Prikkelgeleidingsblokkeringsinstrument.
De uitvinding heeft betrekking op een instrument voor het in een of meer wanden van de boezems van het hart aanbrengen van tenminste een streepvormige transmurale laesie die de elektrische prikkelgeleiding, in een richting die de streepvormige transmurale 5 laesie kruist, in hoofdzaak blokkeert.
Allerlei vormen van hartritmestoornissen en in het bijzonder chronisch en paroxysmaal boezemfibrilleren zijn tegenwoordig chirurgisch te behandelen.
Een bekende operatieprocedure (MAZE) is ontworpen om atrium-10 fibrilleren permanent op te heffen. In bedoelde procedure worden met behulp van een scalpel incisies in de wanden van de hartboezems gemaakt teneinde door de aldus ontstane onderbreking van de weefselcontinuïteit elektrische prikkelgeleiding in een richting die de incisies kruist te blokkeren. Door de navolgende littekenvor-15 ming krijgen deze elektrische blokkades een permanent karakter.
Deze bekende techniek wordt vanwege de complexiteit van de ingreep wereldwijd nog maar in beperkte mate uitgevoerd. Het verhoogde risico is vooral gerelateerd aan de duur van de ingreep en aan de wijze waarop de ingreep moet plaatsvinden.
20 De operatieduur en met name de zogenaamde Cross-Clamp tijd (X-Clamp) is zodanig lang dat de kans op beschadiging van de hartspier groot is.
De Cross Clamp tijd die nodig is voor alleen de MAZE procedure bedraagt op dit moment nog gemiddeld 68 min. (gebied 50-102 min.) 25 en de benodigde tijd aan de hart-long machine gemiddeld 182 min. (gebied 130-256 min) . Voor verdere gegevens wordt verwezen naar Atrial Fibrillations:Mechanisms and Therapeutic Strategies, Futura Publishing Co, Inc .Armonk,N.Y. 1994 .J.L.Cox:Surgical Interruption of atrial reentry as a cure for atrial fibrillation.
30 De wijze waarop de operatie wordt uitgevoerd met de scalpel geeft vanwege het grote aantal en de lokalisatie van de daarbij behorende vaatnaden een verhoogde kans op naadlekkages en nabloedingen.
De onderhavige uitvinding heeft ten doel een instrument van het bovenaangegeven type te verschaffen dat de genoemde 35 1003024 2 nadelen opheft en dat met name de voor de operatie benodigde tijd bekort, de kans op bloedingen en beschadiging vermindert en derhalve het risico voor de patiënt in een open hart operatie die benodigd 5 is vermindert.
Volgens de uitvinding wordt het instrument dat is aangegeven gekenmerkt doordat door de aspecten zoals verwoord in het kenmerkende gedeelte van conclusie 1.
Het instrument volgens de uitvinding is een sonde waarmee in open 10 hart operaties een permanente verandering van een boezemwand teweeggebracht kan worden die transmuraal is d.w.z. zich over de volledige dikte van de wand uitstrekt.
Zoals later nog zal worden besproken wordt bij de uitvoering van de behandeling de elektrode aan het uiteinde van de sonde in 15 aanraking gebracht met de te behandelen boezemwand en daarlangs bewogen in een lijnvormig patroon. Bij bekrachtiging van de elektrode met RF vermogen vindt diëlektrische (RF) verwarming van het wandweefsel plaats. Door de RF behandeling treedt verandering van de celstructuur van de boezemwand op waardoor prikkelgeleiding in 20 een richting die de transmurale laesie kruist wordt geblokkeerd. Om goed met de sonde te kunnen werken moet deze een relatief stijf karakter bezitten zodat plaatsing en beweging van de elektrode op en langs de boezemwand met nauwkeurigheid kunnen plaatsvinden. In de behandeling treedt geen desintegratie van het weefsel van de 25 boezemwand op en bestaat er geen kans op nabloedingen. De behandeling kan naar wens aan de buitenzijde of de binnenzijde van de boezem plaatsvinden.
