Description
Przedmiotem wynalazku jest sposób otrzymywania implantów hydroksyapatytowo-tytanowych, które łączą zalety bioaktywnej powłoki ceramicznej i biokompatybilnego metalu. The subject of the invention is a method of obtaining hydroxyapatite-titanium implants that combine the advantages of a bioactive ceramic coating and a biocompatible metal.
W przypadku porowatego tytanu o odpowiednio zaprojektowanej mikrostrukturze można obserwować wrastanie tkanki kostnej w pory materiału, dzięki czemu połączenie implant-kość staje się znacznie mocniejsze. Dodatkową poprawę osteointegracji można uzyskać przez modyfikację materiału fazami bioaktywnymi, umożliwiającymi wytworzenie z tkanką wiązania o charakterze chemicznym. Wytworzone powłoki ceramiczne o wysokiej biozgodności mają na celu poprawę osteointegracji z otaczającymi tkankami. In the case of porous titanium with an appropriately designed microstructure, bone tissue ingrowth into the pores of the material can be observed, thanks to which the implant-bone connection becomes much stronger. Additional improvement of osseointegration can be obtained by modifying the material with bioactive phases, which enable the formation of a chemical bond with the tissue. The highly biocompatible ceramic coatings produced are intended to improve osseointegration with the surrounding tissues.
The most common ceramic coating is synthetic hydroxyapatite (HA). Hydroxyapatite is a mineral that is an inorganic scaffold of the connective tissue responsible for the mechanical strength of bones and teeth. Implants with a synthetic HA coating have been used for many years. The advantages of this type of implants include rapid adaptation of bone tissue, stimulation of osteoblast proliferation and faster maturation of bone tissue in the vicinity of the implant. In the case of implants with a surface covered with hydroxyapatite, the interface between the bone tissue and the surface of the implant, compared to the surface of pure titanium, is characterized by a reduced amount of fibrous tissue, the growth of which is inhibited by hydroxyapatite. Bone tissue is mineralized directly on the hydroxyapatite coating, which shows that the coating is bioactive. HA-coated implants also have the property of maintaining the height of the alveolar ridge or reducing bone resorption. Sun et al. And Biesbrock et al also report bone growth in the vicinity of HA-coated implants. Due to these properties, implants with HA surface are characterized by better osseointegration, i.e. faster and stronger connection with bone tissue.
The beneficial effects of using implants with an HA surface shortly after implantation, however, do not translate into long-term success. The HA surface, with prolonged use, may be subject to resorption and degradation, which causes deterioration of the implant stabilization due to reduced contact between the implant and bone tissue. Due to the insufficient bond strength to the titanium surface, the shell fragments may peel off and cause the formation of local inflammatory foci, which in turn leads to bone tissue resorption. The high roughness of the HA coating makes it susceptible to bacterial colonization. This is especially important in cases of bone tissue resorption in the vicinity of the implant causing the HA coating to be exposed to the oral environment. This coating is then colonized by bacteria that can cause periimplantitis. In such situations, hygienic procedures are also hampered by the high roughness of the coating.
The quality of the HA coating has a very significant impact on both the early healing of implants and the effects of their long-term use. Both the growth of bone tissue in the vicinity of the shell and the resorption of the ceramics are important here. These parameters are influenced by both the chemical composition and physical properties of the coating, as well as its mechanical characteristics.
The HA coating on implants can be obtained in several ways, incl. by means of plasma spraying, ion sputtering, sol-gel process, electrophoresis, laser technologies such as PLD (Pulsem Laser Deposition) method, IBAD (Ion Beam Assisted Deposition) method or laser ablation. The most commonly used, standard method is the plasma spraying method. The coating produced by this method is up to 200 μm thick, it is an isotropic surface with a roughness of 8.2 - 10.2 μm. The disadvantage of this method is the need to use high temperatures, which affects the decomposition of the coating, the formation of chemical and structural heterogeneity and the formation of the undesirable amorphous phase of HA. These changes lead to the degradation of the coating, its delamination as a result of the reduction of the bond strength with titanium and reduce its biocompatibility. In addition, the thermal treatment also mechanically degrades the titanium itself.
