Description
Formålet med foreliggende oppfinnelse er å beskrive
en ny, hurtig metode for å bestemme kalsiumnivået i blod.
Det finnes menneske- og dyresykdommer der symptomene forårsakes av et unormalt høyt kalsiuminnhold i blodet. En hurtig og pålitelig metode til å bestemme kalsiumnivået er nødvendig for at man skal kunne stille riktig diagnose.
Spesielt når det gjelder dyr bør diagnosen helst gjøres på
stedet. Metoden ifølge foreliggende oppfinnelse er utviklet uttrykkelig for dette formål.
Mayer, G. P. et al (J. Am. Vet. Med. Ass. 146/8,
839-842) har beskrevet en bestemmelsesmetode som er basert på etylendiamintetraacetatets (EDTA) evne til å danne kelater med kalsium og på det forhold at blodets koaguleringsmekanisme er avhengig av frie kalsiumioner. Prøven kan utføres slik at en gruppe prøverør med stigende mengder EDTA fylles med blod. Eftersom koaguleringen bare skjer i de prøverør hvor det finnes
et overskudd av kalsium, kan blodets kalsiuminnhold bestemmes.
Prøven er imidlertid besværlig å utføre, og det er vanskelig å bestemme når koaguleringen i prøverørsserien har tatt slutt og hvilket av prøverørene som svarer til det riktige kalsiuminnhold, eftersom ekvivalenspunktet er uklart. Dette beror tydelig på at det kalsium som er bundet til blodets proteiner, frigjøres relativt langsomt.
Foreliggende oppfinnelse beskriver en ny, hurtig metode, hvor man på kort tid får et tydelig ekvivalenspunkt og med hvis hjelp man kan avgjøre om blodets kalsiuminnhold er, over eller under en bestemt grenseverdi.
Ved metoden ifølge foreliggende oppfinnelse binder man det kalsium som finnes i blodprøven, med et overskudd av EDTA. Denne blanding settes til, eller til denne blanding settes en bestemt mengde kalsiumioner. Den EDTA-mengde som ikke forbrukes ved kelatdannelsen med blodets kalsiumioner, reagerer nu med de txlsatte kalsiumioner. Hvis alle Ca 2 +-ioner har reagert med EDTA, skjer ingen koagulering. Hvis det er ubundne Ca 2 +-ioner tilbake, finner koagulering sted.
Prøven utføres i et prøverør med et overskudd av EDTA. Man kan anvende forskjellige EDTA-salter, men kaliumsaltet
har vist seg å være mer fordelaktig på grunn av sin hurtige oppløselighet. Prøverøret er forsynt med en graderingsskala som svarer til blodets forskjellige kalsiuminnhold. Fullblod eller plasma settes til prøverøret i en mengde som svarer til grenseverdiens gradering. Prøverørets innhold rystes godt, og til blandingen settes, eller blandingen settes til en bestemt mengde kalsiumioner, f.eks. kalsiumklorid, samt et stoff som påskynder koagulering, f. eks. tromb.oplas tin (fremstilt av storfehjerner).
På 5 minutter har man nådd ekvivalenspunktet. Hvis EDTA-mengden er i stand til å binde elt kalsium, både det i • blodet og det som tilsettes, koagulerer ikke prøven,, men forblir flytende, hvilket betyr at blodets kalsiuminnhold er lavere enn den grenseverdi man har valgt. Hvis prøven derimot inneholder frie kalsiumioner, begynner koaguleringsmekanismen å virke, og prøven størkner eller partiell koagulasjon finner sted, med andre ord er blodets kalsiumnivå høyere enn den valgte grenseverdi.
Ved å variere mengden av EDTA og blodprøve kan man
velge grenseverdier som svarer til forskjellige kalsiuminnhold. Det har vist seg å være enklest å variere mengden av blodprøve. Man kan gjøre dette enkelt ved å forsyne prøverøret med en
•gradering som svarer til bestemte kalsiuminnhold i blodet. Det er naturligvis mest fordelaktig å velge slike sammenlignings-verdier som ligger nær kritiske kalsiuminnhold i blodet, f.eks. 9 mg/100 ml serum (2,25 mmol/1) og 8 mg/100 ml serum (2,00 mmol/1).
