機内のセメントの成分を密閉シールする。蓋体の閉塞は小 瓶の破壊をも引起こし、他のセメント成分(詳細に関しては以下を参照)内に触 媒が放出される。ライナの内部には真空が付与され、セメントが混合されるとき にその内部での気泡形成が防止されると共に、セメント成分から発生された煙霧 も除去される。また、混合チャンバの下方に位置せしめられたセメント供給用の セメントガン又は種々のカートリッジにも真空が付与され、混合チャンバからセ メントガンへとセメントが放出されるときにセメント内に気泡が形成されるのが 防止される。骨用セメント混合機が真空とされた後、混合機が始動される。 予め選択された混合時間の終了時に、混合機は自動的に停止すると共に、本発 明の一実施例においては、警報を発して操作者に警告を行う。上記チャンバには 再び真空が付与され、混合処理中に形成された煙霧が排出される。混合が完了し たとき、真空は解放されると共にチャンバには圧力が加えられて混合機からセメ ントが排出されるが、これは、混合機の底部に位置せしめられたポートを介し、 セメント供給用のセメントガン又は種々のカートリッジ内にセメントを押圧する ことにより行われる。セメントガンに予め真空を付与しておけば混合機からのセ メント排出は促進される。これは、セメントガン内には何らの空気正圧が存在せ ず、したがってセメントを排出すべく使用されつつある圧力を相殺するからであ る。混合機の使い捨て部分、即ちライナは、使用後に混合機ハウジングから取り 外して廃棄される。以下、混合機の構成要素及び作用を詳述する。 混合機ハウジング12(図1及び図2)は、ヒンジ(図示せず)又は他の適切な 取付手段によりハウジングボディに取付けられた一対の蓋体14を備えて成る。蓋 体は各々、混合用の動力を供給するモータ(図示せず)を収納する。本発明で使 用するに適したモータは、一般的な歯車モータ、パンケーキ(pancake)モータ等 のモータである。モータは可逆式とし、所定時間に渡り一方向に混合を行ってか ら、所望に応じて逆方向にも混合を行い、確実にセメント混合を完了するのが好 適である。本発明で使用するに適したモータは、AL州のGlobe of Dothan 社によ り販売された12V、120rpmの可逆式モー タであるモデル番号407A6008-2などの歯車モータである。 本発明の一実施例においては、モータは電流検知機構により制御される。混合 用モータを作動するに必要な電流が所定レベルを超えたとき、上記装置は自動的 に混合サイクルを終了させ、警報を鳴らすと共に混合機からのセメント排出を開 始する。従って、この機構はセメントの粘度を間接的に検知し、その粘度が高く なり過ぎて混合機から流出できなくなる前に確実に排出すると共に、早過ぎるセ メント硬化に繋がるセメントの過混合を防止する。 好適実施例においては、本発明の動力式混合機はプログラマブルであり、異な る稠度及び/または組成のセメントを効率的に混合し得る。例えば、脛骨(けい こつ)/髀臼(ひきゅう)の交換手術用のセメントは濃密な即ち高粘性稠度が一 般的に好適であり、かつ、セメント成分を約2分間混合すると、この用途に適し たセメントが生成される。また、大腿骨に使用するセメントは、脛骨/髀臼用の セメントよりも薄い即ち低粘性稠度のものが好適である、と言うのも、それは大 腿骨内に載置さねばならないからである。この用途に関しては、セメント成分を 約1分間混合すると適切なセメントが生成される。更に、幾つかの骨用セメント は、それが適切に「凝結する(set up)」様に混合の間に「停止期間」を必要とす る。これらの混合要件は、混合機の「不揮発性(permanent)」メモリ内に記憶さ れ得る。代わりに、「カスタム」混合プログラムをユーザがプログラムすること も可能である。適切なプログラマブルメモリ機構は当業界で公知であり、かつ、 本発明で使用するに適したデバイスは、NEC により販売されている形式番号8748 等のデバイス、又は、他のプログラマブルコントローラの製造業者により販売さ れている他の同様のデバイスである。プログラマブルコントローラはハウジング 内に収納される。本発明の他の実施例においては、「プログラム」 はプログラマブルメモリ機構内に取入れられるのでは無く、「ハートワイヤード 」とされる。 混合機ハウジングの前面には、キーパッド16が位置せしめられている(図23 参照)。このキーパッドは、経過した混合時間と、チャンバ内の真空レベルすな わち圧力レベルを表示するディスプレイを含み得る。同様に、混合、混合一時停 止、真空付与及び真空解放、ならびに、プログラムオプション選択などの機能を プログラムする為のボタンを含ませても良い。また、予めプログラムされた又は カスタム混合プロトコルの選択、及び、混合機プログラムのリセットを許容する 付加的なボタンを含ませても良い。キーパッドは更に、混合機が実施している混 合サイクル段階を表示するLED を含んでも良く、かつ、経過した混合時間及びチ ャンバ内の真空レベルを表示するLED セグメントを含んでも良い。