Description

Analogi beta-karbonylowe zwiazków wytwarzanych sposobem wedlug wynalazku zostaly ujawnio¬ ne w polskim opisie patentowym nr 142 143.2 147 399 Sposób wedlug wynalazku dotyczy wytwarzania nowyoh pochodnyoh spiro-imidazolidyny o wzorze 1 i o wzorze 2 i ioh dopuazozalnyoh farmakologicznie soli, w którym to wzorze R1 oznacza atom wodoru, fluoru lub chloru albo grupe metylowa, a Y oznacza atom tlenu lub grupe CH2.vKp?zystna grupa zwiazków o wzorze 1 sa te, w których Y oznacza atom tlenu, a szczególnie korzystny Jest zwiazek, w którym R oznacza atom fluoru.Sposób wedlug wynalazku wytwarzania nowych pochodnych spiroimidazolidyny o wzorze 1, w którym R1 i Y maja wyzej podane znaczenie polega na reakcji zwiazku o wzorze 2 z wodor¬ kiem dwuizobutyloglinu, w rozpuszczalniku obojetnym wobeo srodowiska reakcji.Sposób wedlug wynalazku sprowadza sie do redukcji odpowiedniego zwiazku karbonylowego przy uzyciu wodorku glinu jako srodka redukujacego, korzystnie wodorku dwuizobutyloglinu.W praktyce, odpowiedni zwiazek karbonylowy poddaje sie reakoji z nadmiarem srodka reduku¬ jacego w rozpuszczalniku obojetnym wobec srodowiska reakcji, takim jak tetrahydrofuran, w temperaturze reakoji od -78 do -40°C. Many previous efforts have been made to develop more effective oral antidiabetic agents. Typically, these efforts have involved synthesizing new organic compounds, particularly sulfonylureas, and determining their ability to significantly lower blood sugar levels when administered orally. However, little is known about the effect of organic compounds in preventing and alleviating chronic complications of diabetes, such as diabetic cataracts, neuropathy, and retinopathy. U.S. Patent No. 4,463,391 U.S. Patent No. 3,821,383 discloses aldose reductase inhibitors of the type 1,3-dioxo-1H-benzyl-isoquinoline-2(3H)-acetic acid and derivatives thereof as being useful in the treatment of these diseases. U.S. Patent No. 4,117,230 relates to the use of certain hydantoins for the treatment of diabetic complications as aldose reductase inhibitors. Such aldose reductase inhibitors act by inhibiting the activity of the enzyme aldose reductase, which is primarily responsible for regulating the reduction of aldoses, such as glucose and galactose, to the corresponding polyols, such as sorbitol and galactitol, in humans and animals. This prevents or limits the accumulation of galactitol in the lenses of galactosemic patients and of sorbitol in the lenses, peripheral nerve fibers, and kidneys of various diabetic patients. Accordingly, such compounds have therapeutic value as aldose reductase inhibitors for combating certain chronic complications occurring in diabetes, including ocular complications, since it is known from the literature that the presence of polyols in the eye lens leads to the formation of cataracts with accompanying loss of lens transparency. Beta-carbonyl analogues of the compounds prepared by the method according to the invention are disclosed in Polish patent specification No. 142 143.2 147 399. The method according to the invention relates to the preparation of new spiro-imidazolidine derivatives of formula 1 and formula 2 and their pharmacologically acceptable salts, in which formula R 1 is a hydrogen, fluorine or chlorine atom or a methyl group, and Y is an oxygen atom or a carboxylic acid group. A preferred group of compounds of formula I are those in which Y is an oxygen atom, and a compound in which R is a fluorine atom is particularly preferred. The process according to the invention for preparing new spiroimidazolidine derivatives of formula I, in which R1 and Y have the meanings given above, consists in reacting a compound of formula II with diisobutylaluminum hydride in a solvent inert to the reaction medium. The process according to the invention comes down to the reduction of the corresponding carbonyl compound using aluminum hydride as a reducing agent, preferably diisobutylaluminum hydride. In practice, the corresponding carbonyl compound is reacted with an excess of the reducing agent in a solvent inert to the reaction medium, such as tetrahydrofuran, at a reaction temperature of from -78 to -40°C. After completion of the reaction, which requires about 30-60 minutes, the reaction mixture is added to 10% hydrochloric acid and the product, which consists of a mixture of epimeric alcohols, is filtered or extracted with a water-immiscible solvent, such as ethyl acetate. Purification is carried out by recrystallization or chromatography. Because of the acidic hydrogen atom in the spiro-heterocyclic ring of the compounds of formula I, salts of these compounds with pharmacologically acceptable cations can be prepared by known methods. Thus, these salts can be readily prepared by treating a compound of formula I with an aqueous solution of a pharmacologically suitable cation and evaporating the resulting solution to dryness, preferably under reduced pressure. Alternatively, a lower alkyl alcohol solution of the compound of formula I is mixed with the alkoxide of the required metal and the solution is then evaporated to dryness. Suitable pharmacologically acceptable cations for this purpose include, but are not limited to, alkaline earth metal cations such as potassium and sodium, ammonium or the cation of water-soluble