Abstract

본 발명에서는 약 10% 양으로 존재하는 항진균 알릴아민 화합물, 유기산 또는 그 에스테르, 디올 및 격리제로 구성되는 네일의 진균 감염 치료용 제약학적 조성물이 제공되며, 상기 약학적 조성물은 본질적으로 물이 없다(water-free).

Description

신규 항진균 조성물 {NOVEL ANTIFUNGAL COMPOSITION}
본 발명은 네일(nail)의 진균 감염 치료용 조성물로 구성된다. 상기 조성물은 네일의 국소 치료를 위한 것이며 네일 속으로 항진균 물질이 침투될 수 있도록 한다.
네일의 파손으로 이어지는 네일의 진균 감염(손발톱 진균증(onychomycosis))의 문제는 수많은 노력이 주어졌지만, 아직까지 임상적으로 만족스런 어떠한 해결책도 얻어지지 않았다. 그러나, 충분한 양의 강력한 항진균 화합물이 네일 전체에 걸쳐, 그리고 손발톱 바닥(nail bed)에 분포될 수 있다면, 감염은 치료되고 네일의 파손은 종료될 것이라는 것이 일반적으로 공감되는 바이다.
선행 기술
선행 기술에는 항진균제의 침투를 증가시키기 위한 몇몇 시도가 개시되어 왔다.

미국 특허 제7,074,392호는 서방형 치료용 네일 광택제 조성물을 개시하며, 이는 (a) 항진균 유효량의 항진균제; (b) 상기 항진균제의 네일 내 침투를 증가 및 촉진시키기에 충분한 양의 각질 용해제; (c) 3 중량% 초과량의 보습제; (d) 상기 네일을 수화시키기에 충분한 양의 물; (e) 폴리머 필름 형성제 및 휘발성 용매로 구성되는 액체 네일 라커 성분(liquid nail lacquer component) (상기 형성제는 상기 조성물을 네일에 적용하고 상기 휘발성 용매가 증발되면 서방형 필름을 형성하도록 선택됨)으로 구성된다.
높은 침투도를 갖는 손발톱 진균증 치료를 위한 항진균 조성물을 갖는 것이 바람직하다.
또한, 고농도의 활성 항진균 화합물을 갖는 제제를 갖는 것이 바람직하다.
또한, 상기 제제 내에 용해된, 즉 침전되지 않은 항진균 화합물을 갖는 것이 종종 바람직하다.
본 발명의 목적은 앞서 기재된 문제들 중 적어도 일부를 해결하는 것이다. 따라서, 본 발명의 제 1 주요 측면에서는, 네일의 진균 감염 치료용 제약학적 조성물이 제공되며, 이는 5% 보다 많은 양으로 존재하는 항진균 알릴아민(allylamine) 화합물, 유기산 또는 그 에스테르, 디올 및 격리제(sequestering agent)로 구성되고, 상기 제약학적 조성물은 본질적으로 물이 없다(water-free).

달리 언급되지 않는 한, 여기서 사용되는 성분들의 퍼센트 양은 중량 퍼센트를 말하며 조성물의 총 중량을 기준으로 한다.
"약"이라는 용어는 적용가능한 경우, 언급된 값의 +/- 10%의 편차를 나타내기 위해 사용된다. 예를 들어 "약 20%"는 18% 내지 22%의 값을 나타낸다.
상기 조성물은 본질적으로 물이 없다. 상기 제제에 물이 첨가되지 않는다. 그러나, 그럼에도 불구하고 상기 조성물에 미량의 물이 있을 수 있는데, 이는 특정의 성분이 소량의 물을 함유할 수 있기 때문이다. 상기 미량의 물은 5% 미만, 좀 더 바람직하게 3% 미만, 좀 더 바람직하게 2%, 좀 더 바람직하게 1% 미만, 좀 더 바람직하게 0.5% 미만, 가장 바람직하게 0.3% 미만이다.

