また, 請求の範囲 1 2項記載の発明においては, フィルタの入り口, 出口に圧 力を検出するセンサを取付け, 圧力をモニタすることによりパージ液循環回路の 閉塞, フィルタの目詰まりなどを検出できる。
また, 請求の範囲 1 3項記載の発明においては, パージ管に温度を検出するセ ンサを設けパージ液の温度を計測することにより, 次の役割を果たすことが出来 る。
1 . パージ液の循環量を変化させパージ液の温度を血液のたんぱく成分の熱変 成温度に達しないよう管理することにより, 人工心臓の可搬駆動システムの安全 性を高めることができる。
2 . 生体と人工心臓の可搬駆動システムとの間の熱量の移動が少なくなるよう にパージ液流量を制御することにより, 生体への負荷の少ない人工心臓の可搬駆 動システムが構築できる。
また, 請求の範囲 1 4〜請求の範囲 1 9項記載の発明においては, 患者の運動 負荷を検出する機構を設け, 負荷の大きさに応じて血液ポンプの回転数を制御す ることにより, 患者の生活の質を向上できる。 運動負荷は搬送部の車輪に設けられた回転検出器から搬送部の移動速度を計算 することにより容易に検出できる。 さらに, 生体の心拍動数に同期した血液ポン プの負荷変動周期から推定される心拍動数に応じて血液ポンプの回転数を変化さ せることもできる。 これらをそれそれ単独に用いても良いが, 両方用いれば運動 負荷による心拍動数の変化と, 精神的な緊張, 興奮等に起因する心拍動数の変化 を分離でき, さらに詳細な血液ポンプの制御が可能となる。 これにより患者の生 活の質的向上が得られる。
また, 請求の範囲 2 0〜請求の範囲 2 1項記載の発明において, 人工心臓の各 構成要素にセンサを付加し, 表示手段を用いることにより作動状態を常に表示, 監視ができ, またセンサから得られた値と設定値とを比較し, 設定値から外れた 場合, 警報を発すると共に前記表示手段に表示される。 さらに前記通信手段を用 いて外部の管理センタ一へ情報が転送され, さらに外部の管理センタ一より適切 な指示が折り返し転送されることにより, 患者の病院外での生活の安全性が確保 される。
また, 請求の範囲 2 2項記載の発明において, 接続部の体内埋設部分と体外部 分は接続ターミナルにより接続されており, 接続ターミナルはベルト状の腹帯に 固定されることから, 体動による外力が接続部の体内埋設部分へ直接伝わらない。 さらに, 接続部は接続ターミナル部で分割できるため, 搬送部の交換, メンテナ ンスが容易にできる。
また, 請求の範囲 2 3〜請求の範囲 2 5項記載の発明において, 接続部は伸縮 自在であり患者の活動を妨げない。 さらに, パージ管およびモ一タケ一ブルを補 強された保護管で覆うことにより, 破損が防止できる。 また, 保護管の伸び量が パージ管およびモ一タケ一ブルの伸び量より小さいことから, パージ管およびモ —タケ一ブルの引っ張り破損を防止できる。
また, 請求の範囲 2 6〜請求の範囲 2 7項記載の発明において, 前記パージ液 循環手段のトラブル等によりパ一ジ液の循環が停止した場合には, パージ管内に 設けられた弁が自動的に閉鎖され, 前記血液ポンプの血液シール背面のパージ液 の流出を防止できるるため, 血液シールの軸封作用を安全に維持することが可能 となる。 さらに, 人工心臓の可搬駆動システムのうち, 体外露出部分が破損の危 険性が大きいことを考慮して, 前記弁機構は体内埋設部に設置することにより、 より安全性が高まる。
また, 請求の範囲 2 8項記載の発明において, 人工心臓の可搬駆動システムの うち, 破損の危険性が低い接続夕一ミナルから体内埋設部に前記ポンプ制御手段 および電力供給手段の 1系統を体内埋設部に設置することが好ましい。
また, 請求の範囲 2 9項記載の発明において, 表面に多数の小孔を有する炭素 繊維からなる保持部を接続部の体内埋設部の表皮貫通部分に設けたことにより, 前記保持部が表皮と強固に癒着し経皮感染を防止できる。
また, 請求の範囲 3 0〜請求の範囲 3 2項記載の発明において, 体外に配設さ れる各手段は, 搬送部上に搭載され人体と共に搬送が可能となり, かつ, 防水防 染構造を有し, 搬送に伴う振動, 衝撃を吸収する機構を有することにより, 搬送 部の安全性を高めることができる。
また, 請求の範囲 3 3項記載の発明において, 搬送部の電力供給手段は商用交 流電源, 自動車等の車載電源, または搬送部に内蔵される 2次電池等で駆動する ことが可能であり, 患者の活動範囲を制限しない。 図面の簡単な説明

第 1図は本発明の一実施例における人工心臓の可搬駆動システムの構成図であ る。 図において人工心臓の可搬駆動システムは, 体内に埋設される血液ポンプ 1 と, 血液ポンプ 1を体外から制御するシステム駆動部 4 5 , および血液ポンプ 1 とシステム駆動部 4 5を連結する接続部 7 0から構成される。