amine addition salts, such as the lower alkanoyl ammonium cation and other cations derived from basic salts of organic amines which are pharmacologically acceptable, and alkaline earth metal cations such as calcium and magnesium. The term "pharmacologically acceptable salts" encompasses both acid addition salts and salts formed with the appropriate cations as described above. The novel compounds of formula I and their pharmacologically acceptable salts are useful as inhibitors of the enzyme aldose reductase in the treatment of chronic complications of diabetes, such as Diabetic cataracts, retinopathy, and neuropathy. The term "treatment" encompasses both the prevention and amelioration of such conditions. The compounds may be administered to patients in need of treatment by various conventional routes of administration, such as oral, parenteral, and topical. Typically, they will be administered orally or parenterally in doses ranging between about 0.05 and 25 mg/kg of body weight of the patient being treated per day, preferably about 0.1 to 10 mg/kg. However, some variation in dosage may be required depending on the condition of the patient being treated. In any case, the person responsible for the dosage will determine the appropriate dose for an individual patient. The novel compounds of the invention can be administered alone or in combination with pharmaceutically acceptable carriers, in single or multiple doses. Suitable pharmaceutical carriers include inert, solid diluents or fillers, sterile aqueous solutions, and solutions in various organic solvents. Pharmaceutical compositions prepared by combining the novel compounds of formula I and pharmaceutically acceptable carriers are then administered in various dosage forms, such as tablets, powders, lozenges, syrups, injectable solutions, and the like. These pharmaceutical compositions may, if desired, contain additional ingredients such as flavoring agents, binders, excipients, and the like. Thus, for oral administration, tablets containing various excipients such as sodium citrate, calcium carbonate, and calcium phosphate may be employed together with various disintegrating agents such as starch, alginic acid, and certain complex silicates, together with binding agents such as polyvinylpyrrolidone, dextrose, gelatin, and acacia. Additionally, emulsifying agents such as magnesium stearate, sodium lauryl sulfonate, and talc are often useful for tabletting purposes. Solid compositions of a similar type may also be employed as fillers in soft and hard-filled gelatin capsules. Preferred materials for these gels are lactose or milk sugar and high molecular weight polyethylene glycols. If aqueous suspensions or elixirs are required for oral administration, the principal active ingredient may be combined with various sweetening or flavoring agents. A coloring agent or dye and, if necessary, emulsifying or suspending agents, together with diluents such as water, ethanol, propylene glycol, glycerin and combinations thereof. The pharmaceutical compositions comprise a pharmaceutically acceptable carrier and a compound selected from the compounds of formula 1, wherein the weight ratio of the pharmaceutically acceptable carrier to the active compound is in the range of 1:4 to 20:1. A method of treating a diabetic subject for the prevention or alleviation of chronic ocular and neurotic complications associated with diabetes comprises administering to the diabetic subject a lesionally or prophylactically effective amount of the compound of formula 1. For parenteral administration, solutions of the new compound of formula 1 in oil may be used. sesame or peanut oil, aqueous polyethylene glycol, or sterile aqueous solution. Such aqueous solutions may be suitably buffered if necessary, and the liquid diluent first rendered isotonic by the addition of salt or glucose. These particular aqueous solutions are particularly suitable for intravenous, intramuscular, subcutaneous, and intraperitoneal administration. Consequently, sterile aqueous substances are readily available using known, standard techniques readily apparent to those skilled in the art. The compounds of formula I may be used not only advantageously in the preparation of aqueous pharmaceutical compositions for parenteral administration, as described above, but even more advantageously in the preparation of pharmaceutical compositions suitable for use as ophthalmic solutions. Such ophthalmic solutions are of value primarily in the treatment of diabetic cataracts by topical administration. Thus, for the treatment of diabetic cataracts, the compounds of the invention are administered to the eye of a patient in need of treatment in the form of ophthalmic preparations prepared in accordance with conventional pharmaceutical practice. See, for example, Remington's Pharmaceutical Sciences, 15th Edition, pages 1488 to 1501 (Maok Publishing Co., Easton, Pa.). The ophthalmic preparations will contain a compound of formula I or a pharmaceutically acceptable salt thereof in a concentration of about 0.01 to about 1% by weight, preferably about 0.05 to about 0.5% by weight in a pharmaceutically acceptable solution, suspension or ointment. Naturally, some variation in concentration will occur depending on the particular compound