Description
Brystprotese for implantasjon.
Nærværende oppfinnelse vedrører brystproteser, som anvendes for implantasjon i kvinnebryst. Det finnes tidligere to gene-relle typer av brystproteser. Den første bæ-res utvendig i en slags bystholder og anvendes generelt, om brystet eller deler av det er f j ernet kirurgisk. En annen type omfatter en protese beregnet på å innføres i brystet for å forandre dets kontur. Denne type anvendes vanligvis av estetiske grun-ner, og det er denne protesetype som nærværende oppfinnelse i første rekke er beregnet ipå.
Tidligere har proteser, beregnet på å ■implanteres i brystet, vært fremstilt av svampmaterial. Denne fremgangsmåte er beheftet med to alvorlige ulemper. Den vik-tigste av disse er at kroppsvevet (trenger inn i svampen under dannelse av et hårdt arr, som er plagsomt og på ingen måte ligner det naturlige brysts konsistens. Den andre ulempen er at svampimplantatet selv uten Inntregn.ing av kroppsvevet, ikke har samme følelse som det naturlige brystet.
Hovedformålet med nærværende oppfinnelse er å fremskaffe en brystprotese som er egnet for innføring i brystet, og som nesten fullstendig har den naturlige ibryst-vevikonsistens, hvorved arrvevet bare dannes mot brystkasseveggen for å feste protesen sikkert til 'kroppen. Ifølge oppfinnelsen omfatter brystprotesen dels et silisium-gummi- hylster, som 1 f ormen ligner et kvinnebryst, dels et mykt sillsiumgel, som fyller hylsteret og dels et skikt av porøst materiale som er anbragt på hylsterets ene side, slik at kroppsvevet kan vokse inn i det porøse materialet for å feste protesen til den bærende persons brystkasse.
Oppfinnelsen beskrives i det følgende nærmere med henvisning til de vedlagte tegninger, hvilke, som eksempel, viser en utførelsesform. Fig. 1 er et snitt som viser protesen på plass. Fig. 2 er et perspektdv-bilde som viser montasjen av hylsteret. Fig.
3 og fig. 4 er snitt etter linjen 5—5 i fig. 2
og viser en måte å anbringe et porøst skikt på protesens ene side.
Fig. 1 viser protesen på plass. Hylsteret består av to deler, nemlig en skål 6 som har samme form som kvinnebrystet, og et foak-stykke 7 som er stort sett plant og beregnet på å passe mot brystkassen 8. Hylsteret er fylt med et mykt gel 11 som gir protesen egnet konsistens. Dette gel kan bestå av vilkårlig, egnet materiale, men man fore-trekker et silisiuimgel slik som beskrives nærmere i det følgende. På batøtykket 7 er det festet et porøst materiale, i hvilket kroppsvevet kan vokse inn for derved å feste protesen sikkert til brystkassen. Ifølge fig. 1 består dette porøse materiale av tøy som er lagt i bølger og f orbundet med bakstykket 7 ved hjelp av flekkvis anbragt bindemiddel 10.
Det porøse skikt består av et eller an-net egnet materiale, i hvilket 'kroppsvevet kan trenge inn og hvilket ikke opptas av kroppen. Det foretrukne materiale beistår av tøy av polyesterfibere av den type som forhandles under benevnelsen «Dacron». Etter at protesen er innført i 'brystet, slik
det skal beskrives nærmere i det følgende,
lukkes snittet og det naturlige brystvevet 12 medvirker .til å understøtte protesen og holde den på rett plass.
Protesen kan monteres på enhver eg-
net måte og en foretrukken fremgangsmå-
te vises i fig. 2. Den bøyelige skål 6 og bak-
stykket 7 formes separat, hvoretter begge settes sammen enten ved vulkanisering el-
ler anvendelse av et egnet bindemiddel.
Etter at hylsteret er satt sammen, bringes
det porøse skikt, f. eks. tøy, til å feste seg på ibakstykket 7. Ved anvendelse av tøy foretrekkes et tøy som er lagt i bølger, slik som vist i fig. 3 og 4. For dette formål kan tøyet plaseres på bakstykket 7, hvoretter det Urnes fast på dette med flekkvis anbragt bindemiddel 10, se fig. 3. Tøyet skyves sam-
men så det dannes bølger og bindemidlet 10 anbringes flekkvis i rader, se fig. 4, i bunnen av bølgene 9. Dette gjentas inntil en serie bølger er festet over hele lengden av bakstykket 7.
