Abstract

Der Marknagel (10) hat einen Hohlraum (12) in dem ein Innenteil (20) längsverschieblich und verdrehungssicher angeordnet ist. In der den Hohlraum (12) bildenden Wand (11) des Marknagels (10) ist auf deren gegenüberliegenden Seiten im Bereich des Eintreibendes (14) ein quer durchgehendes Langloch (16) vorgesehen. In dem Innenteil (20) ist wenigstens ein fluchtend zu dem Langloch (16) ausgerichtetes Fixierungsloch (22) ausgebildet. Wenn die Verbindungsbolzen in die im Abstand angeordneten Fixierungslöcher (15, 16 und 22) eingeschlagen sind und sich eine Osteotomie zwischen den Verbindungsbolzen (15, 22) befindet, können die Ränder der Osteotomie durch Längsverschieben des Innenteils voneinander weg bewegt werden, wobei sich der sich langsam erweiternde Zwischenraum zwischen den Rändern der Osteotomie mit Knochengewebe füllt.

Description

Marknagel
Die Erfindung betrifft einen Marknagel mit einer einen Hohlraum bildenden Wand, mit einem sich verjüngenden Ende, mit einem Eintreibende und mit im Abstand über seiner LängβerStreckung angeordneten, durch seine Wand quer hindurchgehenden Fixierungslöchern.
Bekanntlich verwendet man die Marknagelung dazu, im Bereich von langen Röhrenknochen nach Knochenbrüchen die beiden Frakturenden im Sinne einer inneren Schienung gegeneinander ruhigzustellen. Dabei wird nach Aufbohrung des Markraums der Marknagel meist vom proximalen Ende des Röhrenknochens eingeschlagen und nach endgültiger Po¬ sitionierung mittels mehrerer Querbolzen, die durch die Knochenkortikalis und Fixierungslöcher im Marknagel hindurchgehen gegen axiale Verschiebung und Verdrehung verriegelt. Da die den Knochen außen umgebende Knochen¬ haut bei dieser Behandlungsart nicht wesentlich beschädigt wird, ist die für den Heilungsprozeß wichtige Ernährung begünstigt und eine Infektvorbeugung gegeben.
Im fortgeschrittenen Stadium der Knochenbruchheilung kann man die Verriegelungsbolzen auf einer Seite des Frakturspaltes entfernen. Eine axiale Belastung führt nun zu einem Gleiten des entriegelten Knochenanteils und somit zu einer axialen Kompression am Frakturspalt, wodurch sich der neugebildete Knochen verfestigt.
Verlängerungen von Röhrenknochen oder Defektüberbrückungen nach Trümmerbrüchen, einer Knochenentzündung oder nach Entfernen von Tumoren im Bereich langer Röhrenknochen werden bisher meist mit Hilfe von äußeren FixierungsSys¬ temen durchgeführt. Die Knochenenden werden dabei mit Knochenschraube oder Drähten gefaßt, die durch die Haut, nach außen mit einem Rahmen verbunden sind. Durch Verschie¬ bung der den jeweiligen Knochenenden zugeordneten Rahmen¬ teile mit Hilfe von Zugschrauben oder Drahtwinden, wird eine Relativbewegung zwischen den Knochenenden zwangs¬ weise erreicht, wobei die geringen Verschiebungsdistanzen fortlaufend durch sich neubildendes Knochengewebe über¬ brückt werden. Sowohl zur Verlängerung als auch für eine Segmentverschiebung ist eine Osteotomie erforderlich, wobei bei der Segmentverschiebung das abgetrennte Knochen¬ segment in die Defektzone gezogen wird.
Die bekannten äußeren Fixierungssysteme haben einen ungünstigen Kraf ansatzpunkt, so daß Verkantungen auf¬ treten, erfordern aufwendige, den Patienten behindernde Aufbauten, bilden eine ständige Infektionsgefahr durch Eindringen von Keimen über die Knochenschrauben oder die Drähte und ergeben unschöne Narben.
