Abstract
Buluş, kalça eklemlerindeki femural ve asetabulum kemiklerinde ortaya çıkan deformasyonu tedavi etmek amacıyla kullanılan kalça protez sistemleri ile ilgili olup; - Akma ve uzama değerleri yükseltilmiş dolayısıyla mukavemeti arttırılmış Ti6Al4V ELI (Grade 23) malzemeden müteşekkil, özel kimyasal pürüzlendirme işlemi ve kumlama yapılarak protezin kemik içerisine çakılan bölgeye tutunumunun arttırıldığı, elektrolitik ortamda elektrotlar üzerinde oluşan reaksiyonlar sonucunda anodik oksidasyon prosesi gerçekleştirilerek biyouyumluluk özelliği arttırılmış proksimal femur gövde (5), - İstenilen açıda sabitlenebilmesi için açı ayar çentiğine (14) sahip olan, ürünlerin implante edilebilmesi için komşu kemik dokulardan destek almak amacıyla kemik doku içerisine yerleştirilen stem (destek çivisi) (10), - Kalça protezi sistemini kemikte yer alan asetebulum eklem kıkırdağına bağlayan bipolar cup (1) ve - Total kalça protez sistemini ilium kemiğine bağlayan asetabulum (12) içermesi ile karakterize edilmektedir.

çikan defoimasyonu tedavi etmek amaciyla kullanilan kalça protez sistemleri ile
Önceki Teknik
Kalça ekleminin fonksiyon göremedigi durumlarda kullanilan ve kalça
ekleminin yerini alan vücuda uyumlu metalden üretilmis platine kalça protezi ismi
verilmektedir. Kalça protezleri kalça kiriklarinda ve kanser gibi durumlarda kalça
ekleminin çikarilmasi gereken hallerde kullanilmaktadir.
Mevcut teknikte yer alan kalça protezleri farkli miktarlardaki
deformasyonlara uygulanamamaktadir. Farkli deformasyon miktarlarina çözüm
olamadigi için ameliyat sürelerini oldukça uzatmaktadir.
Mevcut teknikte yer alan kalça protezlerinde kullanilan el aletlerinin
tasarimlari yetersizdir. Tasarimlarin yetersiz olmasi da yine ameliyat sürelerini
oldukça uzatmaktadir.
Mevcut teknikte yer alan EP2512379 kodlu Avrupa patent dokümaninda
kismi kalça protezinden bahsedilmektedi r. Bahsedilen patent dokümaninda yer alan
protez, protetik bilesenin yüzey islemi ve artikülasyonun optimize geometrisinin
birlestirilmesi ile kismi kalça protezinde sürtünme ve asinmayi azaltmaktadir.
Asetabulum, kemige oyulmaktadir. Ya oymali asetabulumun ya da femoral bas
protezininki olan artiküle yüzeylerden biri küreseldir, böylelikle akiskan ile dolu
bir bosluk, önemli yük aktarim alaninda olusturulmaktadir. Akiskan ile
doldurulmus bosluk, içbükey ve disbükey bilesenlerin uyumlu oldugu dairesel bir
temas alani ile sizdimiaz hale getirilmektedir. Tercih edilen yüzey islemi, kemige
karsi çok düsük sürtünme katsayisina ve yüksek yipranma direncine neden olan,
elmas benzeri kaplama vasitasiyla olmaktadir. Protetik bas, delikli bir kabuk
tarafindan geleneksel bir kök ile veya femur üzerine vidalanan bir femoral boyun
protezi ile femura sabitlenmektedir, böylelikle kismen kemik tarafindan
kaplanmaktadir ve kalkar bölgesinin oyulmus korteksine dayandigi merkeze dogru
asagi yönde kismen maruz kalmaktadir. Söz konusu patent dokümaninda yer alan
protez farkli miktarlardaki deformasyonlarda uygulanamamaktadir. Ayrica
protezin tasarimindan dolayi montaj sirasinda kullanilan el aletleri oldukça fazladir.
