For instance, as illustrated in Figure 1, stem 42 extends from outer surface 28 of sidewall 20. In an alternative implementation, stem 42 can extend from exterior surface 24 of backing element 18. Turning briefly to Figure 8, stem 42 can have a proximal end 44 connected to casing element 12 (e.g., sidewall 20 and/or backing element 18) and a distal end 46 opposite proximal end 44.
