뒤셴 근이영양증(DMD)은 근이영양증의 가장 흔하고 심각한 형태이다. 3500명의 신생아 중 1명에게 영향을 미치는 DMD는 인간 DMD 유전자의 넌센스(nonsense) 돌연변이 및 아웃 오프 프레임(out of frame) 돌연변이에 의해 야기되는 중증 x-연관 근육 소모 질환이다. DMD 환자는 전형적으로 10세 내지 12세까지 휠체어를 필요로 하고 10대 후반 내지 30대 초반에 사망한다.

시험관내 인간 세포에서의 연구 및 생체내 동물 질환 모델에서의 연구는 DMD에 대한 잠재적 치료법으로서 엑손 스키핑의 원리를 입증하였다. 고무적인 결과를 제공하는, 2종의 상이한 AO 화학물질(포스포로다이아미데이트 모폴리노 올리고머(PMO) 및 포스포로티오에이트-연결된 2'-.베타.-메틸 RNA(2'OMePS))을 사용한 초기 임상 시험이 최근에 수행되었다. 엑손 51에 대한 2'OMePS AO가 주입된 4명의 DMA 환자의 TA 근육에서 디스트로핀 발현의 복구가 반 듀테콤(van Deutekom) 등에 의해 보고되었다.

실시예
하기 실시예에서, 안티센스 올리고뉴클레오티드 분자의 효능을 시험하기 위해 3종의 상이한 시험관내 모델 및 생체내 모델을 이용하였다: 1) DMA 근모세포; 2) 정상 인간 근모세포; 및 3) (거의 정상 수준의 디스트로핀을 발현하는 전체 인간 디스트로핀 유전자를 함유하는) 인간화된 DMD(hDMD) 마우스. 상기 3종의 모델을 이용하여 특정 인간 디스트로핀 엑손을 표적화하는 안티센스 올리고머를 스크리닝하였다.
DMD 근모세포 및 정상 인간 근모세포를 24웰 플레이트에서 생장시켰고, 2.5 μM의 PMO 또는 2'-O-메틸 올리고뉴클레오티드(2'OMePS)를 각각의 안티센스 올리고뉴클레오티드에 대한 초기 이중 스크리닝에 사용하였다. AO 처리로부터 24시간 후, 세포를 분화 배지에서 추가 2일 동안 배양하였다.
hDMD 마우스에서의 시험을 위해, 20 ㎍의 PMO 안티센스 올리고뉴클레오티드를 40 ㎕의 식염수에서 제조하였고 근육내 주사를 통해 이중으로 투여하였다. 상기 안티센스 올리고뉴클레오티드는 폴록사머와 폴리에틸렌이민(PEI)의 접합체, 또는 근육 섬유 전달을 용이하게 하기 위한 덴드리머성 옥타구아닌딘 중합체와 접합된 PMO를 포함하는 중합체와 함께 전달되었다. 폴록사머는 폴리옥시프로필렌(폴리(프로필렌 옥사이드))의 중심 소수성 쇄 및 폴리옥시에틸렌(폴리(에틸렌 옥사이드))의 2개 측면 친수성 쇄를 포함한다. 도 7을 참조한다.
단회 주사로부터 7일 후 처리된 근육을 모아 액체 질소-냉각된 이소펜탄에서 급속 냉동시키고 -80℃에서 저장하였다. 상기 근육으로부터 절편을 잘라내어 미세원심분리기 튜브 내로 모은 후, RT-PCR을 위해 RNA를 추출하였다.
각각의 모델 시스템에서 엑손 스키핑 효율을, DMD 유래의 세포 및 인간 근모세포에서는 RT-PCR 및 네스티드(Nested)-PCR로 측정하였고 hDMD 마우스에서는 RT-PCR로 측정하였다. 하기 절차를 이용하였다: 올리고머로 처리된 세포 또는 근육 조직으로부터 RNA를 제조자의 설명서에 따라 트라이졸(TriZol) 시약(인비트로겐(Invitrogene), 미국 캘리포니아주 칼스바드 소재)으로 추출하였다. 추후 사용을 위해 RNA를 -80℃에서 저장하였다. RT-PCR을 RT-피델리택 마스터믹스(RT-Fidelitaq MasterMix)(USB, 미국 오하이오주 클리블랜드 소재)로 수행하여 관심 있는 서열을 증폭하였다. 100 ng의 주형 RNA를 각각의 25 ㎕ RT-PCR 반응에 사용하였다.