본 발명의 또 다른 잇점은 그것이 5% 보다 큰, 고농도의 항진균 물질을 함유한다는 것이다. 이는 상기 항진균 조성물의 효능을 증가시킨다.
상기 항진균 알릴아민은 상기 조성물에 용해된다. 따라서 상기 제제는 용액으로서, 즉 단상계(one phase system)로서 존재한다.
알릴아민 항진균제, 특히 테르비나핀 및 나프티핀이 본 발명의 바람직한 항진균제이다. 이는 진균 에르고스테롤 생합성에서의 중요 효소인 효소 스쿠알렌 에폭시다아제(enzyme squalene epoxidase)를 저지(blocking)함으로써 진균의 성장을 억제시킨다. 적당한 알릴아민 항진균제의 예는 아모롤핀(amorolfine), 부테나핀(butenafine), 테르비나핀(terbinafine) 및 나프티핀과 이들의 혼합물로 구성되는 군으로부터 선택되는 알릴아민 항진균제를 포함한다. 이들은 알릴아민 항진균제의 비제한적 예이다. 테르비나핀이 본 발명에 따라 가장 바람직한 알릴아민 항진균제이다.
상기 제제 내 알릴아민의 양은 1% 내지 12%이다. 바람직하게 알릴아민의 양은 약 10%이다. 12%가 본 발명의 제제 내 알릴아민의 용해도의 대략적 한계이다. 바람직하게 알릴아민의 양은 5% 내지 12 %, 좀 더 바람직하게는 8% 내지 12%, 가장 바람직하게는 10.5% 내지 12%이다. 그러나, 알릴아민의 용해도는 포함된 화합물들의 질(quality) 및 온도에 따라 달라질 수 있다. 선택적으로, 상기 조성물 내 알릴아민 용해도의 상한이 11.5%인 경우, 알릴아민의 바람직한 양은 5% 내지 11.5 %, 좀 더 바람직하게는 8% 내지 11.5%, 가장 바람직하게는 10.5% 내지 11.5%이다.

상기 유기산에 대한 대안으로서, 상기 조성물은 적당한 유기산의 C1-4 알킬 에스테르, 또는 적당한 유기산 및 상기 유기산의 에스테르의 혼합물을 포함할 수 있다. 바람직한 에스테르는 젖산의 에스테르이다. 적당한 에스테르의 비제한적 예는 메틸 락테이트, 에틸 락테이트, 부틸 락테이트 및 프로필 락테이트이다.

적당한 디올은 프로판디올, 부탄디올, 펜탄디올 및 헥산디올이고, 프로판디올 및 부탄디올이 특히 적당하다. 상기 디올들의 혼합물도 또한 적당하다.
상기 디올 또는 그들의 혼합물은 바람직하게 50% 초과의 양, 좀 더 바람직하게는 50% 내지 95%, 좀 더 바람직하게는 50% 내지 90%, 좀 더 바람직하게는 60% 내지 90%, 가장 바람직하게는 67.5% 내지 약 84%의 양으로 사용될 것이다.

본 발명에 따른 바람직한 조성물은 또한 실시예 부분에서의 실시예 A, B, C, D, E, F, G, H, I, J, K, L, ISM09024, ISM09017, ISM09018, 및 ISM09016의 조성물이다.
상기 제제는 상기 제제의 효과 및 안정성에 유익할 기타의 성분들로 구성될 수 있다. 그러한 성분들의 예는 우레아(urea), 설프히드릴기(sulfhydryl group)-함유 아미노산 및 기타의 각질 분해제(keratin degrading agents)이다. 각질 분해제의 예는 시스테인(cysteine), 아세틸 시스테인 및 메르캅토산(mercapto acids)이다.

본 발명의 제제는 네일에 적용되도록 의도된다. 상기 제제는 네일의 진균 감염을 치료하기 위해 사용될 것으로 의도된다. 그러나, 본 발명의 제제는 또한 기타 유형의 각질화된 조직, 예를 들어 굳은살(callous)의 진균 감염을 치료하기 위해 사용될 수 있다.
본 발명의 제 2 주요 측면에서는, 네일의 진균 감염 치료를 위해 본 발명의 조성물을 사용하는 것이 제공된다.
본 발명의 제 3 주요 측면에서는, 네일의 진균 감염 치료 방법이 제공되며, 여기서 본 발명에 의한 조성물은 환자의 네일에 투여된다.
실시예
본 발명의 제제의 효과를 평가하기 위하여 체외(in vitro) 침투법, 프란츠 셀(Franz cell)을 이용하였다. 네일 대신 발굽 막(hoof membranes)을 이용하였다. 상기 발굽은 이러한 유형의 실험에서 인간 네일을 대신한 허용가능한 모델이다(Mertin, D. Lippold, B. C. "인간의 네일 및 소 발굽으로부터의 각질막의 체외 투과도(in-vitro permeability): 네일 플레이트에 대한 항진균제의 침투 속도 및 그 효능의 예측" J Pharm Pharmacol, 1997, 49 (9), 866-72)(Mertin and Lippold 1997 인용).
상기 체외 약물 침투 실험은 Mertin and Lippold(1997)에 기재된 바와 같이 실행한다. 실험은 다음과 같이 수행하였다. 0.1 M 시트르산염(citrate) 버퍼, pH 3.7를 상기 프란츠 셀에서 수용체 용액(receptor solution)으로 사용하였다. 상기 수용체 용액은 실험 전에 헬륨으로 10분간 탈기하였다. 소 발굽 바닥으로부터의 발굽 막만을 사용하였다. 테르비나핀은 테르비나핀 염산염(terbinafine hydrochloride)의 형태로 사용하였다.
상기 발굽 막은 15분간 수화한 후에 확산 셀에 장착하였다. 6시간 동안 확산되도록 한 후 샘플링하였다. 모든 체외 침투 실험은 3회 수행하였다.
플럭스(flux)는 1% 테르비나핀의 플럭스로 정상화하였다. 따라서 상기 플럭스는 여기서 테르비나핀/%tbf*h*cm2 의 ㎍으로 기재되며 상기 침투 실험으로부터의 결과는 아래 식에 따라 계산되었다:
정상화된 플럭스 = Δm/(Δt*A*%tbf)

실시예 1: 제제 내 젖산의 서로 다른 레벨에서의 테르비나핀 플럭스
상기 제제 내에서 젖산의 효과를 본 실험에서 보여준다. 젖산을 0%에서 20%로 증가시키면 2배 더 높은 플럭스가 발생하였다(표 1에서 조성물 C와 조성물 E 비교).