Kullanilan el aletlerinin fazla olmasi, ameliyat sürelerini uzatmaktadir.
Bulusun Amaci
Bulusun amaci; farkli miktarlardaki deformasyonlarda uygulanabilen bir
kalça protezi elde etmektir.
Bulusun bir diger amaci; biyouyumluluk Özelligi arttirilmis bir kalça protezi
elde etmektir.
Bulusun bir diger amaci; protezin uygulanmasi sirasinda kullanilan el
aletlerinin minimize edilmesini saglayan
Bulusun bir diger amaci; Ti 4 malzemesine göre akma ve % uzama degerleri
1,5 kat daha fazla artirmaktir.
Bulusun bir diger amaci; akma degeri ve % uzama degeri dikkate alindiginda
küçük çaplardaki implantlarda daha fazla mukavemet saglamaktir.

çikan deformasyonu tedavi etmek amaciyla kullanilan kalça protez sistemleri ile
ilgilidir. Bulus konusu kalça protezi sistemi; bipolar cup (1), asinma plakasi (2),
segman (3), head (4), proksimal femur gövde (5), doku delikleri (6), Civatalar (7),
kalça protezi kapagi (8), kalça protezi konnektör bölgesi (9)I destek çivisi (stem)
(10), vidalar (ll), asetabulum (12), total sistem asinma plakasi (13) ve açi ayar
çentigi (14) içermektedir.
Kalça protezi proksimal femur gövde (5) malzemesi olarak Ti6AI4V ELI
(Grade 23) kullanilmistir. Mevcut teknikte yer alan kalça protezlerinde kullanilan
Ti 4 malzemesine göre akma ve % uzama degerleri 1,5 kat daha fazladir. Akma
degeri ve % uzama degeri dikkate alindiginda küçük çaplardaki implantlarla daha
fazla mukavemet saglanmaktadir.
Kalça protezi sistemleri 250 mm”lik deformasyonlara kadar
uygulanabilmektedir. Kalça protezi proksimal femur gövde (5) ve destek çivisi (10)
birbirine spacer parçasi (baglanti modülü) ile baglanmaktadir. Bu durumda kalça
protezinin yerlestirileçegi bölgede deformasyon arttikça spacer parçalari uç uça
puzzle parçasi gibi birbirine takilmakta ve protezin boyunu uzatabilmektedir.
Spaçer parçalari 250 mm”ye kadar çözüm sunmaktadir.
Kemik-implant tutunumu için kalça protezi gövde yüzeylerinde Ti6Al4V
E LI (Grade 23) malzemesine özel kimyasal pürüzlendirme islemi uygulanmaktadir.
Kimyasal pürüzlendiniie isleminden sonra yüzeyler kumlama yapilarak protezlerin
kemik içine çakilan bölgeye tutunumu artirilmaktadir.
Kalça protezi gövde yüzeylerinin, elektrolitik ortamda elektrotlar üzerinde
olusan reaksiyonlar sonucunda anodik oksidasyon prosesi gerçeklestirilerek
biyouyumluluk özelligi artirilmaktadir.
Bulus konusu kalça protezi sistemlerinde yer alan kalça protezi kapagi (8)
ve proksimal femur gövdedeki (5) tek tip konvektör sayesinde farkli çap ve
boylardaki destek çivileri (10) gövdeye baglanabilmektedir.
Bulus konusu kalça protezi gövdesi üzerinde doku delikleri (6)
bulunmaktadir. Doku delikleri (6), tendonlarin baglanmasi için kullanilmaktadir.
Ameliyat esnasinda kaslarin kemige baglandigi yerler kesildiginden, kaslarin
baglanacagi yer kalmadigindan, tendonlarin baglanabilmesi için protez üzerinde
doku delikleri (6) birakilmistir.
