Wstepne wygiecie plytki niszczy Jej warstwe pasywna i inicjuje mikropek¬ niecia, które moga powlekazac sie w miare przenoszenia róznych obciazen, prowadzac do cal¬ kowitego pekniecia plytki. Powoduje to niejednoznaczne ustalenie zespolu srubowo-trzpieniowego w polozeniu prostopadlym do powierzchni czolowej plytki i moze sie ono zmieniac zarówno w osi podluznej jak. i poprzecznej. Wystepuje tu wiec kolej¬ ny czynnik destabilizujacy zespolenie. Naklad 100 egz.Cena 220 zl PLThe present invention relates to a pressure-locking bone assembly consisting of a pressure plate, bone screws and nuts. The known set for pressure-locking bones from Polish patent specification No. 124 448 consists of screw-pin assemblies with thrust washers, a pressure plate with holes for the threaded ends of the bolts and with seats for profile nuts. The nut seats and the holes for the threaded ends of the bolts have an elongated shape along the longitudinal axis of the plate and have oriented sliding surfaces. The side surfaces of the seats have a U-shape in cross-section. The characteristic feature of the production of these seats is the displacement of the spherical cutter by a specific segment in relation to the axis of the seat, by which the pin will move during its tightening. Due to the fact that they have smooth shanks, they will be terminated on one side with a normal thread, they will additionally have a cortical thread on the opposite end. On the ends of spindles with a normal thread, pore plates are placed, which, after the plate with a groove in the bottom part is placed, engage the groove. The well-known set is assumed that in the holes made in different directions in the bones, sets of screws are inserted spindle. Thrust washers and a pressure plate, screwed with nuts, are applied to the screw ends. The well-known set for the pressure fixation of bones has a number of disadvantages, which do not allow to achieve the assumed goal. The smooth pins do not give any connection between the fastener and the bone, as they are only inserted into the hole. Even a short cortical thread on the end of one of the long pins does not allow for a stable connection because the self-tapping end connects too little to the bone. Applied to the ends of the spindles, with a normal thread, the thrust washers separate the plate from the bone, reducing its pressure on the bone, but not eliminating it. Due to the lack of connection of the smooth pins with the bone, it is not possible to lift the plate above the bone 1 the whole assembly must be placed on an operationally open fracture. The thrust washers are a sliding element in the direction of the longitudinal axis of the mandrel, which further confirms their function as indirect pressure-transmitting elements on the bone. The situation does not change even when the pin has a diameter greater than the diameter of the threaded end, since the pin does not allow the assembly to be lifted because of its smooth surfaces. In the area of the thrust washers, which in each case transmit the pressure indirectly on the bones, the phenomenon of osteolysis of the bones will occur, further destabilizing the entire joint. Moreover, there is no possibility of stable tightening of the nuts on the threaded ends, because the spindles, due to their smooth surfaces and lack of connection with the thrust plate when tightened with a torque Ms ¦ 15 Nm, they simultaneously rotate when they are perpendicularly inserted, or damage the bone tissue when inserted at an angle. Because the screw-pin set does not allow for the elastic deformation of the self-locking plate, required for correct pressure on the bone fragments, it is necessary to bend the plate in advance. The initial bending of the plate destroys its passive layer and initiates micro-cracks, which may spread as different loads are transmitted, leading to a complete cracking of the plate. U-shaped slots and cause the sludge to slide off. The contact of the seat surface with the surface of the spindle nut is linear and occurs only on parts of it. This causes an ambiguous fixation of the screw-pin assembly in a position perpendicular to the face of the plate and it can change both along the longitudinal axis. and transverse. Therefore, there is another factor that destabilizes the anastomosis. Such a large number of factors influencing the destabilization of the bone joint connection, with the use of a known device, does not allow to obtain the pair of forces assumed by the solution with the same direction of action, but with the opposite direction. When the connection of the device is too tight and the bone fragments slide together, the phenomenon of osteolysis occurs within the supporting plates and pins and the fracture gap is impaired. However, if the connection of the device and the clamping of the fragments is too weak, the fracture gap will not heal. Similar to the known holes, the plate described in the German patent specification No. 2 340 880 has holes. These holes are oblong in the longitudinal axis of the plate, or show, when retained longitudinally, V-shape. They have an oblique line from outside to inside into the bones, sliding surfaces and support surfaces for the head of the fastening screw. The sliding and supporting surfaces pass at the inner end on the side of the bone into the abutment plane, approximately horizontal, matching the lower side of the screw head. The support surface serves during assembly as a support for the shank of the bolt and during subsequent assembly as the sliding surface of the bolt head. The known hole is not a symmetrical hole, it shows a significant deviation from the symmetry axis. Accordingly, it has the same disadvantages as the bore known from Polish patent specification No. 124 448, that is, the screw head contacts the plate bore linearly, not the entire surface. Due to this, the screw is not unequivocally fixed in a position perpendicular to the face of the plate, but it can assume different positions, both in the longitudinal axis and in the transverse axis. The consequence of this is the lack of stability of the joint and improper pressure of the fragments. The plate, according to the invention described, adheres to the surface of the bones with its entire surface, causing the phenomenon of osteolysis with a long range. 