さらに, 血液ポンプ 1の回転軸 1 3の軸受け 1 9への血液の浸入を防止するた め, 前記血液ポンプ 1に内存する血液シール 1 8の接触部へパージ液を循環供給 するパージ液循環手段 2 0 , モータケーブル 7 1を介し前記血液ポンプ 1の駆動 を電気的に制御するポンプ制御手段 3 0, 前記各手段の動作状態およびデ一夕を 表示する表示手段 4 0 , 外部と情報交換を行う通信手段 5 0 , 前記各手段へ電力 を供給する電力供給手段 3 3, および前記各手段を制御する制御手段 1 0 0とか らなるシステム駆動部 4 5 , 前記システム駆動部 4 5を搭載し可搬可能とする搬 送部 6 0とから構成される。
また, 接続部 7 0は血液ポンプ 1とシステム駆動部 4 5のパージ液循環手段 2 0を連結するパージ管 2 4 , 2 5および血液ポンプ 1とポンプ制御手段 3 0を連 結するモ一夕ケーブル 7 1からなり, 接続部 7 0の体内埋設部と体外露出部を接 続する接続ターミナル 8 0, パージ管 2 4, 2 5およびモー夕ケーブル 7 1の体 内埋設部と患者の表皮 Fを強固に癒着させる保持部 9 0から構成される。 また, 接続ターミナル 8 0には, 予備のポンプ制御手段 3 0および電力供給手段 3 3を 有する。
本人工心臓の場合, 心臓ポンプの停止は患者の生命へ重篤な影響を及ぼす。 一 方, 通信手段 5 0や運動負荷検出機構 3 1などの高度な機能は一時的に停止して も患者の生命を直接脅かすことはない。 また血液ポンプ 1の回転軸 1 3の血液シ —ル 1 8に関しても, 血液シール 1 8背面のパージ液が消失しなければ,パージ 液の循環が途絶えても数時間程度は問題ない。 従って, 本システムにおいて 2重 3重の安全性が必要なのは血液ポンプ 1の回転に要する根幹機能であるポンプ制 御手段 3 0と電力供給手段 3 3であり, この根幹機能を重複して装備することに より人工心臓の可搬駆動システムの安全性を高めている。 しかしながら, 接続部 7 0の切断やシステム駆動部 4 5そのものが大破した場 合は, システム駆動部 4 5に設けた 2重 3重の安全対策は何の役にも立たない。 この点を考慮し, 予備のポンプ制御手段 3 0と電力供給手段 3 3を前記のような 事故発生の危険性の少ない患者の体表近傍に設置している。
第 2図に血液ポンプ 1の一例を, 第 3図に人工心臓の可搬駆動システムの人体 への装着例を示す。
血液ポンプ 1は円筒形のモータ部 1 1を有するポンプ基部 1 0と, 前記ポンプ 基部 1 0に接続されるポンプ部 1 2から構成され, ポンプ部 1 2にはモータ部 1 1より延伸した回転軸 1 3を介して駆動されるポンプ翼 1 4とポンプ翼 1 4を覆 う様にポンプ基部 1 0に接続されるケ一シング 1 5から構成され, ケ一シング 1 5の端点に設けられた流入口 1 6から左心室 A内の血液がケ一シング 1 5内へ流 入し, ケ一シング 1 5内のポンプ翼 1 4によりエネルギーを付加された後, ケ一 シング 1 5の側面に設けられた流出口 1 7および人工血管 Cを経て大動脈 Bへ吐 出される。
ポンプ基部 1 0とポンプ翼 1 4の間には端面接触式の血液シール 1 8 (以下メ 力二カルシ一ル) があり, 血液成分の浸入による回転軸 1 3の軸受け 1 9の凝着 を防止している。 また、 ポンプ基部 1 0にはパージ液の入口 8および出口 9が形 成されており、 これらパージ液の入口 8および出口 9はパージ管 2 4 , 2 5を介 して後述するパージ液の循環装置に接続される。 パージ液循環手段 2 0によりメ 力二カルシ一ル 1 8内部にパージ液を循環させ, メカニカルシール摺動面の潤滑, 冷却, および拡散によってパージ液中へ浸入する微量の血液成分を除去すること により, メカニカルシール 1 8および軸受け 1 9の摺動面は常にクリーンに維持 される。
本例では血液ポンプ 1に遠心ポンプを用いた例を示したが, 軸流ポンプまたは 斜流ポンプでも実施可能である。 さらに, 血液シール 1 8はメカニカルシ一ルに 限定されるものではなく, オイルシール等他の接触式シールでも実施可能である。 また、 前記血液ポンプ 1に内存するメカニカルシール 1 8へパージ液を供給す るパージ液循環手段 2 0, モ一タケ一ブル 7 1を介し前記血液ポンプ 1の駆動を 電気的に制御するポンプ制御手段 3 0 , 前記各手段の動作状態およびデータを表 示する表示手段 4 0 , 外部と情報交換を行う通信手段 5 0, 前記各手段へ電力を 供給する電力供給手段 3 3, および前記各手段を制御する制御手段 1 0 0とから なるシステム駆動部 4 5はコンパクトな筐体 6 1内に構築され, 車輪 6 2および ハンドル 6 3を有する手動式の搬送部 6 0に搭載される。
また, 本例においては搬送部 6 0は手押し車の形態をなすが, その他に車椅子 状のもの, バッグ状などもあり, 患者の容体および生活環境に適した形態のもの が用いられる。 さらに, 車輪 6 2にモ一夕を付加した電動移動型の搬送部 6 0も 用いられる。
