Claims (1)
- 【特許請求の範囲】 1.拡張可能なステントを搬送し、かつ展開するためのステント・デリバリ・シ ステムであって、 A)ステントを患者の体内の選択された位置へ配置するためのデリバリ手段と 、前記デリバリ手段は、収縮した搬送状態にあるステントの上側に常には配置す るシースと、収縮した搬送状態にあるステントの下側に常には配置する内側コア とを有することと、 B)ステントをシースから展開することと、ステントをシース内へ引っ込める ことを選択的に実施するための展開手段と、前記展開手段がリングを有し、同リ ングは前記内側コアに結合され、かつ同内側コアから延出し、さらには収縮状態 にあるステントに対して係合すること を含むステント・デリバリ・システム。 2.前記リングの外径は収縮状態にあるステントの最小内径より大きい請求項1 に記載のステント・デリバリ・システム。 3.前記リングは収縮したデリバリ状態にあるステントの近位端付近において同 ステントに係合する請求項2に記載のステント・デリバリ・システム。 4.前記デリバリ手段はシース及び内側コアの両者の近位端に配置されたハンド ルを更に有し、前記ハンドルはシースを内側コアに対して近位方向へ後退させる ための第1のアクチュエータ手段と、内側コアをシースに対して遠位方向へ移動 させるための第2のアクチュエータ手段とを有し、第1のアクチュエータ手段及 び第2のアクチュエータ手段の選択的操作はステントをシースの外側において拡 張状態へ展開することと、ステントを部分的に展開した状態からシース内へ引っ 込めることを選択的に実施可能にする請求項3に記載のステント・デリバリ・シ ステム。 5.前記デリバリ・システムは観察用チャネルを有するタイプの内視鏡装置の作 業チャネル内での使用に適合しており、前記カテーテルは内視鏡装置の観察用チ ャネルを通じて視認できる視認可能目印を同カテーテルの遠位端付近に有し、前 記目印はステントの展開の範囲を示す請求項1に記載のステント・デリバリ・シ ステム。 6.前記デリバリ手段はシース及び内側コアの両者の近位端に配置されたハンド ルを更に有し、前記ハンドルはシースを内側コアに対して近位方向へ後退させる ための第1のアクチュエータ手段と、内側コアをシースに対して遠位方向へ移動 させるための第2のアクチュエータ手段とを有し、第1のアクチュエータ手段及 び第2のアクチュエータ手段の操作はステントをシースの外側において拡張状態 へ展開することと、ステントをシース内の収縮状態へ引っ込めることを選択的に 実施可能にする請求項5に記載のステント・デリバリ・システム。 7.前記第1のリングから軸線方向に離間した第2のリングを更に有し、前記第 1のリング及び第2のリングは収縮したデリバリ状態にあるステントの近位端付 近において同ステントに係合する請求項1に記載のステント・デリバリ・システ ム。 8.前記デリバリ手段はシース及び内側コアの両者の近位端に配置されたハンド ルを更に有し、前記ハンドルはシースを内側コアに対して近位方向へ後退させる ための第1のアクチュエータ手段と、内側コアをシースに対して遠位方向へ移動 させるための第2のアクチュエータ手段とを有し、第1のアクチュエータ手段及 び第2のアクチュエータ手段の操作はステントをシースの外側において拡張状態 へ展開することと、ステントを部分的に展開した状態からシース内の収縮状態へ 引っ込めることを選択的に実施可能にする請求項7に記載のステント・デリバリ ・システム。 9.前記デリバリ・システムは観察用チャネルを有するタイプの内視鏡装置の作 業チャネル内での使用に適合しており、前記カテーテルは内視鏡装置の観察用チ ャネルを通じて視認できる視認可能目印を同カテーテルの遠位端付近に有し、前 記目印はステントの展開の範囲を示す請求項8に記載のステント・デリバリ・シ ステム。 10.前記ステントを展開状態へ選択的に拡張するために同ステントの下側に配 置された機械的拡張が可能な手段を有する請求項1に記載のステント・デリバリ ・システム。 11.前記デリバリ・システムは観察用チャネルを有するタイプの内視鏡装置の 作業チャネル内での使用に適合しており、前記カテーテルは内視鏡装置の観察用 チャネルを通じて視認できる視認可能目印を同カテーテルの遠位端付近に有し、 前記目印はステントの展開の範囲を示す請求項1に記載のステント・デリバリ・ システム。 