本発明で使用 するに適したLED は、Kingbrite 社製のもの、または、Hewlett-Packard 社製の 「mil-spec」高温耐性LED である。本発明で使用するに適したキーパッドは、3M 社により販売されている様な多層ポリカーボネート、または、シリコーンラバー 、等の上カバーを有する膜付きキーパッドである。 カスタム混合が選択されたとき、プログラムパラメータもキーパッドを使用し て指定することが出来る。一実施例においては、カスタム混合時間に対するキー パッドは、混合時間を指定する為の数字0〜9として指定されたキー群を含んで いる。このキー群には、プログラムを入力するキーが含まれる。また、キーパッ ド上には、混合処理が完了したことを示すアラームが位置せしめられている。更 に、電力が混合機に供給されつつあることを示すランプも含ませ得る。 本発明の一実施例においては、混合機の動力はハウジング内の電 池から供給される。本発明で使用するに適した電池は、再充電可能なNi-Cd 電池 (12V、2.8 Amp)である。これらの電池は、再充電を行う前に3回の混合サイク ルまで混合機を作動させるのが典型的である。本発明の代替実施例においては、 混合機はDCコンバータを用いてAC電源に接続され得る。斯かるコンバータは当業 界で公知である。電源として電池が使用される場合、「ロー電池」表示ランプが キーパッドに含まれる。 また、ハウジング内には、混合中にライナ内部を減圧する真空圧ポンプが含ま れる。本発明で使用するに適したポンプは、真空モードでは約70cmHgを吸引する と共に圧力モードでは100psiを発生するGA州ナルクロス市のASF 社製の膜ポンプ であり、これを、MI州Kalamazoo 市のHumphrey Valve社により販売されているソ レノイドバルブ、及び、所望の圧力レベルを制御かつ維持する検出機構を提供す るNY州ニューヨーク市のOmega 社製の真空変換器と組合せて使用する。ソレノイ ドバルブは、真空モードでは約0〜76cmHgの範囲で作動すると共に圧力モードで は120psiで作動すべく選択される。 使用の為に混合機を消毒すべく、ハウジングが圧熱滅菌される。ハウジングは 、真空圧ポンプ、プログラムコントローラ、電池及びモータを内蔵するが、これ らの取り外しは容易でないことから、ハウジングはこれらの要素の覆いも含み、 圧熱滅菌中の高温からそれらを保護している。適切な覆いは、NJ州、Chatham 市 のHoechst Celanese社により販売されるガラス充填FORTRON グレードの硫化ポリ フェニリン、キングスポートのEastman Performance Plastics社により販売され ている15% ガラス充填の熱可塑性コポリマー樹脂であるEKTAR、及び、General E lectric社のTN(約255 ℃までの温度での使用に適切である)又はULTEM、などの 材料から成る保護「容器(can)」内に熱的不安定要素を囲繞することにより提供 される。 蓋体14の各々の下側は、蓋体が閉じられたときにハウジングに対して蓋体を密 封するシール17となっている。また、各々の蓋体の下側には、モータをライナの パドル又はインペラシャフト92に接続する駆動ソケット18も位置せしめられてい る。好適実施例におけるシャフトの端部は五角形断面を有することによりシャフ トと駆動ソケットとの間の滑りを防止している。五角形断面が好適ではあるが、 三角形、四角形、六角形などの他の形状も適切であることは理解される。 また、蓋体の下側にはブレード20も位置せしめられており、従って、蓋体が閉 じられたときにブレードは小瓶用キャビティ32内に延出し、このキャビティ内に 収納された小瓶を破壊開成する。 ハウジングボディは、ライナが載置される一対のチャンバを備える(これらの チャンバは、所定位置のライナと共に示されている)。チャンバの各々は底部( 図示せず)にて開口され、ハウジング外部へのライナの延出とセメント収集アダ プタの取付けとを許容している。これらの開口の回りにはO−リングが位置せし められ、ライナを所定位置で密封している。 ハウジングボディは、ハウジングを支持する幅広の水平基部24から上方に突出 するスタンド22上に取付けるのが好適である。ハウジングは、General Electric 社製のULTEM、又は、American Celanese 社製のVECTRAなどの高温プラスチック 材料から作成するのが好適である。但し、混合チャンバの直下領域などの、セメ ントと接触し得る領域はステンレス鋼などの材料により作成し又は被覆し、セメ ント調製の間に零れたセメントを容易に洗浄除去し得る様にするのが好適である 。 本発明の一実施例(図1)においては、真空/圧力ポート26も蓋体に備えられ ている。この真空ポートは、混合機のハウジング内に 位置せしめられた真空ポンプに対して耐圧チューブ又は真空チューブにより接続 されている。また、真空ポートは、真空ポンプに接続されると共に、又は、真空 ポンプに接続する代わりに、「据付(house)」真空ラインに接続しても良い。他 の実施例(図2)においては、ライナの周縁回りに複数個の真空/圧力孔48が位 置せしめられている。