第 4図にポンプ制御手段 3 0の構成図の一例を示す。
制御手段 1 0 0は, 演算器 1 0 1 , メモリ 1 0 2, 各種インタフェイス 1 0 3 を有するワンポ一ドマイコン等からなる。 A/D変換器 1 0 4を介して演算器 1 0 1へ入力された前記各手段に設けられた各センサの出力信号はメモリ 1 0 2の R O Mに記憶された制御プログラムに従い処理がおこなわれ、 計算結果は D /A 変換器 1 0 5を介して出力される。 また、 インタフェイス 1 0 3を介して各構成 手段の演算器 1 0 1とデータの交換もおこなわれる。 前記各デ一タはメモリ 1 0 2の R AMに保存され、 前記表示手段 4 0により表示される。
通常血液ポンプ 1の制御は制御手段 1 0 0によりおこなわれる。 制御手段 1 0 0は, ポンプ制御のうち口一夕 1 0 6の回転位置に対応した位置信号の計算をお こなう。 血液ポンプ 1のモータ部 1 1に内存するホールセンサ 1 0 8の出力から 口一夕 1 0 6の回転を検出し, 血液ポンプ 1の回転数が一定になる様に制御手段 1 0 0の演算器 1 0 1で計算し, 接続ターミナル 8 0部に内存する電力供給器 1 0 7で位置信号に応じた電力をモータの固定子卷線 1 0 9に供給する。
本実施例ではホールセンサ 1 0 8を用いた回転数制御をおこなっているが, セ ンサレス駆動も実施可能であり, 制御方法も回転数一定制御に限定するものでは ない。
また, 予備のポンプ制御手段 3 0が接続ターミナル 8 0内にあり, 前記制御手 段 1 0 0と予備のポンプ制御手段 3 0はインタフェイス 1 0 3を介し, それぞれ の演算器 1 0 1を相互に監視している。 例えば, 制御手段 1 0 0の演算器 1 0 1 に異常が発生した時には予備のポンプ制御手段 3 0へ血液ポンプ 1の制御が切換 えられ, インタフェイス 1 0 3を介して制御手段 1 0 0の演算器 1 0 1のリセッ トがおこなわれる。 制御手段 1 0 0の演算器 1 0 1のリセット後正常になった時 は, 心臓ポンプの制御は制御手段 1 0 0へ移管し, 予備のポンプ制御手段 3 0は 制御手段 1 0 0の演算器 1 0 1の監視のみをおこなう。 逆に予備のポンプ制御手 段 3 0の演算器 1 0 1に異常を検出した場合は, 予備のポンプ制御手段 3 0をリ セヅトする。
本実施例ではポンプ制御手段 3 0および電力供給手段 3 3を 2系統有している が、 これに限定するものではなく、 さらに重複化することも可能である。
また、 運動負荷検出機構 3 1は, 搬送部 6 0の車輪 6 2に回転検出器 3 2を設 けてなり、 制御手段 1 0 0は回転検出器 3 2の信号から人工心臓の可搬駆動シス テムの移動速度を計算し, 移動速度に応じて血液ポンプ 1のモ一夕の回転数を増 減させる。 患者の運動量を容易に検出できるため, 患者の生活の質の向上が得ら れる。
また, 生体の拍動に応じて変化する血液ポンプ 1のモータの負荷変動から心拍 数を計算し, 心拍数の変化に応じて血液ポンプ 1のモ一夕の回転数を増減する様 制御することも可能である。
第 5図は生体の心拍動数の推測方法を説明するものであり、 第 2図に示した遠 心ポンプを回転数一定で制御した場合の特性を示している。
生体の左心室 Aより脱血し大動脈 Bへ送血する本血液ポンプ 1は, 生体の心拍 動による左心室 A内圧の変化に伴いモ一夕部 1 1の負荷は大きく変動する。 例え ば, 血液ポンプ 1の流入圧力に相当する左心室 A内圧と、 流出圧に相当する大動 脈 B圧の差が心拍動に伴い P 1から P 2の間で変動すると, 血液ポンプ 1の流量 は Q 1から Q 2の間で変動する。 血液ポンプ 1の流量が Q 1から Q 2の間で変動 する時, 血液ポンプ 1の電流値も I 1から I 2の間で変動するため, 電流値の変 動の周期をモニタすれば生体の心拍動数が推測できる。 図は示さないが血液ポン プ 1を電流値一定で制御した場合も同様の特性を示し, 血液ポンプ 1の回転数の 変動から生体の心拍動数が推測できる。
これらにより, 患者の運動負荷量に応じて血液ポンプ 1の動作点を自動的に増 減させることにより, 患者の生活の質が改善可能となる。 なお, 前記制御方法は それぞれ単独で用いても, 同時に用いても良い。
また、 前記検出手段はいずれも極めてシンプルな構造にて実現できるため, 誤 作動, 故障も少なくでき, 患者の生活の質を向上させることが可能となる。 第 6図に電力供給手段 3 3の構成図の一例を示す。