12.長尺管状ステント・デリバリ・システムであって、同デリバリ・システム の遠位端部に収縮状態で支持されたステントの上側及び下側にそれぞれ配置され たシース及び可撓性コアを有する遠位端部と、前記ステントは患者の管内で拡張 状態へ選択的に展開すべく患者の体内を搬送されることと、軸線方向に沿ったシ ース及びコアの相対的移動を可能にすべく、シースに結合された第1のハンドル 部及びコアに結合された第2のハンドル部を有する近位端部とを含む長尺管状ス テント・デリバリ・システムにおいて、前記コアに結合され、かつ同コアから延 出する肉薄リングを有し、前記肉薄リングはシース内において収縮状態で配置さ れたステントに対して係合し、シースをコアに対して移動させた際、ステントは リングと一緒にシースに対して移動する長尺管状ステント・デリバリ・システム 。 13.前記リングは外径Drを有し、Dr>IDminの関係が存在し、前記IDmin は収縮状態にあるステントの複数の部分の最小内径である請求項12に記載の管 状ステント・デリバリ・システム。 14.前記リングは収縮したデリバリ状態にあるステントの近位端付近において 同ステントに係合する請求項13に記載の管状ステント・デリバリ・システム。 15.前記ステントは最大内径IDmaxを備えた部分を有し、同部分は最小内径 IDminを有するステントの複数の部分の間に位置し、IDmin<Dr<IDmaxの 関係が存在し、リングは前記複数の最小内径部分の間に配置されている請求項1 4に記載の管状ステント・デリバリ・システム。 16.前記デリバリ・システムは観察用チャネルを有するタイプの内視鏡装置の 作業チャネル内での使用に適合しており、前記管状ステントの遠位端は目印をコ アの遠位端付近に有し、前記目印は内視鏡装置の観察用チャネルを通じて視認で きるタイプであって、かつステントの展開の範囲を示す請求項15に記載の管状 ステント・デリバリ・システム。 17.前記第1のリングに隣接し、かつ軸線方向に沿って同第1のリングから離 間した位置においてコアに固定された第2のリングを有し、前記第1のリング及 び第2のリングは収縮したデリバリ状態にあるステントの遠位端から近位方向へ 離間している請求項12に記載の管状ステント・デリバリ・システム。 18.前記ステントを収縮状態から機械的に拡張させる手段を更に有する請求項 17に記載の管状ステント・デリバリ・システム。 19.収縮状態にあるステントを機械的に拡張させる手段を更に有する請求項1 2に記載の管状ステント・デリバリ・システム。 20.ステントをデリバリし、かつ選択的に展開させる方法であって、 A)軸線方向に沿って延びるカテーテルを患者の体内へ挿入する工程と、前記 カテーテルは外側シースを有し、収縮状態にあるステントがカテーテルの遠位端 付近に取付けられ、前記ステントはシースの下側であって、かつリングの上側の 位置に配置されていることと、 B)カテーテルの遠位端を選択された位置へ配置すべく同遠位端を患者の体内 を通って押し進める工程と、 C)ステントをシースの遠位端から拡張することと、ステントをシースの遠位 端内へ引っ込めることを選択的に実施可能にすべく、ステントをシースに対して 強制的に移動させるために前記リングをシースに対して選択的に移動させる工程 と を含む方法。 21.収縮状態にあるステントの下側に位置し、かつリングの上側に位置するバ ルーンをステントを機械的に拡張すべく膨張させる工程を有する請求項20に記 載のステントをデリバリし、かつ選択的に展開させる方法。 22.前記ステントは自己拡張性を有し、前記ステントをシースから遠位方向へ 移動させる工程はステントの展開を可能にし、係合工程は最小内径を有するステ ントの複数の部分の間に位置するステントの内周面を係合させることを含む請求 項20に記載のステントをデリバリし、かつ選択的に展開させる方法。 23.ステントを引っ込め得るか否かを決定すべくカテーテル上の目印を前記移 動工程中に見る工程を有し、前記係合工程は前記内周面を隣接し、かつ離間した 第2の環状リングに係合させる工程を有する請求項20に記載のステントをデリ バリし、かつ選択的に展開させる方法。