Die der Erfindung zugrundeliegende Aufgabe besteht nun darin, einen Marknagel derart zu modifizieren, daß er unter Aufrechterhaltung seiner Funktion zur Stabilisierung als innere Schiene gleichzeitig eine axiale Verschiebung von getrennten Knochenteilen voneinander weg ermöglicht.
Diese Aufgabe wird ausgehend von dem Marknagel der ein¬ gangs genannten Art durch ein in dem Hohlraum verdrehungs- sicher angeordneten und durch einen Antrieb längsver- schiebliches Innenteil und durch ein im Bereich Eintreib¬ endes vorgesehenes, quer durchgehendes Langloch entweder in den gegenüberliegenden Seiten der den Hohlraum bildenden Wand oder in dem Innenteil und durch wenigstens ein fluchtend zu dem Langloch ausgerichtetes Fixierungsloch entweder in dem Innenteil oder in den gegenüberliegenden Seiten der den Hohlraum bildenden Wand gelöst.
Der Antrieb für die Längsverschiebung des Innenteils in den Hohlraum kann ein mechanischer, pneumatischer oder hydraulischer oder auch ein elektrischer, elektromagneti¬ scher oder piezoelektrischer Antrieb sein, der innerhalb und/oder außerhalb des Hohlraums installiert ist. Bei¬ spielsweise kann bei einem geschlossenen Hohlraum das Innenteil die Funktion eines im Hohlraum längsverschieb¬ baren Kolbens übernehmen, der in vorgegebener Weise mit Druck beaufschlagt wird, der die geforderte Verschiebung bewirkt. Es kann auch eine LängsVerschiebung mit Hilfe einer Gasdruckfeder durchgeführt werden.
Wenn das Innenteil mit einer sich von seiner einen Stirn¬ seite aus erstreckenden Bohrung mit Innengewinde versehen ist und am Eintreibende eine Stange mit einem Außenge¬ winde drehbar gelagert ist, die mit dem Innengewinde in der Bohrung des Innenteils in Gewindeeingriff steht, kann die Drehung der Stange durch einen Drehantrieb in Form einer biegsamen Welle erfolgen, die nach einer genügend langen Strecke im Weichteilgewebe durch die Haut nach außen geführt wird, und die an ihrem proxi alen Ende für das Drehen mit einem Sechskant versehen ist. Als Drehantrieb kann auch ein geeigneter Elektrogetriebemotor eingesetzt werden, der an dem proximalen Ende des Marknagel angeflanscht ist oder sogar in dem Lumen des Marknagels untergebracht werden kann. Die Energieversorgung kann mit Hilfe von Batterien als Einheit mit dem Elektromotor oder separat durch Einpflanzung eines Batteriegehäuses in das subkutane Fettgewebe erfolgen, wobei zum Anschluß eine flexible Litze verwendet wird. Die Aktivierung des Elek¬ tromotors kann durch Auflage eines Magneten mit Hilfe eines Reed-Kontaktes oder im kontinuierlichen Betrieb erfolgen.
Nach Aufbohren des Markraums und Anlegen einer Osteotomie an einer geeigneten Stelle mit einer Innensäge wird der erfindungsgemäß ausgebildete Marknagel einschlagen, und zwar, wenn sich das Langloch in den gegenüberliegen¬ den Abschnitten der Wand des Marknagels befindet, nur so weit, daß der um die zu verlängernde Knochenstrecke aus der Einschlagstelle vorsteht. Nach Setzen der Ver¬ riegelungsbolzen beiderseits der Osteotomie wird dann durch Längsverschiebung des Innenteils die Distraktion der Knochenenden an der Osteotomiestelle durchgeführt.