145 631 3 a self-locking plate for the correct pressure of bone fragments, - obtaining a stable connection of the set with the bones, - lifting the set above the bone * The essence of the invention consists in the fact that the screw with a head in the form of a nut at the point where the cortex thread passes into the normal thread with a bead larger than the clearance of the oval slot in the plate * The pressure plate has sockets in the upper part in the form of a taper cone around the entire circumference * The set according to the invention allows to avoid the phenomenon of oatolysis of the bone tissue * thanks to the possibility of lifting the plate, based on the screw thickening * above the bone even lifting it above the skin * Required * for proper bone fixation, the plate elasticity is obtained thanks to the construction of screws and holes in the self-locking plate according to the invention * Screws with cortical thread give a strong connection of the fixing element with the bone * The grooves located at the junction of the cortical and normal threads are located above bones so that they do not touch it * Nuts * tightening the plate to the screw thickening * has a conical surface around the entire circumference, thanks to which it adheres to the socket also around the entire perimeter * causing the plate to bend resiliently * The cooperation of the uninterrupted conical surface of the socket guarantees self-locking of the nut * securing it against unscrewing during variable stresses * This gives a very strong connection * resistant to loosening during the load of forces acting in different planes * The set takes the form of a clamp stabilizer * Moreover, this design allows for a controlled the pressure of the bone fragments, that is, the selection of such the position of the screws in relation to the plate, but always perpendicular to it and to the bone, that each time it is possible to determine the size of the fracture pressure required for the proper course of treatment. None of the so-far known designs of bone-fixing devices guarantee the achievement of such effects * The subject of the invention is shown in an example in the drawing *, in which Fig. 1 shows the bone-tightening device mounted at the fracture site in longitudinal section, Fig. 2 - set as in fig * 1 in cross-section, fig * 3 - - pressure plate in longitudinal section, fig * 4 - pressure plate in top view, fig * 5 - screw in side view, fig * 6 - screw in view from top, fig * 7 - a single socket with an oval hole, fig * 6 - operations of making a socket with a drill, fig * 9 - - insertion of the milling cutter in the taper hole axis, fig * 10 - milling of an oval hole by displacing the milling cutter along the axis long plate, fig * 11 - initial phase of attaching the plate to the screw, fig * 12 - condition after tightening the nut, fig * 13 - distribution of forces in the anastomosis * The set for pressure-fixation of bones consists of a self-seaming plate 1, screw 2 1 shaped nuts 3 * The pressure plate 1 shown in Figs 3 and 4 has a slot 4 and a groove 5 * The seat 4 has an oval hole 6 in the lower part and a cone-shaped upper part 7 and a slide 6 * Netomiest shown in Fig. 5 16 the screw 2 has a cortex thread 9 at one end and a normal thread 10 at the other end. At the transition between cortical thread 9 and normal thread 10, the screw 2 has a square-shaped bead 11 * Twisted on a normal thread 10 shaped nut 3 has a conical shape 12 in the lower part, and cross notches 13 in the upper part. * The execution of the seat 4 with an oval hole 6 is shown in Figs. 6-10. kowa 7 * In the socket 4 prepared in this way, the cutter 16 is inserted in the axis of the hole * Then, the oval hole 6 is milled by shifting the cutter 16 by a segment x of a length that allows the continuity of the conical surface 7 of the broken bone of Fr. the formation, the screws 2 are screwed in such a way that the grooves 11 do not touch the bones 14, and after placing and tightening the pressure plate 1, its ends are bent to the value f, within the limits of the elasticity of the self-pressure plate 1, as is shown in Fig. 13 * A self-pressure plate 1 is put on the screwed screws 2 until the entry of the thickeners 11 into the groove 5, which wedges the rotation of the screws 2, Then, using an appropriate screwdriver, screw 3 shaped nuts onto the normal thread 10. This operation is shown it is shown in Figures 11 and 12. During screwing on, the nut 3 contacts the lower tapered surfaces 12 with the slide 8, which causes the displacement of the screw 2 in the oval hole 6. When the nut 3, the entire tapers 12 contact the tapered surfaces 7 of the socket 4, the displacement of the screw 2 reaches the value x. At the same time there is a descent and compression of the bone fragments * Patent claims 1.