電力供給手段 3 3は, 使用する電源の切換えおよび内蔵するバッテリ 3 5の充 放電を制御する電力供給手段制御部 3 4、 ニッケル水素 2次電池またはリチウム イオン 2次電池からなる複数のバッテリ 3 5、 充電器 3 6などを有し、 システム 駆動部 4 5および接続ターミナル 8 0内に設けられいる。 商用交流電源または車 載用電源などの外部電源 3 7が利用できる場合は、 可搬駆動システムは外部電源 3 7で駆動し、 同時に内蔵する複数のバッテリ 3 5の充電を充電器 3 6を用いて おこなう。 外部電源 3 7が利用できない場合は、 システム駆動部 4 5に複数有す るバッテリ 3 5を順次使用する。 接続ターミナル 8 0内にあるバッテリ 3 5はシ ステム駆動部 4 5の破損時などシステム駆動部 4 5内の電力供給手段 3 3が利用 できない場合に用いられるものであり、 通常動作時はシステム駆動部 4 5の電力 供給手段 3 3を用いる。 また、 システム駆動部 4 5のバッテリ 3 5の残留値が規 定値を下回った場合は, システム駆動部 4 5を構成する各手段への電力供給を停 止し, 接続ターミナル 8 0内の予備のポンプ制御手段 3 0のみへ電力を供給する ことにより, 可搬駆動システムの駆動時間の延長をはかる。
内蔵されるバッテリ 3 5にはそれそれバッテリ制御回路 3 8が設けてなり、 ノ、' ッテリ 3 5の残量と充放電回数, 電圧値などをモニタしている。 電力供給手段制 御部 3 4はバッテリ制御回路 3 8より前記データを受信し、 バッテリ毎に順次切 り替えて充放電を制御している。
また、 バッテリ毎に設けられた温度を検出するセンサ 3 9の出力を制御手段 1 0 0により監視し、 バッテリ 3 5の異常発熱による発火、 爆発事故を防止する。 制御手段 1 0 0と電力供給手段 3 3の演算器 1 0 1とはィンタフヱイス 1 0 3 を介して相互の構成要素の動作情報の交換および動作状態を監視する。 例えば, 電力供給手段 3 3の演算器 1 0 1から使用する電源の種類, バッテリ 3 5の残量 と充放電回数, 電圧値, バッテリ 3 5の温度などの情報を受信し, 制御手段 1 0 0の演算器 1 0 1で動作内容を判断し, 前記表示手段 4 0に情報を表示する。 ま た, 電力供給手段制御部 3 4の演算器 1 0 1に異常が発生した場合は、 インタフ ェイス 1 0 3を介して電力供給手段制御部 3 4の演算器 1 0 1のリセットがおこ なわれる。 制御手段 1 0 0の構成は前記と共通でありここでは説明は省略する。 第 7図はパージ液循環手段 2 0の構成図の一例である。
パージ液循環手段 2 0は, 血液に対する血液ポンプ 1の回転軸 1 3の軸封を維 持するため前記メカニカルシールの接触面へパージ液を循環供給するものであり, 消毒剤が添加されたパージ液は, パージ液を蓄えるリザ一バ 2 1からパージ液を 循環させるパージ液循環ポンプ 2 2により, パージ液をろ過するフィルタ 2 3を 通過し, 一方はパージ管 2 4を通り体内に埋設された血液ポンプ 1の内部へ流入 し, メカニカルシールの接触面へ達した後, 再びパージ管 2 5を通りリザ一パ 2 1へ戻る。 さらにフィルタ 2 3から流出するもう一方のパージ液はバイパス流路 2 6を通りリザ一バ 2 1へ戻る。
また、 パージ管 2 4 , 2 5の途中には弁機構 7 5を有する。 弁機構 7 5はパ一 ジ液循環が停止した瞬間にパージ管 2 4 , 2 5を閉鎖するものであり、 血液ポン プ 1内にパージ液を保留することにより血液シール 1 8の機能を維持する。
前記血液ポンプ 1のメカニカルシールの接触面の潤滑薄膜を介して拡散により パージ液中へ浸入した血液のたんぱく成分は, パージ液が循環する流路の途中に 設けられたフィルタ 2 3によりろ過され, パージ液は常にクリーンに維持され血 液ポンプ 1の回転軸 1 3の軸受け 1 9の凝着を防止できる。 さらに, フィルタ 2 3に限外ろ過膜を用いることにより, 細菌, ウィルス, 細菌毒素も除去可能とな り, パージ液の無菌化, 無毒化を達成する。
また, パージ液の交換作業, 搬送部 6 0の破損などによりパージ液中への細菌, ゥィルスの浸入の恐れもありパ一ジ液そのもが殺菌作用を有する方が好ましいこ とから, パージ液の消毒手段として, 次亜塩素酸などを用いた塩素消毒法を用い る。 前記次亜塩素酸は 1 p p mの低濃度でほとんどあらゆる細菌, ウィルスを殺 菌することが可能であり, 生体内では有機物と結合して有機塩素化合物となり無 毒化する。 さらに消毒剤をパージ液中に添加することにより, リザ一バ 2 1およ びフィル夕 2 3も消毒する。
これらにより人工心臓の血液ポンプ 1の内部へ流入するパージ液は物理的, ィ匕 学的に消毒されることになり, 二重の安全機構を有することになる。
細菌, ウィルス, 細菌毒素を除去し, さらに軸受の凝着を引き起こすフイブリ ノーゲンを除去するために, 限外ろ過膜を用いたフィルタ 2 3のポアサイズは分 子量 3 4万以下に設定する。
また, フィルタ 2 3の目詰まりを予防するためバイパス流路 2 6が設けられた クロスフ口一循環路とすることにより, クロスフ口一循環路は次の 3つの役割を 兼ねることが可能となる。
一つはホロファイバ型の限外ろ過フィル夕 2 3を用いる場合に, ファイバ内の エアは自動的にバイパス流路 2 6を通して抜気されるため, エアトラップによる フィル夕 2 3の機能低下を起こさない。
