Abstract
L'invention concerne un appareil et des procédés permettant de mettre en place et de faire progresser un instrument de diagnostic ou de traitement dans un organe creux du corps humain, d'une anatomie tortueuse ou non supportée, comprenant une poignée, un tube de recouvrement disposé dans un manchon hydrophile, et une région distale présentant une extrémité atraumatique. Le tube de recouvrement peut être retiré de la poignée et présente un axe longitudinal disposé obliquement par rapport à la poignée. Le manchon est jetable afin de pouvoir réutiliser le tube. Des mécanismes de tension à sécurité intégrée peuvent être prévus pour raidir sélectivement le tube de recouvrement, afin de réduire les distensions de l'organe causées par la progression dudit instrument de diagnostic ou de traitement. Les mécanismes de tension à sécurité intégrée réduisent le risque de reconfiguration du tube, en cas de défaillance du système de tension et, dans une forme d'exécution, rigidifient le tube de recouvrement, sans déplacement proximal sensible de la région distale. La région distale permet le guidage passif du tube, résultant de la déflexion de l'instrument de diagnostic ou de traitement, cependant que l'extrémité atraumatique empêche que la paroi de l'organe soit saisie ou pincée durant la manipulation dudit instrument.An apparatus and methods for positioning and advancing a diagnostic or treatment instrument in a hollow organ of the human body, with a tortuous or unsupported anatomy, comprising a handle, a cover tube disposed in a hydrophilic sleeve, and a distal region having an atraumatic end. The cover tube can be removed from the handle and has a longitudinal axis disposed obliquely to the handle. The sleeve is disposable so that the tube can be reused. Safety integrated tensioning mechanisms can be provided to selectively stiffen the covering tube, in order to reduce the distensions of the organ caused by the progression of said diagnostic or treatment instrument. The fail-safe tensioning mechanisms reduce the risk of reconfiguring the tube in the event of a tension system failure and, in one embodiment, stiffen the covering tube, without appreciable proximal displacement of the distal region. The distal region allows passive guidance of the tube, resulting from the deflection of the diagnostic or treatment instrument, while the atraumatic end prevents the wall of the organ from being grasped or pinched during the manipulation of said instrument.
