PL PL PL.111.1967 Denmark Published: 20.XII.1971 64196 KI. 30 d, 16 MKP A 61 f, 5/44 13(KD U- cdw Pate*t*»#0O ' Inventor: John Henning Schiott Nielsen Patent holder: Pharma — Piast A/S, Lille Vaerlose (Denmark) Colotomy device The invention relates to a device for use after colostomy, ileotomy or similar surgical procedures, comprising an annular connecting element attached to a flange adapted to be attached to the body around an artificial anal opening, and a detachable reservoir or container having an opening provided with a retaining ring attached to the container and adapted to receive said connecting element. Post-colotomy devices are known comprising Ring-shaped connecting elements made of metal or rubber, adapted to maintain their position on the body by means of a belt, and to which a container can be attached. Such a device is easy to clean because it can be completely removed at any time when its replacement is required. Attaching the container to the connecting element is, however, inconvenient, and, moreover, due to the movement of the body, it is very difficult to ensure a tight seal between the connecting element and the body. Post-colotomy devices are also known in which the connecting element is attached to the body around the artificial anus by adhesion, in which an external circumferential groove is used, while the inlet opening of the container is provided with a roller holder or rubber band, by means of which the edge of the inlet opening can be tightly connected to the groove of the connecting element. This causes Sufficient compression of the rubber makes it possible to achieve tightness in such devices, but on the other hand, this can cause difficulties in removing the container and also in keeping the convex rim of the connecting element clean. The aim of the invention is to design a device after colostomy that would enable the container to be replaced quickly and conveniently, so that absolute tightness of the container would be immediately and automatically achieved after its installation, and at the same time, convenient cleaning of the connecting element would be ensured. This is achieved according to the present invention in such a way that the annular connecting element, made of a hard, elastic and deformable material, comprises a radially shaped retaining flange that is tightly connected to the contact flange. an annular support element extending axially from the retaining collar and having an outer conical shape tapering from the collar; a locking collar on the outer edge of the retaining element, extending radially outward from the element. Furthermore, the device according to the invention comprises a retaining ring also made of a hard, resilient and deformable material, which is provided with a radial connecting collar by which the ring is sealed to the container around its opening, an axially shaped annular clamping collar extending inwardly of the container and having an inner surface corresponding in shape to the outer surface of the support element, so that when the container is seated on the connecting element, the support element is compressed by the clamping collar. The retaining ring can be quickly pushed through the locking collar into the connecting element due to the elasticity of the materials used. Once the retaining ring is pushed into place, its clamping collar tightly embraces the support element, and because the clamping collar has an internal diameter slightly smaller than the external diameter of the support element, the clamping collar automatically moves outward from the support element until its edge presses against the inside of the locking collar of the support element. In this way, a seal is established between the connecting element and the a completely tight connection, the retaining ring being at the same time securely fastened to the connecting element. On the other hand, when the container needs to be replaced, it is easy to remove the retaining ring from the connecting element, since this operation only requires exerting an outward pull at a point on the circumference of the retaining ring. Such pulling cannot be caused by the container itself, even if the user moves the device vigorously. To facilitate the fitting and removal of the retaining ring, the ring can be made of a flexible material that is more resilient than the material used for the connecting element, ensuring that the main parts of the retaining ring behave in this way during fitting and removal of the container. which protects the connecting element, usually used for a relatively long period of time, against any permanent deformations. The inner surface of the locking collar, located opposite the rebate collar, forms an angle not exceeding 90° with the axis of the connecting element, and this angle should preferably be slightly smaller, thanks to which the closing effect of the locking collar on the clamping collar of the retaining ring increases without causing any difficulties in connection with removing and installing the retaining ring. The subject of the invention is shown in an example embodiment in the drawing, in which Fig. 1 shows a vertical section of the connecting element, which is part of the device after colostomy, Fig. 2 - a vertical section of the container of the device, provided with a retaining ring, and Fig. 3 - part of the cross-section a retaining ring, shown in Fig. 2, mounted on the connecting element shown in Fig. 1. The upper part 10 of the device container, shown in Fig. 1, provided with an opening 12, contains a retaining ring 14 made of a resilient and deformable material, for example a medium-hard plastic - polyvinyl chloride. A flat, for example, or spherical, contact flange 16 is made of