人工心臓の血液ポンプ 1と体外のシステム駆動部 4 5を接続する接続部 7 0が 外力 (例えば踏まれるとかドアに挟まれる) により閉塞された場合, クロスフ口 —循環路を持つパージ液循環手段 2 0では, このような緊急時にパージ液のバイ パス流路 2 6が安全弁として働き, パージ圧の上昇を一定値以下に保ちパージ循 環手段の破損を防止することができる。
クロスフ口一循環路を持つパージ回路では, 体外にあるパージ循環手段を流れ るパージ流量がバイパス流路 2 6を通るパージ流量分だけ大きくなる。 パージ液 の冷却はパージ液と外気の温度較差に起因する熱放散によって行われるのでパ一 ジ循環流量が大きくなるほどパージ液の放熱効果が高まる。
さらに, パージ液はメカニカルシールの接触面からの微量の漏れにより減少す るため, リザ一バ 2 1を一定期間毎に交換する必要がある。 また, フィルタ 2 3 もたんぱく質が付着すると細菌の増殖の原因となる恐れがあるため一定期間毎に 交換する必要がある。 リザ一バ 2 1 , パージ液循環ポンプ 2 2, 限外ろ過膜を用 いたフィル夕 2 3をュニットイ匕し, 定期的な交換作業を極めて簡便かつ清潔に行 うことが可能である。
パージ液循環ポンプ 2 2は小型で効率が高く, シール不要で消毒できる信頼性 の高い容積式ポンプが適する。 図においてはローラポンプを用いているが, 口一 ラポンプはパージ液の流れるパイプをパイプ外部よりしごくことにより内部のパ —ジ液を送り出す構造となっており, パージ液に接する部分がパイプの内側だけ のため好ましい。 図には示さないが他には小型のダイアフラム型ポンプ, ギヤポ ンプなども用いられる。
さらに, フィル夕 2 3の入口および出口に圧力を検出するセンサ 2 7 a , 2 7 bを取付け, センサ 2 7 a, 2 7 bの出力を制御手段 1 0 0でモニタすることに よりパージ液循環回路の閉塞, フィルタ 2 3の目詰まりなどを監視する。
また、 血液ポンプ 1からパージ液循環手段 2 0へ環流するパージ液の温度を検 出するセンサ 2 8 aをパージ管 2 5に取り付け, パージ液の温度が血液のたんぱ く成分の熱変成温度に達しないようパージ液循環ポンプ 2 2の運転条件を制御す る。 本実施例では制御部 1 0 0の演算器 1 0 1で運転条件を計算し、 パージ液循 環手段制御部 1 1 0の演算器 1 0 1が制御手段 1 0 0の指示内容に従いパージ液 循環ポンプ 2 2を制御する。
さらに、 パージ液循環手段 2 0から血液ポンプ 1へ循環するパージ液の温度を 検出するセンサ 2 8 bをパージ管 2 4に取り付け、 血液ポンプ 1へ流入、 流出す るパージ液温度を比較することにより生体と人工心臓の可搬駆動システムとの間 の熱の移動が把握できる。 この熱の移動を少なくするようパージ液循環ポンプ 2 2の運転条件を制御することにより、 生体への負荷の少ない人工心臓の可搬駆動 システムが構築できる。
圧力センサ、 温度センサの他に, 流量センサ, 血液センサ等を付加することに よって, パ一ジ液循環手段 2 0の動作状態を更に詳しく把握することも可能であ る。
さらに, パージ液の入ったリザ一バ 2 1は柔軟な袋状の形態をなし, パージ液 の消費に伴う容積変化, 搬送部 6 0の移動に伴う姿勢変動, 変動などに対応でき る。
さらに、 リザ一パ 2 1にはパージ液の容積を検出するセンサ 2 9が設けてあり、 センサ 2 9の出力を制御手段 1 0 0でモニタすることによりパ一ジ液の漏れを監 視する。
また, 制御手段 1 0 0はパージ液循環手段制御部 1 1 0の演算器 1 0 1をイン タフェイス 1 0 3を介して監視をおこない, パージ液循環手段制御部 1 1 0の演 算器 1 0 1にエラ一が発生した場合, パージ液循環手段制御部 1 1 0の演算器 1 0 1のリセットをおこなう。
表示手段 4 0は情報を表示する表示部 4 1、 表示情報の切り替えおよびデータ 入力のための入力手段 (図示せず) を有する。 表示部 4 1は, 制御手段 1 0 0で 処理された人工心臓の各構成手段の作動情報を常に表示する。 また, 前記作動情 報が設定値から外れた場合, 警報を発すると共に前記表示部 4 1に対応策などの 詳細な情報が表示される。
第 8図 (a ) 〜第 8図 (c ) は表示部 4 1の表示例を示したものである。
第 8図 (a ) は通常の表示状態である。 最上段は本人工心臓の可搬駆動システ ムの使用日数を示す。 左列は上から外気温度、 心拍数、 接続ターミナル 8 0の動 作状態を表す。 右列は上から血液ポンプ 1、 パージ液循環手段 2 0、 バッテリ 3 5の状態を示す。 右下は充放電、 外部電源 3 7の使用を示すものである。 通常は 前記の様な基本的な動作情報が表示されており、 各構成手段のより詳細な動作情 報も入力手段により表示できる。