Um dies zu erreichen, ist eine Erweiterung der den Hohl¬ raum bildenden Wand zu dem Eintreibende hin vorgesehen, wobei im Bereich der Erweiterung Lamellen bildende, U-för- mige Einschnitte ausgeführt sind, deren Bodenverbindungs- abschnitt dem Eintriebsende zugewandt ist. Durch ein in dem Hohlraum axial verschiebbar angeordnetes, an den Lamellen angreifendes Keilelement lassen sich die Lamellen nach außen in einen Formschluß mit dem proximalen Röhren¬ knochenabschnitt ausspreizen, wodurch dieser gegen axiale Verschiebungen gesichert wird.
Der Hohlraum des Marknagels kann in bekannter Weise ganz oder teilweise offen sein, bevorzugt im verjüngungs- seitigen Bereich, er kann aber auch völlig geschlossen sein und einen zylindrischen, polygonalen oder anders geformten Querschnitt, beispielsweise einen kleeblatt¬ förmigen Querschnitt aufweisen.
Anhand von Zeichnungen werden Ausführungsbeispiele der Erfindung näher erläutert. Es zeigen:

Der in den Figuren 1 bis 6 gezeigte Marknagel 10 hat eine einen zylindrischen Hohlraum 12 bildende Wand 11, deren eines sich erweiternde Eintreibende 14 mit einem Einsatz 17 versehen ist, und deren anderes Ende 13 sich zu einer Nagelspitze verjüngt. Der Einsatz 17 hat eine zentrale Bohrung 18. In der Wand 11 des Marknagels 10 sind im Bereich seines sich verjüngenden Endes 13 im Abstand angeordnete Fixierungslöcher 15 vorgesehen, die sich quer durch ihn hindurch erstrecken. In dem zylindrischen Hohlraum 12 des Marknagels 10 ist ein zylindrisches Innenteil 20 längsverschiebbar, ver¬ drehungsgesichert und koaxial zur Achse 40 des Markna¬ gels 10 angeordnet. Von der dem sich erweiternden Ein¬ treibende 14 zugewandten Stirnseite des Innenteils 20 erstreckt sich koaxial in das Innenteil 20 eine Bohrung 21 mit einem Innengewinde, in die das Außengewinde 33 einer Stange 30 eingeschraubt ist, die sich durch, die Bohrung 18 im Einsatz 17 erstreckt und in einem Kopf 31 mit einem Imbusloch 32 endet. Durch Drehen des Kopfes 31 verschiebt sich das Innenteil 20 axial in dem Hohl¬ raum 12 des Marknagels 10.
Bei der in den Figuren 1 und 2 gezeigten Ausführungs¬ form ist im Abstand von dem Ende der Bohrung 21 mit dem Innengewinde ein sich quer durch das Innenteil 20 hindurch erstreckendes Fixierungsloch 22 ausgebildet. Fluchtend zu diesem Fixierungsloch 22 ist in der Wand 12 des Marknagels auf den gegenüberliegenden Seiten jeweils ein Langloch 16 ausgebildet. Der Marknagel 10 in der Ausführungsform der Figuren 1 und 2 wird so weit eingeschlagen, daß er proximal um die zu verlängernde Knochenstrecke aus der Einschlagstelle vorsteht. Nach Setzen der nicht gezeigten Verriegelungsbolzen durch die Fixierungslöcher 15, die sich auf der einen Seite der Osteotomie befinden, und durch die gegenüberliegen¬ den Langlöcher 16 in der Wand 12 des Marknagels 10 und durch das Fixierungsloch 22 auf der anderen Seite der Osteotomie werden durch Drehen des Kopfes 31 der Stange 30 die Ränder der Osteotomie allmählich voneinander entfernt, wobei der zur Knochenverlängerung führende, sich erweiternde Spalt sich mit neugebildetem Knochen¬ gewebe füllt. Bei der in den Figuren 3 und 4 gezeigten Ausführungs¬ form ist die Wand 12 des Marknagels 10 an einer Trenn¬ fuge 19 durchtrennt, so daß die dadurch gebildeten bei¬ den Teile des Marknagels 10 axial zueinander geführt auf dem Innenteil 20 verschiebbar sind. Bei dieser Aus¬ führungsform ist das Langloch 16 in dem Innenteil 20 ausgebildet, während in der Wand 12 des eintreibseitigen Abschnitts des Marknagels 10 im Abstand in der Längser¬ streckung jeweils zwei fluchtende quer verlaufende Fixie¬ rungslöcher 22 auf den gegenüberliegenden Seiten fluchtend zum Langloch 16 im Innenteil 20 ausgebildet sind. Bei dieser Ausführungsform wird der Marknagel 10 vollständig in den Röhrenknochen ohne proximalen Oberstand eingeschlagen. Anschließend werden die Verriegelungsbolzen gesetzt, wobei sich die Osteotomie zwischen dem Abschnitt mit dem sich verjüngenden Ende 13 und dem Abschnitt mit dem sich erweiternden Eintreibende 14 des Marknagels 10 befindet.
Um bei einer Segmentverschiebung mit einer Ausführungsform wie in den Fig. 1 und 2 dargestellt das proximale Knochen¬ segment separat gegen axiale Verschiebung zu sichern, sind in dem sich erweiternden Bereich des Marknagels 10 U-förmige Einschnitte 52 ausgebildet, deren Schenkel parallel zur Achse 40 verlaufen und deren Bodenverbin¬ dungsabschnitt 53 dem Eintreibende 14 zugewandt ist. Durch die ü-förmigen Einschnitte 52 werden Lamellen 51 gebildet. Auf der Stange 10 ist ein hülsenförmiges Keilelement 50 mit Außenkeilen geführt, das durch Drehen einer im Einsatz 17 am Eintreibende 14 in Gewindeeingriff gehaltenen Schraube 54 axial verschoben wird und durch seine Außenkeile die Lamellen 51 in die in Figur 6 gezeigt Verkeilungslage nach außen spreizt, wodurch ein Form¬ schluß mit dem proximalen Röhrenknochen und somit dessen Lagesicherung erreicht wird.