Werkwijzen van RF verwarming of diëlektrische verwarming zijn gebaseerd op het benutten van warmte die wordt opgewekt in materialen 3 0 die relatief slechte elektrische geleiders zijn wanneer deze worden geplaatst in hoogfrequente electromagnetische velden.De warmte wordt opgewekt als gevolg van diëlektrische verliezen die optrden in een materiaal dat zich bevindt tussen metalen elektroden die een condensator vormen en die verbonden is met een hoogfrequent(RF) 35 generator. Een dergelijke verwarming is in hoge mate gelijkmatig en derhalve voor de toepassing van het instrument, de sonde, volgens de uitvinding buitengewoon geschikt. Bij gebruik van de sonde wordt een van de condensator"platen" gevormd door de elektrode aan het uiteinde van de sonde terwijl de andere "plaat" een tegenelektrode 40 is die bijvoorbeeld op de rug van de patient wordt geplakt; bij 1003024 3 voorkeur wordt bij aanbrengen van laatstgenoemde elektrode gebruik gemaakt van een contactgelei die elektrisch geleidingsvermogen bezit. De tegenelektrode kan uiteraard ook, wanneer bijvoorbeeld de elektrode van de sonde in aanraking wordt gebracht met de binnenzijde 5 van de hartboezemwand, aan de buitenzijde van bedoelde wand worden geplaatst.
In verband met de uitvinding wordt gewezen op WO 95/03742 waarin een catheter is beschreven die aan het distale uiteinde een metalen elektrode omvat waarmee weefselerosie kan worden uitgevoerd die 10 ook wel ablatie wordt genoemd.
In verband met de uitvinding wordt voorts gewezen op US-A-5013312. Deze publicatie beschrijft een instrument van het aangegeven type dat echter een bipolaire uit delen opgebouwde electrode bezit terwijl in het onderhavige instrument de electrode een gesloten electrode 15 is.
WO 95/19148 beschrijft de toepassing van temperatuuropneemmiddelen en een terugkoppelingsmechanisme om de temperatuur van een electrode te regelen in een catheter. Dit instrument is niet een relatief stijve sonde zoals het instrument volgens de uitvinding.
20 WO 95/17222 beschrijft eveneens een catheter die middelen omvat om vloeistof langs een uiteinde-electrode te laten stromen.
Ook dit instrument is niet een relatief stijve sonde zoals het instrument volgens de uitvinding.
Een dergelijke catheter bezit typisch een lengte van ongeveer 25 lm; een diameter van ongeveer 2 mm met een elektrode van ongeveer 2 mm diameter en is door zijn geringe dikte zeer flexibel zodat deze zonder problemen een ader kan volgen. Voor locale puntvormige ablatie is deze catheter geschikt. Voor toepassing als sonde voor het aanbrengen van streepvormige transmurale laesies in een 30 boezemwand is een dergelijke catheter niet geschikt.
In het bijzonder omvat de sonde volgens de uitvinding tenminste een handvat; een uiteinde; een relatief stijve schacht tussen het handvat en het uiteinde alsmede aansluit- en geleidingsmiddelen voor het verbinden van het uiteinde van de sonde met een RF 35 vermogensbron.
In het instrument volgens de uitvinding is zoals aangegeven nabij het uiteinde van de sonde een temperatuuropnemer aanwezig die in werking onder terugkoppeling met de RF vermogensbron de 1003024 4 temperatuur van het uiteinde van de sonde kan regelen op een vooraf ingestelde waarde. Door invoer van het RF vermogen zal in het algemeen de temperatuur van het uiteinde van de sonde stijgen; terugkoppeling met de RF vermogensbron maakt het mogelijk er voor 5 te zorgen dat de temperatuur van het uiteinde niet boven een voorafbepaalde waarde stijgt.
Bij toepassing van RF vermogen is het van groot belang dat zich in het lichaamstrajekt tussen het uiteinde van de sonde en de tegene lekt rode aan de buitenzijde van het lichaam zo weinig 10 mogelijk elektrische blokkades bevinden. In verband daarmee wordt er bij voorkeur voor gezorgd dat de sonde kan samenwerken met middelen voor het naar het uiteinde ervan toevoeren van een fysiologisch aanvaardbare vloeistof. In zijn meest simpele vorm wordt via een leiding,die geen deel uitmaakt van de sonde,een 15 dergelijke vloeistof toegevoerd nabij de elektrode van de sonde.De vloeistof heeft enerzijds een functie voor het koelen van de elektrode en voorkomt anderzijds het ontstaan van elektrisch isolerende luchtspleten die de effectiviteit van de RF werking nadelig beïnvloeden.
2 0 Voorts omvat de sonde volgens de uitvinding zoals aangegeven middelen voor het nabij het uiteinde van de sonde laten uitstromen van een fysiologisch aanvaardbare vloeistof. Bedoelde vloeistof zal in het algemeen bij voorkeur een zekere mate van elektrische geleiding bezitten en is doelmatig een fysiologische zoutoplossing.