第 8図 (b ) はパージ液循環手段 2 0の動作状態の詳細を表したものである。 上段はパージ液循環手段 2 0を構成するの血液ポンプ 1 , リザーバ 2 1 , パージ 液循環ポンプ 2 2, フィルタ 2 3とパージ管 2 4, 2 5の動作状態を表す。 下段 は左から外気温度、 パージ液制御手段 1 0 0の演算器 1 0 1の動作状態、 パージ 液温度を表す。
異常発生時には第 8図 (c ) に示すようなメッセージが表示され、 異常内容の 詳細は入力手段により切り替えることにより表示できる。 (図示せず)
その他には、 運転パラメ一夕の変更等も入力手段により変更できる。 通信手段 5 0は電話回線に接続するための接続端子が設けられ、 病院等のコン ピュ一夕と接続可能となっており、 この病院のコンピュータによって、 人工心臓 の可搬駆動システムの制御、 監視が可能となる。 携帯電話などの移動通信手段 5 0を組み込むことにより、 人工心臓の可搬駆動システムの情報は適宜病院等へ転 送され、 適切な指示が折り返し転送されるため, 患者の病院外での生活の安全性 はさらに高まる。
第 9図に接続ターミナル 8 0の一例を示す。
接続ターミナル 8 0は前記接続部 7 0の体内埋設部と体外露出部を接続するも のである。 接続ターミナル 8 0はベルト状の腹帯 8 1に固定され, 接続部 7 0の 体外露出部は伸縮自在に形成されているため, 体動による外力は接続部 7 0の体 内埋設部分へ直接伝わらない。
また、 接続ターミナル 8 0は表示部 4 1を有し, 血液ポンプ 1のモータの回転 数, バッテリ残量などの情報が操作ポ夕ン 4 2で切り替えることにより順次表示 される。 また, エラー発生時はエラ一表示器 4 3の点灯および警報により告知さ れる。
また, 接続ターミナル 8 0の内部には予備のポンプ制御手段 3 0と電力供給手 段 3 3が内蔵されており, システム駆動部 4 5が何らかのトラブルで停止した際 は、 接続ターミナル 8 0内部の予備のポンプ制御手段 3 0と電力供給手段 3 3に より血液ポンプ 1は駆動される。
また, 図示してはいないが,接続部 7 0の体外露出部は接続ターミナル 8 0で 分割可能に接続さており、 また接続夕一ミナル 8 0にはパージ管 2 4 , 2 5およ びモ一タケ一ブル 7 1の予備端子が内蔵されており, システム駆動部 4 5のメン テナンス時も容易にアクセスできる。
第 1 0図に保持部 9 0の一例を示す。 保持部 9 0は表面に多数の小孔を有する 複数の炭素繊維からなる構造物であり, 円筒部 9 1とつば部 9 2からなる。 つば 部 9 2は腹直筋 Dと腹直筋鞘 Eの間に設置され, 円筒部 9 1の一端は表皮 Fに達 する。 保持部 9 0は接触する組織と強固に癒着し体動による接続部 7 0の剥離を 防止し, 接続部 7 0を介した感染症を防止する。 第 1 1図に接続部 7 0の一例を示す。 接続部 7 0はパージ管 2 4, 2 5, およ びモ一タケ一ブル 7 1と, それらを覆う保護管 7 2から構成される。 パージ管 2 4 , 2 5 , およびモ一タケ一ブル 7 1は螺旋状, 保護管 7 2は蛇腹状に形成され てなり, それぞれが伸縮性を有し, 患者の体動を妨げないようになっているが, パージ管 2 4, 2 5, およびモータケ一ブル 7 1の伸び量より, 保護管 7 2の伸 び量を短くし, 保護管 7 2の最大伸び量にて接続部 7 0の伸び量を制限すること により, パージ管 2 4 , 2 5 , およびモータケ一ブル 7 1の破損を防止する構造 になっている。 さらに, 保護管 7 2にはケプラー繊維等の破損に強い材料が用い られ, その表面には防水処理が施される。 さらに, 保護管 7 2には金属, 樹脂等 の円形リング 7 3を複数個埋設し接続部 7 0に荷重が直接作用してもパージ管 2 4, 2 5およびモ一タケ一ブル 7 1の変形, 破損を防止する構造となっている。 以上説明したように本発明によれば、 .搬送部上にポンプ制御手段, パージ液循 環手段, 制御手段, 表示手段, 通信手段, 電力供給手段をコンパクト化し載置さ せたことにより病院外での生活が可能となる。
また、 ポンプ制御手段および電力供給手段を複数設けたことにより, 異常時で も血液循環を安全に維持できる。
更に、 パージ液中に消毒剤を添加させることにより血液の凝固をなくし, 限外 ろ過フィルタと共に雑菌を除去, 減殺することができ, 清浄なパージ液を循環さ せることができる。
また、 制御手段により運動負荷に応じて血液ポンプの回転数の制御ができ, 常 に安定かつ安全な血液循環を維持でき、 また、 表示手段により人工心臓の駆動状 況をモニタリングできるため, 自己管理ができ異常時にも自己または第三者によ り対応でき、 また、 通信手段により人工心臓の異常時に対する医師, 病院の対応 が迅速になされるため, 病院外の生活での安全性が大きく向上する。