これらの孔は、真空ポンプ及び/または据付真空ラインに 接続されたライナ内マニフォルドと連通される。他の実施例においては、小瓶用 キャビティの端部においてライナ内に真空/圧力孔31(図3)が配備されている 。この実施例においては、これらの孔がハウジング内のマニフォルドに結合し、 蓋体(図示せず)内に載置されたキャビティを介してライナのチャンバと連通す る。 図3〜図8に示された本発明のライナ30はポリエチレン等の材料から成り、使 用後は廃棄され得る。 概略的には、上記ライナはカップ状チャンバ36を備え、チャンバは、混合用の セメント成分が投入載置される略円形断面の凹所を形成している。ライナの頂部 の片側又は両側には小瓶用キャビティ32が位置せしめられており、その中にはセ メント用触媒を含む小瓶が位置せしめられる。また、ライナ底部は、混合処理の 最後にセメントを収集するポート130 となっている。更に、ライナの内部には、 セメント混合用のインペラ90が配備されている。 本発明の一実施例(図6〜図8)においては、ライナの外側及び底部に沿って リブ50が位置せしめられ、ライナを補強すると共に所望の作動真空下でそれが潰 れるのを防止している。本発明のこの実施例においては、ライナの外側回りには 相互に等間隔で離間された4個のリブが位置せしめられている。一方、ライナを 装入する為にハウジングチャンバの壁部には整合溝を位置せしめ得ることから、 上記リブはハウジング内でライナを整列する役割を果たし得る。但 し、リブの配備は、長期間の使用又は高真空/高圧力による応力上昇に繋がる可 能性もある。 本発明の一実施例においては、蓋体14内に位置せしめられた真空ポート26を介 してライナが減圧される。また、更なる実施例(図6〜図8参照)においては、 ライナを真空/圧力ポンプに接続する為にライナ内にポート46が位置せしめられ ている。ライナ内には一連の孔48が位置せしめられるが、これらの孔は、真空ポ ートコネクタに非常に近接すると共に、真空ポートに接続されたマニフォルドに 通じている。これらの孔により、必要に応じてライナ内部は減圧かつ通気され得 る。 壊する如き位置に置かれている。図3〜図5の実施例においては、この破壊の結 果として小瓶が深底部上に直立し、小瓶の内容物の吐出が確実に行われる。 触媒は、種々の長さを有する種々の小瓶により市販されている。従って、小瓶 用キャビティは種々の寸法の小瓶を収容し得るのが好適である。長寸の小瓶の場 合、小瓶用キャビティの寸法はこのキャビティ内に小瓶が遊挿される如きものと される。また、小寸の小瓶が使用される場合、小瓶はライナの側壁54に緊密嵌合 することにより保持される(図3及び図6)。 更なる実施例においては、小瓶を所定位置に保持する為に、プランジャ56を備 える機構(図6〜図8)が使用される。このプランジャは小瓶用キャビティの丈 に沿うと共に、端部57により小瓶用キャビティの外部まで延在している。また、 このプランジャはスプリング58に取付けられている。小瓶用キャビティが空のと き、プランジャはキャビティの端壁60と接触し、スプリングは弛緩されている。 プランジャが小瓶用キャビティの端壁から離間する様に引出されたとき、スプリ ングは圧縮されて小瓶用キャビティ内には小瓶が載置され得る。スプリングによ る力を受けているプランジャは、小瓶用キャビティの端部における所定位置に小 瓶を保持する。大寸の小瓶を使用するときは、小瓶用キャビティの全長に渡って プランジャが引出され、大寸の小瓶が小瓶用キャビティ内に載置され得る。 本発明の別の実施例(図示せず)においては、小瓶用キャビティ内に小瓶を保 持する為に引張スプリングが使用される。この場合には小寸のプランジャが引張 スプリングに取付けられる。また、スプリングは、小瓶用キャビティの内部の端 壁60に取付けられる。小瓶 を収納すべくプランジャが引出されたとき、このスプリングは伸張されてプラン ジャに力を加えるので、プランジャが小瓶を所定位置に固定する。 蓋体が閉じられたときに小瓶は破壊され、小瓶の液体内容物は放出されてダク トを通してライナチャンバ内に流出し、そこで他のセメント成分と混合される。 本発明の各実施例において、小瓶用キャビティはダクト52によりライナチャン バと連通されている。本発明の好適実施例においては、ダクト内にフィルタが配 置され、小瓶の破壊から生ずるガラス裂片がセメント混合物内に入り込むのを防 止している。 ライナの内部は混合インペラとなっている。本発明の一実施例(図9及び図1 0)においては、インペラはシャフト96の両側に延在する2枚のブレード94を備 えている。これらのブレードはそれらの先端が相互に略平行となる如く約45°の 動及び他の所望パラメータのプログラムシーケンスが入力される。次にハウジン グの蓋体14が閉じられ、セメント成分が混合機内に密閉される。 蓋体の閉塞により小瓶が破壊され、他のセメント成分内に触媒が放出されると 共に予めプログラムされた機能が開始される。ライナの内部には真空が付与され 、セメント混合時の気泡形成が防止されると共に、セメント成分から発生した煙