2 5 In een zeer aantrekkelijke uitvoeringsvorm omvat het instrument tussen het handvat van de sonde en de schacht inlaatmiddelen voor het invoeren van de fysiologisch aanvaardbare vloeistof die binnen de schacht elektrisch geïsoleerd blijft van de in de schacht aanwezige aansluit- en geleidingsmiddelen terwijl nabij het uiteinde 30 uitstroommiddelen voor de fysiologisch aanvaardbare vloeistof omvat. Met behulp van deze uitvoeringsvorm is de werking van de sonde nog verder te optimaliseren en kan worden gewaarborgd dat de grootste RF energiewerking zich concentreert in de wand van de te behandelen boezem onder vorming van de gewenste transmurale laesie.
35 De geleidings- en aansluitmiddelen van de sonde volgens de uitvinding omvatten aan de van het uiteinde van de sonde afgekeerde zijde van het handvat een daarmee verbonden connector met kontaktmiddelen voor aansluiting van de elektrode aan het uiteinde 1003024 5 van de sonde en de daarin aanwezige temperatuuropnemer aan de RF vermogensbron.
De connector is bij voorkeur van het snelkoppelingstype zodat gemakkelijke koppeling aan de RF vermogensbron wordt toegelaten.
5 Teneinde de hantering van het instrument volgens de uitvinding in de vorm van een sonde door de behandelende chirurg tijdens een open hart operatie zo gemakkelijk mogelijk te maken omvat de schacht van de sonde bij voorkeur een eigen kromming die doelmatig ongeveer 140° bedraagt.
10 De uitvinding zal nu worden toegelicht aan de hand van de tekening waarin: - figuur 1 een schetsmatig beeld toont van de transmurale laesies die met het instrument volgens de uitvinding kunnen worden aangebracht en die elektrische prikkels in richtingen die bedoelde 15 laesies kruisen kunnen blokkeren, - figuur 2 een instrument volgens de uitvinding toont in een eerste uitvoeringsvorm, - figuur 3 een instrument volgens de uitvinding toont in een tweede uitvoeringsvorm.
20 In figuur 1 zijn schematisch in een twee-dimensionale weergave de twee boezems van een menselijk hart gegeven waarbij de tranmurale laesies zijn aangegeven met verwijzingsletter C, de ongestoorde elektrische prikkels met A en de geblokkeerde elektrische prikkels met B. De laesies C hebben het karakter van littekenweefsel dat 25 is gevormd na behandeling met behulp van de sonde volgens de uitvinding.
Figuur 2 toont een sonde volgens de uitvinding in een eerste uitvoeringsvorm en toont een handvat l, een werkzaam metalen uiteinde 2 met de aanduiding van de plaats van een temperatuurvoeler 3. De 30 schacht van de sonde 5 heeft een kromming van ongeveer 140° 4 en in de schacht verloopt de elektrische bedrading 6 voor bekrachtiging van het elektrodevormig uiteinde 2 en bedrading 7 voor verbinding met de temperatuurvoeler die is aangebracht ter plaatse van het verwij zingscijfer 3. Het uiteinde 2 is volledig gesloten en is bij 35 voorkeur vervaardigd uit een inert metaal zoals platina of goud.
Binnen het handvat 1 zijn niet nader aangeduide elektrische schakelingsmiddelen 10 om de sonde te kunnen aansluiten op de niet getekende RF generator. Met 8 en 9 is voorts een connector aangeduid 1003024 6 die het mogelijk maken om de sonde te koppelen aan de RF generator.
Figuur 3 toont een bijzonder voordelige uitvoeringsvorm van de sonde volgens de uitvinding die identiek is aan de sonde uit figuur 2 maar waarbij met 11 een y-connector is aangegeven die het 5 mogelijk maakt om een fysiologisch aanvaardbare vloeistof via een poort 12 toe te voeren binnen de schacht 5 waarbij bedoelde fysiologisch aanvaardbare vloeistof door de schacht 5 wordt geleid buiten aanraking met de geleidingsmiddelen 6 en 7. De fysiologisch aanvaardbare oplossing stroomt via de poort 12 naar een binnenschacht 10 13 en vandaar via de schacht 5 naar de uitstroompoorten 14 die zijn aangebracht in de nabijheid van het metalen uiteinde 2. Bij uitstroming van de vloeistof uit de uitstroompoorten 14 stroomt deze langs en over het oppervlak van het metalen uiteinde 2 van de sonde. De stroming vindt druppelsgewijs plaats b.v. 1-5 ml 15 oplossing per minuut alhoewel grotere of kleinere hoeveelheden mogelijk zijn. De fysiologisch aanvaardbare vloeistof is doelmatig een fysiologische zoutoplossing die door het lichaam goed verdragen wordt.