Jedno z rozwiązań opisane jest w patencieCurrently, research is conducted on implants coated with HA nanolayers, obtained by electrodeposition. This type of coating is thin, and the size of the crystals forming it is in the nanometer dimension. It consists entirely of HA. Thanks to these features, the osseoconductive nature of the shell is preserved, while the risk of bone tissue loss around the implant due to the separation of shell fragments is eliminated. The research also aims to obtain coatings with high crystallinity, ensuring lower solubility of the coatings and improvement of fixation. To achieve the intended goals, various solutions are used that accompany the electrochemical processes of applying HA coatings to titanium implants. One of the solutions is described in the patent
PL 218 412 B1PL 218 412 B1
RU 2385740 (C1). Said patent relates to an electrochemical process using the method involving the stepwise pulse for applying a titanium anode lasting 200 ms current pulses 2 of the current density range of 0.2 to 0.25 A / mm 2. The difference between the method described in the said document and the present invention mainly concerns the electrochemical technique used, thus the process conditions and composition of the bath and the treatment preceding the electrochemical process. RU 2385740 (C1) describes an anode process (oxidation processes), while the patent application relates to cathodic reduction processes under constant current conditions, i.e. a constant current is applied to the titanium electrode throughout the process. The bath used in the process described in RU 2385740 (C1) consisted of H3PO4, Ca10 (PO4) 6 (OH) 2, CaCO3 and very clearly different from that proposed in the present invention (CaCl2, NaH2PO4, NaCl). Also the procedures for the preparation of titanium implants (including sandblasting and etching) differ significantly in terms of the materials used and their duration (in document RU 2385740 - sandblasting with Al2O3 and SiO2 and chemical treatment with HCl (30%) and H2SO4 (60%), and in the present invention sandblasting with corundum and etching with 0.5M H2SO4).
The processes of cathodic electrodeposition of HA on titanium plates are also described in the scientific literature, e.g. by Suping Huang et al. (J. Mater. Sei: Mater. Med. 2008, 19, 437-442). In this case, both the composition of the bath used during the HA electrodeposition and the type of treatment preceding this process (preparation of titanium implants) and the hydrothermal treatment ending the entire process determine the essential differences between the described process and the process being the subject of this application.
Istotą wynalazku jest sposób otrzymywania implantów hydroksyapatytowo-tytanowych, polegający na tym, że powierzchnię tytanowego implantu, będącego katodą w układzie dwuelektrodowym, piaskuje się korundem o średnicy ziarna 53-75 μm, dalej wytrawia w 0,5M H2SO4, a następnie nanosi się warstwę nanohydroksyapatytu w procesie elektrochemicznej depozycji przebiegającej w elektrolicieThe essence of the invention is a method of obtaining hydroxyapatite-titanium implants, which consists in sandblasting the surface of a titanium implant, which is a cathode in a two-electrode system, with corundum with a grain diameter of 53-75 μm, then etched in 0.5M H 2 SO 4 , and then applied the nanohydroxyapatite layer is formed in the process of electrochemical deposition in the electrolyte
2,08 x 10-4 M CaCl2, 1,25 x 10-4 M NaH2PO4 i 0,1 M NaCl, przy pH 6,3, w temperaturze nieprzekracza-5 2 jącej 110°C, i gęstości prądu powyżej 0,5 x 10-5 A/mm2, w czasie co najmniej 60 min.2.08 x 10 -4 M CaCl2, 1.25 x 10 -4 M NaH2PO4 and 0.1 M NaCl, at pH 6.3, at a temperature not exceeding 110 ° C, and a current density above 0.5 x 10 -5 A / mm 2 , during at least 60 min.

Do uzyskania nanowarstwy HA na powierzchni implanta tytanowego wykorzystano układ dwuelektrodowy. Gwintowany implant wykonany z tytanu o średnicy 4 mm i długości 7 mm stanowił katodę, natomiast platynowa siatka stanowiła anodę. Bezpośrednio przed operacją elektrochemicznego nanoszenia warstwy HA, powierzchnia implantu została wypiaskowana korundem o średnicy ziarna 53-75 μm (granulacja F220) i wytrawiona w 0,5M H2SO4. Elektrolit zawierał 2,08 x 10-4 M CaCl2,A two-electrode system was used to obtain the HA nanolayer on the surface of the titanium implant. A threaded implant made of titanium with a diameter of 4 mm and a length of 7 mm served as the cathode, while the platinum mesh was used as the anode. Immediately before the operation of electrochemical deposition of the HA layer, the implant surface was sandblasted with corundum with a grain diameter of 53-75 μm (granulation F220) and etched in 0.5M H 2 SO 4 . The electrolyte contained 2.08 x 10 -4 M CaCl 2 ,
1,25 x 10-4 M NaH2PO4 i 0,1 M NaCl, przy pH na poziomie wartości 6,3 przy użyciu 0,1M NaOH. Po2 wierzchnie implantu poddano kontaktowi z elektrolitem, który wynosił 101,24 mm2. W trakcie procesu elektroosadzania warstwy HA, trwającego 90 min. stosowano temperaturę 100°C oraz gęstość prą-5 2 dową wynoszącą 7 x 10-5 A/mm2.1.25 x 10 -4 M NaH2PO4 and 0.1 M NaCl, at pH 6.3 using 0.1M NaOH. Two of the tops of the implant were contacted with the electrolyte, which was 101.24 mm 2 . During the electroplating process of the HA layer, lasting 90 minutes. a temperature of 100 ° C was used and a current density of 7 x 10 -5 A / mm 2 was used .
Uzyskano powierzchnie składającą się z nanohydroksyapatytu bez innych związków fosforu.A surface consisting of nanohydroxyapatite without other phosphorus compounds was obtained.