a thin and flexible material, for example a synthetic plastic. This flange has a central opening 18 and, on the other side, a contact cover by means of which it can be attached to the body skin in such a way that the opening 18 encompasses the artificially created anal opening. On the front side of the contact flange 16, there is arranged, enclosing the opening 18, a connecting element 20. This element 15 is provided at its end, on the opposite side of the flange 16, with a shaped retaining collar 22 on the outside, attached to the flange 16 by welding or a similar tight manner. The connecting element 20 comprises a conical, annular supporting element 24 projecting from the retaining collar 22. The external shape of this element should be conical in every case, while it is not absolutely required that the inside of this element has the same taper as the outside. Thus, the supporting element 24 may be thinner at its front edge than at its rear edge, and its center may be cylindrical. The supporting element 24 is provided with at the front edge with a radially shaped locking collar 26, preferably having a rounded front outer edge 28. The lateral side 30 of the locking collar 26 opposite the retaining collar 22 can either be shaped as a plane at a right angle to the axis of the connecting element 20 or, as shown in Fig. 1, more conveniently as a conical surface forming an acute angle with the axis of the connecting element 20 with its apex situated at the front of said element, in such a way that the interior 30, having a sharp annular transition 32 to the outer side of the connecting element 24, is undercut. The retaining ring 14 of the container 10 is then provided with a radially shaped connecting collar 34, which is tightly attached to the outside of the container 10 by welding or in some other way. A clamping collar 36 is formed axially from the connecting collar 34, 50, extending into the interior of the container 10. This collar in each case has an interior 38 conically shaped with the same taper as the external shape of the support element 24, 55 of the connecting element 20 and has slightly smaller cross-sectional diameters than the aforementioned internal shape of the connecting element 24, so that the clamping collar 36, when placed on the connecting element, clamps tightly thereon as shown in Fig. 3. The clamping collar 36 has a free end portion 40 which is tapered in the axial direction. The outer surface 42 of the part 40 is arranged on a tapered surface flared outwardly towards the connecting collar 34 and has a slightly rounded transition to the inside of the end portion. When the container 10 is fitted onto the annular connecting member 20, which is preferably made of a deformable material slightly harder than the material used for the retaining ring 14, for example polyvinyl or polypropylene, the lower part of the retaining ring 14 is first pushed into the inside of the locking collar 26 of the connecting member 20 until the end portion 40 is positioned above the collar. This pressing-in during the insertion of the container does not cause any difficulties, thanks to the fact that the axial length of the clamping collar 36, measured from the inside of the connecting collar 34 to the extreme edge of the end part 40, is slightly less than the axial distance between the mounting collar 22 of the connecting element 20 and the outer edge of the interior 30 of the locking collar 26. When a slowly increasing pressure is exerted along the circumference of the edge of the end part 40, exerted by the container 10, the final insertion of the retaining ring 14 completely outside the locking collar 26 does not cause any difficulties, thanks to the elasticity of the materials used. The retaining flange 14 is in contact with the outer side of the connecting flange 34 against the retaining flange 22 of the connecting element 20, but as a result of the compression, the compression flange 36 on the annular supporting element 24 and the compression flange 26 of the fastening ring 14 will move slightly outwards of the annular element 24 until the end part 40 presses against the locking flange 26. Due to the small contact area of said end part 40 and due to its tapered shape, a high surface pressure is created on the locking flange 26, ensuring a tight seal due to the clamping effect of the fastening flange around the connecting element 24. At the same time, a small annular space is created between the rounded end part 40 and the locking flange 26. The inner edge of the end portion 40 and the bottom of the sharp annular transition 32 are formed, thereby creating a kind of labyrinth seal. In this way, the tightness becomes effective even in the case of a relatively high overpressure being created in the container 10, and even in the case of the vent valve 44 in the container being closed with an odor filter, which closes the container so as to prevent any reduction of the overpressure. In this way, no malodorous air can pass between the members 14 and 20. The invention is not limited to the example described above. For example, the locking collar 26 of the annular connecting element 20 may have a slight outward inclination, or the clamping collar of the retaining ring 14 may be tapered along its entire inner side. The retaining ring 14 and the container 10 can be easily manually removed from the annular connecting element 20 by first pulling on the portion of the retaining ring protruding from behind the locking collar 26, then on the entire retaining ring. As shown in the drawing, the retaining ring protects the connecting element from contamination, which can furthermore be easily cleaned even when the attachment collar 16 remains attached to the patient.