Ameliyatta hekime 45mm ile 250mm arasinda 10”ar mm araliklarla kesi
imkâni sunulmaktadir. Ameliyatta hekimlere verilen 45 mm ile 250 mm arasindaki
kesi imkâni, kalça protezinin 250 mm°ye kadar olan deformasyonlarda
uygulanabilmesi ile ilgilidir. Deformasyonlu bölgenin ölçüsü kadar ameliyat
sirasinda kesi atilmaktadir. Spacer parçalari 250 mm°ye kadar çözüm oldugu için
hekim 45 mm ile 250 mm arasindaki bölgeye cerrahi operasyon
uygulayabilmektedir.
Kalça protezi kapagi (8) içerisinde bulunan açi ayar çentigi (14] sayesinde
kemik içine yerlestirilen stemler (10) istenilen açiya getirilmektedir.
Cerrahi operasyonda kullanilmak üzere uygulama seti içerisinde kalça
protezine ait özel el aletleri bulunmaktadir. Mevcut teknikte kullanilan kalça
protezlerinde 158 adet el aleti kullanilirken, bulus konusu kalça protezi sisteminde
bu adet 58”dir. Ba'hsedilen adetin düsürülmesi ile ameliyat sürelerini %40 oraninda
kisaltilmistir. Kalça protezinin çok fazla genis ölçüde bulunan deformasyonlara
çözüin olmasi tek cerrahi operasyonla ve tek set ile protezin çözüm olmasini
saglamaktadir. Tek setin el aletlerinin yeterli olmasi el aleti sayilarini aza
indirgemektedir.
Bulus konusu kalça protezi, uzuvlarda bulunan kemik dokularinin tamir
edilemeyecek ölçüde çesitli nedenlerle kaybi sonucunda dokunun rekonstrüksiyonu
için kullanilan Diafizer ve Artrodez Endoprotez sistemleri ile uyumlu
çalismaktadir. Kalça protezleri ile Diafizer ve Artrodez Endoprotez Sistemlerin
birbiri ile uyumlu çalismasini, ayni tasarima sahip stemler (10] ve konnektör
bölgeleri (9) saglar. Stemler (10); ürünlerin implante edilebilmesi için komsu kemik
dokulardan destek almak amaciyla kemik doku içerisine yerlestirilen parçalardir.
Kalça protezi konnektör bölgesi (9) ise kemik doku kaybinin ürün boyutlarindan
fazla oldugu durumlarda birden fazla protez parçasinin birlestirilmesi için
kullanilmaktadir. Konnektörün bulundugu bölgeye genel olarak konnektör bölgesi
(9] ismi verilmistir.
Tasarimi ve üretimi gerçeklestirilen kalça protez sistem basamaklari bipolar
ve total sistem basamaklari olarak 2,ye ayrilmaktadir. Femur hat ve femur
kemiginde deformasyon varsa bipolar sistemler, kalça kemiginde deformasyon var
ise total sistemler kullanilmaktadir.
Kalça protezinde bipolar sistem asagidaki islem basamaklari seklinde
çalismaktadir;
- Hasarli kemik kesilir ve uzaklastirilir,
- Femur kemigi içerisinde kalça protezi sistemlerine ait olan destek çivisi
çakilir ve sabitlenir.
Proksimal femur gövde (5) ideal açi ve pozisyonda sabitlenir.
Proksimal femur gövde (5) kalça protezi konnektör bölgesinde (9) yer
alan konnektör parçasina kalça protezi kapagi (8) takilir ve Civatalar (7)
ile sikilir.
Proksimal femur gövde (5) içerisine head (4) çakilir.
Bipolar cup (1) parçasina asinma plakasi (2) çakilir ve segmanla (3)
sabitlenir. Hazirlanan sistem head”in (4) üzerine oturtulur.
Son olarak sistem hekim tarafindan asetabulum (12) bölgesine oturtulur.
Eger kalçada olusan deformasyonda sadece femur sorunu varsa sekil-1°de
yer gösterilen kalça protezi sistemi kullanilmaktadir. Kalça protezi sistemi, kemikte

basamaklari seklinde çalismaktadir;
Hasarli kemik kesilir ve uzaklastirilir.
Femur kemigi içerisinde kalça protezi sistemlerine ait olan destek çivisi
(10) çakilir ve sabitlenir.