used, the condition of the patient being treated, and similar factors. The ophthalmic preparation will preferably be in the form of a sterile aqueous solution containing, as necessary, additional ingredients, for example, preservatives, buffers, tonicity agents, antioxidants and stabilizers, nonionic wetting or clarifying agents, viscosity-increasing agents, and the like. Suitable preservatives include benzalkonium chloride, benzethonium chloride, chlorobutanol, thimerazole, and the like. Suitable buffers include boric acid, sodium and potassium bicarbonate, sodium and potassium borate, sodium and potassium carbonate, sodium acetate, sodium hydrogen phosphate, and the like, in amounts sufficient to maintain a pH between about 6 and 18, preferably between about 7 and 7.5. Suitable tonicity agents are Dextran 40, dextran 70, dextrose, glycerin, potassium chloride, propylene glycol, sodium chloride, and the like, so that the sodium chloride equivalent in the ophthalmic solution is within the range of 0.9 plus or minus 0.31. Suitable antioxidants and stabilizers include sodium bisulfite, sodium pyrophosphate, sodium metabisulfite, sodium thiosulfite, thiourea, and the like. Suitable wetting and clarifying agents include polysorbate 80, polysorbate 20, poloxamer 282, and tyloxapol. Suitable viscosity-increasing agents include dextran 40, dextran 70, gelatin, glycerin, hydroxyethylcellulose, hydroxymethylpropylcellulose, lanolin, methylcellulose, petrolatum, polyethylene glycol, polyvinyl alcohol, polyvinylpyrrolidone, carboxymethylcellulose, and the like. The ophthalmic preparation is administered externally to the eye of the patient requiring treatment by conventional means, for example, in the form of eye drops or eye baths in an ophthalmic solution. The activity of the compounds of the invention as agents for the treatment of chronic diabetic complications can be determined by a number of standard biological and pharmacological tests. Suitable tests include (1) measuring their ability to inhibit the enzymatic activity of isolated aldose reductase, (2) measuring their ability to reduce or inhibit the accumulation of sorbitol in the sciatic nerve and in acutely streptozoonized lenses, i.e., (3) measuring their ability to reverse the already elevated sorbitol levels in the sciatic nerve and in the lenses of rats with chronic streptozotocin-induced diabetes, (4) measuring their ability to prevent or inhibit galactitol formation in the lenses of rats with acute galactosemia, (5) measuring their ability to delay cataract formation and reduce the severity of opacity in the lenses of rats with chronic galactosemia, (6) measuring their ability to prevent sorbitol accumulation and cataract formation in isolated lenses of rats incubated with glucose, and (7) measuring their ability to reduce the already elevated sorbitol levels in isolated lenses of rats incubated with glucose. The method of the invention is illustrated by the following: EXAMPLES* Proton nuclear resonance (NMR) spectra were measured at 60 MHz (unless otherwise noted) for solutions in perdeuterodimethyl sulfoxide (DMSOnlg) and peak positions are expressed in parts per million (ppm) downstream of the tetrame tylosilane field. The peak shapes were designated as s - singlet, d - doublet, t - triplet, q - quartet, m - jultiplet and b - broad. Example I. 2-Hydroxy-6-fluorospiro^chroman-4,4'-imidazolidinoJ-2',5'-dione For suspension 640 mg (0.002 mol) 6-fluorospiro?chromane-4,4'-irnidazolidinJ-2,2',5'-trione in 40 ml of toluene at -78°C was added 852 mg (0.006 mol) of diisobutylaluminum hydride in hexane and the mixture was stirred at -78°C for 30 min. The reaction mixture was allowed to warm to 0°C and stirred for 24 h. The reaction mixture was recooled to -40°C. 6 ml of dry tetrahydrofuran was added and the reaction mixture was warmed to 0°C. Additional diisobutylaluminum hydride (1.0 ml) was added in 0.2 ml portions. The reaction mixture was added to 10% hydrochloric acid and the mixture was extracted with ethyl acetate. The combined extracts were washed, dried and evaporated to dryness. The residue was chromatographed, giving 67 mg of compound, m.p. 247- 251° (decomposition). The NMR spectrum (DgDMSO) shows absorption at 11.05 (bs» 1H), 8.85 and 8.3 (s, 1H) 7.2 - - 7.08 (m, 1H), 6.93 - 6.80 (m, 2H) 5.79 - 5.71 and 5.36 - 5.27 (m, 1H) and 2.38 - 1.98 (m, 2H) due to the existence of a pair of diastereomers. Example II. Starting from the appropriate reagents, Spiro/chroman-4-4'-imidazolidine/-2,2',5'-trione was obtained with a melting point of 273° - 276°C, 6-chlorospiro/chroman- 1. A method for preparing new spiroimidazolidine derivatives of the formula 1, wherein R1 is a fluorine, chlorine or hydrogen atom or a methyl group and Y is an oxygen atom or a CHp group, characterized in that the compound of the formula 2, wherein R1 has the meaning given above, is reacted with diisobutylaluminum hydride in a solvent inert to the reaction environment. 2. A method according to claim 1, characterized in that the reactions are carried out at a temperature of -78°C to -40°C in tetrahydrofuran as an inert solvent. 3. A method according to claim 1, characterized in that the reactions are carried out at a temperature of -78°C to -40°C in tetrahydrofuran as an inert solvent.