更に、 接続ターミナルを腹帯状のベルトに取り付けることにより外見を奇にす ることなく, 日常生活を行うことができる。 また, 体動による表皮への影響も少 なくできる。
また、 炭素繊維からなる保持部を体内表皮近傍に埋設したことにより, 保持部 と細胞が強固に癒着し感染症を防止することができる。
また、 .接続部に伸縮性と強度を同時に保持させることにより,使い勝手と安全 性の両立を達成できる。
本発明について人工心臓に用いた好適な実施例について説明したが, 本発明は 、 その技術的な範囲内において上記実施例に様々な変形 ·変更を加えて実施でき ると共に、 人工心肺, 人工透析等に使用することができる。

Claims

請 求 の 範 囲
1 . 人体に使用される人工心臓を駆動するための駆動軸を有する円筒形のモータ 部 1 1、 前記モータ部 1 1より伸延し軸受け 1 9に軸承される回転軸 1 3を介 して駆動され血液を流動させるポンプ翼 1 4と, 前記軸受け 1 9への血液成分 の浸入を防止する接触式の血液シール 1 8を具備し、 前記モータ部 1 1に接続 されポンプ翼 1 4を覆うケ一シング 1 5により構成され、 血液を流動させる血 液ポンプ 1と,
血液に対する前記回転軸 1 3の軸封を維持するため前記血液シール 1 8の接触 部へ流体 (以下パージ液) を循環供給するパージ液循環手段 2 0 , モ一タケ一 ブル 7 1を介し前記血液ポンプ 1のモ一夕部 1 1の駆動を電気的に制御するポ ンプ制御手段 3 0 , 各手段へ電力を供給する電力供給手段 3 3, 前記各手段の 動作状態およびデ一夕を表示する表示手段 4 0 , 外部との情報交換をおこなう 通信手段 5 0、 および前記各手段を駆動制御する制御手段 1 0 0とからなるシ ステム駆動部 4 5と, 前記システム駆動部 4 5を搭載し移動可能な搬送部 6 0 と,
前記血液ポンプ 1とシステム駆動部 4 5を接続する接続部 7 0, および前記接 続部 7 0を表皮 F上に保持する保持部 9 0からなる接続手段 7 4とを有してな ることを特徴とする人工心臓の可搬駆動システム。
2 . 前記血液ポンプ 1は, 血液シール 1 8としてメカニカルシール, またはオイ ルシールを用いることを特徴とする請求の範囲 1項記載の人工心臓の可搬駆動 システム。
3 . 前記システム駆動部 4 5は, 前記各手段に優先度を設けてなり, 前記電力供 給手段 3 3の電力残留値が規定値を下回った場合, 前記構成手段のうち優先度 が高い手段のみ電力を供給することを特徴とする請求の範囲 1項記載の人工心 臓の可搬駆動システム。
4 . 前記システム駆動部 4 5は, 各手段のうち前記ポンプ制御手段 3 0の優先度 を高くすることを特徴とする請求の範囲 3項記載の人工心臓の可搬駆動システ ム。
5 . 前記システム駆動部 4 5は, ポンプ制御手段 3 0および電力供給手段 3 3を 複数系統有することを特徴とする請求の範囲 1項記載の人工心臓の可搬駆動シ ステム。
6 . 前記システム駆動部 4 5は, 最低 1系統のポンプ制御手段 3 0および電力供 給手段 3 3を搬送部 6 0以外の箇所に設置することを特徴とする請求の範囲 5 項記載の人工心臓の可搬駆動システム。
7 .前記パージ液循環手段 2 0は, パージ液を貯留するリザ一バ 2 1と前記血液 ポンプ 1の血液シール 1 8との間でパージ液を循環させるパージ液循環ポンプ 2 2と, パージ液をろ過するフィルタ 2 3と, 前記各要素を接続するパージ液 循環経路を有して構成され, パージ液をフィルタ 2 3を介して前記血液ポンプ 1の内部に循環させてなることを特徴とする請求の範囲 1項記載の人工心臓の 可搬駆動システム。
8 . 前記パージ液は, 純水もしくは生理食塩水からなり, パージ液を無菌状態に て維持するために消毒剤を添加させることを特徴とする請求の範囲 7項記載の 人工心臓の可搬駆動システム。
9 . 前記パージ液の流路中には, 目詰まり防止のバイパス流路 2 6を設け, パ一 ジ液の循環路をクロスフ口一させてなることを特徴とする請求の範囲 7項記載 の人工心臓の可搬駆動システム。
1 0 . 前記パージ液循環ポンプ 2 2と前記フィルタ 2 3および前記リザ一パ 2 1 をュニット化させてなることを特徴とする請求の範囲 7項記載の人工心臓の可 搬駆動システム。
1 1 . 前記パージ液循環手段 2 0において, リザ一バ 2 1は柔軟な材質であるこ とを特徴とする請求の範囲 7項記載の人工心臓の可搬駆動システム。
1 2 . 前記パージ液循環手段 2 0において, フィルタ 2 3の前後に少なくとも 1 つづつ圧力を検出するセンサ 2 7 a、 2 7 bを設けてなることを特徴とする請 求の範囲 7項記載の人工心臓の可搬駆動システム。
1 3 . 前記パージ液循環手段 2 0において, パージ液の温度を検出するセンサ 2 8 a , 2 8 bを少なくとも 1箇所以上に設置することを特徴とする請求の範囲 7項記載の人工心臓の可搬駆動システム。