Claims

Patentansprüche
1. Marknagel (10) mit einer einen Hohlraum (12) bildenden Wand (11), mit einem sich verjüngenden Ende (13), mit einem Eintreibende (14) und mit im Abstand über seiner Längserstreckung angeordneten, durch seine Wand (11) quer hindurchgegehenden Fixierungslöchern (15, 16, 22), g e k e n n z e i c h n e t durch einen in den: Hσhlrauπr (12) verdrehungssicher angeordneten und durch einen Antrieb längsverschieblichen Innenteil (20), durch ein im Bereich des Eintreibendes (14) vorgesehenes, quer durchgehendes Langloch (16) entweder in den gegenüberliegenden Seiten der den Hohlraum (12) bildenden Wand (11) oder in dem Innenteil (20) und durch wenigstens ein fluchtend zu dem Langloch (16) ausgerichtetes Fixierungsloch (22) entweder in dem Innenteil (20) oder in den gegenüberliegenden Seiten der den Hohlraum (12) bildenden Wand (11).
2. Marknagel nach Anspruch 1, d a d u r c h g e k e n n ¬ z e i c h n e t , daß das Innenteil (20) durch einen mechanischen, pneumatischen, hydraulischen, elektrischen, elektromagnetischen oder piezoelektrischen Antrieb längs¬ verschiebbar ist.

Marknagel nach Anspruch 2, d a d u r c h g e ¬ k e n n z e i c h n e t, daß das Innenteil (20) mit einer sich von seiner einen Stirnseite aus erstrecken¬ den Bohrung (21) mit Innengewinde versehen ist, und daß am Eintreibende (14) eine Stange (30 mit einem Außen¬ gewinde (33) drehbar gelagert ist, das mit dem Innengewinde in der Bohrung (18) des Innenteils (20) in Gewindeeingriff steht.
4. Marknagel nach Anspruch 2 oder 3, d a d u r c h g e k e n n z e i c h n e t , daß der Antrieb inner¬ halb des Hohlraums (12) angeordnet ist.
5. Marknagel nach einem der Ansprüche 1, bis 4, g e - k e n n z e i c h n e t durch eine Erweiterung der den Hohlraum (12) bildenden Wand (11) zu dem Eintreibende (14) hin, durch im Bereich der Erweiterung in der den Hohlraum (12) bildenden Wand (11) ausgeführte, Lamellen (51) bildende, U-förm±ge Eiπschnitte= (52), deren Bodenverbin¬ dungseinschnitt (53) dem Eintreibende (14) zugewandt ist, und durch ein in dem Hohlraum (12) axial verschiebbar angeordnetes, an den Lamellen (51) angreifendes Keilelement (50).