Etter at hylsteret og det porøse mate-
rialet er satt sammen, innføres i hylsteret på egnet måte en væske, som bringes i gel-
form. Det beste er å sprøyte inn en væske gjennom hylsterets vegger ved 'hjelp av en injeksjonssprøyte. Etter at væsken er bragt i gelform har den ingen eller ubetydelig tendens til å lekke igjennom det hull som er dannet av injeksjonssprøytenålen. Om hullet skulle være for stort, kan det lett forsegles ved hjelp av et bindemiddel eller lignende. Protesens størrelse kan anpasses etter de individuelle behov. For en protese av mellomsitørrelse innsprøytes ca. 270 ml væske i hylsteret for siden å bringes i gel-
form 1 dette. Større eller mindre proteser kan imidlertid også fremstilles, om således ønskes.
Slik som ovenfor nevnt kan man an-
vende 'Vilkårlig egnet material som ikke forårsaker reaksjon med vevet og som er mykt og bøyelig. Et egnet material for hylsteret er silisium-gummi og et egnet gel er et silisiumgel. Et slikt gel kan fremstilles ifølge det kanadiske patent nr. 640 244. Det må imidlertid fremheves at silisiumgelet ikan fremstilles på annen måte, f. eks. ved at en blanding av et hydroxylert slloxan og ethylsilikat innsprøytes 1 hylsteret sammen med en egnet katalysator, som tinn (II)
caprylat. Væsken stivner så på sin plass under dannelse av et mykt gel. Uavhengig av den anvendte type gel, bør gelet ha myk konsistens målt med et penetrometer, se Presiision Scientific Company, katalog nr.
73, s. 510. I stedet for de standard kjegler som leveres med penetrometeret, monteres et messinghode med diameter 6,4 mm og
høyde 4,8 mm og avrundete kanter på spin-
delen. Den totale vekt for denne og hodet er 51,9 ig. En prøve av gelet ved værelsetem-
peratur plaseres mot undersiden av spin-
delhodet. Akselkoblingen trykke® ned 5 sek.,
hvoretter inntregningsdybden i gelet må-
les. For den foretrukne utførelsesform iføl-
ge nærværende oppfinnelse bør penetra-
tjonen være 30,0-—20,0 'mm, skjønt for visse formål er høyere eller lavere penetrasjon ønskelig.
Den ovenfor beskrevne utførelsesform
foretrekkes i øyeblikket, men i visse tilfelle kan man med fordel anvende hårdere eller mykere materiale. Uavhengig av den eksak-
te penetrometerverdi bør gelet være til-
strekkelig mykt og bøyelig for å etterligne det naturlige brystets 'konsistens. Det for-
gelete material bør videre være inert i for-
hold til hylsteret.
For innføring av protesen utføres et
vanlig, ca. 7,6 om langt snitt på brystets underside og brystet skjæres fritt fra bryst-
mUskelen og brystkassen. Hulrommet bør være tilstrekkelig stort for fritt å kunne ta imot protesen. Implantatet skyves opp med den tynne kanten øverst og rettes noe inn mot den forreste hj elpefolden. Etter at pro-
tesen er satt inn, trer ofte implantatets øv-
re del altfor sterkt frem. Dette kan korri-
geres etter at såret er sydd igjen, ved at et sterkt trykk settes mot, ved hjelp av noen skumgummimellomlag og heftplaster tvers over protesens øvre del. Herved presses ge-
let til den nederste del under strekning av overliggende vev, slik at brystet fremtrer sterkere slik som ønsket. Dette trykk kan holdes ved i en uke til 10 døgn. Brystet har nå naturlig utseende med oversiden svakt konkav når brystet understøttes med en bystholder.
Det porøse skikt, f. eks. «Dacron»-gas,
som limes fast på baksiden av protesen bør helst skjæres så nær implantatets kant som mulig. Herved forhindres altfor sterk dan-
nelse av arrvev mens protesen festes sik-
kert til brystkassen. Et særskilt egnet teks-
tilmateriale for anvendelse ifølge nærvær-
ende oppfinnelse, er en grovmasket poly-estertrikotvare som selges under betegnel-
sen «D200» av Mohawk Fabr-ic Company.
Claims (4)
1. Brystprotese for implatasjon i kvin-
nebryst, karakterisert ved at den omfatter dels et silisium- gummi-hylster, hvis form i hovedsak ligner et kvinnebryst, dels et mykt silisiumgel, som fyller hylsteret og dels et skikt av porøst tekstil-mate-rlal festet på hylsterets ene side, slik at kroppsvevet kan vokse linn i det porøse materialet for å feste protesen til den bærende persons brystkasse.
2. Brystprotese ifølge påstand 1, karakterisert ved at det porøse ma-terialskiktet består av *et bølgeformet tekstilmateriale, som er festet på hylsterets ene side.
3. Brystprotese ifølge påstand 2, karakterisert ved at det bølgeformete tekstilmaterialet er et polyestertekstlima-teriale.
4. Brystprotese ifølge påstand 3, karakterisert ved at det bølgeformete tekstilmaterialet med mellomrom er festet til hysterets bakstykke, slik at kroppsvevet kan vokse gjennom tekstilmaterialet i de deler, der det er fritt fra bakstykket.