1 4 . 前記制御手段 1 0 0は, 前記可搬駆動システムを用いる患者の運動負荷を 検出する運動負荷検出機構 3 1を有し, 前記検出機構の出力に応じて前記血液 ポンプ 1の運転条件を制御することを特徴とする請求の範囲 1項記載の人工心 臓の可搬駆動システム。
1 5 . 前記運動負荷検出機構 3 1は, 前記搬送部 6 0の移動速度を検出すること を特徴とする請求の範囲 1 4項記載の人工心臓の可搬駆動システム。
1 6 . 前記運動負荷検出機構 3 1は, 生体の心臓と接続された前記血液ポンプ 1 の, 生体の心拍動数に同期した前記モータ部 1 1の負荷変動を検出し, 推定さ れた生体の心拍動数から患者の運動負荷を検出することを特徴とする請求の範 囲 1 4項記載の人工心臓の可搬駆動システム。
1 7 . 前記運動負荷検出機構 3 1は, 前記搬送部 6 0の移動速度を検出する手段 と, 前記生体の心拍動数を検出する手段の 2つを有することを特徴とする請求 の範囲 1 4項記載の人工心臓の可搬駆動システム。
1 8 . 前記運動負荷検出機構 3 1は, 前記血液ポンプ 1のモータ部 1 1の電流値 の変動を検出し, 推定された生体の心拍動数から患者の運動負荷を検出するこ とを特徴とする請求の範囲 1 6項記載の人工心臓の可搬駆動システム。
1 9 . 前記運動負荷検出機構 3 1は, 前記血液ポンプ 1のモータ部 1 1の回転数 の変動を検出し, 推定された生体の心拍動数から患者の運動負荷を検出するこ とを特徴とする請求の範囲 1 6項記載の人工心臓の可搬駆動システム。
2 0 . 前記表示手段 4 0は, 前記人工心臓の可搬駆動システムを構成する各要素 の動作状態をモニタに表示し, 設定値を外れた場合に警告を発することを特徴 とする請求の範囲 1項記載の人工心臓の可搬駆動システム。
2 1 . 前記システム駆動部 4 5は, 外部と情報交換を行う通信手段 5 0を有し, 患者と病院もしくは医師との連絡, 指示可能な送受信機能を有してなることを 特徴とする請求の範囲 1項記載の人工心臓の可搬駆動システム。
2 2 . 前記接続部 7 0は, 前記血液ポンプ 1と前記パージ液循環手段 2 0を接続 するパージ管 2 4, 2 5と, 前記血液ポンプ 1と前記ポンプ制御手段 3 0を接 続するモ一タケ一ブル 7 1とからなり, 患者の腰部近傍に卷かれたベルト状の 腹帯 8 1上に設置された接続ターミナル 8 0で分割可能に接続されることを特 徴とする請求の範囲 1項記載の人工心臓の可搬駆動システム。
2 3 . 前記接続部 7 0は, 前記パージ管 2 4, 2 5と前記モ一タケ一ブル 7 1と を螺旋状に形成し, それらを覆う蛇腹状の保護管 7 2を有してなり, 伸縮自在 なることを特徴とする請求の範囲 2 2項記載の人工心臓の可搬駆動システム。
2 4 . 前記接続部 7 0は, 前記保護管 7 2の伸び量を, 前記パージ管 2 4 , 2 5 , およびモ一夕ケーブル 7 1の伸び量より短く形成させてなることを特徴とする 請求の範囲 2 2項記載の人工心臓の可搬駆動システム。
2 5 . 前記保護管 7 2は円形なるリング 7 3を複数埋設させてなることを特徴と する請求の範囲 2 2項記載の人工心臓の可搬駆動システム。
2 6 . 前記パージ管 2 4 , 2 5には, パージ液の流れを遮断する弁機構 7 5を有 することを特徴とする請求の範囲 2 2項記載の人工心臓の可搬駆動システム。
2 7 . 前記弁機構 7 5は, 前記接続ターミナル 8 0と前記血液ポンプ 1との間に 設置されてなることを特徴とする請求の範囲 2 6項記載の人工心臓の可搬駆動 システム。
2 8 . 前記接続夕一ミナル 8 0には, 最低 1系統のポンプ制御手段 3 0および電 力供給手段 3 3を有することを特徴とする請求の範囲 2 2項記載の人工心臓の 可搬駆動システム。
2 9 . 前記保持部 9 0は, 表面に多数の空隙を有する炭素繊維構造物から構成さ れ人体の表皮 Fの内部に装着されることを特徴とする請求の範囲 1項記載の人 ェ心臓の可搬駆動システム。
3 0 . 前記搬送部 6 0は, 体外に配設される前記システム駆動部 4 5を搭載し搬 送可能な車輪 6 2付き載置台を有し, 自動または手動操作により人体と共に搬 送されることを特徴とする請求の範囲 1項記載の人工心臓の可搬駆動システム。
3 1 . 前記搬送部 6 0は, 前記システム駆動部 4 5への汚水, ほこり等の侵入を 防止する防水, 汚染防止機構を有することを特徴とする請求の範囲 3 0項記載 の人工心臓の可搬駆動システム。
3 2 . 前記搬送部 6 0は, 移動に伴う振動, 衝撃を前記システム駆動部 4 5へ伝 達しないように衝撃吸収機構を有することを特徴とする請求の範囲 3 0項記載 の人工心臓の可搬駆動システム。
3 3 . 前記電力供給手段 3 3は, 商用交流電源, 自動車等の車載電源, または前 記搬送部 6 0に内蔵した 2次電池などの複数の電源を切り替えて使用すること を特徴とする請求の範囲 1項記載の人工心臓の可搬駆動システム。