Hvis man setter kalsiuminnholdet på 8 mg/100 ml storfe-plasma som grenseverdi og som prøve anvender 10 ml fullblod, hvis koagulering forhindres av 10,1 mg K2EDTA-2H20 (25,0 yM) kan. man ved hjelp av storfeets, middelhematokritt (33,5%) beregne den kalsiumtilsetning som behøves. Herved får man verdien 11,7 yM Ca, som svarer til 1,72 mg CaCl-2H20.
Når man har beregnet de reagensmengder som behøves,
har man gått ut fra blodlegemenes volum på 33,5 volumprosent og fra at hvis kalsiuminnholdet er 6 mg/100 ml, inneholder 1 ml fullblod 40 yg kalsium, hvilket svarer til en konsentrasjon på 1 ymol/ml. Hvis man til dette legger 1 ml 0,001N EDTA opp-løsning nås ekvivalenspunktet eller alt kalsium danner kelater.
Hvis blandingen av blod og EDTA settes til kalsium,
trenger man to prøverør for å utføre prøven. Hvis tilsetningen skjer på motsatt måte, kan man.for å unngå pipetteringer til-
sette kalsium i fast form, som pulver eller piller.
Valget av grenseverdi beror på det man vil diagnostisere. Hvis man f.eks. vil forsikre seg om det lave kalsiuminnholdet i. blodet hos en ku, velger man grenseverdien 9 mg%. Hvis man vil bestemme blodets kalsiuminnhold i forbindelse med andre sykdomsårsaker, f.eks. ved muskeldegenerering eller septisk colimastitt, hvor til og med et lite overskudd av kalsium kan være livsfarlig, velger man grenseverdien 8 mg%.
Eksempel 1
Påvisning av hypokalcemi hos storfe.
Prøverør A, hvor det finnes EDTA, fylles med blod
direkte fra blodkaret, til den valgte grenseverdi (8 eller 9 mg%). Røretusvingés opp og ned 20 ganger, men får ikke rystes, eftersom skum forhindrer tolkningen av prøven. Til glassampullen B,
som inneholder kalsiumklorid og tromboplastin, settes ca.
10 dråper destillert vann, og ampullen rystes. Blod-EDTA-blandingen helles fra prøverøret A i glassampullen' B. Ampullen lukkes, og innholdet blandes ved å svinge den opp og ned 20 ganger. Ampullen får stå på stedet i 5 minutter, hvorefter man undersøker
om koagulering har funnet sted ved å svinge ampullen vannrett.
Hvis prøven har koagulert, overskrider prøvens kalsiuminnhold
den valgte grenseverdi, og storfeet har hypokalcemi.
Ved en feltprøve (159 undersøkelser) har metoden vist
seg å være enkel og pålitelig ved diagnostisering av kalvnings-lammelse. Da tilsvarende serum ble analysert med atom-absorpsjonsspektrofotometer, kunne man konstatere at 100% sikkerhet oppnås hvis man tolererer en variasjon på ± 0,5 mg/100 ml. Forskjellene i kalsiumverdiene oppnådd med de forskjellige metoder beror hovedsakelig på variasjoner i hematokrittverdiene. Den metode som beskrives i henhold til foreliggende oppfinnelse, måler
kalsiumnivået i fullblod eller plasma, mens klassiske metoder måler kalsiuminnholdet i serum, eller plasma.
Claims (4)
1. Fremgangsmåte for bestemmelse av kalsiumnivået i blod eller plasma, basert på etylendiamintetraacetatets evne til å danne kelater med kalsium, samt blodets koaguleringsmekanismes avhengighet av frie kalsiumioner, karakterisert ved at blodets kalsium bindes med et overskudd av etylen-diamintetraacetat, og denne blanding settes til eller til denne blanding settes en bestemt mengde kalsiumioner og et stoff som påskynder koaguleringen.
2. Fremgangsmåte som angitt i krav 1, karakterisert ved at etylendiamintetraacetatet anvendes i form av natrium- eller kaliumsalt.
3. Fremgangsmåte som angitt i krav 1,
karakt'e ris' 'ert. ved at man anvender kalsiumklorid som kalsium.
4. Fremgangsmåte som angitt i krav 1, karakterisert ved at man for å påskynde koaguleringen anvender tromboplastin.