Proksimal femur gövde (5) ideal açi ve pozisyonda sabitlenir.
Proksimal femur gövde (5 ), kalça protezi konnektör bölgesinde (9) yer
alan konnektör parçasina kalça protezi kapagi (8) takilir ve eivatalar (7)
ile sikilir.
Proksimal femur gövde (5) içerisine head (4) çakilir.
Total kalça protezlerinde bulunan asetabulum (12) parçasi head°e (4)
çakilir.
Son olarak kalça kemigi boslugu kazilir ve asetabulum (12) parçasi
hekim tarafindan oturtulur.
Eger kalçada olusan deformasyon, kemikte yer alan asetebulum eklem
kikirdaginda da görülüyorsa, yani asetebulum eklem kikirdaginin da degistirilmesi
gerekiyorsa sekil-4”te bulunan total kalça protezi sistemleri kullanilmaktadir.
B ahsedilen total kalça protez Sistemi, kemikte yer alan asetebulum eklem kikirdagi
deforme oldugu için total kalça protezi ilium kemigine asetabulum (12) ile
baglanmaktadir.
Bulus konusu kalça protezinin bipolar cup (1) ve head (4) parçasi krom
kobalttan, segman (3) ve asinma plakasi (2) UHMWPE (Ultra High Molecular
Volume Polyethylene] malzemeden mütesekkildir. Kullanilan malzeme sayesinde
sistemin ömrü oldukça uzamaktadir. Total sistemlerde kullanilan asetabulum (12)
parçasi da krom kobalttan üretilmektedir.
Sekil-l yer alan kalça protezi sistemlerinde bulunan bipolar cup'larda (1)
liner ile fiksasyonun saglanabilmesi için segman (3] kullanilmaktadir. Sekil-1`de
bulunan head (4) parçasi ile bipolar cup (1), asinma plakasi (2) ile birbirine
baglanmaktadir. Bahsedilen asinma plakasi asinmaya engel olmaktadir.
Kalça protezi sistemlerinde çocuklarda kullanilan pediatrik proksimal femur
gövdesi (5) ve yetiskinlerde kullanilan normal proksimal femur gövdesi (5] Olmak
üzere 2 farkli femur gövde çesidi kullanilabilmektedir. Pediatrik ürün grubunun
ölçüsü @27 mm, yetiskinlerde kullanilan gövdenin ölçüsü ise @30 mm°dir. Kalça
Bahsedilen araliklar arasinda seçilen ölçü kisinin yetiskinlik, pediatrik, yas ve
cinsiyete bagli olarak degisebilmektedir.

- ISTEMLER . Bulus, kalça eklemlerindeki femural ve asetahulum kemiklerinde ortaya çikan deformasyonu tedavi etmek amaciyla kullanilan kalça protez Sistemleri ile ilgili olup özelligi; Akma ve uzaina degerleri yükseltilmis dolayisiyla mukavemeti arttirilmis Ti6A14V ELl (Grade 23) malzemeden mütesekkil, özel kimyasal pürüzlendirme islemi ve kumlama yapilarak protezin kemik içerisine çakilan bölgeye tutunumunun arttirildigi, elektrolitik ortamda elektrotlar üzerinde olusan reaksiyonlar sonucunda anodik oksidasyon prosesi gerçeklestirilerek biyouyumluluk özelligi arttirilmis proksimal femur gövde (5), Istenilen açida sabitlenebilmesi için açi ayar çentigine (14) sahip olan, ürünlerin implante edilebilmesi için komsu kemik dokulardan destek almak amaciyla kemik doku içerisine yerlestirilen Stem (destek çivisi) (10), Kalça protezi sistemini kemikte yer alan asetebulum eklem kikirdagina baglayan bipolar cup (Il ve Total kalça protez sistemini ilium kemigine baglayan asetabulum (12) içermesidir, Istem 1”de bahsedilen kalça protez sistemi olup özelligi; proksimal femur gövde (5) ve destek çivisi (10) birbirine baglayan spacer parçasi (baglanti modülü) içermesidir. .