이 실시예에서는 프로판디올을 다른 디올로 부분적으로 치환하였다. 펜탄디올이, 침투도 관점에서 헥산디올보다 더 우수한 성능을 보이는 프로판디올과 침투도에 대하여 동일한 효과를 갖는 것으로 보인다.

우레아의 양과 테르비나핀 침투도 사이의 관계를 알아보았다. 제제 내에 우레아를 도입한 것은 테르비나핀의 침투도에 아무런 영향을 주지 않았다.

이 실험에서는 4개의 조성물을 제조하였다. 조성물들은 표 4에 나열되어 있으며 프로판디올에 성분들을 용해시킴으로써 제조하였다.
표 4에서, 아세틸 시스테인을 제제에 포함시키면 플럭스가 40% 증가된다. 아세틸 시스테인의 농도의 증가는 플럭스를 증가시키지 않는다. 그러나, 우레아 대 아세틸 시스테인의 비율을 1:1로 유지하고 양측의 농도를 10%에서 15%로 증가시키면 플럭스가 40% 증가된다.
실시예 5: EDTA 의 효과
본 발명의 조성물 성분들을 안정화시키기 위한 시도로 EDTA를 제제에 첨가하였다. 체외 실험에서 놀랍게도 테르비나핀의 플럭스가 이미 높은 레벨에서 30% 증가하였음을 발견하였다. 데이터는 표 5에 제시된다.

Claims (12)

  1. 5% 보다 많은 양으로 존재하는 항진균 알릴아민 화합물, 유기산 또는 그 에스테르, 디올 및 격리제로 구성되고, 본질적으로 물이 없는(water-free), 네일의 진균 감염 치료용 제약학적 조성물.
  2. 청구항 1에 있어서, 상기 항진균 알릴아민 화합물은 상기 조성물에 용해된 것을 특징으로 하는 네일의 진균 감염 치료용 제약학적 조성물.
  3. 청구항 1 또는 2에 있어서, 상기 유기산은 젖산인 것을 특징으로 하는 네일의 진균 감염 치료용 제약학적 조성물.
  4. 청구항 1 내지 3 중 어느 한 항에 있어서, 상기 유기산 또는 그 에스테르는 5% 내지 25%의 양으로 존재하는 것을 특징으로 하는 네일의 진균 감염 치료용 제약학적 조성물.
  5. 청구항 1 또는 4 중 어느 한 항에 있어서, 상기 디올은 프로판디올, 부탄디올, 펜탄디올 및 헥산디올로 구성되는 군으로부터 선택되는 것을 특징으로 하는 네일의 진균 감염 치료용 제약학적 조성물.
  6. 청구항 1 내지 5 중 어느 한 항에 있어서, 상기 디올은 50% 보다 많은 양으로 존재하는 것을 특징으로 하는 네일의 진균 감염 치료용 제약학적 조성물.
  7. 청구항 1 내지 6 중 어느 한 항에 있어서, 상기 알릴아민 항진균 화합물은 테르비나핀인 것을 특징으로 하는 네일의 진균 감염 치료용 제약학적 조성물.
  8. 상기 청구항 중 어느 한 항에 있어서, 상기 알릴아민 항진균 화합물은 5% 내지 12%의 양으로 존재하는 것을 특징으로 하는 네일의 진균 감염 치료용 제약학적 조성물.
  9. 청구항 8에 있어서, 상기 알릴아민 항진균 화합물은 8% 내지 11.5%의 양으로 존재하는 것을 특징으로 하는 네일의 진균 감염 치료용 제약학적 조성물.
  10. 청구항 1 또는 2에 있어서, 약 67.5% 내지 약 84%의 프로판디올, 8% 내지 20%의 젖산, 0.03% 내지 0.1%의 EDTA 및 8% 내지 12%의 테르비나핀으로 구성되는 네일의 진균 감염 치료용 제약학적 조성물.
  11. 네일의 진균 감염 치료를 위한 청구항 1 내지 10 중 어느 한 항에 의한 제약학적 조성물의 이용.
  12. 청구항 1 내지 10 중 어느 한 항에 의한 조성물을 환자의 네일에 투여하는, 네일의 진균 감염 치료 방법.