De fysiologische zoutoplossing zorgt enerzijds voor koeling 20 van de elektrode 2 en verlaagt anderzijds de elektrische weerstand tussen de elektrode van het uiteinde 2 en de boezemwand. Met de in figuur getoonde sonde worden buitengewoon goede en reproduceerbare resultaten verkregen. De bron van RF vermogen is typisch een generator die bij een frequentie van 500 kHz een vermogen van 25 bijvoorbeeld maximaal 50 watt kan af leveren. Het af gegeven vermogen is een funktie van de ingestelde temperatuur en het weefselkontakt van de elektrode die het uiteinde van de sonde vormt. De gewenste temperatuur is bij de generator in te stellen en ligt in het algemeen in het gebied van 50 tot 70°C.Bij toelaten van temperaturen hoger 30 dan het aangegeven gebied zal verbranding van het weefsel (coagulatie) optreden waardoor een isolerende laag wordt gevormd; bedoelde laag zal verdere inwerking van de RF energie bemoeilijken waardoor onderliggend weefsel minder volledig of niet wordt behandeld.
35 Het uiteinde 2 van de sonde omvat doelmatig platina en heeft typisch een cylindervorm met een diameter van 4 mm. De diameter kan in het algemeen liggen tussen 3 en 6 mm.
De totale lengte van de sonde zonder aansluitmiddelen is typisch 1003024 7 ongeveer 35 cm waarbij het handvat ongeveer 20 cm lang is; de schacht ongeveer 10 cm en het uiteinde ongeveer 2 cm. In het algemeen heeft de schacht 5 een lengte tussen 8 en 15 cm en bezit een diameter tussen 3 en 6 mm en is uit een fysiologisch aanvaardbare kunststof 5 vervaardigd. Geschikte kunststoffen zijn nylon 66, polypropyleen en hoge dichtheid polyethyleen.

Claims (8)
1. Instrument voor het in een behandeling in een of meer wanden van de boezems van het hart aanbrengen van tenminste een streepvormige transmurale laesie die de elektrische prikkelgelei-ding in een richting die de streepvormige transmurale laesie kruist in hoofdzaak blokkeert, 10 met het kenmerk, dat het instrument een sonde is waarvan het uiteinde (2),dat bij de behandeling in aanraking komt met de te behandelen wand, een gesloten elektrode is die met een RF vermogensbron kan samenwerken terwijl de sonde een relatief stijf karakter bezit,· 15 dat in het uiteinde (2) van de sonde een temperatuuropnemer (3) aanwezig is die in werking onder terugkoppeling met de RF vermogensbron de temperatuur van het uiteinde (2) van de sonde kan regelen op een vooraf ingestelde waarde, en dat de sonde kan samenwerken met middelen voor het naar het uiteinde (2) ervan 20 toevoeren van een fysiologisch aanvaardbare vloeistof.
2. Instrument volgens conclusie 1, met het kenmerk, dat de sonde tenminste omvat een handvat (1) ; een uiteinde (2) ; 25 een relatief stijve schacht (5) tussen het handvat (1) en het uiteinde (2) alsmede aansluit- en geleidingsmiddelen (6, 7) voor het verbinden van het uiteinde (2) van de sonde met de RF vermogensbron. 1 2 3 4 5 6 1003024
3. Instrument volgens conclusie 1, 2 met het kenmerk, 3 dat de middelen voor het naar het uiteinde (2) van de sonde 4 toevoeren van een fysiologisch aanvaardbare vloeistof deel 5 uitmaken van de sonde. 6

Instrument volgens conclusie 1-3, met het kenmerk, dat tussen het handvat (1) van de sonde en de schacht (5) inlaat- middelen(12) zijn opgenomen voor het invoeren van de fysiologisch aanvaardbare vloeistof en binnen de schacht(5) elektrisch geïsoleerde doorvoer van deze vloeistof naar het uiteinde van de sonde kan plaatsvinden terwijl nabij het uiteinde(2) 5 uitstroommiddelen (14) voor de vloeistof aanwezig zijn.
5. Instrument volgens een of meer van de voorgaande conclusies 2-4, met het kenmerk, dat de geleidings- en aansluitmiddelen (6, 7) van de sonde aan 10 de van het uiteinde (2) van de sonde afgekeerde zijde van het handvat (l) een daarmee verbonden connector (8, 9) omvatten met contactmiddelen voor aansluiting van het uiteinde(2) en de temperatuuropnemer(3) aan de RF vermogensbron.
